发明内容

参照图1,本公开文本的系统采用一些连续计算机执行模块来制备和施用诸如化疗化合物的处方流体药物化合物。这些模块均包括代码,该代码允许用户进行输入、产生输出并且计算和确定可在一个或更多个合适的计算装置的一个或更多个处理器101上执行的用于制备和施用药物化合物的指令。更具体地,系统允许内科医生输入针对患者的处方,随后检验处方的准确性,基于计算机辅助指令来准备处方,基于测得的重量来检验处方并且将处方施用于患者。此系统包括以下详细描述的特定模块。这些模块包括:(A)计算机化的医嘱输入(CPOE)模块100;(B)药剂师检验模块200;(C)药物制备模块300;(D)药物最终检验模块400;以及(E)临床(例如施用)模块500。这些模块可均在设置在单个计算装置上的单个处理器或多个处理器上分别执行或者均可在其自身具有处理器的独立计算装置上执行,在该计算装置中,使用诸如但不限于以太网(Ethernet)、WiFi、蜂窝、Bluetooth等的任何合适的有线或无线通信协议,在计算装置之间进行数据和信息的通信。
A.CPOE模块
CPOE模块100使内科医生能够输入针对患者的处方治疗医嘱,医嘱包括与特定患者关联的处方药物。特别地,内科医生将针对患者的处方信息输入计算机中,并且通过医院内网来传输数据并且存储数据,以便由本文中描述的后续模块获取和使用。处方信息可包括一种或更多种药物和对应这些药物的相应剂量/数量。CPOE模块100是可选的组件,可能并非在实现整体系统的每种情形下都能利用。
B.药剂师检验模块
系统的药剂师检验模块200使药剂师能够查看内科医生输入CPOE模块100中或者在没有利用CPOE模块100的情形下来自某种其他来源的处方信息数据,并人工检验针对特定患者的处方治疗。如以上所讨论的,药剂师检验模块200可以与CPOE模块100在同一计算装置上实现。或者,药剂师检验模块200可在与实现CPOE模块100的计算装置远离的计算装置上实现。
C.药物制备模块
参照图2和图3,药物制备模块300包括软件和诸如药物制备系统1的关联硬件,用于指导药剂师或非药剂师技术人员遍历诸如化疗药物的处方流体药物化合物的步骤。药物制备系统1辅助药剂师或非药剂师技术人员制备含有一种或更多种处方药物化合物的注射器或静脉注射(IV)袋。药物制备系统1有效连接到计算装置4,计算装置4包括具有显示器5和诸如键盘、鼠标等用户输入装置7的用户界面3。可选地,用户界面3的显示器5可以实现为设置在药物制备系统1的一部分上的小LED投影仪,用于将所显示的信息投影到层流罩25的后壁上,由此不再需要如图2中所示的用作显示器5的显示屏。具有秤输出界面11的秤9可以有效连接到用户界面3。秤9可以实现为当物体被放置在其上时检测质量或重量变化的任何合适装置。因此,秤9可以简单构造为当物体的质量或重量大于或小于预定阈值时发送信号的装置,或者提供放置在其上的物体的重量的准确读数的高精度秤。
在一个实施例中,条形码扫描仪13可以有效连接到用户界面3和秤9中的至少一个,使得条形码扫描仪13可以扫描放置在秤9的一部分上的带有条形码的药瓶。在另一个实施例中,图像采集装置15可以有效连接到用户界面3和秤9中的至少一个,使得图像采集装置15可以拍摄诸如放置在秤9的一部分上的药瓶、IV袋或注射器等物品的照片。在一个实施例中,图像采集装置15可以采集整个药物化合处理期间放置在秤9的一部分上的物品的多个静止图像或运行视频,以便记录和/或后续审核药物配制过程。
在又一个实施例中,条形码扫描仪13和图像采集装置15中的至少一个可以至少部分密封在密封外壳17内。在某些构造中,外壳17可完全密封条形码扫描仪13和图像采集装置15。可选地,外壳17可以只包括条形码扫描仪13和图像采集装置15中的一个。
外壳17可以设置在秤9的一部分上方,外壳17例如由支撑臂19支撑。如图2中所示,药物制备系统1可以设置在层流罩25内,层流罩25具有用于在层流罩25的内部29内形成空气层流的进气源23和出气口27。外壳17的外表面21可以具有流线型形状和/或轮廓,优化流线型形状和/或轮廓,以减少层流罩25内空气流的扰动。
