BASIC SAFETY INFORMATION
INCIDENT-NEAR MISS -ACCIDENT
Near
INCIDENT / NEAR-MISS : Miss
“ An undesired event that has caused, or could have potentially
caused personal injury, illness and/or damage (loss) ”
ACCIDENT :
“An unwanted , unforeseen , unplanned event which
result in a loss of some kind”
HAZARD :
“ A condition or action that has the potential for an unplanned
release of, or unwanted contact with, an energy source that may
result in harm or injury to people, property or the environment ”
RISK :
“ The likelihood that harm will occur and the severity of the harm ”
(Risk=Probability of occurrence x Severity of damage)

Work At Height
Jobs carried out 1.8 meters above ground level is known as work at heights.
Use of full body harness is mandatory to prevent fall.
ANCHORAGES 1.8 m. high (6ft) & above
100% TIE-OFF TWO LANYARDS,
FULL BODY HARNESS
•Strong enough.
•Above head .
•100% tie-off.
•Not in conduit.
•Not in electrical cable.
•Not in air ducts.
•Not in valve handles.
•Harnesses and
Lanyards are
to be inspect
on commencing the job.
•Destroy the harness

Fire Prevention
The Three elements
FIRE Heat - Ignition sources, lighter, cigarette .
sparks from grinding, welding etc.
Fuel - paper, oils, clothes wood, gases.
Oxygen- From Air
HOW TO PREVENT A FIRE ?
best way is to put off the fire before it starts.
• Do not smoke in the work areas \ Restricted areas.
• Do not use mobile phones in flammable areas.
• Maintain proper housekeeping before & after
job comp.
Always do the gas testing before carrying out any
welding/cutting/grinding in flammable areas.
Do not use fire extinguishers unless you are
trained & competent in fire fighting.
PASS Technique.
P - Pull the Pin
A - Aim the nozzle at the base of the fire
S - Squeeze the handle To save human life and
to protect the property
S - Sweep side by side pass

Scaffolding
Scaffolding must be erected to provide safe working access to work site
or when working from ladder is not feasible
Scaffold shall be erected by trained, qualified person
Use scaffold after inspected and “Green Scaffold” tag displayed
Scaffold and platform elevated 1.2 m or more must have handrail and
toe boards
Scaffold platform must be secured and extended at least 15 cm (6
inches). Mobile scaffold height shall not exceed 3 timed the smallest base
dimension
Climb and descend from through access ladders only
The ladder must conform to ANSI standards and extend 3 feet beyond
the top resting point
Scaffold pipes, clamps and planks must be in good condition.
Employees shall not work on scaffolds during storms or high winds.
Scaffolding shall be inspected and tagged by scaffold supervisor

Compressed Gas Cylinders
If not handled properly it can lead to major accidents and injuries.
Cylinders must always be transported on wheeled cylinder carts with retaining
straps or chains.
Store cylinders upright and secure them with a chain, strap, or cable to a
stationary building support (i.e. Structural Beam) or to a cylinder cart to
prevent cylinders from tipping or falling.
Caps used for valve protection should be kept on the cylinders at all times,
except when the cylinder is actually being used or charged. Cylinder valves
should remain closed.

Electrical Safety
Don't touch electric tools, equipment, or cords that are wet,
or with wet hands.
Inspect tools and appliances for wear and damage prior to use .
Do not repair any electrical equipments unless you are trained
and qualified.
Use ladders made of wood instead of metal for electrical jobs.
Do not use electrical outlets or cords with exposed wiring.
Use insulated tools or Wear rubber gloves and rubber boots when
working around electrical components.
Know where breakers and electrical boxes are in case of an
emergency .

Confined Spaces
CONFINED SPACE
It Has limited/restricted means of entry and exit. Not designed for continuous
occupancy. Large enough and so shaped that one can Bodily Enter and perform
the work.
Examples: Storage tanks, process vessels, drums, boilers, sewers, Deep manhole or pits, underground utility tunnels, excavation deeper than 1.2 m depth, AC
ducting system, large diameter piping and duct, etc.

Hazards of Confined Spaces

Material Handling
Do not lift any load which is out of your capacity. max load
that you can lift is the one third of your body weight. Ask
help or use mechanical equipments.

Hand Power Tools
Power Tools & Non -powered. They include anything
from axes to wrenches. The greatest hazards posed by hand
tools result from misuse and improper maintenance.
A wrench must not be used if its jaws are sprung. Using a screwdriver
as a chisel may cause the tip of the screwdriver to break and fly.
Safety TIPS:
Keep all tools in good condition with regular maintenance.
Use the correct tools for the job.
Examine each tools for damage before use.
Operate according to the manufacturer's instructions.
Do not operate any tools/equipments unless you are trained
and qualified.
Provide and use the personal protective equipment.

Energy Sources
Energy Sources
Gravity The force caused by the attraction of all other masses to the mass of the
earth.
Motion The change in position of objects or substances.
Mechanical The energy of the components of a mechanical
system, i.e. .rotation, vibration, motion, etc.
Electrical The presence and flow of an electric charge.
Pressure Energy applied by a liquid or gas which has been
compressed or is under a vacuum.
Temperature The measurement of differences in the thermal
energy of objects or the environment
Chemical The energy present in chemicals that inherently,
or through reaction
Biological Living organisms that can present a hazard.
Radiation The energy emitted from radioactive elements or
sources and naturally occurring radioactive materials
Sound Sound is produced when a force causes an object or substance to vibrate–
the energy is transferred through the substance in waves.

is any facility. Proper safe handling of chemicals
by employees and safe chemical operations will
prevent accidents, chemical exposures of chemical
release to the environment.
• Labeling of all chemicals..
• Proper chemical storage containers & areas.
• Segregation of incompatible chemicals.
• Personal Protective Equipment.
