SHS TEACHER III RECLASS REQUIREMENTS

1. Education relevant master’s degree


2. Experience: 1 year of relevant teaching/industry work experience
3. Training: 4 hours of training relevant to the subject area specialization
4. Eligibility: RA 1080 (teacher)

5. Education: bachelor’s degree holder; or graduate of technical vocational course in the area of
specialization

6. Experience: 1 year relevant teaching/ industry work experience

7. Training: at least NC III + TMC appropriate to the specialization

8. Eligibility: RA 1080 (TEACHER)

Interested and qualified applicants must submit the following documents to the office of the HRMO:

1. Letter of intent addressed to the School’s Division Superintendent


2. Fully accomplished Personal Data Sheet (PDS) CS Form 212, Revised 2017
3. Performance rating in the last rating period
4. Photocopy of certificate of eligibility/rating/license
5. Photocopy of transcript of records and CAV
6. Photocopy of the latest approved appointment
7. Certificates of other supporting documents

Deadline of submission: august 4 (WEDNESDAY)
