EXECUTIVE ORDER NO. 10
SERIES OF 2019

“AN EXECUTIVE ORDER DESIGNATING THE BARANGAY KASAMBAHAY


DESK OFFICER OF BARANGAY SAN ISIDRO, PALOMPON, LEYTE”

WHEREAS, Republic Act No. 10361 otherwise known as “Domestic Workers Act”
or “Batas Kasambahay”, provides for the registration of all Kasambahay nationwide;

WHEREAS, DILG MC No. 2013-61, dated July 08, 2013 enjoins the Punong
Barangay to initiate the conduct of registration of all kasambahay within their jurisdictions
and to require every employer to register his or her domestic workers under his or her
employment in the Kasambahay Masterlist (Registry of the Domestic Workers) in the
barangay where the employer’s residence is located.
NOW THEREFORE, I, IAN JAY L. AREVALO, Punong Barangay of Barangay
San Isidro, Palompon, Leyte by virtue of the power vested in me, do hereby order and
designate MARJORIE A. ENCARNACION as the KASAMBAHAY DESK OFFICER of
Barangay San Isidro, Palompon, Leyte.

I. Functions: The Kasambahay Desk officer shall perform the following functions:

 Receive the two (2) copies of accomplished Kasambahay Registration Form


(KR Form 1) with the attached Kontrata sa Paglilingkod sa Tahanan;
 Review and assess the completeness of data entries;
 Stamp the date of receipt and assign a registration control number at the upper
right corner of the KP Form 1;
 Provide the employees or their authorized representatives duplicate copy or
KR Form 1 to serve as proof of registration;
 Safe keep registration forms;
 Encode or record the information about the Kasambahay in the Kasambahay
Masterlist (KR form 2); and
 Prepare the monthly Kasambahay Report (KR Form 3) which includes data
regarding the registration of the Kasambahay, to be submitted to the Municipal
Public Employment Service Office (PESO)

II. Funding. Corresponding budget shall be allocated for this purpose for effective
implementation of the Batas Kasambahay at the Barangay level.

III. This Executive Order shall take effect immediately.

DONE, in the Barangay San Isidro, Palompon, Leyte this 30th day of August in the
year of our LORD, Two Thousand Ninteen.