如图3中所示的空气动力学流线型外壳17以使因在层流气流中有装置而形成的气流扰动降至最少这样的方式进行设计。该构造允许装置被放置在秤的上游附近并且出于检验药物制备目的而仍然具有可接受的重量分析准确度(即,+/-0.05g)和稳定时间(即,不另外超过2秒)。
较小和/或更具流线型的外壳17导致流扰动较小,因此满足准确性和稳定性需求的可能性较高。流线型外壳17具有使流扰动和阻力最小的形式,从而实现了出于药物制备目的而需要的稳定且准确的重量分析读数。另外,在输入装置(即,图像采集装置15和条形码扫描仪13)放置在相对于秤9的上游气流附近的同时,外壳17实现了重量分析秤具有所需的准确性和稳定性。通常,将这些物体放置在秤9附近出于多种原因是理想的区域。流线型外壳17的另一个优点是提供并保持对于药物的无菌制备而言洁净的工作环境。在使用中,流罩中的气流的目的是出于卫生原因形成洁净的区域。气流旁边或上游的物体所形成的湍流区会导致在药物制备期间有可能有污染风险。结果,使输入装置具有空气动力型外壳使层流气流扰动量最小并且降低了任何类型的污染机会。
继续参照图3,秤9可以包括压盘31,诸如秤9的称重表面的一部分,压盘31可以向技术人员提供关于图像采集装置15待采集图像的中心的可视指示,诸如十字槽35。这样允许技术人员在图像采集装置15的视场内正确定位药品配制相关药物37和相关物资,图像采集装置例如是密封在设置在秤9的压盘31上方的外壳17内。压盘31的上表面41可以限定多个凹槽39和/或从压盘31的表面伸出的突起,以将药品配制相关药物37和相关物资摩擦性限制在压盘31的上表面41上。在另一个构造中,压盘31的上表面41可以包括增粘剂或其他增强摩擦表面,以类似地将药品配制相关药物37和相关物资限制在压盘31的上表面41上。凹槽39和/或突起的布置可以不费力地向用户指示可以布置成与图像采集装置15的视场中心重合的压盘31的中心。
多个凹槽39和/或从压盘31的表面伸出的突起可以构造成,限制在药品配制过程期间意外洒到压盘31的上表面41上的任何液体材料。多个凹槽39可以限定接收井(receiving well)47,接收井47用于在可以采用正确的处置技术之前将意外洒出的材料收集和限制在压盘31内的受约束区域内。压盘31的表面可以涂覆耐用组分,耐用组分抵抗因暴露于腐蚀剂,诸如化疗化合物和药品以及清洁剂,诸如漂白剂、异丙醇等而造成的劣化。在某些构造中,耐用组分可以是环氧树脂或基于环氧树脂的涂料或涂层。
参照图4、图14A和图14B,在操作中,通过用户界面3的显示器5上设置的图5A至图5D、图6A至图6D、图7A至图7B、图8A至图8D、图9A至图9D、图10A至图10C、图11A至图11D、图12A至图12C和图13A至图13C中的一个或更多个中示出的一系列显示屏截屏来提示药剂师/技术人员采取以下步骤来制备药物化合物。图4提供了重组有效成分的制备的第一阶段的流程图。首先,操作人员用条形码扫描仪13扫描药物容器上的第一条形码,药物容器包括将被重组以制备处方药物化合物的药品(框301),如图5C中所示。然后,将药物容器放置在秤9上(框302)。如图6A中所示,在用户界面3的显示器5上显示该步骤的展示。一旦重量稳定下来,系统就核实所测得的重量满足重量目标+/-预定容差。另外,图像采集装置15拍摄药物容器的图像并且在用户界面3的显示器5上向用户显示该图像(框304),如图6B中所示。然后,用户取下药物容器并且将图像保存于药品制备的数据记录(框306)。
接下来,技术人员扫描其流体将与待重组药品混合的流体容器的第二条形码(框308),如图6C中所示。然后,将流体容器放置在秤9上(框310),并且一旦重量稳定下来,图像采集装置15就拍摄流体容器的图像并且在用户界面3的显示器5上向用户显示该图像(框312),如图6D中所示。然后,用户取下流体容器并且将图像保存于药品制备的数据记录(框314)。
