Description
Die Erfindung betrifft einen Katheter mit einer über einen Füllkanal aufblasbaren, einen Teil des Katheters
umgebenden Manschette.
Bei den bekannten Kathetern dieser Art ist die Manschette ballonartig ausgebildet. Bei eingeführten
Kathetern wird über den Füllkanal die Manschette aufgeblasen, welche dadurch zum Anliegen an eine
Körperinnenwand kommt und so eine gas- und flüssigkeitsdichte Abdichtung zwischen dem eingeführten
Katheterende und der Außenseite des herausführenden Katheterteils darstellt. Um eine sichere Abdichtung
zu gewährleisten, ist es erforderlich, daß die Manschette fest gegen die Körperinnenwand andrückt. Hierbei
besteht die Gefahr, daß die Blutversorgung der Körperinnenwand unterbrochen wird, was zu Nekrosen
führen kann. Wird durch das Aufblasen der Manschette das fasrige Gewebe der Körperinnenwand überdehnt,
kann dies zu einer Stenose führen, welche nur operativ entfernbar ist. Derartige Kathetei finden in erster Linie
als Trachealkatheter Anwendung.
Aus dem British Medical Journal, Januar 1966, Seite 296 ist ein Katheter mit zwei getrennt aufblasbaren
Manschetten bekannt, welche im Abstand axial hintereinander angeordnet sind. Hierbei wird die eine
oder die andere Manschette während der Intubationszeit aufgeblasen. Hierdurch ist es möglich, daß gegen
zwei getrennte Bereiche der Körperinnenwand jeweils eine Manschette während der halben Intubationszeit
anliegt. Die Gefahr des Auftretens von Nekrosen wird hierdurch verringert, jedoch ist auch hier der Nachteil
zu verzeichnen, daß die Manschetten mit relativ hohem Druck gegen die Körperinnenwand anliegen.
Bei beiden Ausführungsformen ist zudem nachteilig, daß wenn eine Nekrose auftritt, diese sich über einen
erheblichen Bereich der Körperinnenwand erstreckt, was durch die bailonförmige Ausbildung der Manschetten
bedingt ist.
Der DE-AS 1122 673 ist ein Blutstillkatheier zu
entnehmen, bei welchem die Manschette zwei getrennt oder gleichzeitig aufblasbare Ringwülste aufweist.
Dieser Katheter dient dem speziellen Zweck, nach Einführen in die Harnröhre einen Druck auf den
Blasenhals auszuüben. Zu diesem Zweck ist der Querschnitt der Manschette dem Querschnitt des
Blasenhalses angepaßt.
Es besteht die Aufgabe, den Katheter so auszubilden, daß der von der Manschette auf die Körperinnenwand
ausgeübte Druck geringer ist und auch die Boreiche, bei denen dieser Druck ausgeübt wird, kleiner sind als bei
Verwendung einer ballonförmigen Manschette.
Gelöst wird diese Aufgabe mit den Merkmalen des Anspruches 1. Vorteilhafte Ausgestaltungen sind den
Unteransprüchen entnehmbar.
Durch die gewellte faltenbalgartige Oberfläche der Manschette entsteht eine Labyrinthdichtung, bei welcher
sich zwischen den Ringwülsten, die gegen die Körperinnenwand anliegen, Druckkammern bilden. Da
ίο die Druckunterschiede zwischen benachbarten ringförmigen
Druckkammern gering sind, können die Ringwülste mit geringem Druck gegen die Körperinnenwand
anliegen. Die Gefahr des Auftretens von Nekrosen oder gar von Stenosen wird hierdurch erheblich verringert

