Description

Claims (35)
- 허혈을 갖거나 허혈의 위험이 있는 피검체에서 중추신경계에 대한 허혈의 손상 효과를 치료하는데 사용하기 위한 PSD-95 억제제를 포함하는 약학적 조성물로서, 여기서 피검체에 대해 재관류 요법이 수행되고, PSD-95 억제제 및 재관류 요법은 피검체의 중추신경계에 대한 허혈의 손상 효과를 치료하며, 재관류 요법은 혈전용해제의 투여를 포함하고, PSD-95 억제제는 KLSSIESDV(SEQ ID NO:5) 또는 KLSSIETDV(SEQ ID NO:43)로 구성되거나 이를 포함하는 아미노산 서열을 갖는 펩티드를 포함하며, 상기 펩티드는 내재화 펩티드에 연결되거나 지질화됨으로써, 세포막 또는 혈액뇌장벽을 가로지르는 펩티드의 통과가 촉진되는 것인, 약학적 조성물.
- 제 1항에 있어서, PSD-95 억제제는 미리 정해진 당 분야에서 승인된 기준에 따라 진단 없이 뇌졸중 또는 다른 허혈성 질환이 의심되는 경우에 투여되는, 약학적 조성물.
- 제 1항에 있어서, 완전 경색의 존재가 CT 스캔에서 검출되거나, 추가 이익이 관찰되지 않을 때까지 다수의 투여량으로 투여되기 위한, 약학적 조성물.
- 제 1항에 있어서, 허혈의 손상 효과가 경색 또는 기능 결손인, 약학적 조성물.
- 제 1항에 있어서, PSD-95 억제제가 재관류 요법이 수행되기 전에 투여되거나, PSD-95 억제제가 허혈의 발생 전에 허혈의 위험이 있는 피검체에 투여되고 재관류 요법이 허혈의 발생 후에 수행되거나, 허혈의 발생 후에 PSD-95 억제제가 투여되고 재관류 요법이 수행되는, 약학적 조성물.
- 제 1항에 있어서, 허혈이 대뇌 허혈이거나, 피검체가 뇌졸중을 갖거나, 허혈이 CNS로의 혈류 또는 CNS로부터의 혈류를 억제함으로써 중추신경계에 영향을 미치는 심장, 폐 또는 주요 사지(major limb) 허혈인, 약학적 조성물.
- 제 1항에 있어서, 피검체가 PSD-95 억제제의 투여 및 재관류 요법의 수행 사이에 대뇌 허혈의 존재 또는 대뇌 출혈의 부재에 대해 시험되거나, 피검체가 PSD-95 억제제의 투여와 재관류 요법의 수행 사이에 출혈의 존재 또는 출혈의 위험에 대해 평가되는, 약학적 조성물.
- 제 1항에 있어서, PSD-95 억제제가 tat-NR2B9c인, 약학적 조성물.
- 제 1항에 있어서, 혈전용해제가 알테플라제(alteplase), 레테플라제(reteplase), 테넥테플라제(tenecteplase), 스트렙토키나제(streptokinase), 유로키나제(urokinase), 또는 데스모타플라제(desmotaplase)인, 약학적 조성물.
- 제 1항에 있어서, PSD-95 억제제가 재관류 전에 투여되고, PSD-95 억제제 투여와 재관류 요법 사이의 간격은 30분 내지 6시간인, 약학적 조성물.
- 제 5항에 있어서, 혈전용해제는 손상된 혈류 부위로의 국소 투여에 의해 투여되거나, 재관류 요법이 허혈의 발생 3시간 초과 후에 수행되는, 약학적 조성물.
- 제 1항에 있어서, 재관류 요법이 CT, MRI 또는 PET 분석에 의해 나타나는 바와 같은 완전 경색의 결손, 허혈성 반음영(ischemic penumbra) 및 출혈의 결손을 기초로 하여 재관류에 적격인 피검체를 결정한 후에 수행되거나, 재관류 요법이 허혈의 발생 적어도 12시간 후 또는 적어도 24시간 후에 수행되거나, 재관류 요법이 허혈의 발생 275-690분 후에 수행되는, 약학적 조성물.
- NMDAR 2B에 결합하는 PSD-95를 억제하는 작용제가 투여되는 피검체에서의 재관류 요법에 사용하기 위한 혈전용해제를 포함하는 약학적 조성물로서, 상기 작용제는 KLSSIESDV(SEQ ID NO:5) 또는 KLSSIETDV(SEQ ID NO:43)로 구성되거나 이를 포함하는 아미노산 서열을 갖는 펩티드를 포함하며, 상기 펩티드는 내재화 펩티드에 연결되거나 지질화됨으로써, 세포막 또는 혈액뇌장벽을 가로지르는 펩티드의 통과가 촉진되고, 재관류 요법 및 작용제가 중추신경계에 대한 허혈의 손상 효과를 치료하는, 약학적 조성물.
