We, ELMER D. MODEQUILLO and HERMENIA C. LOYLOY, both
Filipinos, of legal ages, and residents of Corazon, Catmon, Cebu, after having been duly sworn in accordance with law, hereby depose and state:
1. That we personally know the person of “KRISTINE JOY C.
EMATONG” who is the daughter of EDUARDO M. EMATONG and CRISTINA L. COMAINGKING for being our neighbor and close acquaintance;
2. That we know for a fact that the person by the name of “KRISTINE JOY C. EMATONG” was born a FEMALE on 11 November 1988 in Cebu Puericulture Center & Maternity House, Inc. Cebu City, hence, her gender as indicated in her Certificate of Live Birth as MALE is nothing but a clerical error;
3. That we are not in any way related to any of the foregoing person within the fourth civil degree either by consanguinity or affinity, nor do we have any pecuniary interest in this matter whatsoever;
4. That we are executing this affidavit in order to attest to the truth
of the facts aforementioned, and for any legal purpose this may serve.
