This paper is a concise review of the available data on the potential importance of MDR1/P-gp in respiratory disease pathogenesis and treatment. The literature includes references to experimental in vitro and in vivo investigations, and clinical results on MDR1 genetic polymorphisms and P-gp expression levels which aim to elucidate their influence on chronic obstructive pulmonary disease and different types of lung tumor pathogenesis and treatment.

This is a review of the current data on Fetuin-A, a multifunctional plasma glycoprotein with diagnostic potential as a biomarker for selected internal diseases. The review covers key information on the pathophysiological relations and associations between Fetuin-A level and liver diseases, metabolic syndrome and its components, type 2 diabetes mellitus, atherosclerosis and cardiovascular diseases. Fetuin-A is discussed as a predictor of mortality from cirrhosis, hepatocelluar cancer and cardiovascular disorders.

Parathyroid carcinoma is a rare tumor, typically presenting with marked elevations of serum calcium concentrations and associated renal and skeletal symptoms. It is slowly growing and may recur in regional lymph nodes or metastasize to the lungs. The authors describe an unusual case of a patient with parathyroid carcinoma with aggressive biological behavior, synchronous lung metastases and manifestation of brain metastases within 18 month after the initial diagnosis. These metastases could be detected with [18F] fluorodeoxyglucose positron-emission tomography/computed tomography as well as with 99mtechnetium-sestamibi scan. Other cases of patients with brain metastases of parathyroid carcinoma reported in the literature are reviewed.

This systematic review was aimed at analyzing the role of heat shock proteins in oral carcinogenesis. It elucidates the importance of heat shock proteins as diagnostic and prognostic markers for oral squamous cell carcinoma. The therapeutic implications of heat shock proteins in oral squamous cell carcinoma are highlighted.

This study provides a unique view on the clinical impact of different immunosuppressive regimens used in cardiac transplant care on bone marrow suppression. The authors confirm that use of contemporary medication regimens lead to significant lymphocytopenia and leukopenia. In the case of significant myelotoxicity, reducing or stopping of MMF leads to normalization of blood count without increased incidence of rejections.

One strategy for new platinum-based candidate development is the synthesis of compounds which combine platinum-based moiety with another biologically active structure in augment anticancer effects. An important aspect of a new candidate´s route to clinical application is studies of interactions with liver microsomal cytochromes P450. In this work, the authors studied platinum(II) complexes which carry roscovitine derivatives and their interactions with nine human liver microsomal CYP forms.

The primary aim of this study was to investigate whether osteoblasts from diabetic patients are able to stimulate proliferation in human umbilical cord mesyncymal stem cells. It confirmed that osteoblasts from diabetic and nondiabetic patients have the similar effect in promoting the differentiation of umbilical cord mesynchymal stem cells.

These authors evaluated the effect of lacosamide on bone mineral density, bone turnover markers, and bone mechanical strength in a rat model. To their knowledge there are no other full-text studies evaluating the effect of lacosamide on bone tissue.

Innovative application of rheohaemapheresis technology and its effects on antioxidant capacity were studied in patients with age related macular degeneration. The character of the treatment raised the suspicion that there is decrease not only in cholesterol but also antioxidants such as vitamin E and vitamin A. Twenty-three patients with the progressive dry form of age-related macular degeneration treated with 8 procedures of rheohaemapheresis were studied. The results showed that rheohaemapheresis did not result in a negative antioxidant balance; Rrheohaemapheresis might have had favourable additive anti-atherogenic effects due to the relative increase in content of vitamin E in the lipoprotein fraction, in addition to causing a significant reduction in content of the atherogenic LDL.

The aim of this study was to demonstrate the outcomes of the anti-VEGF treatment in patients with the wet form of AMD in real clinical practise in Czech Republic. A comparison of the results with clinical trials was surprising. A sub analysis of the results identified parameters that could predict treatment success.