此后,用户通过将流体容器中的流体注入药物容器中来将待重组药品与流体容器中的流体混合(框316),如图7A中所示。然后,药物容器返回秤9并且检验药物容器的重量(框318),如图7B中所示。一旦重量稳定下来并且已核实(框320),图像采集装置15就基于从秤接收的信号来自动拍摄药物容器的图像并且在用户界面3的显示器5上显示该图像(框322)。如果技术人员判定图像没有满足特定要求,则选择请求新的或另外的图像(框324)。请求另一个照片将把图像采集装置15自动切换成用户界面3处显示的“直播视频模式”(框326)。技术人员此时可以将秤9上的药物容器移至优选的位置并且通过用户界面3来触发图像采集(框328)。如前文所述,将在用户界面3处示出所采集的图像并且通过从秤9上取下物品,技术人员接受图像并且系统自动移至下一个配制步骤(框330)。
一旦完成了药品制剂,系统就打印将布置在重组药品制剂上的条形码标签。
参照图14A和图14B,将描述使用药物制备模块300来制备药物化合物的第二阶段。首先,操作人员用条形码扫描仪13来扫描重组药品制剂上的条形码(框332),如图8A中所示。然后,将重组药物制剂放置在秤9上(框334)并且在秤9上添加空注射器(框336),如图8C中所示。一旦重量稳定下来,系统就检验所测得的重量满足重量目标+/-预定公差。另外,图像采集装置15拍摄药物容器的图像并且在用户界面3的显示器5上向用户显示该图像(框338),如图8D中所示。然后,用户取下重组药物制剂和空注射器并且将图像保存于药品制剂的数据记录(框340)。
接下来,如图9A中所示,指示技术人员用注射器从重组药品制剂中抽取预定量(框342),并且将注射器放回秤9上(框344)。然后,检验重量(框346)并且采集图像(框348),如图9B中所示。如图14和图14B的流程图中所示,如果确定重量太低,则指示技术人员取下注射器(框350和图9C)并且抽取额外量的重组药品制剂(框352和图9D)。
一旦将额外量的重组药品制剂抽取到注射器中,就将注射器放回秤9上(框354),如图10A中所示。然后,检验重量(框356)并且采集图像(框358),如图10B中所示。然后,从秤上取下注射器(框360)。
然后,技术人员扫描其中含有盐水的流体容器,诸如IV袋的条形码(框362)。然后,将流体容器放置在秤9上(框364),并且一旦重量稳定下来,图像采集装置15就拍摄流体容器的图像并且在用户界面3的显示器5上向用户显示该图像(框366)。如果技术人员判定图像没有满足特定要求,则选择请求新的或另外的图像(框368)。请求另一张照片将把相机自动切换成用户界面3处显示的“直播视频模式”(框370)。技术人员此时可以将秤9上的药物容器移至优选的位置并且通过用户界面3来触发图像采集(框372)。如前文所述,将在用户界面3处示出所采集的图像并且通过从秤9上取下物品,技术人员接受图像(框374)并且系统自动等待药剂师授权来引导(框376)。图11A至图11C中的屏幕截图示出该过程。
一旦提供了药剂师授权(框378),用户就将注射器的内容物注入流体容器中(框380),如图12A中所示。然后,将药物容器返回秤9并且检验药物容器的重量(框382)。一旦如图12B中所示重量稳定下来并且已核实(框384),图像采集装置15就基于从秤接收的信号来自动拍摄药物容器的图像并且在用户界面3的显示器5上显示图像(框386),如图12C中所示。如前文所述,将在用户界面3处示出所采集的图像并且通过从秤9上取下物品,技术人员接受图像(框388)。一旦完成了药品制剂,系统就打印将布置在成品药品制剂上的条形码标签,该条形码标签包括表示药物名称和患者信息的编码信息。
药物制备模块300还包括软件指令,该软件指令致使计算装置4的处理器在药品制备期间执行以下动作:(i)从医院内网络获取由内科医生输入CPOE模块100中的处方信息数据;(ii)检验被扫描的条形码与处方信息对应;(iii)确定注射器和/或IV袋的重量是否在待施用药物的量的预定阈值精度内;(iv)确定如果重量不准确的话必须进行的调节;以及(v)向医院内网络发送回与注射器和/或IV袋的重量相关的数据。
D.药物最终检验模块
