Activity Intolerance NCP

The client presented with easy fatiguability and generalized weakness. Nursing assessments identified the client's activity intolerance was related to an imbalance between oxygen supply and demand. Over 2 weeks of nursing interventions such as assessing activities of daily living, fall risk, nutrition, emotional response, and encouraging exercise and rest, the client was able to demonstrate active participation in care, increase daily activities, and identify methods to reduce activity intolerance.
