Affidavit of Lost Land Certificate

AFFIDAVIT OF LOSS

I, NORMA NORTH, of legal age, widow, Filipino citizen and a resident of Sta.
Maria , Zamboanga City, Philippines, after having been duly sworn to in accordance with
law, depose and say, that:

1. I am the widow of MR. Peter North who died last 20 May 2010 and was a
grantee of that certain parcel of land which is for interment purposes situated at
Forest Lake Memorial Park-Zamboanga City, Philippines and as such he was
issued a Certificate of Ownership;

2. Said certificate was kept in a certain cabinet in our residence for safe keeping.
However, recently, discovered that said certificate was missing;

3. I exerted diligent efforts to look for the above-mentioned Certificate in other
places where I might have misplaced it, but the same proved futile;

4. For all intents and purposes, I am now declaring said Benefit Certificate lost and
beyond recovery;

5. I make these statements under oath and in good faith in order to establish the
fact of loss of the said Benefit Certificate;

6. I execute this Affidavit to attest to the truth of the foregoing facts based on
personal knowledge and authentic records for whatever purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand on this ____ day of May
2010 at Zamboanga City, Philippines.
