Description
本发明涉及一种设备,该设备用于对伤口进行负压治疗。负压治疗的原理是基于这样一个事实,即对一个敞开的伤口连续地或间歇地进行抽吸,便可能由于刺激了伤口停滞区域的血液流动而促使伤口较快愈合。这种治疗方法现在已经被大家接受作为一种高效的途径来对褥疮、溃疡、局部深度烧伤及手术后伤口不能快速愈合的病人进行治疗以改善其伤口的愈合。
PCT专利申请WO 96/05873描述了一种对伤口进行负压治疗的设备,该设备包括一个与吸入管相连的多孔垫以及设在抽吸装置和多孔垫之间的容器,该容器是用来收集伤口渗出物的。英国专利申请9623743.3(公布号No.2307180)又公开了上述PCT申请所述设备的一改进,该设备感受作用到伤口部位上的实际压力。尽管上述申请中的设备能有效地工作,但当需要将从病人上断开和再连接吸入管时存在一些实际应用中缺点。
本发明提供一种经过改进的设备,其包括一简便的系统以用于吸入管线与伤口部位连接和断开。
根据本发明的一个方面,提供了一种用于对伤口进行负压治疗的设备,其包括:一用来作用于伤口的多孔垫;一连接多孔垫与负压源的吸入管;一用于收集伤口渗出液的容器,其设在多孔垫与负压源之间;以及一可快速断开的连接器,其将通向多孔垫的吸入管的第一部段与通向该容器的第二部段连接起来,该连接器包括两个可分离的且不可互换的部分,吸入管的该第一部段包括一多腔管,其中一个腔设置成用来对多孔垫进行抽吸,而另一个腔设置成用来监测压力,该多腔管在一端处固定地连接到容器上,而在另一端处固定地连接到该可分离的部分的其中一个上,该可分离部分包括密封装置,由此各个腔以气密的方式连接到吸入管的该第二部段上。

这里的术语“固定地连接”的意味着,所述部件彼此连接成,如果不将连接部件中的一个或两个都破坏则不能将其拆开。连接器的各个部分以及吸入管通常由塑料材料制成,并且各部件可以用焊接或粘结剂来进行固定地连接。一个简便的固定连接方法是使用部件的互溶剂。
本发明的一优点是:容器组件和固定地连接的吸入管仅与正确且配套的吸入管段和相关的多孔垫结合使用。因此,用来进行抽吸的腔道的两部段之间形成有适合且有效的密封,并且在一个管段中的“传感”腔与另一个管段中的对应“传感”腔或与该管段中的“抽吸”腔将密封地连接起来。结果,可以对伤口部位及附近区域的压力进行可靠的监测。

参见附图,特别是从图1中可见本发明的设备包括一个多孔垫1(例如敞开孔聚合泡沫,诸如具有互连孔的聚氨酯或聚乙烯醇泡沫),该垫1打算放到待治疗伤口之中或之上。该多孔垫通过适配器3及吸入管2与一个诸如泵或真空罐的负压源103相连,连接方式如前面提到的英国专利申请No.2307180和我们的正在审理中的英国专利申请No.9819678.5(公布号No.2329127)所述。适配器3优选为具有如英国专利申请No.9819678.5(公布号No.2329127)所述的构造。多孔垫及适配器3使用如英国专利申请No.2307130或No.2329127所述的手术单(surgical drape)以气密的方式与伤口部位密封。吸入管2在一端4处与适配器3的管口部5固定连接。可以使用诸如焊接或粘结剂粘接等任何适用手段来实现端部4和管口部5之间的固定连接及密封。合适的做法是将一种用于适配器和吸入管材料的溶剂涂覆在吸入管的外表面上,并将该管子插入适配器以形成粘结剂的粘接。吸入管2的另一端和伤口渗出物容器6的进口端7以适用的方式气密地固定密封。吸入管2由两个部分组成,这两部分之间以连接器8来连通。连接器8包括两个可分离的部分9及10,这使得病人可以容易地并快速地和设备的主体部分脱离开,例如为了接受其它治疗。将适配器3连接到连接器半部9的吸入管1的部段11完全地延伸进入连接器半部9之内,并以例如粘结剂粘接或焊接的方法将该部段密封于其中。因此,该管部段11在其两端处都是密封的。
