Abstract

본 발명의 접골 플레이트는 근위상완골 골절의 고정에 사용된다. 길이가 폭 x 보다 더 큰 손잡이 형상부(1)와 폭 y가 상기 손잡이 형상부(1)의 폭 x 보다 더 큰 스픈 형상부(2)로 구성되며, 상기 손잡이 형상부(1)와 스픈 형상부(2)는 공통의 수직 중심축(3)을 가지며 전이영역(26)을 통해 서로 연결된다. 적어도 두 개의 스크류홀들(9, 10, 16, 17; 24)이 각각 손잡이 형상부(1)와 스픈 형상부(2)에 배치된다. 손잡이 형상부(1)에 있는 적어도 하나의 상기 스크류홀들(24)은 적어도 부분적인 내부 나사산(28) 혹은 부분적인 원주상의 홈을 구비하고, 스픈 형상부(2)에 있는 적어도 하나의 상기 스크류홀들(9, 10, 16, 17)은 내부 나사산(27) 혹은 부분적인 원주상의 홈을 구비한다. 상기 내부 나사산(27; 28) 혹은 부분적인 원주상의 홈의 피치는 0.5mm 에서 1.1mm의 범위에 있다. 본 발명을 통해 각이 고정된 조골 상태를 얻을 수 있다. 이 접골 플레이트의 주요한 이점으로는 나산이 두부에 형성된 스크류와 결합하는 스크류홀의 각안정성, 취급의 용이성, 더 나은 해부학적 적용성, 근위상완골과 관련된 해부학적 구조에 악영향을 주지 않는다는 점 등이 있다.

