AFFIDAVIT OF LOST PASSBOOK

I, GLENDA G. SANTOS, of legal age, married and a resident of


Brgy. San Juan, Kalayaan, Laguna, after being sworn to in accordance with
law hereby depose and state:

1. That I am a depositor of ________________;

2. That as a savings account depositor, I was issued by the said bank a


passbook under account number 2955-003-01-005484-0;

3. That said passbook was placed in a filing cabinet at my office.


However, sometime on the last week of September 2021, when I was
about to check the entries in the said passbook I discovered that said
passbook was already missing and I cannot locate the same;

4. That I have exerted all efforts to locate the whereabouts of the said
passbook and all other possible places where I may have placed it
but all my efforts were in vain as I still cannot locate the same
anymore;

5. That I am executing this affidavit to attest to the truthfulness of the


foregoing statements and to request for the issuance of a new
passbook in lieu of the lost one.
