The principle of IMRT is to treat a patient from a number of different directions (or continuous arcs) with beams of nonuniform fluences, which have been optimized to deliver a high dose to the target volume and an acceptably low dose to the surrounding normal structures (Khan, 2010). This study intends to provide information to the physicist regarding the application of different dosimeters type, phantoms and analysis technique for Intensity Modulated Radiation Therapy (IMRT) dose distributions evaluation. The measures were performed using dosimeters of LiF:Mg,Ti and Al2O3:C evaluated by techniques of thermoluminescent (TL) and Optically Stimulated Luminescence (OSL). A polymethylmethacrylate (PMMA) phantom with five cavities, two principal target volumes considered like tumours to be treated and other three cavities to measure the scattered radiation dose was developed to carried out the measures.

ABSTRACT Carcinosarcomas are biphasic tumors consisting of an admixture of malignant epithelial and stromal elements without areas of transition between both elements, and each of these elements showing distinct inmuhistochemical features. This rare tumor is most commonly seen in the female genital tract. Pulmonary carcinosarcoma was first described in 1951 and is the fourth common site; it constitutes 0.2% of primary pulmonary malignancies. Metastases have been reported to adrenals, kidney, bone, brain and liver and occur especially from the sarcomatous component. We present a case of pulmonary carcinosarcoma with metastases from the sarcomatous component to the soft tissues of the face Case Report: Female 64 years-old with a 1 year history of cough and hemoptysis, during assessment a chest × ray revealed a right pulmonary nodule and a subsequent chest CT showed a tumor depending on the medial segment from the middle lobe, no metastasic mediastinal nodes and no adrenal gland metastases were identified. A transthoracic fine needle aspiration was performed and a diagnosis of adenocarcinoma was emitted by our cytologist. A PET scan showed only a lesion in the right lung and discarded nodal and metastasic disease. Then she was carried out to surgical resection and superior bi-lobectomy with mediastinal node mapping was performed. At pathological examination a carcinosarcoma of the lung was reported, tumor size 5.5× 3.7 cm, no nodal involvement (pT2 pN0 M0 stage IB). One month later she developed a preauricular cutaneous nodule, a cytology reported a spindle-cell neoplasm and a subsequent excisional biopsy confirmed a metastatic spindle-cell neoplasm from the sarcomatous component of the carcinosarcoma. Actually she is under palliative chemotherapy with doxrrubicin and cisplatin after three months form initial surgery.Discussion: Carcinosarcoma of the lung remains an unusual tumor. It occurs most commonly in males with a 3:1 ratio, particularly in smokers, with a mean age of diagnosis of 63 years. Clinical symptoms depend on the size and localization of the tumor. The earliest and most frequent are usually due to bronchial obstruction. Peripheral lesions can be asymptomatic for a long time. The most accepted theory about the pathogenesis is that there is a malignant transformation of a single cell line, which can undergo clonal evolution and differentiation into a variety of carcinomatous and sarcomatous components. Inmunohistochemical and ultraestructural studies support this theory. Preoperative assessment and even intraoperative findings can not differentiate carcinosarcoma from other lung malignancies. Most cases are operable when first discovered and treatment should be surgery, the prognosis is poor, with an average postoperative survival of nine months and only 10% will survive two years.

Despite the numerous differences among the distinct diseases of the chest wall, the surgery of this area shows certain common features. Treatment has progressively changed in the last few years due to advances in diagnostic techniques, minimally invasive procedures and reconstruction materials, and especially due to the multidisciplinary management of many diseases. Nuss' minimally invasive correction of pectus excavatum has gained devotees, although open approaches are performed with increasingly small incisions, almost comparable to the lateral incisions in Nuss' technique. Surgeons supporting the open approach also cite the evident disadvantages of the need for a steel implant for 2 or 3 years and for a second intervention to remove this implant. En-bloc resections with reconstruction using materials, which are increasingly better and covered by myocutaneous grafts in collaboration with plastic surgery departments, constitute a major advance in the treatment of chest wall tumors. Trimodal therapy for Pancoast tumors, consisting of induction chemotherapy and radiotherapy and subsequent surgical treatment of the tumor, currently provides the best results in terms of resectability and survival.

This paper is a report of a second round of RNA-Puzzles, a collective and blind experiment in three-dimensional (3D) RNA structure prediction. Three puzzles, Puzzles 5, 6, and 10, represented sequences of three large RNA structures with limited or no homology with previously solved RNA molecules. A lariat-capping ribozyme, as well as riboswitches complexed to adenosylcobalamin and tRNA, were predicted by seven groups using RNAComposer, ModeRNA/SimRNA, Vfold, Rosetta, DMD, MC-Fold, 3dRNA, and AMBER refinement. Some groups derived models using data from state-of-the-art chemical-mapping methods (SHAPE, DMS, CMCT, and mutate-and-map). The comparisons between the predictions and the three subsequently released crystallographic structures, solved at diffraction resolutions of 2.5-3.2 Å, were carried out automatically using various sets of quality indicators.
