AFFIDAVIT OF LOSS

I, _____, of legal age, married, Filipino and resident of _____, after having been sworn
to in accordance with law, hereby depose and say, THAT:

1. That I am the true and registered owner of a certain motor vehicle, more particularly

2. That on or about ____ of ______, I discovered that the license plate of the said
motor vehicle is lost or no longer attached to the vehicle;

any other law enforcement agency;

5. That despite diligent search and efforts to locate the said license plate, I could not
find the same such that I now believe that it is now lost beyond recovery.

6. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing
and to support the application for the issuance of a new one in lieu of the one that
was lost.
