Objectives Our objective was to demonstrate microbial regulation of hepatic genes implicated in drug metabolism and transport using germ-free (GF) mice and to explore the impact of a microbial metabolite, butyrate, and a prebiotic dietary intervention on hepatic gene expression in mice. Methods Using reverse-transcriptase PCR, we investigated cytochrome P450 (CYP) and multidrug-resistance protein 1 (MDR1) expression in conventional, GF and colonised GF mice. To investigate the effects of butyrate, sodium butyrate (3 g/l) was administered for 21 days to conventional or GF mice. In the prebiotic study, young adult and middle-aged mice received diet enriched with 10% fructo-oligosaccharide (FOS)-inulin for 14 weeks. Key findings Colonisation of GF animals normalised expression of Cyp3a11 and Mdr1b to conventional levels. Objective: To evaluate the current status of stem cell therapy in the treatment of liver diseases, including efficacy, challenges, and future perspectives based on recent advances and ongoing clinical studies. Methodology: Bibliographic review through the PubMed database applying the following search strategy: “((Liver Cirrhosis) OR (Human Viral Hepatitis) OR (Acute Liver Failure)) AND (Stem Cells) AND (Treatment)”, generating a total of 710 articles published between 2014 and 2024, 24 of which were selected for this review. Review: It is noteworthy that mesenchymal stem cells are capable of modulating the immune system, inducing tissue repair and reducing liver fibrosis, due to their ability to differentiate into different cell types and release bioactive factors. Final considerations: There is a need for more clinical studies to overcome limitations related to the standardization of the stem cell preparation process, in order to guarantee the efficacy and safety of the treatment.

Racional: Tumor Fibroso Solitário é uma entidade rara que se origina das meninges e tecidos moles. Apresenta expressão para marcadores como SATAT6 e CD34 em métodos imuno-histoquímicos. Mais presente em mulheres na faixa etária de 50-60 anos de idade. Objetivo: reportar o caso de uma paciente de 49 anos, que iniciou com uma dor e distensão abdominal. A imagem de tomografia computadorizada demonstrou uma massa em região do lobo hepático esquerdo medindo 15,3x13,7x11,1 cm. Foi optado por ressecção cirúrgica, tendo em vista a sintomatologia da paciente e o achado em exame de imagem. Após análise patológica e imuno-histoquímica, foi dado o diagnóstico de tumor fibroso solitário hepático. Discussão: na literatura ainda há algumas controvérsias frente ao melhor tratamento, porém muitos estudos sugerem que a ressecção cirúrgica com margem de segurança seja a melhor opção. Conclusão: trata-se de uma entidade rara, com poucos relatos na literatura e esse relato pode contribuir para um melhor entendimento dessa entidade.

Augmented-and mixed-reality technologies have pioneered the realization of real-time fusion and interactive projection for laparoscopic surgeries. Indocyanine green fluorescence imaging technology has enabled anatomical, functional, and radical hepatectomy through tumor identification and localization of target hepatic segments, driving a transformative shift in the management of hepatic surgical diseases, moving away from traditional, empirical diagnostic and treatment approaches toward digital, intelligent ones. The Hepatic Surgery Group of the Surgery Branch of the Chinese Medical Association, Digital Medicine Branch of the Chinese Medical Association, Digital Intelligent Surgery Committee of the Chinese Society of Research Hospitals, and Liver Cancer Committee of the Chinese Medical Doctor Association organized the relevant experts in China to formulate this consensus. This consensus provides a comprehensive outline of the principles, advantages, processes, and key considerations associated with the application of augmented reality and mixed-reality technology combined with indocyanine green fluorescence imaging technology for hepatic segmental and subsegmental resection. The purpose is to streamline and standardize the application of these technologies.

This article reports a pelvic hydatid cyst in a 10-year-old boy who initially presented with right flank pain. Computed tomography (CT) revealed a cystic lesion containing internal densities, which was found to be a hydatid cyst between the bladder and rectum during surgery. The lesion was excised and sent to a pathology laboratory for further examination.

The liver is one of the most commonly injured solid organs in blunt abdominal trauma. Non-operative management is considered to be the gold standard for the care of most blunt liver injuries. Angioembolization has emerged as an important adjunct that is vital to the success of the non-operative management strategy for blunt hepatic injuries. This procedure, however, is fraught with some possible serious complications. The success, as well as rate of complications of this procedure, is determined by degree and type of injury, hepatic anatomy and physiology, and embolization strategy among other factors. In this review, we discuss these important considerations to help shed further light on the contribution and impact of angioembolization with regards to complex hepatic injuries.

