瘻中の血栓の血管中電解血栓形成のための方法及び装置
に関するものである。

的に3通りの治療方法、すなわち血管外、血管内及び血
管内外アプローチがある。
治療部位の手術又はマイクロ(顕)微手術からなる。こ
の療法は、頭蓋内漿果動脈瘤において通常使用されてい
る。この方法は、動脈瘤の頚を切除し、頚の縫合結紮を
行い又は動脈瘤全体を包むステップから構成される。こ
れらの手術処置それぞれは、体内への侵入によって遂行
され、また動脈瘤又は目標となる部位の外から遂行され
る。全身麻酔、直線的開頭術、脳押圧(brain retracti
on)及び動脈瘤の頚周辺及び切除部位での蜘網膜解剖並
びにクリップの配置が、これらの手術処置において通常
必要となる。頭蓋血管動脈瘤の手術療法は、死亡率4な
いし8%、罹病率18ないし20%の可能性がある。この死
亡率及び罹病率の可能性のため、手術処置は、手術のた
めの最良の時期を待っている間にしばしば遅滞し、結果
としてさらに多くの患者が手術前に潜在的な疾病又は欠
陥によって死亡することとなる。このため、従来技術
は、この代替となる治療方法を求めてきた。

ーテル・ガイドワイヤー」、米国特許第4,884,579号に
示されているマイクロカテーテル及びイングルソン、
「ガイドワイヤー・トラッキングのためのカテーテ

攣が消散するまで処置を遅延しなければならず、これに
よって再出血の危険が生ずる。

は、体外から動脈瘤にアプローチするために、何らかの
侵入的な処置を伴うものである。
この液体接着剤は、血液と接触した時点で堅固な魂を形
成するためにすばやく重合するイソブチル−シアノアク
リレート(IBCA)である。液体接着剤は、小さい針で嚢
に穴をあけることによって動脈瘤に注入される。IBCA注
入中に親動脈にIBCAがこぼれることを回避するために、
親動脈の血液の流れは、瞬間的に減少するか又は中断し
なければならない。一方で、膨張した風船を、注入を受
ける動脈瘤の頚の水準で動脈に配置されるであろう。親
動脈の一時的な閉鎖によって生ずる危険に加えて、これ
が結果として生ずる動脈の閉塞によって完全に閉鎖され
ない場合には、かかる重合接着剤の親動脈への浸透の危
険が存在する。
瘤壁を通じてホッグ・ヘアーを注入するよう空気銃を利
用している。この処置の成功には、空気銃の注入を可能
とするに十分な動脈瘤の露出が必要となり、血栓形成に
成功するとの納得の行くような結果は示されていない。
気プローブの動脈瘤の嚢に対する定位配置及びその後に
続く鉄製のマイクロ球体の注射針による動脈瘤への注入
からなる。血管外磁石を通じたマイクロ球体の集合の後
に動脈瘤内血栓が生ずる。この療法は、血管外磁石が撤
去されるときに金属性血栓の分裂の危険性があることに
より、完全に成功しているとはいえない。メチル・メタ
クリレートにおける鉄粉の懸濁が分裂を防ぐために使用
されている。この療法は、動脈瘤に穴をあけることが必
要であること、親動脈閉塞の危険、特殊かつ高価な設備
の使用、頭蓋局部切除及び全身麻酔が必要であること、
そして動脈瘤に到達するために脳組織を貫通することが
必要であることを理由として、好まれてはいない。

ルによって動脈瘤内にチップを配している。チップは、
マイクロカテーテルを通じて配置された光ファイバに接
着されている。光エネルギーがマイクロカテーテルの近
接端におけるリモート・レーザーから光ファイバに沿っ
て移送される。光エネルギーは、動脈瘤の頚又は閉塞さ
れるべきその他の血管の開口周辺の組織を焼灼するため
にチップを加熱する。カテーテルには、焼灼及び閉塞が
なされる部位への血液の流れを遮断するために、その末
端又は末端周辺に、風船が設けられている。通常、血液
の流れは、カテーテル・チップにおける熱を奪う可能性
があり、これによって焼灼を妨げる。また、チップ内の
熱は、光ファイバの末端にチップを固着するために使用
されている接着剤を溶解するようにも作用する。すべて
が上首尾に行けば、チップは光ファイバから分離し、動
脈瘤の頚中の適所に放置されることができる。但し、こ
れは、焼灼が熱溶解接着剤が溶解すると同時に完了する
ことを条件とする。

結合性組織を形成するための蛋白質の変性であり、卵白
が加熱され、透明な液体から不透明な白色の固体に凝固
するときと類似している。従って、組織の特性及び凝固
した組織の構成は、赤血球及び白血球、血小板並びにフ
ィブリノゲンの血栓集合によって形成される血栓とは実
質的に異なったものである。凝固組織は、実質上血栓塊
よりも柔軟なものであり、従ってさらに容易に除去でき
る。

