school forms that I need to know so that when I am already in the field, I
will be able to accomplish them properly, accurately and legibly.

MY BIG IDEAS (LEARNING ESSENTIALS)
As a future teacher, I need to know and accomplish the various school
forms properly, accurately and legibly. It is expected that I am able to
follow the Revised Guidelines on the Preparation and Checking of DepEd
Forms (Div. Memo. No. 29 s. 2013)

MY TASK (ACTIVITIES)
A. Ask my cooperating teacher regarding the required school forms of
the Department of Education. List down the school forms cited by
my Cooperating Teacher.

LIST OF THE SCHOOL FORMS IMPORTANCE OF THESE SCHOOL FORMS


SCHOOL FORM 1 This provides detailed information per learner of the
Register summary data being reported to EBEIS (GESP/GSSP).
SCHOOL FORM 2 This serves as the daily updating of students’
Daily Attendance attendance and to monitor absenteeism.
SCHOOL FORM 3 To monitor all textbook being used which should be
Books Issued and reported regardless of number of books per subject
Returned area.
SCHOOL FORM 4 It is lifted from SF 2 submitted by teacher/adviser.
Monthly Learner and Division office furnishes a copy of this report for the
Movement and months of June (attachment to GESP/GSSP-BoSY)
Attendance October and March (attachment to GESP/GSSP-EoSY).
SCHOOL FORM 5 This is used for all grade/year levels regardless of

MY ANALYSIS (LEARNING EPISODE DIRECTIONS)
A. WHY DO I NEED TO PREPARE THE DEPED SCHOOL FORMS PROPERLY, ACCURATELY
AND LEGIBLY?
I need to prepare the DepEd school forms properly, accurately and
legibly because to these were the important records of the
students. In School Register (Form 1) for example, I should write it
accurately and properly to avoid voids with regards to legal
matters if so. For Class Records, I should write it legibly to
accurately assess all the scores without any mistakes.

MY REFLECTION / MY INSIGHTS
A. GOING OVER THE VARIOUS SCHOOL FORMS, I HAVE LEARNED THAT:
Going over with the various school forms, I have learned that being
a teacher involves lots and lots of documents. You make sure you
have the details of each of your students and you accurately
record ALL of their scores.
