Re: Andy Williams, aged 65 years

I am writing to refer Mr Andy Williams for further evaluation and possible treatment regarding his obesity.

Mr Williams, on 15/06/2019 consultation, presented with a BMI of 46.6 kg/m2. He is 183 cm tall and weights 155,5 kg. He
has type 2 diabetes, hypertension and sleep apnea, for which he takes lisopronil, nifedipine and NPH insulin 30 units
before breakfast and 70 units at night.

On last blood exam, Mr William´s A1c was within normal range and his trtglyceride was high. His father and sister have
obesity and he has been having the disorder since childhood, with a steady increase of weight throught the decades,
reaching the highest weight in adulthood.

He has enrolled in commercial and medical weight-loss programs but regained the weight after discontinuing the progrmas.
Currently, he eats 3 large meals per day with dinner being his largest meal, at 8:30 pm. He consumes fast-food 4 times a
month and drinks no alcochol. Binge eating was reported and triggered by stress.

Mr Williams is concerned about his health and wants to learn about surgical options, although he was reluctant regarding
weight-loss surgery in the pass due to posible complications. He is now ready to consider surgical approach if you
recommend it.

In view of the above, I would apreciate if you could evaluate Mr Williams for a posible bariatric surgery.