チップを光ファイバに固着している熱溶解接着剤が溶解
し、隣接する血液組織に分散する。かかる血液組織にお
いて、これは、頭蓋内血液流内で自由粒子(free parti
cles)を形成して再凝固し、強磁性電気血栓の分裂から
生ずる不都合とほぼ同様の結果を生ずる。

に、分離を遂行するための追加期間において同一又は異
なったレベルの電流を適用して血液中で電解による分解
を生ずるために、分解できる部分を露出するよう、ガイ
ドワイヤーが再配置される。

イドワイヤー10は、その末端においてテーパ状になって

形成し、かかる部分14は、ガイドワイヤー10の直径16に
沿って縦に延在している。そして部分16は、第1接着部
20から第2接着部22までの薄い糸状部分18まで徐々に細
くなっている。

いる。ステンレス鋼コイル26は、通常長さ3ないし10cm
であり、ガイドワイヤーと同様に、通常0.010ないし0.0
20インチ(0.254ないし0.508mm)の直径を有している。
糸状部分18の末端に、また第2接着部22においてプラチ
ナ二次コイル28の近接端にはんだづけされている。二次
コイル28自体が通常直径2ないし10mmのらせんを形成し
ている。二次コイル28によって形成されるらせん状のエ
ンベロープは、円筒形又は円錐形であっても良い。ガイ
ドワイヤー10及びステンレス鋼コイル26と同様に、二次
コイル28は、直径約0.010ないし0.020インチ(0.254な
いし0.508mm)である。ステンレス鋼コイル26及びコイ
ル28を形成するワイヤー自体の直径は、約0.001ないし
0.005インチである。

のチップ外に配されれば、第1図に示される形状を形成
し、また動脈瘤の内部形状に対して同様に緩やかに変形
できる。

外の電圧源からガイドワイヤーに対して直流を適用す
る。動脈瘤の腔内の二次コイル28上の正電荷によって、
電気血栓によって動脈瘤内での血栓の形成を生ずる。チ
ップ分離は、以下のいずれかの方法で生ずる。(1)部
分18が血液に露出しているときにあらかじめ定めた時間
において電流を継続的に適用する方法、又は(2)部分
18を血液に露出するためにワイヤーを移動し、次にあら
かじめ定められた時間において電流を継続的に適用する
方法。最終的に、糸状部分及びステンレス鋼コイル部分
26の両方が少なくとも1地点で完全に分解し、よってガ
イドワイヤー10を血管スペースから撤去し、一方動脈瘤
内に形成された血栓内に付着した二次コイル28を放置す
る。第2図は、部分的な拡大断面図において、本発明の
第2の実施態様を図解している。ステンレス鋼核32は、
円錐形の末端部分34において終結している。断面図に示
されているステンレス鋼コイル36は、接着部38において
ガイドワイヤー32の末端部分34にはんだづけされてい
る。ステンレス鋼コイル36の反対の端には、はんだづけ
された球体のプラチナ・チップ40が設けられている。図
解される実施態様において、ステンレス鋼核ワイヤー32
は、直径約0.010インチであり、ステンレス鋼コイル36
の長さは約8cmであり、プラチナ・チップ40の縦の長さ
は、3ないし10mmである。チップ40から近接端までのガ
イドワイヤー32の全体的な長さは、約150cmである。

は、コイル36からチップ40までステンレス核32が延長し
ていないことによって第1図に示される実施態様と区別
される。第2図の実施態様の場合には、ステンレス鋼コ
イル36内に内部核又は強化材が設けられていない。糸状
部分18は、第1図の実施態様においてガイドワイヤーの
引張強度の増大を可能とするために設けられている。し
かしながら、ガイドワイヤーの柔軟性の度合いは、糸状
チップ18を含めることによってでさえ犠牲になるので、
第2図の実施態様は、少なくともステンレス鋼コイル36
を構成するマイクロガイドワイヤーの部分について、さ
らに柔軟性のあるチップを設けている。

2号(1985)及びサムソン等、「短スプリング・チップ
を伴ったカテーテル・ガイドワイヤー及びその使用方
法」、米国特許第4,554,929号(1985)において示され
ている。

ている。プラチナ・コイル56は、あらかじめプレバイア
スされておらず、また内部強化材も有していないが、自
由かつ開放的なコイルである点において第2図の実施態
様のステンレス鋼コイル36に類似している。

