Subject: Dropped Object Incident Involving Lube System Pump Motor Fan Cover on TDS-11HP

Assemblies: Pump/Motor Assembly (P/N 114113, 30178472, 30179191 & M61001499; see Table 1)
Objective: Advise customers of a dropped object incident, recommended inspection and of
upgrade kit for Lube System Pump Motor Fan Covers.
Issue: NOV received a report that the fan cover on a Lube System Pump Motor fell to the drill
floor. No injuries were reported as a result of this incident.
The lube system pump is driven by an AC motor, which is cooled by a fan that draws
cooling air across the motor (see Figure 1). The fan, which is mounted on the end of the
motor, is enclosed by a protective cover and is attached with multiple fasteners that are
typically lock wired. In this instance the fasteners did not have lock wire installed for
secondary retention. The fasteners appear to have loosened during operation of the
Top Drive allowing the fan cover to fall to the rig floor.
Solutions: NOV strongly recommends that the Maintenance section of the service manual be
adhered to regarding periodic inspection of the top drive. If any parts are found to be
worn, damaged or missing they should be immediately replaced before the top drive is
returned to service.
NOV has reviewed the secondary retention aspects of the design of the lube pump
motors and has developed an upgrade kit to enhance the current design in the event
that the fasteners are damaged during normal drilling, as a result of rough drilling,
jarring or if lock wire is compromised in any way. The Motor Cover Lanyard Kit (P/N
10738809) included a lanyard assembly, new drilled head fasteners, lock wire, hole saw
and instructions for installation of the kit.

In addition, current production documentation has been revised to incorporate these
enhanced features as well as individual motors and pump motor assemblies which may
be ordered as replacement spares. See Table 1 to determine which kit is applicable to
which pump/motor assembly and motor.
