Abstract

添付図面を参照して、本発明の或る例示実施形態を以下に説明する。一般に、このような実施形態は、好ましくは脊椎の腰領域に茎ねじを用いて背側脊椎固定手術を行うための固定システム(非制限的な例として態様小侵襲性または最小侵襲性固定システム)に関する。例えばインプラントおよび機器を含む固定システムは、脊椎固定または背側脊椎固定に限定されるものではなく、他の用途を有しかつ種々の形態および構造にすることができる。例えば、骨アンカ−は、一般に茎ねじとして示されかつ茎ねじと呼ばれ、多軸または単軸茎ねじ、フック(単軸および多軸)または他のファスナ、クランプまたはインプラントとして構成できる。
或いは、ラテラルインプラントホルダ1のフランジ14およびスリ−ブ20のフランジ28は互いに隣接して配置でき、親指ノブ12はいずれかの側、例えばフランジ14、28より近位側に配置できる(図示せず)。

- 第二部材は、アクチュエーション機構を第一部材に向かって移動させると第一および第二ねじを離れる方向に移動させかつアクチュエーション機構を第一部材から離れる方向に移動させると第一および第二ねじを一緒に移動させるように形成されていることを特徴とする請求項33記載の固定システム。
- 第二部材はシーソー形状でありかつ第一部材にピボット連結されていることを特徴とする請求項32記載の固定システム。
- 第一部材は、1つの機器を受入れるサイズおよび形状をもつ貫通孔を有していることを特徴とする請求項32記載の固定システム。
- 移動機構は、第二部材に関連して作動する第三部材を更に有し、第二および第三部材は、第一部材に向かう第三部材の移動により、第二部材の遠位端が第一部材に向かって移動されるように構成されかつ配置されていることを特徴とする請求項32記載の固定システム。
- 第二および第三部材は第一部材にピボット連結されていることを特徴とする請求項39記載の固定システム。
- バーは、第一および第二部材の方向を互いに固定するロッキング機構を有していることを特徴とする請求項32記載の固定システム。
- 骨内に挿入されるサイズおよび形状を有するねじを有し、該ねじはねじヘッドを備え、
遠位端を備えたラテラルインプラントホルダを有し、該ラテラルインプラントホルダはねじヘッドを保持する複数のジョーを備え、
遠位端および長手方向軸線を備えたスリーブを更に有し、該スリーブは、ラテラルインプラントホルダに関連して作動しかつラテラルインプラントホルダに対して移動でき、スリーブの遠位端はラテラルインプラントホルダの複数のジョー上に配置されて、ラテラルインプラントホルダがねじヘッドから分離することを防止し、スリーブは、該スリーブの長手方向軸線の回りで部分的に延びている組織保護部分を備えていることを特徴とする最小侵襲性固定システム。 - 第一および第二スリーブを互いに隣接して配置したとき、第一および第二組織保護部分の相互の干渉が小さくなることを特徴とする請求項42記載の最小侵襲性固定システム。
- 組織保護部分は、ヘリカル形状のカットアウトを有していることを特徴とする請求項43記載の最小侵襲性固定システム。
- 第一組織保護部分は第一方向に延びているヘリカルカットアウトを有し、第二組織保護部分は第一方向とは反対の第二方向に延びているヘリカルカットアウトを有していることを特徴とする請求項44記載の最小侵襲性固定システム。
- 細長い複数のラテラルインプラントホルダを用意し、各ラテラルインプラントホルダが、遠位端と、ねじヘッドを備えたねじを受入れるための、前記遠位端に設けられた受入れ部分とを備え、
遠位端を備えた複数のスリーブを用意し、各スリーブの遠位端は、ラテラルインプラントホルダの係合部分と接触するサイズおよび形状を有し、各スリーブは、隣接する組織保護部分との干渉が小さいサイズおよび形状を有し、
ラテラルインプラントホルダをねじに取付け、
スリーブを、ねじをラテラルインプラントホルダから取外すことができる、ラテラルインプラントホルダ上の第一位置から、ねじがラテラルインプラントホルダにより保持される、ラテラルインプラントホルダ上の第二位置まで移動させ、
ラテラルインプラントホルダおよびスリーブを、身体の切開部を通して体内に挿入し、
駆動機構を用いてねじを骨内に挿入し、
ロッドを第一ロッド位置において体内に挿入しかつロッドがねじヘッド内に挿入されるときにロッドを第二ロッド位置に移動させ、
ねじに対してロッドを固定することを特徴とする、体内の骨に取付けられたねじ内にロッドを移植する方法。
