Introduction:
IVF is becoming now the gold standard for solving the problem of infertility, nearly million IVF babies were
born. The time of embryo transfer still rises a big question and is still a field of great debate whether to
transfer it early as day 3 or wait till day 5 to give a good chance for embryo selection this question have made
us to design our study and created a prospective randomized clinical trial study aimed to determine whether
embryo transfer on day 3 versus day 5 shows a significant difference in implantation, clinical pregnancy, live
birth either miscarriage and multiple pregnancy rates among women undergoing intracytoplasmic sperm
injection.
Patients:
We included in our study 240 infertile women retrieved from a;exandria university hospital and were
randomly divided into two groups at the day of ovum pick up. Group I underwent day 3 embryo transfer,
while group II underwent day 5 embryo transfer. With the following criteria : age less than 35 years of the
female , mild male factor and normal uterine cavity. We excluded women with endometriosis.
Results:
For day 3 embryo transfer we had 65 pregnancies while for day 5 transfer we had 66 pregnancies which
showed no significant difference between two groups. . Concerning multiple pregnancy rates day 3 embryo
transfer was 16 in compared to 12 in day 5 transfer which showed no significant difference between two
groups. There was only slight difference between live birth in two groups, for day 3 we had 43 single living
babies while for day 5 we had 36 life birth . Using Pearson Chi-Square there was no significant difference
between the two groups regarding live birth (X2=1.385, p=0.500).
Conclusion:
There was no great difference in success rates and outcome of embryo transfer between 2 groups.From out study, we can conclude that there is no significant difference in either pregnancy or delivery rates
between day 3 and day 5 when patients are appropriately randomized .Delaying embryo transfer to day 5
offers no advantage to the typical patient who going IVF. The numerous pregnancy rates observed in most
programs with day 5 transfer can most likely be explained by the fact that physicians advise patients with
many high embryos to delay their transfer.

Background: Currently, controlled ovarian hyperstimulation (COH) is monitored by serum estradiol (E2) levels which are believed to primarily detect functional activity of follicles.

Objective: To evaluate estradiol level as a predictor of ovarian response and pregnancy outcome during COH in women from Gaza Strip.

Methods: This prospective cohort study consisted of 75 women attending in vitro fertilization (IVF) at Al-Basma Fertility Center in Gaza City. Blood withdrawal for E2 hormone measurement was performed in all the patients and the number of oocytes and embryos were recorded for each female and the occurrence of pregnancy was followed for three months. Obtained data were computer analyzed using SPSS statistical package version 18.
Results: The mean age of the study population was 29.2±5.9 years. Questionnaire interview showed that the cause of infertility was mostly referred to husbands. More than half of women seeking IVF had no children and had repeated IVF. The mean level of E2 showed the highest value of 2194.4 (pg/ml) at age group 26-35 years. However, the difference in E2 levels among the age groups was not significant (F=0.940 and P=0.395). When related to the number of oocytes retrieved, E2 level showed general increase with increase ovarian response, recording values of 1642.7, 1665.1, 2156.8 and 1798.7 pg/ml with <4, 4-8, 9-16 and >16 oocytes, respectively, but this change was not significant (F=0.219 and P=0.883). The mean level of E2 showed its maximum value of 2143.6 pg/ml in positive
pregnancy. However the difference in E2 levels among the different categories of IVF outcome was not significant
(F=0.423 and P=0.656). The numbers of total and mature oocytes, and embryo were significantly increased with
increased levels of E2, showing a good response at E2 level=1000-2000 pg/ml. The number of mature oocytes
showed positive correlations with E2 and number of embryo (r= 0.159, P= 0.177 and r= 0.890, P=0.000, respectively)
and negative correlation with age (r=-0.276, P=.017).

Conclusion: Estradiol level 1000-2000 pg/ml at mean age of 27.8±4.9 years, could be a predictor of ovarian response and pregnancy outcome during COH. Consequently, for women to undergo a more likely successful IFV program, it is recommended to optimize E2 level at 1000-2000 pg/ml.

ABSTRACT Abstract:
Objective: to correlate the ultrasound parameters to the Intra-cytoplasmic Sperm Injection outcome in polycystic ovarian syndrome cases treated by non-agonist chronic step up stimulation protocol.
Setting: Agial and Integrated Fertility Centers, Alexandria, Egypt.
Patients: Twenty five cases of polycystic ovarian syndrome fulfilling Rotterdam criteria.
Materials & Methods:
Patients received a protocol started with administration of urinary human menopausal gonadotropin (hMG) per day, follow up was done by ultrasound and serum estradiol level with increasing dose until the required response was achieved and if ovarian hyperstimulation syndrome (OHSS) occurred , gonadotrophin was withheld till estradiol declined. Human chorionic gonadotropin (hCG) was given intramuscularly to achieve oocyte maturation. After (34-36 hours) of hCG administration oocytes retrieval was performed. The patients received luteal phase support in the form of natural progesterone vaginally. Transvaginal ultrasound was performed two weeks after positive pregnancy test to confirm clinical pregnancy.
Main Outcome measures: number of ampoules, stimulation days, serum estradiol level and progesterone level on day of hCG, number of oocytes retrieved, grading of embryos obtained, endometrial thickness, pregnancy rate, implantation rate, abortion rate, cancellation rate, multiple pregnancy rate and cost.
Results: patients with higher number of basal antral follicles were associated with lower number of ampoules needed for stimulation (14.63 vs. 21.64 ampoules) and less stimulation days (7.07 vs. 10.73 days). Patients with higher number of mature follicles were associated with higher estrogen level (1561.53 vs. 658.14 pg/ml) and higher progesterone level on day of hCG (1.10 vs. .62 ng/ml), higher number of oocytes retrieved (7.18 vs. 4.13 oocytes) and higher number of embryos transferred (3.06 vs. 2 embryos). The number of mature follicles and the endometrial thickness were higher in pregnant group (19.78 follicles and 12.94 mm, respectively). The pregnancy rate was (36%), the chemical pregnancy rate was (33.3%), the abortion rate was (22.2%) and the ongoing pregnancy rate was (44.4%).
Conclusions: The higher the number of the basal antral follicles, the less the number of the ampoules needed for stimulation and the less the stimulation days. The higher the number of mature follicles, the higher the estrogen level on day of hCG, the greater the number of oocytes retrieved and the greater the number of embryos transferred. The stromal thickness has no effect on the pregnancy outcome but the number of mature follicles and the endometrial thickness has a positive correlation with the pregnancy outcome.
