Description
本出願は、2007年12月21日に出願された米国仮特許出願第61/016,342号の利益を主張し、この出願の全開示は本明細書中に参考として援用される。
本発明は、全体として医療装置および方法に関し、より詳しくは蛍光X線透視法および磁気共鳴法において見ることができるハイドロゲルフィラメント、および生物医学的治療におけるそのような材料の使用のための方法に関する。
現在、脳血管疾患および/または末梢血管疾患の患者に対して、介入的神経放射線医/神経外科医には3つの主要な塞栓デバイスの選択肢、すなわち白金コイル、ハイドロゲル/白金コイル、または分解性高分子/白金コイルがある。3種類のコイルはすべて動脈瘤の中に留置され、それぞれに固有の利点および弱点がある。白金コイルは標準的なマイクロカテーテルを通して留置しやすく、広い範囲の柔らかさのものが手に入り、サイズが小さいかまたはくびれのある動脈瘤に最も適している。ハイドロゲル/白金コイルも標準的なマイクロカテーテルを通して留置しやすい。ハイドロゲル/白金コイルは白金コイルと比べると固く、動脈瘤の内部に留置するのが困難なことがあるが、より広い範囲の嚢およびくびれサイズの場合に満足な結果が得られる。分解性高分子/白金コイルは容易に移動し、動脈瘤嚢の中に留置されるが、サイズが小さいかまたはくびれのある動脈瘤の場合にしか満足な結果が得られない。
本明細書には、1つ以上の脳血管疾患および/または末梢血管疾患の結果として生じる構造物および機能不全部を閉塞するための装置、組成物、システム、および関連する方法が記載されている。遅延制御された膨脹速度を有する1つ以上の可視化剤を含むハイドロゲルフィラメントを用いてこれらの構造物および機能不全部を処置し、それによって構造物または機能不全部の内部に入ったデバイスの位置の移動を可能にする。さらに、1つ以上の可視化剤、例えば制御された膨脹速度を有する放射不透過成分または充填材を含むハイドロゲルフィラメントによって、フィラメントを適切に配置するのに十分な時間が外科医にもたらされ、フィラメントが直ぐに膨脹するせいで急ぐ必要がなくなる。
PEG1000ジアクリルアミドの調製
最初に、200mLのトルエンとの共沸蒸留によって18gのポリエチレングリコール(PEG)1000を脱水した。次に、7.0mLのトリエチルアミンを4.6mLのメシルクロリドとともに加えて4時間撹拌した。次に、この溶液をろ過して塩を除去し、溶媒を蒸発させた。得られた生成物を150mLの25%アンモニア水酸化物に加え、2日間撹拌した。水を取り除き、トルエンとの共沸蒸留によって生成物を脱水した。得られた無水PEGジアミンを20mLのジクロロメタンおよび50mLのトルエンに溶かした。次に、7.0mLのトリエチルアミンおよび4.9mLのアクリロイルクロリドを加え、撹拌しながら4時間反応させた。得られた溶液をろ過し、溶媒を除去してPEG1000ジアクリルアミドを得た。
放射不透過モノマーの調製
最初に、9gのトリヨードフェノールをアルゴン下150mLのジクロロメタンに溶かした。次に、撹拌しながら3.15mLのペンテノイルクロリドを加えた。次に、トリエチルアミンをゆっくり加え、4時間撹拌した。この溶液を100mLの水で洗浄し、蒸発乾固させて2,4,6−トリヨードフェニルペンタ−4−エノエートを得た。
クロロホルム中の放射不透過ハイドロゲルフィラメントの調製
有機溶媒中の放射不透過ハイドロゲルを調製するために、2gの2,4,6−トリヨードフェニルペンタ−4−エノエート、0.67gのアクリル酸、1.2gのPEGジアクリルアミド400、24mgのN,N−メチレンビスアクリルアミド、および75mgのアゾビス(2−メチルプロピオニトリル)を2.5mLのクロロホルムに溶かした。次に、溶液にアルゴンを10分間吹き込んだ後、3ccの注射器を用いて0.020インチのポリエチレン管の中に注入した。チューブの両端を熱で封止し、80℃のオーブンの中に一夜置いて溶液を重合させた。
バリウム充填放射不透過ハイドロゲルフィラメントの調製