Description

근위상완골 골절 고정용 접골 플레이트{Bone plate for the fixation of fractures of the proximal humerus}
본 발명은 청구항 1의 전제부에 기재된 근위상완골(proximal humerus) 골절 고정용 접골 플레이트(bone plate)에 관한 것이다.
이러한 접골 플레이트는 모든 형태의 근위상완골 골절의 고정에 사용될 수 있는데, 근위상완골에서의 골절 탈구(dislocation)는 물론이고 특히 삼상 및 사상 골절에 사용된다.
어린 나이에는 근위상완골 골절이 심한 충격을 받을 때 자주 일어난다. 이런 형태의 골절이 실제로는 나이가 들어감에 따라서도 발생되는데 그 이유는 근위상완근 영역의 뼈 구조가 심각하게 약해지기 때문에 손상되지 않은 뼈 구조는 근위상완근 외주영역에만 남아있게 된다. 성인이 되어 과도하게 팔을 뻗은 상태에서 충격을 받을 때 근위상완골은 그 외주부분에서 부러지기 쉽다.
US Patent 6,096,040 ESSER에 일반적인 접골 플레이트가 공지되어 있다.
상기 시스템의 단점으로는 상기 접골 플레이트가 스픈 형태이고 홀이 구비되는데 이로 인하여 근위상완골 외주부에 잘 맞는 것으로 알려진 뼈 구조를 고정시키기 어렵게 한다. 뿐만 아니라, ESSER이 제시한 임플란트는 상기 홀에 고정각을 선 택할 수 없는 플레이트이다. 이 플레이트를 사용하게 되면, 접골 스크류(bone screw)를 통해 뼈에 압박이 가해진다. 이러한 과정을 압축 조골(compression osteosynthesis)이라 하는데, 발생되는 힘이 마찰에 의해 임플란트와 뼈 사이에 전달되며, 뼈가 대부분의 부담을 견뎌내야한다. 상기 플레이트를 뼈에 안정하게 고정하기 위하여 접골 스크류에 가해지는 장축 방향의 압력 때문에 동적인 상태에서는 뼈에서 뼈 스크류가 빠져버릴 수도 있으며, 따라서 플레이트-뼈 구조의 안정성이 상실된다.
이와 같은 관련 기술의 내용은 본 발명의 배경을 설명하기 위하여 제시하는 것일 뿐 본 출원 또는 그 우선권 주장이 되는 선출원 당시에 공지되었다는 것을 의미하지는 않는다.
본 발명이 해결하고자 하는 것은 바로 이것이다. 본 발명은 근위상완골 골절 고정을 위한 접골 플레이트에 관련된 것으로 이를 통하여 고정된 각을 갖는 조골을 얻을 수 있다. 본 발명의 주요한 이점으로는 나산이 두부에 형성된 스크류와 결합하는 스크류홀의 각안정성(angle stability), 취급의 용이성, 더 나은 해부학적 적용성, 근위상완골과 관련된 해부학적 구조에 악영향을 주지 않는다는 점 등이 있다. 두부에 나사산이 형성된 스크류를 사용함으로써 각이 고정된 접골 플레이트 구조체를 만들어내며 동적인 상태에서도 균형을 유지한다.
본 발명은 청구항 1의 특징을 갖는 접골 플레이트를 사용하여 상기한 문제점을 해결한다.
나사산 피치를 작게하여 스크류가 플레이트에 매우 짧은 거리로, 보통은 1mm 에서 2mm의 범위로, 견고하게 고정되도록 한다. 이렇게 함으로써 플레이트-스크류 구조체의 안정성에 영향을 주지 않으면서도 플레이트 두께를 매우 얇게 유지할 수 있다.
바람직한 실시예에서, 상기 효과를 더욱 증진시키기 위하여 내부 나사산 혹은 부분적인 원주상의 홈의 피치는 0.7mm 에서 0.9mm의 범위로 한다.
다른 바람직한 실시예에서, 손잡이 형상부의 폭 x로부터 스픈 형상부의 폭 y에 이르는 전이영역은 점점 넓어지며, 지수함수적으로 넓어지는것이 바람직하다. 이렇게 함으로써 골절되지 않은 근위상완골에 최적으로 적용하는 것이 가능하게 된다. 플레이트는 근위상완골에 말끔하게 정착되며, 관련되는 해부학적 구조를 저해하지 않는다.
플레이트의 두께는 사용의 편의상 1.7mm 에서 2.3mm의 범위, 바람직하게는 1.9mm 에서 2.1mm의 범위이다. 종래의 접골 플레이트와 비교할 때, 상대적으로 작은 두께의 이점은 주로 얇은 플레이트는 관련 주변 구조에 나쁜 영향을 주지 않으며 팔을 들 때 견봉(acromion)에 마찰을 주지 않는다는데 있다.
플레이트의 손잡이 형상부는 중심축 방향으로 볼 때 적어도 일부분이 곡선 형태를 가지는 것이 바람직하다. 다른 바람직한 실시예에서는, 플레이트의 손잡이 형상부는 중심축(3) 방향으로 볼 때 적어도 일부분에 있어 스플라인(spline)(21) 형태로 구부러져 있다. 스플라인(21)과 연관된 타원의 장축(22)의 길이는 바람직하게는 150mm에서 170mm의 범위, 전형적으로는 157mm에서 163mm의 범위를 갖는다. 스플라인(21)과 연관된 타원의 단축의 길이는 바람직하게는 60mm에서 80mm의 범위, 전형적으로는 67mm에서 73mm의 범위를 갖는다. 손잡이 형상부(1)와 스픈 형상부(2)의 자유단은 서로에 대해 평행한 평면상에 놓여있는 것이 바람직하다.
이 모든 치수들은 골절되지 않은 근위상완골에 플레이트가 말끔하게 정착되며 관련된 해부학적 구조를 저해함이 없이 개선된 적용을 가능하게 한다는 이점을 갖는다.
또 다른 바람직한 실시예에서, 접골 플레이트는 적어도 일부분에서 중심축에 직각인 굴곡을 가지며, 바람직하게는 반경이 18mm 에서 22mm로 형성된다.
또 다른 실시예에서, 접골 플레이는 전체 길이에 대하여 중심축에 직각인 굴곡을 가진다. 여기서 얻을 수 있는 이점은 연부조직(soft tissue)에 자극을 주지 않는다는 것이다. 팔을 들 때, 접골 플레이트가 견봉에 마찰을 가하는 것도 방지된다.
또 다른 바람직한 실시예에서, 스픈 형상부의 자유단은 중심축에 대하여 대칭적으로 배치된 평행한 홀 축을 갖는 두 개의 홀을 구비하며, 상기 홀 축은 스픈 형상부가 이루는 표면에 대해 92°에서 98°, 바람직하게는 94°에서 96°의 각을 형성한다. 종래의 임플란트는 각이 고정되는 리포지션닝(repositioning)을 얻기 위하여 각이 고정된 홀로 삽입되는 스크류에 의해 대치되는 블레이드(blade)가 필수적이다. 그러나 블레이드를 사용하는 수술방법은 본 발명에서와 같은 고정각 스크류를 사용하는 것보다 시간이 많이 들고 더 복잡하다. 대략 95°의 접촉각은 고상의 뼈와 접촉하는 표면을 확장시키는데, 그 이유는 고상의 뼈질은 비교적 속이 비어있는 상완골 두부의 가장자리 영역에만 존재하기 때문이다.
또 다른 바람직한 실시예에서, 스픈 형상부의 전이영역에 인접한 부분은 중심축에 대하여 비대칭적으로 배치된 두 개의 홀을 구비하며, 상기 두 홀의 축은 평행면상에 놓여있고 바람직하게는 중심축에 직각으로 배치된다. 하나의 홀 축이 다른 홀 홀 축을 포함하는 평행면으로 투사되었을 때 형성되는 각은 40°에서 60°, 바람직하게는 46°에서 54°이다. 이러한 배치의 이점은 상기 교차하는 홀들로 삽입되는 스크류가 근위상완골의 골절시에는 빈번히 끈어져버리는 작은 결절(tuberculum)의 안착소(pullup)가 되도록 한다는 것이다.
또 다른 바람직한 실시예에서, 중심축에 대하여 대칭적으로 및/또는 비대칭적으로 배치된 홀들중 적어도 하나는 내부 나사산 혹은 부분적인 원주상의 홈을 갖는다. 바람직하게는 중심축에 대하여 대칭적으로 및/또는 비대칭적으로 배치된 홀들의 홀축중 어느 것도 교차하지 않아야 한다. 이것은 홀에 삽입되는 스크류가 서로 간섭하지 않게 되는 이점을 가져다준다.
또 다른 실시예에서, 손잡이 형상부의 홀축은 손잡이 형상부에 수직으로 배치된다. 상기 내부 나사산은 다중 나사산 혹은 이중 나사산으로 형성되는 것이 바람직하다.
본 발명에 따른 접골 플레이트는 왼쪽 및 오른쪽 상완골 모두에 사용될 수 있으며 건강한 근위상완골의 형태에 최적으로 적용될 수 있다.
본 발명 및 본 발명의 개선 내용은 실시예에 관한 부분 모식도를 사용하여 이하에서 상세하게 설명한다.

