Diese betreffen den Patientenschutz, vor allem für vulnerable Patientengruppen, sowie die Bedingungen für eine weitergehende Nutzung von in klinischen Prüfungen gewonnenen Daten. Die Einführung des weitgehend öffentlich zugänglichen Informationssystems CTIS (Clinical Trials Information System) wird die Transparenz über laufende klinische Prüfungen deutlich verbessern, wenn auch die Möglichkeiten zur Schwärzung von kommerziell vertraulicher Information sowie einer Verschiebung der Veröffentlichung von prüfungsbezogenen Daten und Dokumenten um mehrere Jahre den Umfang der Transparenz möglicherweise beeinflussen werden. In recent years a number of new treatment strategies and compounds were developed for the treatment of acute myeloid leukemia. There were several randomized, controlled clinical trials with the objective to improve patients’ management and patients’ outcome in acute myeloid leukemia. Unfortunately, these trials are not always directly comparable, as they do not measure the same outcomes and currently there are no core outcome sets that can be utilized to guide outcome selection and harmonization in this disease area. Background Patient and public involvement (PPI) in research aims to increase the quality and relevance of research by incorporating the perspective of those ultimately affected by the research. Despite these potential benefits, PPI is rarely included in epidemiology protocols. Objective The aim of this study is to provide an overview of methods used for PPI and offer practical recommendations for its efficient implementation in epidemiological research. Methods We conducted a review on PPI methods. We mirrored it with a patient advocate’s viewpoint about PPI. We then identified key steps to optimize PPI in epidemiological research based on our review and the viewpoint of the patient advocate, taking into account the identification of barriers to, and facilitators of, PPI. From these, we provided practical recommendations to launch a patient-centered cohort study. We used the implementation of a new digital cohort study as an exemplary use case. Objectives: The Innovative Medicines Initiative–funded, multistakeholders project Healthcare Alliance for Resourceful Medicine Offensive Against Neoplasms in hematologY (HARMONY) created a task force involving patient organizations, medical associations, pharmaceutical companies, and health technology assessment/regulator agencies’ representatives to evaluate the suitability of previously established value frameworks (VFs) for assessing the clinical and societal impact of new interventions for hematologic malignancies (HMs).
Methods: Since the HARMONY stakeholders identified the inclusion of patients’ points of view on evaluating VFs as a priority, surveys were conducted with the patient organizations active in HMs and part of the HARMONY network, together with key opinion leaders, pharmaceutical companies, and regulators, to establish which outcomes were important for each HM. Next, to evaluate VFs against the sources of information taken into account (randomized clinical trials, registries, real-world data), structured questionnaires were created and filled by HARMONY health professionals to specify preferred data sources per malignancy. Finally, a framework evaluation module was built to analyze existing clinical VFs (American Society of Clinical Oncology, European Society of Medical Oncology, Magnitude of Clinical Benefit Scale, Institut für Qualität und Wirtschaftlichkeit im Gesundheitswesen, Institute for Clinical and Economic Review, National Comprehensive Cancer Network Evidence Blocks, and patient-perspective VF).
Results: The comparative analysis describes challenges and opportunities for the use of each framework in the context of HMs and drafts possible lines of action for creating or integrating a more specific, patient-focused clinical VF for HMs.
Conclusions: None of the frameworks meets the HARMONY goals for a tool that applies to HMs and assesses in a transparent, reproducible, and systematic way the therapeutic value of innovative health technologies versus available alternatives, taking a patient-centered approach and using real-world evidence.

Patienteneinbindung oder "patient engagement" ist in den letzten 5 Jahren zum wahren Trend in der internationalen pharmazeutischen Industrie geworden. Einige Firmen hatten schon seit längerer Zeit Abteilungen für die engere Zusammenarbeit mit Patienten. Andere legen jetzt nach und gestalten ihre internen Strukturen um, damit die Stimme der Patienten besser gehört werden kann. Die Natur der Einbindung hat sich grundsätzlich geändert, wobei Patienten nicht nur in die Marketingtätigkeiten, sondern auch in die Forschung und Entwicklung, also in die wissenschaftliche Arbeit immer stärker einbezogen werden.

