SWORN STATEMENT
We, AMANTE M. CABAMALAN and JOSE M. ASEDILLO, both of legal age,
Filipino Citizens , with present residence and postal address at Real St. Brgy. San Juan,
Kalayaan, Laguna, after having been duly sworn to in accordance with law , hereby
depose and state:
That we know a child named LORETA DE GUZMAN MERCADO born on
August 8, 1974 at Kalayaan, Laguna , and her parents named MAMERTO
RAGASA MERCADO and ROSALINDA REYES DE GUZMAN, both
deceased;
That facts of her birth was duly registered at LCRO-Kalayaan, Laguna and the
same was forwarded to Philippine Statistics Authority (PSA) formerly NSO;
That her parents got married on February 1, 1978 at Kalayaan, Laguna as
evidenced by their marriage contract duly registered at LRCO-Kalayaan, Laguna
and PSA(NSO) under Registry No. 19 (B-78);
That her date of birth as appeared in the Proof of Surviving Legal Heirs was
erroneously entered as August 8, 1972 instead of August 8, 1974 , her correct
date of birth as appeared in her Birth Certificate issued by PSA (NSO);
That the error was merely due to inadvertence;
That discrepancy of her (Loreta) signature appeared in the affidavit of waiver
and in her Identification Card presented was merely due to hasty execution of
handwriting; and
That signatures of her (Loreta) appeared in the Affidavit of Waiver and in the
Identification Card presented refer to signature of one and the same person as
supported by available proofs that the signature appeared in the Affidavit of
Waiver is not the only manner she has ever signed her name ;
That we executed this sworn statement to attest to all the facts above-stated and
to support her mothers death claim at PAG-IBIG.

depose and state:
That we know a child named WENILYN DE GUZMAN MERCADO born on
March 13, 1978 at Kalayaan, Laguna , and her parents named MAMERTO
RAGASA MERCADO and ROSALINDA REYES DE GUZMAN, both deceased
and the facts of her birth was duly registered at LCRO-Kalayaan, Laguna and the
same was forwarded to Philippine Statistics Authority (PSA) formerly NSO;
That in her Certificate of Live Birth (COLB), certain errors appeared as the
COLB filed and kept at the archive of Kalayaan, Laguna Local Civil Registry
Office (LRCO) bears Registry No. 38(d-73), while PSA copy was carrying
Registry No. 38 and that COLB No. 38 (d-73) and 38 is one and the same
document;
That likewise, the childs first name appearing in the LRCO copy of the COLB
clearly stated WENILYN the correct spelling of the childs first name as
appearing in her marriage contract and the first name that she had been using in
all her local and foreign dealings;
That the childs first name in the PSA copy was blurred and difficult to read which
can be read as WINILYN instead of WENILYN, her correct first name;
That LRCO personally and verbally confirmed that the entries were authentic
handwriting of the former midwife, Aida C. Kochensparger, the attendant at the
time of the childs birth and We assumed that the discrepancy on the childs first
name was merely due to wear and tear of document;
That WINILYN (blurred and difficult to read) and WENILYN pertain to one and
the same person who should be WENILYN and that column no. 16b, referred to
her deceased sister MERLINA DE GUZMAN MERCADO born on August 7,
1958 and died at Sta. Cruz, Laguna on October 19, 1973;

deceased, and the facts of her birth was duly registered at LCRO-Kalayaan,
Laguna and the same was forwarded to Philippine Statistics Authority (PSA)
formerly NSO;
That her parents got married on February 1, 1978 at Kalayaan, Laguna as

That in the childs birth certificate, her parents date of marriage was erroneously
entered as November 1, 1958 and not February 1, 1978, her parents correct date
of marriage;
That the childs date of birth as appeared in the Proof of Surviving Legal Heirs
was erroneously entered as February 25, 1978 instead of February 25, 1977, her
correct date of birth as appeared in her birth certificate issued by the PSA(NSO);
That the error was merely due to inadvertence;
