INTRODUCTION
Quality improvement (QI) methods have
been introduced to healthcare to support
the delivery of care that is safe, timely,
effective, efficient, equitable and cost
effective. Of the many QI tools and
methods, the Plan-Do-Study-Act (PDSA)
cycle is one of the few that focuses on
the crux of change, the translation of
ideas and intentions into action. As such,
the PDSA cycle and the concept of iterative
tests of change are central to many
QI approaches, including the model for
improvement,1 lean,2 six sigma3 and total
quality management.4

PDSA provides a structured experimental
learning approach to testing changes.
Previously, concerns have been raised
regarding the fidelity of application of
PDSA method, which may undermine
learning efforts,5 the complexity of its
use in practice5 6 and as to the appropriateness
of the PDSA method to address
the significant challenges of healthcare
improvement.7

This article presents our reflections on
the full potential of using PDSA in
healthcare, but in doing so we explore
the inherent complexity and multiple
challenges of executing PDSA well.
Ultimately, we argue that the problem
with PDSA is the oversimplification of
the method as it has been translated into
healthcare and the failure to invest in a
rigorous and tailored application of the
approach.

The study to determine the effect of patient experience management using the Deming PDSA cycle on the quality of HIV care was conducted in under-resourced settings of Uganda. The interventional study had 437 files studied, and 224 adult HIV clients with poor HIV care outcomes of; missed appointments for more than two weeks, poor adherence to treatment of < 85%, and unsuppressed viral load of > 1000 viral copies per milliliter were interviewed at baseline and end line. The study revealed a 13.6% prevalence of the study HIV care outcomes among all the client files, the majority of which were for patients in care for more than four years. 11.8%, 65.2%, and 46.5% were due to poor adherence to treatment, two weeks missed appointments, and unsuppressed viral load at baseline, which reduced to 4.7%, 25.1%, and 31.5% at end-line assessment, respectively. 80% of the studied clients were retention in care between the baseline and end-line assessments. 7.7% of the study files had never had a viral load done to the clients at baseline, which reduced to 1.4% at the end line. 34.5% of the study client files were overdue for a viral load at baseline but only 16.8% at end-line assessment. The study also showed a general improvement on the patient experience before and after interventions. In conclusion, Patient Experience Management using the Deming PDSA cycles improves the quality of HIV care in under-resourced settings.

Purpose – Hospital-based quality improvement (QI) programs are becoming increasingly common in developing countries as a sustainable method of strengthening health systems. The aim of this paper is to present the results and lessons learned from a QI program in a large, rural, district hospital in Lesotho, Southern Africa. Design/methodology/approach – Over a 15-month period, a locally-relevant, hospital-wide QI program was developed and implemented. The QI program consisted of: planning meetings with district and hospitals staff; creation of multi-disciplinary QI teams; establishment of a QI steering committee; design and implementation of a locally appropriate QI curriculum; and monthly consultation from technical advisers. This article describes an adaptive instructional design project a
professor and two teaching assistants (TAs), implemented in an
online doctoral level leadership course (Leadership in Educational
Organizations, or LEO). Through a collaborative and
iterative process of opportunity analysis, implementation, and
reflective practice based on experience and feedback from students,
this instructional team aimed to promote deeper learning
by building a learning community among themselves and
the 26 students from two sections of this course. The resulting
plan, do, study, and act (PDSA) cycles informed instructional
adjustments using existing technologies and mainstream media
resources.

RESUMO

A dicotomia comumente observada entre a corrida desenvolvimentista e os alertas para as questões ambientais é situação que vem sendo desconstruída com o surgimento do conceito de Desenvolvimento Sustentável, o qual, após inúmeras discussões a nível internacional e consequente inserção em nosso Direito positivo, resultou em inúmeros reflexos positivos diante do ideal de um desenvolvimento consciente. Nesta baila, cumpre destacar os reflexos dessa nova política no Estado do Amapá que, situado na região amazônica brasileira, cumpre papel de destaque na persecução de um desenvolvimento econômico calcado sob a ótica da responsabilidade ambiental.
