Abstract

Description

다중축 뼈-고정 장치{Multi-Axial Bone Anchor Apparatus}
본 발명은 소망하는 공간적 관계로 뼈들(bone member)이 고정시키고, 뼈의 편향성 질환(bone misalignment disorder)을 교정하는 데 사용하기 위한 변형교정 삽입물 조립체(orthopedic implant assembly)에 관한 것이다. 보다 상세하게는, 본 발명은 판(plate)과 같은 연장 부재(elongated member)를 포함한 다중축의 척추 고정 시스템(multi-axial spinal fixation system)에 관한 것이다.
변형교정 수술분야, 특히 척추수술 분야에서, 판(plate) 또는 봉(rod)과 같은 연장 부재를 뼈에 부착시켜 주어진 위치에 뼈를 유지 및 지지하는 기술은 오래 전부터 공지되어 왔다. 예를 들어, 손상되거나 질병에 걸리거나 잘못된 형태를 가지거나 기타 비정상적인 추골(vertabrae)을 일체화(fuse)하는 과정에서, 외과의사는 추골을 교정된 위치에 놓게 된다. 연장 판(elongated plate)을 척추골에 인접하게 위치시키고, 특정 형상의 나사 또는 볼트와 같은 뼈 고정재(bone anchor)를 사용하여 상기 판을 뼈에 고정시킨다. 이러한 고정재의 경우, 뼈에 1 이상의 구멍을 뚫고 구멍 안으로 고정재를 돌려 끼워넣음(threading)에 의해 배치가 이루어진다<참조: 미국특허 5,676,666 (Oxland 등), 미국특허 5,613,967(Engelhardt 등) 및 미국특허 5,603,713(Aust 등)> 고정재는, 판을 관통하여 척추의 구멍(vertebral hole)안으로 끼워 뼈에 연결시키거나, 혹은 대안으로써, 고정재를 뼈에 연결한 후, 전기 고정재 위 또는 그 주위의 올바른 위치에 상기 판을 놓을 수 있다. 이어서, 고정재 및 판을 서로 고정하여 상호 움직임을 최소화하거나 방지하게 된다. 이러한 방법에 의해 뼈를 치료를 위한 적절한 정렬상태로 유지 및/또는 지지할 수 있다.
삽입물 시스템의 연장 부재 또는 기타 다른 고정기구에 대한 볼트 또는 다른 고정재의 각 배향이 수개의 면으로 이루어 질 수 있는 삽입물 시스템이 바람직한 것은 공지된 사실이다. 상기 특징은, 뼈 고정재가 고정에 가장 적합한 각도로 놓일 수 있도록 함으로써, 고정판이 최적의 위치에 놓이면서도 고정재가 느슨해지거나 빠지거나 기타 다른 움직임을 일으킬 수 있는 가능성을 줄어들게 한다. 나아가, 상기 시스템은 편평하지 않은 뼈의 표면 및 교정해야 할 기형도(abnormality)로 인해 척추 수술시 종종 발생하는 조작의 어려움을 줄여주고, 일반적으로, 삽입물에 대한 적응의 필요성을 줄여주므로, 외과의사의 교정 수술을 보다 용이하게 하고 환자에게는 수술 후 고통을 줄여준다.
이러한 다중축능(multi-axial capability)을 달성하기 위해서 여러가지 접근방법이 사용되어 왔다. 예를 들어, Olerud의 미국특허 5,735,853은 볼트 주변에 딱 맞는(snap-fit) 압축성의 구형 칼라(compressible spherical collar)를 구비하여 뼈 볼트가 판과의 관계에서 여러가지 각도로 위치할 수 있도록 한 삽입물 기구를 개시하였는데, 상기 칼라는 판 삽입물(a plate insert) 내의 구형 개구내에서 회전이 가능하고 기울어질 수 있다. 그러나, 판 내에서 볼트 및 칼라를 압축하여 끼워맞추는 것(compression fit)은 기구 조립시, 특히 유체가 널리 퍼진 환경에서 어려 움을 야기할 수 있다.
또 다른 접근 방식은 Wagner의 미국특허 5,304,179에 나타난 것으로, 이는 하나의 금속통 내부에 금속통(bushing)의 길이방향축에 대해 임의의 각도로 고정된 하나의 뼈 나사를 개시하고 있다. 금속통은 이웃하는 봉형태 수단(road-based instrumentation)의 축에 대해 일정한 각도를 가지는 연결자(connector)의 일정 범위내에서 회전할 수 있다. 연결자는 상기 수단의 축 주위로 회전할 수 있다. Wagner 시스템의 경우, 3차원 공간내에서 뼈 나사의 불연속적인 배치만 가능하며, 금속통들로 인해 구조물의 길이 및 윤곽(profile)이 증가할 뿐만 아니라 의사가 다루고 배열해야 하는 부품수도 늘어난다.