与此相似,吸入管2的第二部段12将连接器的半部分10和渗出物收集容器6相连,该第二部段12也密封在其相应的连接器半部分10之内。因此,适配器3和连接器8之间以及连接器8和容器6之间都不可能存在空气泄漏。连接器8设计成当其两半部分组装在一起时便能锁定在密封状态。连接器的构造将在下面作更详细的描述。
吸入管2的部段12是一多腔管,该多腔管具有一个用来对伤口部位进行抽吸的中心腔道以及一个或多个用来感受伤口部位压力的周边腔道。共同未决的英国专利申请No.9819678.5(公布号No.2329127)中描述了一些多腔管的实例。
部段11最好也是一多腔管。但也不是必须如此,特别是当位于连接器半部分9和病人之间的部段11的长度相对较短时。在这种情况下,从连接器8处感受到的压力和从伤口部位感受到的压力基本上相等。
在容器6处,中心腔道与管子20相连,该管子20经由通道21连通到容器6的内部。可以通过例如管子22对容器6进行抽吸,以将抽吸作用施加到伤口部位上。一个或多个周边腔道与周边通道23连通,该周边通道与腔室24相连。腔室24通过通道25连到传感器10上,用于对连接器8处或伤口部位的压力进行测量。通道25还可连接一与大气连通的阀门102,以释放或减小伤口部位的负压。通过打开或不时地关闭阀门102,便可中断或改变作用在伤口上的负压。
管子22连接到负压源,例如泵103。管子22上还连接有压力传感器104来检测吸入管路中的压力。
图2到图5更详细地示出了连接器。
如图2所示,连接器半部分9具有一肩部30,此肩部30进入到连接器半部分10中的一个凹口内,并紧靠O型圈31密封。连接器半部分10具有一管形的突出部32,该突出部32的尺寸设计成:该突出部32能够进入多腔管11的中心腔道33,并且当连接器8处于完全装配状态时一增大部分35在中心腔道33内形成密封。连接器半部分10具有一些通道36,这些通道36的一端与多腔管的一个或多个周边腔道37相连通,而另一端和中心突出部32外侧空间38相通。
两个半部分9及10在其封闭状态中通过一卡口装配件保持在一起,该卡口装配件包括凸起部39及与之接合的槽口40。形成凸起部39和槽口40的相互接合表面确定成:使两个半部分9及10紧固在一起,以压缩O型圈,并迫使增大部分35稳固地进入内腔道33。图3清楚地示出了装配完成的状态。由图可见,当装配完成时,中心腔道33通过多腔管的二部分之间的中空突出部32得到连通,并且此腔道和与吸入管各部段中的周边腔道37相连通的传感器部分是完全隔开的。
适于制作管子的材料包括聚氯乙烯,适于制作连接器的材料包括聚氯乙烯、聚丙烯和ABS。可以借助于诸如环己醇胶的溶剂胶或通过可紫外线固化的粘结剂组合物来将管子密封在连接器半部分内。为了防止多腔管的部段11、12在拆卸时发生泄漏,连接器的两个半部分在分离之前可在管子上夹上夹子50。
图6所示为一个连接器的变型,该连接器在其两个半部分分离时会自动保持密封效果。该连接器和图2及3所示的连接器基本相同,其不同点在于具有一横截于多腔管的中心部分33或吸入管部11的整个开口端部的橡胶膜16。连接器半部分10的突出部分32的形状是具有一个能够刺破橡胶膜60的尖端部61。该突出部分32具有一与其内部连通的侧向孔62。这样,当连接器的两个部分9、10连接为一体时,该尖端部61便穿透橡胶膜60,并将吸入管11、12的两个中心腔道连通。膜60设计成当突出部分32抽出时可自动密封的柔软材料。于是,当吸入管11从连接器半部分10上脱离开时伤口渗出物就不会从管端泄漏出。
在一些情况中,例如对很大的伤口进行治疗或是需要对肢体的两侧同时进行治疗时,就适于使用分开的多孔垫或是必须使用很大的多孔垫。在这种情况下,合适的是设置两个适配器5,这两个适配器5都通过吸入管11连接到抽吸装置及渗出物容器6上。在这些情况下,不必分别地将管子连接到容器上,而可以设置一个Y形或T形的连接件,其中两个管臂连接分开的管子11以通向分开的适配器,第三管臂则连接收集容器。图7中示出了两个Y形连接件。Y形连接件本体的三个管臂上各设置有一个可分离部件80或81,每个可分离部件都分别与一对应部件相连接,该对应部件与相应管部段固定地连接。