My PhD thesis is based on my work as a patient advocate and peer educator in the fields of HIV, viral hepatitis, mental health and general patient advocacy, and gives an account of the key learnings from the personal and professional process in the course of my work and research.
The dissertation discusses the relevance of the affective dimension in doing patient advocacy and, to some extent, in working in the non-governmental sector in general in late-stage capitalist societies, drawing primarily on experience with and literature from patient organisations and expert patients from the global North.
The affective model of patient advocacy is introduced and described as a descriptive but also prescriptive model for the capacity building and organisational development activities of patient organisations. I posit that any such work becomes meaningful and successfully if it acknowledges and includes the affective dimension.
The mixed method research process included interviews, desk research, online surveys, and autobiographical research and analysis to draw some general conclusions about the role of emotions in patient advocacy, and how working as an expert patient or patient advocate will contribute not only to the personal coping of the individual with the trauma of illness, but will also further the development and deepening of citizenship, thus democracy.
Several good practices, such as the European AIDS Treatment Group and its work in biomedical research and development; and the European Patients’ Academy on Therapeutic Innovation EUPATI are described and discussed. The importance of conscious learning, teaching and knowledge production by expert patients and patient organisations is emphasised with particular view to the importance of the democratic and solidary distribution of scientifically sound information about health and illness as a tool to tackle inequalities.

Objectives: The objective of the 1st European AIDS Clinical Society meeting on Standard of Care in Europe was to raise awareness of the European scenario and come to an agreement on actions that could be taken in the future.
Methods: Data-driven presentations were given on specific topics followed by interactive panel discussions.
Results: In Eastern European countries, the epidemic is largely driven by injecting drug use, in contrast with Western Europe where the infection mainly occurs through heterosexual contact. A high proportion of people living with HIV remain unaware of their infection. Substantial differences exist in Eastern Europe and Central Asia with respect to treatment coverage, regimen availability and continuity of drug supply. In 2012, tuberculosis case notification rates were 5–10 times higher in Eastern Europe compared with Western Europe, with an alarming proportion of newly diagnosed multi-drug-resistant cases. Hepatitis C is widespread in selected geographical areas and risk groups.
Conclusions: The key conclusion from the meeting was that a high-priority group of actions could be identified, including: increasing HIV awareness and testing, improving training for health care providers, ensuring equitable patient access to treatments and diagnostics for HIV and comorbidities, and implementing best practices in infection control and treatment of HIV-infected patients coinfected with tuberculosis and hepatitis C virus, for whom direct acting antiviral treatment. should be considered.

Novel forms of knowledge production and dissemination increasingly extend to the involvement of civil society. This research looks at the role of the community of people living with HIV (PLH) in biomedical research and pharmaceutical development by tracing the evolution of such involvement from anger to activism to scientific contributions. The institutional review of the European Community Advisory Board (ECAB)2 has examined the history, working models, relevance, and future perspectives of the organisation. Semi-structured interviews have been conducted with organisation members and stakeholders to explore the history of ECAB and its role in scientific research. The concept and role of the “expert patient” are discussed. A HIV-fertőzötteket támogató sorstárs és önkéntes segítők csoportjaival megvalósított szociodramamtikus játékok didaktikus alkalmazása hatékony és könnyen használható, standardizálható eszköz bizonyos ismeretek és információk gyors megvilágításához és átadásához. A szociodráma alkalmazásának előnyei mellett annak korlátait és jövőbeli alkalmazási, elméleti lehetőségeit is tárgyalja a szerző.

This paper describes certain aspects of the transitions in identity of persons living with HIV (PLWH) who have become peer helpers for other PLWH. Using the method of interpretative phenomenological analysis, two semi-structured interviews were analysed to explore the meaning-making of the experience of living with HIV and becoming a peer helper. It appears from this study that living with HIV involves profound changes in the lifeworld of the individual. The concepts of time, embodiment and intersubjectivity are in the forefront of how the experience is constructed. A different quality and elevated levels of anxiety are induced, which is then turned around into conscious supportive work done in the interest of others. It seems that, building on these common experiences, the development of a common methodology for the support of people living with HIV could be a fruitful and feasible attempt.

While the core objectives of EUPATI do not include health policy-related issues or trends, there is a clear demand from patient communities and public sustaining partners to gain a better understanding of the pricing mechanisms of medicines and diagnostics, and the underlying systems and regulations (e.g. intellectual property regulations, patents, global trade agreements, reference pricing etc.). These questions dominate the discourse of patient communities. It is also clear, however, that the access to and affordability of medicines encompass a much broader field of knowledge than just the price, or even the distinction between price, cost, and value.

