    Corrective action on the engine damage is a temporary emergency measure, so further action is required by conducting maintenance activities, prevention of damage (Preventive maintenance), and able to detect abnormal symptoms before the machine breakdown occurs. The sudden impact of machine damage resulted in disruption of planned production performance so that it is necessary to identify and analyze the factors that cause the machine damage. Establish maintenance method using Autonomous Maintenance, Preventive Maintenance and Reliability Centered Maintenance (RCM) concept to prevent machine failure from the beginning, through fault tree analysis (FTA) method, failure mode effect, and analysis (FMEA), mean time between failure (MTBF ) is an analytical activity for implementing RCM systems on machines and critical components, able to identify and detect symptoms of malfunction before the machine is damaged. The results of the research get better application of maintenance system so that identification of important components can be anticipated from the symptoms of damage. Overall, there is an increase in performance seen from the increase of Overall Equipment Effectiveness (OEE) value, still expected to continue to increase according to JIPM 85% standard value. In conclusion FMEA, FTA, MTBF analysis method can be used to build autonomous maintenance, preventive maintenance and reliability centered maintenance so as to facilitate the production and maintenance in determining the proper maintenance of the machine.

    Every company tries to minimize the cost of production to get the product with the price of flocking. A Chemical Company in Cilegon has a production cost reduction program. However, in the process of creation, still found some factors that cause high production costs, especially the cost of electricity in pumping equipment. This study aims to determine the causes of high electricity consumption in the drainage pump unit, provide suggestions for improvement and evaluate the results of revision. This research uses some new quality tools consisting of affinity diagrams, interrelationship diagrams, matrix diagrams, tree diagrams, and Process Decision Program Chart (PDPC). Based on the results of the research, the factors causing high electricity consumption in the drainage pump unit are DCS-Man forgot to stop at the G-1875 pump, the C-1851 sealing output goes to the slimy pit, and the G-1871 sealing output goes into the dirty hole. The remedial step is to move the C-1851 sealing output from the muddy ditch to the clean trench, moving the G-1871 sealing output from the filthy ditch to the clean ditch, fixing the leak in the sample product box, and the G-1875 pump made auto start-stop. These drainage improvements resulted in drainage units operating two times per week from the previous 18 times per week or decreased by 89% and can save as much as Rp 1,677,205 per year.

    Krebo Jantan Restaurant has plans to conduct business development in the field of food. Krebo Jantan Restaurant has a problem as to whether its business development plan can provide feasibility for future growth of the company. The purpose of this study is to determine the feasibility of the business plan for the development of food business with the limits of research on the assessment of market aspects & financial aspects. Objects in interviews and questionnaires are business owners and residents around the place of business. Data on market aspects were analyzed using Strengths, Weaknesses, Opportunities, Threats (SWOT) Methods, and financial aspect using Net Present Value (NPV) Method, Internal Rate of Return (IRR) Method and Payback Period (PBP) Method. The results of the research on the Market Aspect show that Krebo Jantan Restaurant has Strengths, Weaknesses, Opportunities, and Threats that have been analyzed and anticipated the impact of the risks well. On the Financial Aspect the results of the Net Present Value (NPV) Method indicate cash collected from year to year if assessed in the present circumstances with a positive value at the end of the 5th year of Rp 29,577,421, -, the Internal Rate of Return (IRR) Method shows the ability of the company's business to provide an average annual return/dividend of 20.494% and the Payback Period (PBP) method indicates the company's ability to return all investment costs for 4 years and 8 months. Based on the results of financial analysis shows that all financial feasibility criteria are met so that business development is feasible to be carried out under normal conditions.

    Public satisfaction can be achieved if the government apparatus directly involved in the service, can understand and appreciate and wish to perform excellent service. This study aims to identify the level of public satisfaction with the performance of Members of the House of Representatives Commission III, to determine the priority of expectations or public desires on the quality of service absorption aspirations of members of Parliament with Servqual method and improve the quality of absorption services by QFD method. This study, using questionnaires, interviews, and observations in data collection. Research respondents are citizens who have filed complaints totaling 100 people. The survey results are processed by using validity and reliability test, analysis of Importance-Performance Analyze (IPA), Servqual, and QFD. The results showed that the community was not satisfied with the performance of members of the House of Representatives with a gap of 0.84. The priority of improvement based on the IPA diagram is the level of professionalism in solving aspirations, the level of responsibility for the expectation of society aspirations, the level of fairness in the aspiration settlement, the level of aspiration service speed, the accuracy of responding to aspirations according to the problem, the timeliness of completing aspirations and the level of responsiveness to resolve aspirations. Increasing the quality of absorption services by QFD method can be done by increasing commitment, discipline, skilled human resources, coordination and synergy and policy improvement that is more concerned with the interests of the community.

