Description
例1では、外科的挿入装置は、内腔(lumen)を画定するキャニスタと、キャニスタの近位端部に結合された頂部キャップと、キャニスタの遠位端部に取外し可能に結合された切開ポートとを備える。キャニスタは、内腔に外科装置を受け入れるようにサイズが決められている。頂部キャップには少なくとも1つの内腔が画定され、少なくとも1つの内腔は、支持ロッドを受け入れるように構成されている。切開ポートは、流体シールを維持するように構成された流体封止要素を備えている。

例1に従う外科的挿入装置に関する例5は、キャニスタが、円柱形状、球形状または円錐形状を有するものである。
例1に従う外科的挿入装置に関する例7は、流体封止要素が、封止可能スリーブ装置、可撓性シール要素、取外し可能蓋シール要素またはフラップシール要素を含むものである。

図39Aおよび図39Bに、代替的な外部加圧装置の実施形態を示す。この実施形態では、装置500は単一チューブ502であって、そのチューブ502の頂部には単一アクセスポート504が配置されている。アクセスポート504は、医療装置または外科医の手がポート504を通して挿入されるときに流体シールを確立するのに役立つ。チューブ502の底部からは、2つのカメラポート506も延在している。一実装形態によれば、チューブ502は、本明細書の別の場所に開示した任意の切開ポートまたは任意の既知の切開ポートを含む、切開ポートに結合するように構成されている。
図62Aおよび図62Bは、容器802の底部分に結合される基部結合要素(「基部結合器」とも呼ぶ)850を示す。基部結合器850は、上部溝852、下部溝854、および上部溝852と下部溝854との間の結合器850の一部から延在する3つの結合突起(「結合切欠き」とも呼ぶ)856を有している。
図77Bを続けると、この実施形態の二重ポートシール要素1004は2つのシール要素、すなわち開口部1020を画定する弾性円形シール1018および円形シール1018と流体連通するフラップシール1022を有している。弾性円形シール1018は、開口部1020を通して位置決めされた標準的な腹腔鏡装置の平滑な表面の周囲に強固なシールを形成するように構成されている。一実装形態では、フラップシール1022は、二重ポートシール要素1004を通していかなる装置も配置されていないときに流体シールを提供する二次シールである。すなわち、二重ポートシール要素1004にいかなる装置も配置されていないとき、2つのフラップ1024A、1024Bは、ガスが注入された患者の腔内の圧力によって互いに接触するように付勢され、それにより、2つのフラップ1024A、1024Bが流体シールを形成する。
さまざまな追加の実装形態によれば、本明細書において開示されまたは企図されている挿入装置を、患者の腔内に挿入されている外科装置の挿入深さを測定する1つまたは複数のセンサまたは他のタイプの測定機構を有することができる。
図83は、上述した挿入装置のいずれかで使用することができる切開ポート1120の代替実施形態を示す。この実装形態では、切開ポート1120は、図72A〜図75Cに示す蓋と同様の摺動可能な蓋1122を有している。さらに、ポート1120はまた、切開ポート1120の内腔または開口部と流体連通するガス注入ポート1124も有している。この実施形態では、ガス注入ポート1124は、摺動可能な蓋1122の下方にあるようにポート1120に位置決めされる流量弁ポート1124である。一実装形態では、ガス注入ポート1124を用いて、患者の腔にガスが注入されるか、または処置中に補助的なガス注入が提供される。使用時、蓋1122は、閉鎖位置に位置決めされて、腔内で(かつ、別の場所で上述したように挿入装置内で)流体シールを確立し、その後、ガス注入ポート1124を介して患者の腔にガスが追加される。
Claims (10)
- 外科的挿入装置において、
(a)内腔を画定する可撓性のキャニスタであって、前記内腔に外科装置を受け入れるようにサイズが決められた可撓性のキャニスタと、
(b)前記キャニスタの近位端部に結合された頂部キャップであって、前記外科装置に結合される支持ロッドを受け入れるように構成されている少なくとも1つの内腔が前記頂部キャップ内に画定されており、前記頂部キャップには、前記少なくとも1つの内腔に関連付けられる固定機構であって、前記支持ロッドを前記頂部キャップに取り外し可能に固定するように、前記支持ロッドに取り外し可能に固定可能である固定機構が備えられている、頂部キャップと、
(c)前記キャニスタの遠位端部に取外し可能に結合された切開ポートであって、流体シールを維持するように構成された流体封止要素を備える切開ポートと、
(d)挿入前形態であって、前記可撓性のキャニスタが第1の高さを有するとともに前記頂部キャップが前記切開ポートから第1の距離に配置される前記挿入前形態と、
(e)挿入後形態であって、前記可撓性のキャニスタが前記第1の高さ未満である第2の高さを有するとともに前記頂部キャップが前記切開ポートから前記第1の距離未満である第2の距離に配置される前記挿入後形態とを備え、
前記支持ロッドは、前記支持ロッドが前記挿入前形態と前記挿入後形態との間で移動するように前記頂部キャップが前記挿入前形態と前記挿入後形態との間で移動するときに、前記支持ロッドが前記頂部キャップに対して同じ相対位置を保つように、前記頂部キャップに取り外し可能に固定されることを特徴とする外科的挿入装置。 - 前記内腔は周囲空気に対して流体封止される請求項1に記載の外科的挿入装置。
- 前記キャニスタは可撓性部分および実質的剛性部分を含む請求項1に記載の外科的挿入装置。
- 前記キャニスタは、円柱形、球形または円錐形を有する請求項1に記載の外科的挿入装置。
- 前記キャニスタは少なくとも1つのリブ構造を備える請求項1に記載の外科的挿入装置。
- 前記流体封止要素は、封止可能スリーブ装置、可撓性シール要素、取外し可能蓋シール要素またはフラップシール要素を含む請求項1に記載の外科的挿入装置。
- 前記頂部キャップは、圧力逃し弁、少なくとも1つのねじ切り内腔、脱着式ケーブルハーネス、およびクランプ突起のうちの少なくとも1つを備える請求項1に記載の外科的挿入装置。
- 前記頂部キャップに結合可能な外側ハンドルセットをさらに備える請求項1に記載の外科的挿入装置。
- 前記頂部キャップまたは前記切開ポートに結合された少なくとも1つの測定機構をさらに備える請求項1に記載の外科的挿入装置。
- 前記キャニスタが少なくとも1つのアクセスポートを備え、前記少なくとも1つのアクセスポートが、手アクセスポートおよび側部アクセスポートである請求項1に記載の外科的挿入装置。