在制备了处方药物之后,药物最终检验模块400允许药剂师审核药物制备模块300所形成的包括图像采集装置15所拍摄图像的数据和/或记录并且要么批准要么拒绝向患者发放制剂。如上所述,药物最终检验模块400可以与药物制备模块300在同一计算装置上实现。或者,药物最终检验模块400可以在与药物制备模块300的计算装置远离的计算装置上实现。在图15中示意性示出此远程构造。参照图15,药物最终检验模块400包括系统401,系统401具有处理器402,处理器402配置成从药物制备模块300接收关于药物化合物制备的信息。该信息包括药物化合物制备中的至少一个步骤的至少一个图像和在药物化合物制备中的至少一个步骤期间秤9所提供的重量分析测量信息。系统401还包括用户界面403,用户界面403有效连接到处理器402并且配置成基于来自处理器402的指令来显示药物化合物制备中的至少一个步骤的至少一个图像和基于重量分析测量信息的关于药物化合物的浓度是否在可接受公差范围内的指示。
在图16中提供药物最终检验模块400处提供的示例性屏幕截图。在该示例性画面上,示出所采集图像连同对应的配制指令和关于用数学方法确定的药品浓度是在可接受公差范围内还是其以外的指示。因此,药物最终检验模块400提供与从秤收集的叠加有定量测量值并且用数学方法检验的可视信息(即,制备中的每个步骤的照片),以遵循预定的接受标准。
如图16和图17中所示,药物最终检验模块400包括审核窗口420,审核窗口420具有:第一部分422,显示特定药品制备步骤的所选择图像424;第二部分426,显示药品制备步骤中的每个缩略图;以及第三部分428,其显示特定药品制备步骤的配制指示连同由秤提供的定量测量结果和用数学方法提供的关于药品浓度要么在可接受公差内要么在其以外的指示。另外,图标430可以与特定药品制备步骤的缩略图关联,以向药剂师提供满足特定药品制备步骤的公差要求的指示。例如,如果满足公差要求,则可以提供绿色检测标记,或者如果不满足公差要求,则可以提供红色感叹号。审核窗口420还可以包括第四部分432,第四部分432显示允许药剂师要么确认要么拒绝药品制剂的图标434。
继续参照图16和图17,鼠标指针正悬停在从审核窗口420的第二部分426中的左边起算的第五缩略图上方。对于该特定缩略图,在审核窗口420的第一部分422中的缩略图行上方和缩略图下方示出放大照片,在审核窗口420的第三部分428中示出对应的配制指令435连同定量测量值436和方法检查438的结果(另外参见图2中提供的示例性屏幕截图)。第三部分428还可以包括关于药品制备的其他统计信息,诸如但不限于必须以什么频次来重复配制步骤以满足公差目标或者特定配制步骤是否相比于其他情况花费比常规更多的时间。
另外,缩略图中的图标430指示是否满足特定配制步骤的公差要求,从而向药剂师提供问题出现在配制过程中的什么地方的快速指引。围绕多个缩略图的灰框(未示出)向药剂师提供重新采集表示该配制步骤的图像的指示。
所采集图像与算术方法用重量分析数据生成的检验信息叠加允许药剂师快速审核类型迥异的信息。图像的可视信息内容允许药剂师检查非常显见的信息,诸如但不限于药品颜色、注射器类型或系统是否被不正确使用(例如,用户使用另外的物体来产生满足方法公差要求所必需的重量)。表示方法检查结果的缩略图中的图标430是二进制信息,该信息告诉药剂师,药品的量是在特定配制步骤的公差要求内还是在其以外。红色感叹号形式的缩略图上的图标430的累积向药剂师提供技术人员需要进行多次反复以满足公差要求的快速指示并且当审核此药品制剂时可以触发进行额外的仔细查看。
E.临床模块
在向患者施用处方药物之前,临床模块500允许进行最终检验。由护士或其他技术人员使用毗邻患者设置的条形码扫描仪来扫描注射器和/或IV袋上的条形码标签。条形码扫描仪与计算机进行通信,从而检验带有编码在从医院内网获取的患者信息和/或处方信息的第二条形码上的信息。

Claims (20)
1.一种用于制备药物化合物的系统,所述系统包括:
计算装置,包括用户界面和与所述用户界面有效连接的至少一个处理器,所述用户界面为操作人员提供制备所述药物化合物的指令;
秤,与所述至少一个处理器有效连接;以及
图像采集装置,与所述至少一个处理器和所述秤有效连接并且设置成采集在制备所述药物化合物时使用的组分和所述药物化合物中的至少一个的图像。