    Age and years of service are factors directly related to work productivity. Age and employment perceived by employees can decrease productivity or increase employee productivity. This research was conducted at PT. Oasis Water International Palembang Branch and aims to determine the influence of Age and Duration of employment on employee productivity. The number of sample is 27 respondents by using random sampling technique. Analysis method used is multiple linear regression method using SPSS aid which includes t test and F test. As independent variable (independent), that is age and work period, while dependent variable is work productivity. The result of data analysis showed that the variable of age and duration of work did not have a positive effect on work productivity, where the probability value of F test of significance value for the effect of age and work period simultaneously to productivity was 0.355 > 0.05 and F value count 1.083 <F table 3.39, and from the results of the analysis using the coefficient of determination known that the factor of age and employment only has an effect of 8.3 percent (%) on work productivity in the company PT.Oasis Water International.

    As competition increases, delivering better service becomes more important; it is not with the expception of PT. Telekomunikasi Indonesia, Tbk (PT TELKOM), which is the only state-owned enterprise as well as the largest telecommunication and network service provider in Indonesia. This is because service quality is considered as an important aspect for the success of the service provider. This study aims to assess the service quality of PT. TELKOM for IndiHome products. A preliminary study showed that there are many customers complaining about IndiHome's service quality. In addition, the tight competition and the emergence of several similar service providers as well as attractive promotion will inevitably attract customers not to be loyal if PT. TELKOM does not immediately improve the quality of the service. This research was conducted not only to assess the service quality of IndiHome (using SERVQUAL), but also to give some recommendations to PT. TELKOM especially Regional 1 Sumatra in order to attain customer satisfaction. There were 153 respondents who participated in this study. The results show that on average, customers are not satisfied (the gap value is −1.539). This negative value means that the customers have high expectations of the services that should be delivered by PT. TELKOM, but in reality the performance of the service is not as high as their expectations. Therefore, some recommendations are given to improve the performance of IndiHome’s service quality so that the customers are satisfied.

    This research have objective to determine the application of procedures, mismatches work processes and maintenance of machinery repair and analyze the causes of motor engine damage in the PDAM Tirta Pakuan Bogor City. The data used in this research are primary and secondary data. Primary data obtained through observations and interviews to the parties concerned in the mechanical and electrical parts. Secondary data in the form of data SOP maintenance and repair machine from company. Descriptive, comprative, and evaluative methods are used to analyze the actual condition and working procedures of maintenance and repair of machinery and to compare between the results obtained from this research on the implementation of the maintenance work
    process and the repair of machines there are still stages that are not executed according to the procedure. Using the Fishbone Diagram method, the most dominant type of damage occurring in the motorizer engine is a burning roll with a contribution rate of 53.125%. Using Pareto Diagram, this analysis can provide information for the company to pay more attention to the maintenance and repair activities of the machine so that the work process activities in the mechanical and electrical

    Quality control is an effort to increase customer satisfaction and minimize damage. Super Absorbent Polymer which is a product as a raw material for making baby diapers / elderly and sanitary napkins. In Super Absorbent Polymer, defects usually found in the colour contamination that there is a dark colour on the product, print labels mistakes and packaging defects. This study aims to determine the value of DPMO and sigma level, identify the efforts which is taken to reduce disability, identify the types of disability, and find the main factors causing disability. Six Sigma method is used to analyze data with define, measure, analyze, improve and control. The quantitative data obtained by direct observation of quality problems. By using the method of six sigma can be seen that the quality of the resulting product is quite good that is 3.07 sigma with a damage rate of 58,624 for a million production (DPMO). The three highest defect product causes are color contamination of 93.34%, misprint of 3.55%, and packaging damage as much as 3.11%. The main factor causing defects is the engine factor for the type of colour contamination defect,. The type of improper defects in the packaging labels and main factor for the packaging defects is the human factor.