Objective: Liver is the most important organ in the pivotal role of regulating various physiological processes and several vital functions like metabolism, secretion and storage. Plants are reputed in the indigenous systems of medicine for the treatment of various diseases. Toxicant or drug induced liver injury can be prevented by treating with non toxic hepatoprotective herbs, which can possess membrane stabilizing, hepatoprotective and antioxidant activities. NDMA (N-Nitrosodimethylamine) belongs to nitrosamine compounds which are known hepatic carcinogens. Methods: The aim of the present investigation was to analyse the effects of NDMA on the morphology of hepatic cells, to determine the reversible effect if any, after providing the treatment with the crude extract and isolated glycoside from the roots of Operculina turpethum. The treatment was given in different groups of Swiss Albino Mice. Results: Scanning Electron Microscopy and Light microscopical examinations indicated that NDMA treated mice livers (n = 6) displayed severe vascular and endothelial damage compared to control livers (n = 6). Liver sections appeared with inflammatory cellular infiltration, vacuolated hepatocytes, dilated sinusoids, increased number of Kupffer cells, fibrosis, endothelial fenestrations, intercellular spaces and spaces of Disse, and were accompanied by dilatation of bile canaliculi. Conclusion: These alterations were recovered with the treatment at the dose of 400 mg/kg of crude extract and 50 mg/kg of the isolated compound. Hence, it can be stated that this plant can show significant recovery in NDMA damaged livers.

Orthotopic liver transplantation (OLT) in rats using a whole or partial graft is an indispensable experimental model for transplantation research, such as studies on graft preservation and ischemia-reperfusion injury 1,2, immunological responses 3,4, hemodynamics 5,6, and small-for-size syndrome 7. The rat OLT is among the most difficult animal models in experimental surgery and demands advanced microsurgical skills that take a long time to learn. Consequently, the use of this model has been limited. Since the reliability and reproducibility of results are key components of the experiments in which such complex animal models are used, it is essential for surgeons who are involved in rat OLT to be trained in well-standardized and sophisticated procedures for this model. Darunaka aka dandruff, also known as Pityriasis Captitis, is one of the foremost common and widely seen dermatological maladies that influences most of the world's populace. The cardinal symptoms for Darunaka are Daruna (rough/flakes), Kandu (scalp itching), Rukshata (dry scalp), Kesha Bhumi Prapatana (cracking of the scalp), Kesha Chyuti (hair fall). A clinical study was conducted to assess the comparative effect of Malatyadi Taila over the Dhurdhuradi Taila Shiro Abhyanga in Darunaka (Dandruff). 40 patients with Darunaka were taken and randomly divided into 2 groups consisting of 20 patients in each group such as Dhurdhuradi Taila as Group A and Malatyadi Taila as Group B. The oil application (Shiro Abhyanga) is adopted for both the Taila with a duration of 1 month. Grading of severity were given for clinical signs and symptoms based on the before and after treatment. The overall effect of the study showed 25% of complete cure, 65% of mild cure and 10% of them have felt no change in Group A, and in Group B 80% of complete cure, 15% mildly cure, and 5% have felt no change. Darunaka management certainly benefited from the usage of Malatyadi Taila. There was a substantial improvement in clinical symptoms after just one month, and it was cost effective, adaptable, and safe.

Moringa oleifera, a well known traditional medicinal plant from Moringaceae family has a remarkable reputation among the indigenous medical practitioners. The chemo protective role of hydroethanolic extract of Moringa oleifera and its isolated saponin against 7, 12- dimethyl-benz[a]anthracene (DMBA) intoxicated mice was investigated in the present study with the help of histopathological analysis of soft tissues (liver and kidney). The PAH 7, 12-dimethyl-benz[a]anthracene (DMBA) acts as a potent carcinogen by generating various reactive metabolic intermediates leading to oxidative stress and tissue damage. The aim of this study was to investigate the effect of crateagus flavonoid extract in preventing pulmonary hypertension syndrome (PHS) in broiler chickens reared at high altitude, encountered ascites was evaluated. A 225 day-old broiler chickens (Ross-308) were randomly assigned to three treatments including different drinking levels of crateagus flavonoid extract (0, 0.1, and 0.2 mL per liter of drinking water) in a 42-day trial. Body weight gain were increased and feed conversion ratio were decreased significantly (P<0.05) when crateagus flavonoid extract was consumed by broiler chickens at levels of 0.1 and 0.2 mL per liter of drinking water in the both starting and growing stages, and throughout the trial. Moringa oleifera (M. oleifera) Lam belongs to the family Moringaceae. It is an important multipurpose tree that is largely distributed globally and has been used almost in every aspect of traditional medicine for the treatment of various illnesses including cancers, diabetes mellitus, asthma, arthritis, etc. This study investigated the effects of oral acute and sub-acute administration of M. oleifera hydroethanolic leaf extract (MOHE) in ICR-mice. Its major phenolic compounds were also determined. Ten (10) female, 8-week old mice were grouped into control and treatment groups for acute toxicity study. A dose of 2000 mg/kg MOHE was given once to the treatment group via oral gavage. PurposeTo describe first clinical experience with a directly image-able, inherently radio-opaque microspherical embolic agent for transarterial embolization of liver tumors.MethodologyLC Bead LUMI™ is a new product based upon sulfonate-modified polyvinyl alcohol hydrogel microbeads with covalently bound iodine (∼260 mg I/ml). 70–150 μ LC Bead LUMI™ iodinated microbeads were injected selectively via a 2.8 Fr microcatheter to near complete flow stasis into hepatic arteries in three patients with hepatocellular carcinoma, carcinoid, or neuroendocrine tumor. A custom imaging platform tuned for LC LUMI™ microbead conspicuity using a cone beam CT (CBCT)/angiographic C-arm system (Allura Clarity FD20, Philips) was used along with CBCT embolization treatment planning software (EmboGuide, Philips).ResultsLC Bead LUMI™ image-able microbeads were easily delivered and monitored during the procedure using fluoroscopy, single-shot radiography (SSD), digital subtraction angiography (DSA), dual-phase enhanced and unenhanced CBCT, and unenhanced conventional CT obtained 48 h after the procedure. Intra-procedural imaging demonstrated tumor at risk for potential under-treatment, defined as paucity of image-able microbeads within a portion of the tumor which was confirmed at 48 h CT imaging. Fusion of pre- and post-embolization CBCT identified vessels without beads that corresponded to enhancing tumor tissue in the same location on follow-up imaging (48 h post).ConclusionLC Bead LUMI™ image-able microbeads provide real-time feedback and geographic localization of treatment in real time during treatment. The distribution and density of image-able beads within a tumor need further evaluation as an additional endpoint for embolization.