CALCR - a new modified technique for the treatment of ROP (retinopathy of prematurity) provided greater accuracy when targeting the laser and cryo spots to the avascular part of the retina. It enables better visualisation and treatment precision and reduced the risk of unintended damage to healthy retinal tissue. The possibility of high-resolution photo-documentation of the retina enables precise preoperative, intraoperative and postoperative evaluation of the retina. This results in improved assessment and comparison of retinal conditions on a fully standardized basis.
Ultrasound-guided percutaneous ethanol injection of small and medium-sized thyroid cysts with relatively small amounts of ethanol

Ultrasound-guided percutaneous ethanol injection (US-guided PEI) is used in patients with recurring symptomatic thyroid cysts. It is an alternative non-surgical approach in patients at risk during surgery. US-guided PEI was performed in 33 patients. The initial cyst volume was significantly reduced to a final volume at 6 months after PEI (10.82±5.03 mL vs. 0.78±0.84 mL; P<0.001). In case of medium-sized cysts, further PEI procedures were needed than for small cysts (1.1±0.4 vs. 2.0±0.5). The total amount of 96% ethanol was equal to 22.7±7.9% of the initial cyst volume. Our experiences confirm that thyroid cysts may be successfully treated with relatively small amounts of ethanol.

The authors describe the results of a single-center prospective study assessing the incidence of obstructive sleep apnea syndrome in a population of patients with exacerbation of COPD requiring in-hospital care. The design and outcomes of the study are novel and original as there are very limited data on this clinical scenario published worldwide.

Liver functions in 1 473 patients with acute heart failure (AHF) from the AHEAD registry were evaluated. Liver function tests (LFTs) were associated with acute heart failure severity: the most abnormal values were found in patients with cardiogenic shock. Hepatocellular LFTs pattern (elevated AST and ALT) predominated in left sided forward heart failure, while cholestatic pattern (elevated GGT and ALP) occurred mainly in bilateral and right sided AHF.

The transjugular intrahepatic portosystemic shunt may impair cardiac function and haemodynamics. Serum post-procedural GPBB concentration were increased and significantly correlated with baseline systemic vascular resistence and cardiac index.

Non-alcoholic fatty liver disease (NAFLD) represents a hepatic manifestation of metabolic syndrome which affecting a substantial percentage of modern populations. The study showed that NAFLD is present in 79% of patients with diabetes mellitus type 2 and a number of these patients could have liver cirrhosis/advanced fibrosis. The presence of NAFLD was related to the components of metabolic syndrome but not to the parameters of diabetes compensation. The validity of liver fibrosis scoring systems in this group of patients needs to be assessed.

Acute kidney injury following surgical myocardial revascularization is associated with high mortality and morbidity in octogenarians patients. Surgical myocardial revascularization without extracorporeal circulation is a safe method of therapy and the results of this study show a renoprotective trend in comparison with on-pump surgery.

Diabetes mellitus was an independent predictor of hard events in patients with normal SPECT (single-photon emission tomography). We observed a significant adverse outcome in those diabetic patients who required an additional rest imaging.

This article describes the results of a study, the aim of which was to investigate the frequency of the KRAS-LCS6 variant allele as an important biomarker in different types of solid tumours including colorectal cancer, breast cancer, non-small cell lung cancer and brain tumours. The occurrence of this genetic variant was then correlated with the presence of selected somatic mutations representing predictive biomarkers in the respective tumour types in the Czech population.

Mesothelin or soluble mesothelin-related protein (SMRP) was studied as a potential marker of mesothelioma and other asbestos-related diseases of the respiratory system in at-risk populations. In persons followed up for asbestos exposure, increased mesothelin levels signalized pathological processes in the chest and correlates with disease severity. The study suggests that mesothelin cannot be considered a reliable marker for the early stage of malignant pleural disease but only an additional criterion for examination of followed-up individuals.

Proinflammatory cytokine expresion is significantly higher in obese patients after abdominal surgery (colon resection). Early elevation of visfatin in particular could be a new marker in the diagnosis of postoperative inflammatory complications.

Infants born with congenital diaphragmatic hernia (CDH) demonstrate wide variability in lung hypoplasia and postnatal catch-up lung growth. Prospective follow-up of CDH patients aims at identifying respiratory problems that have the potential to affect the quality of life of CDH survivors into adulthood. To prevent long term remodeling of the airways and improve prognosis, early diagnosis and treatment are necessary. Complex, precise infant pulmonary function testing (IPFT) is needed for an in depth understanding of the main developmental events during both prenatal and postnatal periods in the hypoplastic lung in CDH.

This case report is a rare case of transient branch retinal artery occlusion (BRAO) in a healthy fifteen-year-old girl. The anamnesis showed that the patient began use oral contraception at the time of the occurrence of the eye problems. Fundus examination revealed in the right eye intensive edema of the part of superior half of the retina, but the vessels were absolutely normal. The paper includes a review of the literature.