    PT. XYZ is a manufacturing company that produces the constituent components of HDD (Hard Disk Drive) products. One of the products produced by PT. XYZ made from rubber. The product has a strict specification, both for the accuracy of size and for display. One of the problems faced by PT. XYZ is the output of a 100% unstable check section even though the amount of human resources used is the same every day. There is a variation of the output / clock / operator from 650 to 714.4 pieces. This study aims to analyze the cause of the variation of output / hour / operator and to find solutions to minimize the output variation. The result of this research can be concluded that the cause of variation of output / hour / operator is the number of product type checked by operator not always the same every day. Where more and more types of products are checked then the smaller the output on that day. To get maximum results then the operator only do one type of product every day. If not possible then cultivated to a minimum operator change the type of product checked. It requires expertise from the supervisor to manage the work of the operator in an effort to get the maximum output.

ABP is handling infrastructure development project of the acropolitan area (Right Payment) Sukaci - Baros Village. This project has been late for 19 days from the dead line 126 days. Therefore the cost is getting higher. It’s happened because bad management is the problem source. Restoring the project time need to be focused on the cost and the due date based on the schedule. Scheduling evaluation using Critical Path Method (CPM) and Crashing to find optimal results using Critical Path Method (CPM) can be found the critical paths activity, there are ground digging , bopflang installation, stone installation, agregrate B agrgregation , aggregate A flatting, rigit bavement work, and refining, when the time suppression obtained optimal time results from 126 days to 92 days. It taken an alternative to the addition of working hours with duration 92 day after the crashing. Using two alternatives, additional working hours and the addition of workforce, obtaining the difference of extra hours of work cost Rp 80.128.125 and labourcost increase Rp 55.260 from the regular cost.

The number of defective products for nickel pig iron in a few months getting on upward trend. This research will analyze the cause of the increasing defect product by using Six Sigma methode. Furthermore, the proposed improvement is done by using the failure mode and effect analysis (FMEA) approach. The results shows the main problems are the higher value of Silica (Si) and phospor (P), the sigma level is obtained from the annual pig iron making process in 1.6 sigma, process improvement need to be done are monitoring and scheduling of burnlime powder orders, checking new materials by testing the formula against new materials, and making a repair schedule as well as focusing on changes in pressure on the monitor screen

    Supplier selection is being a strategic activity, especially when the supplier is supplying materials that will be used in the long term. In the world of construction, one part of the supply chain which has a significant contribution to the improvement of the company's effectiveness is the efficiency in the procurement of materials (raw materials) from suppliers. In the research found the problem of 50 TON of welded wire failed. This study aims to determine the criteria based on the company needs in determining the supplier of welding wire and provide the ranking of suppliers as a material consideration in determining the supplier. The study used the Analytical Hierarchy Process (AHP) method for the selection of criteria and the Technique For Order Performance By Similiar To Ideal Solution for rating or evaluating alternative suppliers. Based on the results of AHP calculation we get the weight of each criteria as follows, for the quality of 0.0644, the price 0.056, the delivery time of 0.158, the quantity 0.053, the response to the claim of 0.093, and from the criteria are made reference to the calculation of vendor selection using TOPSIS. Results obtained supplier S2, i.e., PT.Esabindo Pratama ranked first as a priority supplier with a preference value of 0.322. Excellence which is owned by supplier S2 (PT.Esabindo Pratama) are the criteria of quality, price, quantity

    Many consequences caused by the condition of the work environment is not good. This can be caused by noise, lighting and temperature conditions. We need to determine the optimal value of these work environment factors in order to improve operator performance. The correlation between operator performance with work environment factor such as noise, lighting and temperature are found out by develope empirical model to describe relation between operator performance with work environment factor. The model is used to optimize operator performance. This research use surface response method. The experimental design used a central composite design. The research was conducted by experiments in the Laboratory of Analysis of Work Design and Ergonomics of Islamic University of Indonesia Yogyakarta. Variables taken are noise, lighting and temperature. Range for noise is 40 dB to 75 dB, lighting 200 lux to 700 lux, and temperatures between 20ºC to 35ºC. The experiment carried out was the resistor color matching for 5 minutes according to the combination of research variables that have been determined in the surface response method. Output is the number of truths that occur. Using this optimization model, optimal working environment conditions for noise of 60.5 dB, 354.5 lux illumination and temperature of 25.8º.