도 1 내지 4에 도시된 접골 플레이트는 근위상완골의 골절 고정에 사용되는 것이며 길이가 그 폭 x 보다 더 큰 손잡이 형상부(1)와 폭 y가 상기 폭 x 보다 더 큰 스픈 형상부(2)로 구성된다. 손잡이 형상부(1)와 스픈 형상부(2)는 공통적으로 수직 중심축(3)을 가지며 손잡이 형상부의 폭 x로부터 스픈 형상부의 폭 y로 지수함수적으로 확장되는 전이영역(26)을 통하여 서로 연결된다.
손잡이 형상부(1)와 스픈 형상부(2)에는 다수의 스크류홀(9, 10, 16, 17; 24)이 있다. 접골 플레이트의 손잡이 형상부(1)에 위치한 모든 스크류홀(24)은 내부 나사산(28)(부분적인 원주상의 홈으로 대치될 수도 있다.)을 갖는다.
접골 플레이트의 스픈 형상부(2)에 있는 네 개의 스크류홀(9, 10, 16, 17) 역시 내부 나사산(27)(부분적인 원주상의 홈으로 대치될 수도 있다.)을 갖는다.
이러한 내부 나사산(27; 28)(혹은 부분적인 원주상의 홈)의 피치는 0.8mm이다. 접골 플레이트의 두께는 2mm이다.
도 4에 도시된 바와 같이 손잡이 형상부(1)는 중심축 방향으로 볼 때 일부분이 스플라인(21)의 형태로 굴곡을 가진다. 스플라인(21)과 연관된 타원의 장축(22)의 길이는 160mm이며, 스플라인(21)과 연관된 타원의 단축(23)의 길이는 70mm이다. 손잡이 형상부(1)의 굴곡으로 인하여 손잡이 형상부(1)와 스픈 형상부(2)의 자유단(5)은 서로에 대해 평행한 면들 위에 놓이게 된다.
도 2에 도시된 바와 같이, 접골 플레이트는 전체 길이에 대하여 중심축(3)에 대하여 수직인 굴곡(6)을 가지며, 이 굴곡은 반경이 20mm이다.
도 1 및 2에 도시된 바와 같이, 스픈 형상부(2)의 자유단(8)은 중심축(3)에 대하여 대칭적으로 배치되어 있고 평행한 홀축을 갖는 두 홀(9, 10)을 구비하며, 홀축(11, 12)은 스픈 형상부(2)에 의해 형성되는 표면(14)과 95°의 각(13)을 이룬다.

Claims (19)

  1. 길이가 폭(x) 보다 더 큰 손잡이 형상부(1)와 폭(y)이 상기 손잡이 형상부(1)의 폭(x) 보다 더 큰 스픈 형상부(2)를 구비하며, 상기 손잡이 형상부(1)와 스픈 형상부(2)는 공통의 수직 중심축(3)을 가지며 전이영역(26)을 통해 서로 연결되고, 적어도 두 개의 스크류홀들(9, 10, 16, 17; 24)이 각각 손잡이 형상부(1)와 스픈 형상부(2)에 배치되며,
    a) 손잡이 형상부(1)에 있는 적어도 하나 이상의 상기 스크류홀들(24)에는 일부분 혹은 전체에 걸쳐 내부 나사산(28)이 구비되거나, 또는 부분적인 원주상의 홈이 구비되고;
    b) 스픈 형상부(2)에 있는 적어도 하나 이상의 상기 스크류홀들(9, 10, 16, 17)에는 내부 나사산(27) 혹은 부분적인 원주상의 홈이 구비되고;

  18. 제12항에 있어서, 상기 손잡이 형상부(1)의 홀(24)의 축은 손잡이 형상부에 수직으로 배치되는 것을 특징으로 하는 근위상완골 골절 고정용 접골 플레이트.
  19. 제12항에 있어서, 상기 내부 나사산(27; 28)은 다중 나사산으로 형성되는 것을 특징으로 하는 근위상완골 골절 고정용 접골 플레이트.