A Szegedi Tudományegyetem Gendertudományi Kutatócsoportja által szervezett "Gender és szolidaritás" című konferencián 2018. 09. 21-én megtartott előadásban arról beszéltem, hogy hogyan jelenik meg a diszkrimináció a melegközösségen belül, és ennek milyen következményei vannak.
Jóllehet a melegközösség a külső szemlélő számára sokszor homogén, szolidáris egységnek tűnik, a melegek megélt tapasztalatai és a kutatások ennél sokkal összetettebb képet mutatnak. Előadásomban bemutatom a kívülről jövő homogenizációs nyomás („buzilobbi”) hatását és néhány olyan törésvonalat, amik mentén a közösségen belüli stigma és diszkrimináció megvalósul. Példákon keresztül bemutatom ezek megjelenési formáit és tisztázom egyes fogalmak jelentését (PrEP-kurvák, drogos ribancok, nyunyók és bikák). A queerelmélet módszertanát alkalmazva bemutatom, hogy a közösségen belüli diszkrimináció önmagukat felerősítő hurkokon keresztül hogyan erősíti az önstigmatizációt és hogyan roncsolja a közösség mentális és fizikai egészségét. Végül néhány példán keresztül megoldási javaslatokat is teszek.

Ehhez azonban hiányoznak a megfelelően képzett páciensképviselők, aktivisták, szakértők - olyan személyek, akiknek valós, első kézből származó információik vannak egy-egy betegségről. Az Európai Páciensakadémia ilyen szakértő pácienseket képez.

The concurrent use of recreational drugs and excessive sex has become more popular across the MSM community all around Europe, also Central and Eastern Europe. The unprecedented low price of and easy access to drugs, the general status of gay communities in Europe, and increasing institutionalised homophobia in several countries all contribute to this growing phenomenon. This presentation tries to provide an explanation to why this problem is emerging from a queer philosophical stance. The presentation also suggests that the work focus of LGBTQ organisations needs to incorporate health issues in addition to the tradition of rights advocacy. This was presented at the series of the EATG European Networking Zone programs at the IAS2014 Conference.

Ha hozzászámoljuk, hogy valószínűleg még egyszer ennyi embernek nincs diagnózisa, mert nem megy el vagy nem jut el szűrésre, akkor minden nap (!) több mint egy ember lesz HIV-fertőzött Magyarországon. Ehhez képest a Budapest Pride programjából idén is hiányzik a HIV/AIDS, de a fizikai és mentális egészség minden egyéb szempontja is. Miközben a tömeg a nagyvállalatok pénzén vidáman táncol és cirkuszi majmocska módjára szórakoztatja a heteró többséget, addig a problémákról nem esik szó. Maroknyi civil úszik az árral szemben.

This training manual was developed as part of the Affordability Project of AIDS Action Europe. The objective is to educate patient advocates and patient organisations on certain key aspects of the pricing mechanisms of medicines, to elaborate on the use of generic versus original medicines, and to advise on some advocacy tools that patient activists, advocates and their organisations can use to facilitate the affordability of medicines and diagnostics. I use examples from the fields of HIV and related co- morbidities (TB and HCV) in order to demonstrate essential phenomena and tools in this work. However, that much of this knowledge can be easily transferred from one specific disease area to many others, as pharmaceutical developers and manufacturers use very similar pricing schemes in different disease areas, and healthcare systems or other payers face similar challenges when trying to secure access to medicines.

With ever reducing funding in health care and social systems, and the recent steady and continuing increase in medicine prices across the globe (PricewaterhouseCoopers 2016), the role of patient organisations as manifestations of civil society activity has increased in fighting for wider and more affordable access to medicines. However, the neoliberal ideals of autonomy, individualism, growth and competition force these not-for-profit organisations to operate under certain conditions, and to comply with requirements of funders, that are irreconcilable with their inherently altruistic nature. Patient organisations thus either go down the route of professionalization and growth under market requirements, thus turning into corporate entities; or they are destined to remain small, under-funded and not taken seriously.
In this paper, the concept of patient organisations in globalised capitalist settings is described with some analysis of the philosophical and practical consequences of the conflict between capitalistic operational requirements and not-for-profit work. The contradiction between representing interests that are in conflict with those of the funders is discussed. The difficulties around accessing state funding, where available, are mentioned. Although organically related to this topic, the work and relationships of health care providers in the same systems are not subject of this essay.

Várakozással vettem kézbe Feró Dalma és Bajusz Orsolya doktori hallgatók a Miskolci Egyetem Szellem és Tudomány című folyóirata a 2021. év 3. számában megjelent Túlélni a magyar "feminizmust" és "baloldalt" című írását. Fontosnak és érdekesnek tartom, hogy a folyóirat-ha mégoly kevéssé ismert is-teret ad fiatal kutatóknak és szerzőknek, hogy megmutassák, merre halad, haladhat a magyar társadalomtudomány szekere. Az alábbiakat azért írom, mert olyan alapvető módszertani, etikai és tartalmi fenntartások merülnek fel a tanulmánnyal kapcsolatban, amik véleményem szerint nem maradhatnak kritikus említés és válasz nélkül.