Gemäß Fig. 1 weist der endotracheale Katheter einen Schlauch 1 auf, der eine konstante lichte Weite 2
über seint ganze Länge besitzt Dieser Katheter 1 hat ein abgeschrägtes Ende 3. Eine längliche Manschette 4
jo umgibt einen Teil des Katheters 1 nahe dem
abgeschrägten Ende 3. Diese Manschette 4 wird gebildet durch einen gewellten, etwa faltenbalgartigen
Mantel, der relativ kurz ist und eine gleichmäßige Wandstärke hat, wobei diese Manschette 4 an ihren
beiden Enden am Katheter 1 angeklebt ist Diese Manschette 4 mit gewellter Oberfläche mit axial in
Abstand befindlichen Ringwülsten 5 bildet einen Ballon von balgartiger Formgebung, der einen Teil der
Katheterwand 8 rund um eine Öffnung 6 eines
■to Füllkanals 7 umschließt. Dieser Füllkanal 7 verläuft
längs des Katheters 1 innerhalb der Wand 8 und ist am Ende 3 verschlossen. Bei etwa zwei drittel der
Katheterlänge, gerechnet vom Ende 3, ist eine luftdichte Verbindung mit einer äußeren Luftzufuhrleitung 9
hergestellt.
Luft zum Aufblasen der Manschette 4 durch die Öffnung 6 hindurch vom Füllkanal 7 her wird über die
Leitung 9 und über eine Standardkupplung 10 zugespeist. Durch Luftzufuhr dehnt sich die balgartige
Manschette 4 so aus, daß die sanft gekrümmten Ringwülste 5 in Berührung mit der Innenwand der
Luftröhre des Patienten kommen. Diese Berührung stellt eine wirksame Labyrinthdichtung her. Der
Manschettenaufblasdruck wird durch Verwendung eines Verschlußstopfens 11 aufrecht erhalten, der in die
Kupplung 10 eingesetzt ist. Der Druck kann überwacht werden durch Beobachten des Ballons 12, welcher in der
Luftleitung 9 angeordnet ist Die momentane Größe des Ballons 12 entspricht dem Innendruckwert.
Wie in Fig.2 gezeigt, berührt die ballonartige
Manschette 14 nach dem Stand der Technik das härchenbesetzte Epithel E der Schleimhaut M der
Luftröhre über eine erhebliche ununterbrochene Fläche. Der ausgeübte Druck neigt dazu, die Schleimhautkapillaren
C zu verschließen, welche aus der submukosen Schicht L durch die sich längs erstreckenden elastischen
Fasern F hindurchgehen, und welche dazu dienen, das Epithel Emit Blut zu versorgen und zwar unter einem
Blutdruck, der zwischen 12 und 30 Millimeter Quecksilbersäule
liegt Der Aufblasdruck für ein wirksames Dichten der Manschette 14 in der Luftröhre übersteigt

einer Höht von etwa 60 Millimetern Quecksilbersäule ansteigen kann. Ein Verschließen der Kapillaren C mit
anschließender Nekrose und möglicherweise eine Stenose tritt deshalb sehr leicht ein.
Bei einer gewellten Manschette 4 nach F i g. 3 liegt
eine Labyrinthdichtung vor, die mit Hilfe der axial in
Abstand voneinander befindlichen Ringwülste 5 erzielt wird. Bedingt durch diese Labyrinthdichtung ist ein
niedriger Aufblasdruck erforderlich, um die gleiche Dichtwirkung wie bei der Manschette nach Fig.2 zu
erzielen, da sich zwischen den Ringwülsten 5 ringförmige Druckkammern bilden, zwischen denen geringe
Druckunterschiede herrschen. Die Wahrscheinlichkeit des Auftretens von Nekrose und möglicherweise von
Stenose ist hierdurch verringert Tritt eine Nekrose bei der Verwendung der gewellten Manschette 4 auf. dann
ist die Schädigung beschränkt auf ringförmige Bereiche der Luftröhreninnenwand, was die Heilung begünstigt
Gemäß den F ι g. 4 bis 6 wird der gekrümmte Katheter 21 abgeschlossen durcn ein konventionelles Endstück 22 und einen Befestigungsstreifen 23. Die Manschette 24 umgibt ein Teilstück des Katheters 21 in der Nähe von dessen distalem Ende 25. Die Manschette 24 ist über eine Luftleitung 26 aufblasbar, in welcher ein

erstreckt, wobei dieser Füllkanai 28 am distalen Ende 25 verschlossen ist Der Füllkanal 28 kommuniziert mit
dem Inneren der Manschette 24 über zwei öffnungen 29
und 30 in der Katheterwand.
Die Manschette 24 nach Fig.4 hat zwei, diejenigen
nach Fig. 5 und 6 drei obere ringförmige Wülste 31 bzw. 31' und einen schmalen gerundeten Ringwulst 32,
dessen Kontur sanft in einen Endabschnitt 33 von konischer Gestalt übergeht Eine Berührung der
Manschette 24 mit der Luftröhre durch Aufblasen dieser Manschette 24 besieht dann an der Wulst 32 und an den
Wülsten 31 bzw. 3Γ. Der Ringwulst 32' nach Fi g. 6 ist
im oberen Bereich zylindrisch ausgebildet

Claims (3)
1. Katheter mit einer über einen Füllkanal aufblasbaren, einen Teil des Katheters umgebenden
Manschette, dadurch gekennzeichnet, daß die Manschette (4, 24) in aufgeblasenem Zustand
eine gewellte, faltenbalgartige Oberfläche mit mindestens drei Ringwülsien (5,31,32) aufweist
2. Katheter nach Anspruch 1, dadurch gekennzeichnet, daß der dem Katheterende (25) benachbarte
Ringwulst (32) in einen konischen Endabschnitt (33) übergeht.
3. Katheter nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Manschette (4, 24) dünnwandig
ausgebildet ist und im nicht aufgeblasenen Zustand schlapp ist