세번째 접근방식은 Asher 등의 미국특허 No. 5,984,924에 나타나 있는데, 여기서는 2쌍의 와셔사이에 끼워 넣어진 연장된 뼈 정렬부재(elongated bone alignment)를 가진 뼈 정렬 시스템을 보여준다. 각 쌍의 상기 와셔는 여러 위치로 놓일수 있도록 볼-소켓(ball and socket) 형상으로 맞물리는 해당 표면을 가지고 있다. 뼈 고정재의 샤프트(shaft)는 각 쌍의 와셔 및 연장된 부재의 개구를 관통하여 뻗어나오고, 이 때, 당해 와셔 쌍은 상기 샤프트가 연장 부재에 대하여 여러 각도로 배향할 수 있도록 한다. 이러한 접근 방식은 수술 중 취급 및 조립해야 하는 여러 개의 소부품들을 필요로 한다. 나아가, 이러한 시스템에서 와셔는 연장부재의 외부에 존재하기 때문에 와셔에 의해 전체 구조물의 두께가 증가하고, 따라서 좁은 수술공간에서의 사용이 더욱 어려워지며 환자에게 더 큰 불편을 줄 수 있다.
따라서, 당해 기술분야에는 뼈 고정재를 다양한 배향각으로 척추판(spinal plate)과 같은 연장부재에 맞물리기 위해 사용되는 부품의 수를 최소화한 다중축 뼈 고정재 시스템에 대한 필요성이 남아있다. 또한, 뼈 고정재의 위치 선정시 상기 연장부재에 대해 그 공간각 및 선형적 위치가 연속적 범위로 조절될 수 있는 다중 축 뼈 고정재로서, 보다 편평한 윤곽 및 보다 작은 치수를 가지는 고정재에 대한 필요성이 존재한다.
본 발명은, 그 하나의 구현예에 있어, 추골과 같은 1 이상의 뼈들을 따라 인접하게 놓여지기 위한, 길이방향으로 하나의 축을 가진 판 부재(plate member)를 포함한 변형교정 삽입물 시스템을 제공한다. 상기 판 부재는 판의 축에 실질적으로 평행한 1 이상의 슬롯을 포함하며, 이 때, 상기 슬롯은 실질적으로 판부재의 전(全) 길이에 걸쳐 연장된 채널부를 포함한다. 또한, 실질적으로 벽에 의해 한정된 관통 개구를 포함하는 스테빌라이져가 제공되는데, 이 경우, 상기 개구는 길이방향으로 하나의 축을 가지고 있고, 상기 스테빌라이져는 개구의 축에 대하여 측면으로(laterally) 연장된 1 이상의 핑거부를 추가로 포함한다. 이 때, 상기 핑거부는 판의 채널내에 수용되어 스테빌라이져의 개구가 판의 슬롯과 통하도록 한다. 뼈 맞물림부(bone-engaging portion), 매개부(intermidiate portion) 및 나사산을 가진 포스트부(a threaded post protion)를 포함한 뼈 볼트는 스테빌라이져의 개구 및 판의 슬롯을 관통해 뻗어나와(extend) 볼트의 매개부가 스테빌라이져의 벽에 인접하도록 한다. 또한, 둥근 최상부면(rounded top surface), 바닥면(bottom surface) 및, 볼트의 일부의 주위에 맞도록 되어있는 관통 개구를 포함한 와셔, 및 와셔로부터 분리되고, 본체부, 가장자리부 및 볼트와의 맞물림을 위해 본체부를 관통하여 연장된 나사산이 있는 구멍(a threaded hole)을 포함한 너트가 제공되며, 이 때 상기 너트 및 와셔는 볼트에 끼워 맞추기 전에 서로 결합시킨다.

도 7 내지 10은, 연장부재 522 와 함께 사용할 수 있는 본 발명에 따른 뼈 고정재 조립체 524의 한 구현예를 나타낸 것이다. 일반적으로 뼈 고정재 조립체 524 는 뼈 볼트(bone bolt) 540과 같은 고정부재, 호형와셔(arcuate washer) 542, 및 너트 544 를 포함한다. 도시된 구현예의 경우, 와셔 542 및 너트 544 는 쉽게 분리되지 않을 정도로 단단히(affirmatively) 결합되어 있으나, 서로 회전가능(rotatable)하며 병진적으로 움직일 수 있다(tlanslatable).