有機溶媒中のバリウム充填放射不透過ハイドロゲルを調製するために、7gの硫酸バリウム、0.5gのアクリル酸、5gのポリ(テトラメチレンオキシド)ジアクリルアミド1000、1.25gの2−ヒドロキシエチルメタクリレート、212mgのN,N−メチレンビスアクリルアミド、および100mgのアゾビス(2−メチルプロピオニトリル)を3.5mLのイソプロピルアルコールに溶かした。次に、溶液にアルゴンを10分間吹き込んだ後、3ccの注射器を用いて4mmのマンドレルの周りにかぶせた0.010インチのHYTREL(登録商標)管の中に注入した。チューブの両端を熱で封止し、100℃の湯浴中に1時間、次に80℃のオーブン中に一夜置いて溶液を重合させた。
水中の放射不透過ハイドロゲルフィラメントの調製
水中の放射不透過ハイドロゲルを調製するために、5gの2,5−アクリルアミド−2,4,6−トリヨード−n,n’−ビス−(2,3ジヒドロキシプロピル)イソフタルアミド、1.33gのアクリル酸、2.5gのPEGジアクリルアミド400、50mgのn−ビニル−2−ピロリジノン、および100mgの2,2’アゾビス(2−メチルプロピオンアミジン)2塩酸塩を10mLの水に溶かした。次に、この溶液にアルゴンを10分間吹き込んだ後、3ccの注射器を用いて0.020インチのポリエチレン管の中に注入した。チューブの両端を熱で封止し、80℃のオーブンの中に一夜置いて溶液を重合させた。
放射不透過ハイドロゲルフィラメントの洗浄および酸処理
実施例3に従って重合したハイドロゲルの場合には管を3インチ区画に切り、アセトン中に1時間置いた。アセトン中でハイドロゲルはチューブの端を越えて膨脹し、チューブから取り出すことができた。ハイドロゲルをアセトン中で2時間洗浄した。2時間後にアセトンを取り替え、ハイドロゲルをさらに2時間洗浄した。アセトンを除去し、ハイドロゲルを真空オーブン中50℃で2時間乾燥した。

放射不透過ハイドロゲルフィラメントは、V−TRAK(登録商標)(MicroVention Terumo Inc.,Aliso Viejo,CA)または油圧押し具に取り付けることができる。ハイドロゲルをV−TRACK(登録商標)押し具に取り付けるために0.0022インチのポリ(エチレン)管縫合糸をカプラの中に通した。カプラは、一端を中空にしたチタン円筒体と貫通孔とからなっていた。ポリ(エチレン)管縫合糸が引き抜かれてしまわないように結び目をつくった。接着剤を用いてハイドロゲルを結び目の上でカプラに接着した。ポリ(エチレン)管糸の他方の端をV−TRAK(登録商標)押し具の中に通し、結んだ。
座屈力の測定
動脈瘤嚢の内部に留置する実施例6の放射不透過ハイドロゲルフィラメントの能力を他の現在市販されているコイルと比較するために、さまざまなデバイスの座屈力を測定した。この試験では、大体1インチのデバイスのピースをロウ付けまたはポリ(エチレン)収縮管のどちらかによって約15インチのハイポ管のピースに取り付けた。構成物のハイポ管の端を材料の力のデータを測定するために用いたシステムであるInstron 5543 Single Column Testing Systemに取り付けた。構成物をポリ(カーボネート)ブロックの中の行き止まりのチャネルの中に進ませた。デバイスがチャネルの底に達したら、座屈するように力を加え、対応する力を測定した。
曲げ抵抗の測定
測定ベインに取り付けた5gのおもりを有するGurley 4171ET管状試料硬さ試験機を用いて、膨脹していないハイドロゲル試料の曲げ抵抗および注入可能な白金マイクロコイルの曲げ抵抗を得た。試料の長さは1インチであった。3回の繰り返しの平均をそれぞれ以下の表にまとめた。
注入可能な放射不透過ハイドロゲルフィラメントの評価