Mitochondrial protein acetylation increases in response to chronic ethanol ingestion in mice, and is thought to reduce mitochondrial function and contribute to the pathogenesis of alcoholic liver disease. The mitochondrial deacetylase SIRT3 regulates the acetylation status of several mitochondrial proteins, including those involved in ethanol metabolism. The newly discovered desuccinylase activity of the mitochondrial sirtuin SIRT5 suggests that protein succinylation could be an important post-translational modification regulating mitochondrial metabolism. To assess the possible role of protein succinylation in ethanol metabolism, we surveyed hepatic sub-cellular protein fractions from mice fed a control or ethanol-supplemented diet for succinyl-lysine, as well as acetyl-, propionyl-, and butyryl-lysine post-translational modifications. A transhepatic gallbladder is an exceedingly rare finding that presents unique challenges for the general surgeon who often encounters it unexpectedly. A sonogram will not demonstrate it. A gallbladder that wholly penetrates the right lobe of the liver and is clearly visible on the anterior surface is an enigma. We describe an incidental finding at the time of elective surgery of such a phenomenon. This presented the author with a unique problem-how to remove the entire gallbladder without leaving a hole in the right lobe of the liver through which bowel could herniate. A novel technique was utilized which left a small bridge of cauterized gallbladder fundus wall. This article delves into the embryology of the hepatobiliary system in order to surmise how this could have occurred during development. This transhepatic gallbladder also contained a septum, dividing the gallbladder transversely into unequal parts. We hope that our findings and operative description will assist future surgeons that encounter a similar anatomic problem.

Macromolecular contrastenhanced functional CT was performed to characterize early perfusion changes in hepatocellular carcinoma (HCC). Fourteen rats with chemically induced primary liver tumors ranging pathologically from hyperplasia to HCC and 15 control rats were investigated. Two dynamic CT scans using an experimental macromolecular contrast agent were performed on a single slice 11 and 18 weeks after tumor induction followed by pathological examination. A deconvolution mathematical model was applied, yielding the hepatic perfusion index (HPI), mean transit time (MTT), liver distribution volume (LDV) and arterial, portal and total blood flows (FA, FP, FT). Objective: Our study aimed to examine the protective and curative ability of fresh orange juice (OJ) (Citrus sinensis L.) to counteract the adverse side effects of ionizing radiation (IR) on hepatic tissues of female irradiated rats and that has not been studied in advance. Methods: Forty-nine adult female Sprague-Dawley albino rats (170±5 g) were divided into four sets of 12 animals, except the healthy control group contained 10 rats only and the irradiated control group contained 15 rats and was divided as follow Group I: Healthy control; Group II: Irradiated control, rats receiving a single dose (20 gray absorbed dose [Gy]) of whole-body γ-rays; Group III: Protective group, rats received (5 ml OJ/kg body weight) once daily for 14 days and after 24 h exposed to irradiation; and Group IV: Curative group, then rats were submitted to irradiation than after 24 h, treated with (5 ml OJ/kg body weight) once every day for 14 successive days. Results: Our results explored that fresh OJ contains significant amounts of antioxidants as flavonoids and polyphenols and consequently pre-or post-fresh OJ supplementation to female irradiated rats attenuated significantly (p≤0.05) hepatic lipid, protein, and DNA-oxidative damage, hepatic inflammation, and activated inflammatory cyclooxygenase-2/prostaglandin E2 pathway, liver fibrosis, impaired liver functions, and hepatic lipid metabolism when compared with irradiated control rats. Furthermore, fresh OJ improved significantly (p≤0.05) the hepatic antioxidant capacity in protective and curative groups in comparison with the irradiated control group. Conclusion: The current research illustrated that fresh OJ may improve and normalize the various hepatic biochemical abnormalities resulted from irradiation due to its high content of active constituents of flavonoids and polyphenols. It is advised for people who exposed to IR, especially females, to consume about (5 ml OJ/kg body weight) before exposure as the most significant improvements were recorded in the protective group that supplemented with OJ before irradiation.
