Description
本出願は、2016年8月25日に出願された米国仮出願第62/379,667号に対する優先権を主張するものであり、参照によりその全体が本明細書に組み込まれる。
大きすぎて自然には閉じられない、もしくはそうでなければ、創傷の部位への陰圧の適用では治癒しない、開放創または慢性創傷の治療は、当該技術分野ではよく知られている。現在当該技術分野で知られている陰圧創傷療法(NPWT)システムは、創傷の上に流体に対して不透過性または半透過性の被覆を置くことと、創傷を囲む患者の組織に対して被覆を封止する、様々な手段を使用することと、陰圧を被覆の真下に作り出し維持するような手法で、陰圧の源(真空ポンプなど)を被覆に接続することとを伴う。このような陰圧は、創傷部位における肉芽組織の形成を促進し、かつ人体の通常の炎症プロセスを補助しながら、同時に有害なサイトカインおよび/またはバクテリアを含む可能性がある過剰な流体を除去することにより、創傷の治癒を促進すると考えられている。しかしながら、治療の便益を完全に実現するには、NPWTのさらなる改良が必要とされる。
一部の実施形態では、ポンプおよび/またはその他の電子構成要素が未だ、患者に適用されることになる単一の装置の一部であるが、ポンプおよび/またはその他の電子機器が創傷部位から離れて配置されるよう、ポンプおよび/またはその他の電子構成要素は、吸収体および/または透過層に隣接して、またはそれらの隣に配置されるよう構成され得る。一部の実施形態では、ポンプおよび/またはその他の電子構成要素は、上記の吸収層および/または透過層の上に配置され、またはその上に取り付けられてもよい。図2Cは、ポンプおよび/またはその他の電子機器が創傷部位から離れて配置される創傷被覆材を図示する。創傷被覆材は、電子機器エリア1361と、吸収エリア1360とを含み得る。吸収エリア1360は、吸収性材料を含むことができ、創傷部位の上に配置され得る。電子機器エリア1361は、吸収エリア1360から横にずらして配置することなどによって、創傷部位から離れて配置され得る。電子機器エリア1361は、吸収エリア1360に隣接し、且つ、その吸収エリア1360と流体連通して配置され得る。一部の実施形態では、電子機器エリア1361および吸収エリア1360はそれぞれ、長方形状であってよく、互いに隣接して配置されてもよい。電子機器は電子機器エリア1361内に位置し、吸収エリア1360は創傷上に配置され得る。被覆材は、創傷接触層1310と、スペーサ層1311および/または吸収層1317の一つ以上と、吸収層1322と、接触層の上に位置づけられた透湿性フィルムまたは被覆層1313と、スペーサ層、吸収層、またはその他の被覆材の層の一つ以上を含み得る。創傷接触層1310、一つまたは複数のスペーサ層1311および/または透過層1317、吸収層1322、透湿性フィルムまたは被覆層1313は、図2A~図2Bに関して記載された対応する創傷接触層、スペーサ層、透過層、吸収層、および被覆層に関して記載されたものと類似した材料および特徴を含み得る。電子機器エリアの創傷被覆材層および吸収層は、一つの連続した被覆層1313によって覆われ得る。
図2Bで既に述べたような特定の実施形態では、流体(例えば、創傷滲出液)は、穴加工された創傷接触層222を通過することによって、被覆材100によって対処され、透過層226に入って、次いで吸収層221によって吸収されて保持される。次いで、流体は通気性のある裏当て層220を通して蒸発することができる。しかしながら、一部の実施形態では、インビトロ創傷モデル試験中に観察されたもののように、吸収層は予測不可能でしばしば不均一な様式で流体によって満たされる。従って、上述のように、流体が吸収層の特定のエリアに達するのを防止することが望ましい場合がある。
既に図2Cを参照して論じたように、隔壁1362は、ポンプおよび電子機器を含む創傷被覆材の吸収エリア1360と電子機器エリア1361との間に使用され得る。隔壁1362は、吸収層1322および下側空気流スペーサ層1311を、電子機器エリアにおける被覆材の電子ハウジングセグメントから分離することができる。一部の実施形態では、材料は、被覆材内に組み込まれるか、または隔壁1362の代わりに、または隔壁1362に加えて使用して、創傷流体が被覆材の電子機器ハウジングセクションに侵入するのを防止することができる。一部の実施形態では、隔壁は、吸収エリアを電子機器エリアから完全にまたは部分的に分離する任意の材料とすることができる。一部の実施形態では、被覆材層および/または電子構成要素は、疎水性または親水性被覆で被覆されて、吸収エリア1360および電子機器エリア1361の必要な分離を提供することができる。一部の実施形態では、被覆材層および/または電子構成要素は、疎水性または親水性の材料で作製することができる。例えば、図2Cに示す通り、非多孔質ダム1362は、シアノアクリレート接着剤ビードまたは一片のシリコンを含み得る。
Claims (5)
- 創傷被覆材であって、
創傷と接触して配置されるよう構成される創傷接触層と、
前記創傷接触層上の第一エリアであって、下側スペーサ層と吸収層とを含む前記第一エリアと、
任意の複数のスペーサ層と、前記複数のスペーサ層内またはその間に配置される陰圧源および/または電子構成要素とを含む、前記創傷接触層上の第二エリアであって、前記第一エリアが、前記第二エリアに隣接して配置され、前記第一エリアおよび/または前記第二エリアの一つまたは複数の層に適用されたエポキシ材料によって形成された隔壁によって前記第二エリアから分離される、前記第二エリアと、
前記創傷接触層と前記第一エリアと前記第二エリアとを被覆し、前記創傷接触層と前記第一エリアと前記第二エリアとを覆う封止を形成するよう構成された被覆層と、
を含む、前記創傷被覆材において、
前記創傷被覆材が、前記第一エリアおよび前記第二エリアを被覆するよう、且つ、前記隔壁を越えて周ることによって前記第一エリアと前記第二エリアとの間を空気が連通することができるよう構成された上側スペーサ層をさらに含む、創傷被覆材。 - 前記エポキシ材料が可撓性の疎水性材料を含む、請求項1に記載の創傷被覆材。
- 前記エポキシ材料がシリコン接着剤を含む、請求項1に記載の創傷被覆材。
- 前記第二エリアにおける前記複数のスペーサ層が、前記陰圧源および/または電子構成要素の下の第三スペーサ層と、前記陰圧源および/または電子構成要素の上に配置された第四スペーサ層とを含み、前記第四スペーサ層が、前記陰圧源および/または電子構成要素を受容するよう構成された一つまたは複数の切り欠き部または凹部を含む、請求項1~3のいずれか一項に記載の創傷被覆材。
- ユーザが前記陰圧源および/または電子構成要素を操作できるよう構成された、一つまたは複数のユーザインターフェース構成要素をさらに含む、請求項1~4のいずれか一項に記載の創傷被覆材。