도시된 본 구현예의 경우, 가장자리부 577 은 본체부 578 과 일체로서, 아래방향으로 뻗어 있으며, 실질적으로 원뿔형이고, 슬리브부 579 둘레로 연장된, 실질적으로 원뿔형인 내면 584를 가진다. 가장자리부 577 및/또는 내면 584를 위해서는 다른 기하학적 형상을 사용할 수 있다. 본 구현예에서, 슬리브부 579는 대략 가장자리부 577과 본체부 578의 접합부으로부터 가장자리부 577의 하부 가장자리를 넘어서까지 일체로 연장되어 있다. 슬리브부 579는 그 외부 직경이 개구 566의 부축 564보다 약간 작아 와셔 542의 개구 566로 수용되기에 적합하다.
도 10a 내지 도 10d에 나타나 있는 본 발명에 따른 하나의 특정 구현예의 경우, 호형 와셔 542 및 너트 544가, 서로에 대해 회전가능하고 병진적으로 움직일 수 있으나, 쉽게 분리되거나 풀어지지는 않도록 결합되어 있다. 상기 구현예에서는, 우선 너트 544의 슬리브부 579 를 호형 와셔 542의 개구 566으로 삽입하여 가장자리부 577의 내면 584가 호형와셔 542의 헤드 562에 인접하도록 함으로써 결합을 완성한다. 슬리브부 579의 하부는 형철로 구부리거나(swage), 혹은 실질적으로 슬리브부 579의 전체 원주를 따라 플레어식 가장자리(flared edge) 579a 를 형성하도록 확장시킨다. 슬리브부 579는 임의의 종래 방법에 의해 형철로 구부리거나 확장시킬 수 있다. 생성된 플레어식 가장자리 579a 의 직경은 와셔 542의 벽 부분 570 직경보다는 작으나, 벽부분 568의 최소 부분보다는 커서 플레어식 가장자리 579a가 언더컷 569에 의해 와셔 542의 개구 566내에 유지될 수 있다. 이처럼 결합된 경우, 호형와셔 542 및 너트 544는 서로에 대해 자유롭게 회전할 수 있으며, 나아가 서로에 대해 주축 565를 따라 병진적으로 움직일 수 있다.
이러한 방식으로 와셔 542 및 너트 544를 결합함에 의해 외과의사가 다루어야 하는 구성부품의 수를 줄일 수 있어 그 사용시 용이함이 증가한다. 추가로, 결합된 와셔-너트 구성부품은 각각의 별도 구성부품보다 크기 때문에, 삽입물의 취급성이 향상되고, 떨어지거나(dropped), 잘못 꼽히거나(misthreaded) 혹은 잘못 사용되는(misused) 부품의 발생 가능성도 줄어든다. 와셔 542 및 너트 544의 결합시 다른 방법도 사용될 수 있는 바, 예를 들어, 슬리브 579의 말단 부분보다는 중간부분을 확장하거나, 또는 스프링식 래치(spring-loaded latch) 또는 볼-멈춤쇄(ball-dent) 구조, 클램프(clamp) 또는 기타 유사한 홀딩(holding) 구조와 같이, 2개의 부품을 결합하기 위한 종래의 구조를 추가로 제공할 수 있다.

도 1은 추골 521 을 소망하는 위치로 고정 또는 유지하기 위해 본 발명에 따른 변형교정 삽입물 520을 일련의 추골 521에 고정시킨 구현예를 나타낸 것이다. 특히, 2개의 뼈 고정재 조립체 524 각각이 추골 521 각자의 기하에 가장 적합하게 정열한 경우, 각각의 조립체 524 는 서로 평행이 아니거나, 혹은 골판 또는 연장부재 522에 대해 수직이 아닐 수 있다. 본 발명의 와셔 542 및 너트 544는 골판 표면에 대해 실질적으로 수직인 축으로부터의 각 오프셋(angular offset)에 관계없이 각각의 뼈 나사 540을 골 판 522에 고정되게 할 수 있다. 골판 622는, 나타난 바와 같이, 뼈들(bone members) 521의 자연적 윤곽에 보다 적합하도록 길이방향으로 굽어져 있다. 기타 다른 응용의 경우, 굽어진 판이 필요하지 않을 수 있다. 판 522 및 622는 실질적으로 동일한 방식으로 사용되기 때문에, 명확성을 위해 본 발명의 사용은 판 522에 관하여 기술한다.