连接器部件80和一多腔管部段相连,该多腔管部段的另一端和容器6密封地相连。连接器的其它部件可以和吸入管的其它管段固定连接,这些管段中的每一个连通到一适配器3上。对于一个大的伤口,可以设置两个适配器来对同一个伤口进行抽吸。尽管从Y形连接件连接到适配器的吸入管段可以是多腔管,但也不一定必须是多腔管,如果吸入管段是多腔管,这将使Y形连接件的构造非常复杂。假使是这样的话,虽然在伤口部位处感受的压力是两个适配器处压力的平均值,但是它们通常充分地靠近以至于压力差非常小。图7a及7b十分清楚地示出了卡口装配件的情况。
图8a、8b及8c示出了将适配器组件密封到伤口上的布置方式。图8a为适配器组件贴附到伤口200上的截面图。一个例如聚氨脂泡沫材料的多孔包敷件1包扎在创腔内,适配器3压靠在包敷件1的表面上。由图8a至8c可见,适配器凸缘301的下侧面被突起部302覆盖,这些突起部交叉地排列布置以便有助于流体能从大范围的泡沫垫1上流入适配器的管口部5。适配器3及包敷件通过一层弹性体材料薄板303密封在创腔内,这层弹性体材料薄板通过粘性的压敏粘结剂贴在伤口200周围的皮肤304上。弹性体材料薄板303通常是聚氨酯薄膜,该薄板的一端面上整个地涂覆了粘结剂,从而使该薄板如粘接在病人皮肤上同样地粘接在凸缘301的上表面上。
图8b是从适配器上方观察的平面图,其示出了围绕适配器的手术单(surgical drape)组件305。手术单组件和适配器的凸缘是透明的,因此可以看到突起部302。由图8c可以清楚地看到手术单组件包括三层薄板材料。手术单组件的最下层306是一层涂有脱离剂的较硬的材料薄板,这层材料薄板轻度地粘贴在手术单307本身的涂有粘结剂的下端面上。后者材料是一种很薄而柔软的弹性体材料。一层较厚的加强层308覆盖在手术单层307上。层307和层308具有供适配器管口部5通过的孔。使用时,先将泡沫垫1放入伤口,再揭去脱离层306并将手术单的粘接剂面粘贴到垫1周围的皮肤上,以形成气密密封。加强层308可在手术单粘贴到皮肤上之前、同时、或之后揭去。较厚材料的操作条310装接在手术单层的端部和加强层的端部,以助于将手术单粘贴到皮肤上以及便于将加强层从手术单上揭下。在达到密封效果以后,便可从手术单层上除去操作条310,穿孔311便是为此而设置的。
Claims (5)
1.一种用于对伤口进行负压治疗的设备,其包括:一应用到伤口上的多孔垫;一连接多孔垫与负压源的吸入管;一用于收集伤口渗出液的容器,其设在多孔垫与负压源之间;以及一可快速断开的连接器,其将通向多孔垫的吸入管的第一部段与通向所述容器的第二部段连接起来,所述连接器包括两个可分离的且不可互换的部分,吸入管的所述第二部段包括一多腔管,其中一个腔设置成用来对多孔垫进行抽吸,而另一个腔设置成用来监测压力,所述多腔管在一端处固定地连接到容器上,而在另一端处固定地连接到所述可分离的部分的其中一个上,所述可分离部分包括密封装置,由此各个腔以气密的方式连接到吸入管的所述第一部段上。
2.如权利要求1所述的设备,其特征在于,吸入管的所述第一部段也是一多腔管,并且所述可分离的部分及密封装置以基本上气密的方式将第二部段中的腔分别地和第一部段中的相应腔连接起来。
3.如权利要求1或2所述的设备,其特征在于,吸入管的第一部段在一端处固定地连接到一适配器上,该适配器具有一应用于所述垫的凸缘部分和一固定地连接到吸入管的所述第一部段上的管状部分。
4.如上述权利要求中任何一项所述的设备,其特征在于,可分离的部分包括卡口装配件的相应部分,其用于连接和断开。
5.如上述权利要求中任何一项所述的设备,其特征在于,与吸入管的一个部段连接的可分离部分的其中一个部分包括一个管状突出部,该突出部适于延伸进入吸入管的另一部段的腔中并以密封的方式与该腔接合。