バリウム充填放射不透過ハイドロゲル調合物から構成したデバイスを、屈曲血管に合わせた流れモデルの中で評価した。流れ誘導マイクロカテーテル(Boston Scientific Spinnaker 1.5F)を血管の中に配置した。3ccの注射器を用いてマイクロカテーテルの中に5cmから30cmの範囲の長さのデバイスを注入した。導入、移動、留置、および充填についてデバイスを評価した。導入、移動、および留置について合格を実現するために、インプラントは3ccの注射器を用いて支障なく導入、移動、および留置しなければならない。充填について合格を実現するために、インプラントは0.008インチ白金コイルと同様に屈曲血管の中に充填しなければならない。
実験動脈瘤中の放射不透過高分子ハイドロゲルの評価
放射不透過高分子フィラメントによって3つのウサギエラスターゼ動脈瘤を塞栓した。動脈瘤の幅、長さ、およびくびれ部はそれぞれ2.4から3.6mm、4.7から8.8mm、および2.4から4.2mmの範囲であった。マイクロカテーテル(Cordis RAPIDTRANSIT(登録商標),Cordis Corporation,Miami Lake,FL)を動脈瘤嚢の内部に配置した。動脈瘤嚢の中に1つから3つの放射不透過ハイドロゲルフィラメントを留置した。血管造影法は、塞栓の結果として3つの動脈瘤すべての完全な閉塞を示した。塞栓後6週の時点で血管造影法によって3つの動脈瘤すべての完全な閉塞を示した。動脈瘤を回収し、組織学的に処理した。切片は、放射不透過ハイドロゲルフィラメントによる動脈瘤嚢の完全な充填、放射不透過ハイドロゲルフィラメント間の隙間における組織化中/組織化済み線維組織、ならびにマクロファージおよびいくつかの巨大細胞からなる炎症性応答を示した。これらの結果は、放射不透過ハイドロゲルフィラメントを実験動脈瘤の中に留置し、他の塞栓デバイスと同様な異物応答を誘発することができることを示した。
Claims (22)
- 移植のためのデバイスであって、
2官能低分子量エチレン系不飽和成形性マクロマー、ここで当該マクロマーは400グラム/モルおよび1,000グラム/モルの間の分子量を有する;
エチレン系不飽和モノマー;および
40%から60%重量/重量の硫酸バリウム;
を含み、ここで当該デバイスは支持部材を含まず、そしてハイドロゲルフィラメントが0.5±0.4gfの座屈力を有する、前記デバイス。 - 前記ハイドロゲルは、環境応答性である、請求項1に記載のデバイス。
- 前記マクロマーは、ポリエチレングリコール、プロピレングリコール、ポリ(テトラメチレンオキシド)、ポリ(エチレングリコール)ジアクリルアミド、ポリ(エチレングリコール)ジアクリレート、ポリ(エチレングリコール)ジメタクリレート、それらの誘導体、またはそれらの組み合わせを含む、請求項1に記載のデバイス。
- 前記エチレン系不飽和モノマーは、1つ以上のイオン化可能な官能基を含む、請求項1に記載のデバイス。
- 前記エチレン系不飽和モノマーは、N,N’−メチレンビスアクリルアミド、N−ビニルピロリジノン、2−ヒドロキシエチルメタクリレート、それらの誘導体、またはそれらの組み合わせを含む、請求項1に記載のデバイス。

- 前記ハイドロゲルは、生体再吸収性である、請求項1に記載のデバイス。
- a)400グラム/モルおよび1,000グラム/モルの間の分子量を有する2官能低分子量エチレン系不飽和成形性マクロマー、エチレン系不飽和モノマー、40%から60%重量/重量の硫酸バリウム、および溶媒を一緒にしてプレポリマ溶液を調製するステップ、および
b)前記プレポリマ溶液を処理して生理条件下で膨脹可能であり、かつ0.5±0.4gfの座屈力を有するハイドロゲルを調製するステップ
を含む動物における移植のためのデバイスを調製するための方法。 - 前記溶媒は、水、ジクロロメタン、アセトン、イソプロピルアルコール、エタノール、またはそれらの組み合わせを含む、請求項14に記載の方法。
- 前記エチレン系不飽和モノマーは、イオン化可能な官能基を含む、請求項14に記載の方法。
- 前記溶媒は、前記プレポリマ溶液の20%重量/重量から80%重量/重量を構成する、請求項14に記載の方法。
- 前記モノマーは、重量で前記プレポリマ溶液の40%から80%を構成する、請求項14に記載の方法。