金は、特に柔軟性のあるもので、プラチナ・コイル56を
形成するために使用されるワイヤーの直径は、約0.001

には、なめらかな球体のプラチナ・チップ58が設けられ
ており、この点で第1図及び第2図それぞれのチップ30
及び40に類似している。

を使用した、第3図の実施態様の血管66への挿入を図解
している。コイル56は、ステンレス鋼コイル46の少なく
とも1部分がマイクロカテーテル44の末端チップ62を超
えて露出されるまで、動脈瘤64内に送り込まれる。0.1
ないし6ボルトで、約0.01ないし2ミリアンペアの正電
流が、ガイドワイヤー42に適用され、血栓を形成する。
通常、血栓は、3ないし5分以内で形成される。電圧源
70の陰極72が通常皮膚上に、また皮膚と接触して配置さ
れる。

コイルの形状が形状及びエンベロープのバリエーション
をもって設けられ得ることが理解されなければならな
い。さらに、マイクロガイドワイヤー・チップの構成
は、プラチナ以外の要素(ステンレス鋼、ベリリウム、
銅、及びプラチナと同様又は異なった様々な合金を含
む)からも製造できる。さらに、ガイドワイヤーの直
径、上記の多様なガイドワイヤー及び分解可能なチップ
に近接するステンレス鋼コイルは、チップからの電解に
よる分離を遂行するために必要な時間及び電流規模を変
更するために異なった直径又は断面積で設けることがで
きる。さらに、本発明は、末端チップのガイドワイヤー
からの分離の正確な瞬間を決定するためにガイドワイヤ
ーの近接端に接続された従来のエレクトロニクスを含め
ることができる。

Claims (11)
- 【請求項1】血液中で電解によって分解できる末端部分
を有する核ワイヤーと、 血管腔内で閉塞を形成するために適合されるあらかじめ
定められた線状の範囲において、前記核ワイヤーを延長
し、かつ前記核ワイヤーの前記末端部分と連結する細長
いチップ部分であって、前記チップ部分が血管腔内の血
管内挿入のためのものであり、前記の細長いチップ部分
が血液中で電解によって分解できない、細長いチップ部
分と、からなり、 これによつて前記血管腔の血管内閉塞が遂行できる、マ
イクロカテーテルとの組み合わせで使用される、血管腔
内の閉塞形成に使用されるガイドワイヤー。 - 【請求項2】前記核ワイヤーの前記末端部分が、露出し
ているステンレス鋼部分である、請求項1記載のガイド
ワイヤー。 - 【請求項3】前記ステンレス鋼部分が、近接端及び末端
を有し、かつその近接端において前記核ワイヤーに接続
し、またその末端において前記ガイドワイヤーの前記の
細長いチップ部分に接続しているコイルからなる、請求
項2記載のガイドワイヤー。 - 【請求項4】前記核ワイヤーが糸状部分において前記ス
テンレス鋼コイル内で、同心円状に、前記ステンレス鋼
コイルの近接端から、前記ステンレス鋼コイルが前記ガ
イドワイヤーの前記の細長いチップ部分に接続する場所
まで延在する、請求項3記載のガイドワイヤー。 - 【請求項5】前記コイルが内部空間を確定し、該内部空
間が自由かつ補強されていない、請求項3記載のガイド
ワイヤー。 - 【請求項6】前記の細長いチップ部分が長い実質的に柔
軟な部分であり、また血液内で電解によって分解できな
い金属からなる、請求項1記載のガイドワイヤー。 - 【請求項7】前記の細長いチップ部分が479プラチナ合
金からなる、請求項1記載のガイドワイヤー。 - 【請求項8】前記の細長いチップ部分の前記の長い柔軟
な部分が、血管腔に挿入されたときに、実質的に前記血
管腔を満たすに十分な長さである、請求項6記載のガイ
ドワイヤー。 - 【請求項9】血液中で電解によって分解できる末端部分
を有する核ワイヤーと、 あらかじめ定められた線状の範囲において前記核ワイヤ
ーを延長し、かつ前記核ワイヤーの前記末端部分に連結
している細長いチップ部分であって、前記チップ部分が
血管腔内の血管内挿入のためのものであり、前記の細長
いチップ部分が血液中で電解によって分解できない、細
長いチップ部分と、からなり、 前記の細長いチップ部分が長い実質的に柔軟な部分であ
り、かつ血液内で電解によって分解できない金属からな
り、かつ前記の長い柔軟な部分が、マイクロカテーテル
から延長したときに、らせんを形成するようあらかじめ
プレバイアスされており、 これによつて前記血管腔の血管内閉塞が遂行できる、マ
イクロカテーテルとの組み合わせで使用される、血管腔
内の閉塞形成に使用されるガイドワイヤー。 - 【請求項10】前記らせんが円錐形のエンベロープを有
する、請求項9記載のガイドワイヤー。 - 【請求項11】前記らせんが円筒形のエンベロープを有
する、請求項9記載のガイドワイヤー。