2.根据权利要求1所述的系统,其特征在于,所述秤在所述至少一个组分放置在其上时向所述至少一个处理器发送信号,所述信号指示由此检测到的重量发生改变并且基于所述信号,所述至少一个处理器触发所述图像采集装置来采集所述至少一个组分的图像。
3.根据权利要求1所述的系统,其特征在于,所述秤向所述至少一个处理器发送信号,以检验所述操作人员已经基于所述至少一个组分的重量提取了在制备所述药物化合物时至少一个组分的正确使用量并且所述至少一个处理器触发所述图像采集装置在所述至少一个处理器核实所述至少一个组分的正确量时采集所述至少一个组分的图像。
4.根据权利要求1所述的系统,其特征在于,所述图像采集装置被触发,从而一旦所述秤上的所述至少一个组分已经稳定,就采集所述至少一个组分的图像。
5.根据权利要求1所述的系统,其特征在于,在所述用户界面上显示所述至少一个组分的图像,供所述操作人员审核。
6.根据权利要求5所述的系统,其特征在于,从所述秤上取下所述至少一个组分致使所述至少一个处理器接受所述图像,将所述图像与数据记录关联并且在所述用户界面上提供指令,以允许所述操作人员移至制备所述药物化合物的指令的下一个步骤。
7.根据权利要求6所述的系统,其特征在于,所述数据记录和图像提供给药剂师,以便进行检验。
8.根据权利要求5所述的系统,其特征在于,如果所述操作人员认为不能接受所述图像,则所述用户界面配置成为所述操作人员提供重新采集所述图像的能力。
9.根据权利要求1所述的系统,所述系统还包括与所述用户界面有效联接的扫描仪。
10.根据权利要求9所述的系统,其特征在于,所述扫描仪配置成扫描设置在所述至少一个组分上的条形码并且为所述至少一个处理器提供关于所述至少一个组分的信息。
11.一种用于制备药物化合物的系统,所述系统包括:
计算装置,包括用户界面和与所述用户界面有效连接的至少一个处理器,所述用户界面为操作人员提供制备所述药物化合物的指令;
秤,与所述至少一个处理器有效连接;以及
密封件,包括图像采集装置,所述图像采集装置具有设置成采集在制备所述药物化合物期间设置在所述秤上的物体的图像的视场,所述图像采集装置与所述至少一个处理器有效连接,
其特征在于,所述秤向所述至少一个处理器发送信号,以基于所述药物化合物的至少一个组分的重量来检验所述至少一个组分的正确量,以及
其中,当所述至少一个处理器检验所述至少一个组分的正确量时,所述图像采集装置采集设置在所述秤上的所述物体的图像。
12.根据权利要求11所述的系统,其特征在于,所述密封件设置在所述秤上方。
13.根据权利要求11所述的系统,其特征在于,所述密封件还包括条形码扫描仪。
14.根据权利要求13所述的系统,其特征在于,所述条形码扫描仪相对于所述秤成角度。
15.根据权利要求13所述的系统,其特征在于,所述条形码扫描仪具有传感器,所述传感器相对于所述秤偏移。
16.根据权利要求11所述的系统,其特征在于,所述密封件具有流线型形状,以使流罩内的流扰动最少。
17.一种审核药物化合物的检验制备的系统,所述系统包括:
处理器,配置成接收关于所述药物化合物的制备的信息,所述信息包括所述药物化合物的制备中的至少一个步骤的至少一个图像和在所述药物化合物的制备中的至少一个步骤期间秤所提供的重量分析测量信息;以及
用户界面,与所述处理器有效连接并且配置成基于来自所述处理器的指令来显示所述药物化合物的制备中的至少一个步骤的至少一个图像和基于所述重量分析测量信息的关于所述药物化合物的浓度是否在可接受公差范围内的指示。
18.根据权利要求17所述的系统,其特征在于,所述药物化合物的制备中的至少一个步骤的至少一个图像包括所述药物化合物的制备中的每个步骤的图像并且所述用户界面包括显示每个步骤的缩略图的区域。
19.根据权利要求18所述的系统,其特征在于,图形指示叠加在每个所述缩略图上,以指示所述药物化合物的浓度是否在每个所述缩略图中示出的所述药物化合物的制备的步骤的可接受公差范围内。
20.根据权利要求17所述的系统,其特征在于,所述用户界面包括显示制备所述药物化合物的指令的区域,所述区域与显示的所述药物化合物的制备中的至少一个步骤的至少一个图像对应。