외과 의사는 뼈 또는 뼈들 541 안으로 1 이상의 구멍을 판 다음, 뼈-맞물림 부 546의 다공질 나사산 548을 경유해 구멍안으로 볼트(들)540을 끼워 넣는데, 이 경우 볼트 540의 평평한 부분 556가 덮히지 않도록 한다. 삽입된 스테빌라이져 526을 구비한 판 522는 뼈 볼트 540 위에 놓고, 매개부 554가 각각의 스테빌라이져 526의 개구 594내에 있도록 하며, 근위부 550은 판 522의 상부면 523 을 관통하여 뻗어나오도록 한다. 이어서, 판 522는 볼트 540에 대하여, (슬롯 530을 따라 볼트를 이동시키고 채널 530c를 따라 관련 스테빌라이져 526을 움직여) 병진적으로 조정하고, (매개부 554의 상부표면 555상에서 판 522 및 스테빌라이져 526 를 회전시켜) 다중축적으로(multi-axially) 조정할 수 있다. 뼈 볼트 540의 매개부 554의 둥근 상부표면 555는 개구 594 및 스테빌라이져 526의 벽부분 597 및 598과 잘 맞물릴 수 있기 때문에, 뼈 볼트 540은 판 522에 대해 임의의 다양한 각을 형성할 수 있다. 도 3에 도시된 발명의 구현예를 참조하면, 스테빌라이져 526의 하부면 593 이 볼트 540의 평평한 면 556 또는 뼈 맞물림 나사산 548 에 접근하지 않은 것을 알 수 있다.
추골 541의 위치 및 볼트 540 에 대한 판 522의 위치가 소망하는 바대로 놓이면, 결합된 와셔 542 및 너트 554 를 각각의 뼈 볼트 540의 근위부 550위로 돌려서 끼워넣어(thread) 근위부 550이 와셔 542의 개구 566 뿐만 아니라 너트 544의 나사산을 구비한 개구 580과 통로 581을 관통하여 뻗어 나오도록 한다. 와셔 542의 내벽부 568이 테이퍼되어 있기 때문에, 와셔 542 및 너트 544가 그 위에 돌려 끼워질 때, 와셔 542는 볼트 540의 근위부 550에 대하여 다양한 각 위치(angular position)를 차지할 수 있다. 결합된 상태의 와셔 542 및 너트 544가 근위부 550을 따라 판 522에 다가갈 때에, 필요하다면, 와셔 542 를 볼트 540 및 판 522 에 대해 회전시켜 판 522에 대한 위치를 취하게 함으로써, 직사각형의 개구 566이 근위주 550 및 연장부재 522 간의 각 오프셋을 수용하도록 한다. 판 522가 볼트 540에 대하여 원하는 위치로 배향된 경우, 와셔 542의 하부면 563이 판 522의 상부면 523에 실질적으로 정면으로 놓이면, 상기 배치가 완성된다.
너트 544를 더욱 꼭 조임으로써, 가장자리부 577이 헤드 562의 일부에 단단히 맞물리고 이를 누르며(bear), 이로 인해 가장자리부 577의 내면 584 및 와셔 542의 헤드 562간에 안정적이고 단단한 접촉이 제공된다. 이처럼 근위부 550 상에서 너트 544를 꼭 조임으로써, 스테빌라이져 526의 벽부분 597에 대면한 볼트 540의 매개부분 554 에 힘을 가하고, 판 522 의 내부의 실링면 535b 에 대면한 상부면 591 및 핑거부 592에 힘을 가하여, 판 522를 볼트 540에 안정적으로 고정시킨다.
너트 544의 탭 576 은 볼트 540 의 근위부 550 위의 기계 나사산 밑에 놓인 경우(lodge) 너트 544의 반대방향 회전 또는 기타 움직임에 대한 실직적인 저항 및 간섭 끼워맞춤(resistance or interference fit)이 제공된다. 근위부 550의 탭 576위에 남아있는 모든 부분은 부수거나, 절단해버리거나, 잘라내어 주변 조직의 염증을 최소화한다.
상기에서, 와셔-너트 조합 541을 포함한 구현예가 기술되었다. 별개의 호형 와셔 542 및 너트 544를 포함하는 다른 구현예, 예를 들어, 미국 가출원 일련번호 60/129,587 및 60/149,774에 개시된 구현예도 본 발명의 범위에 속하며, 상기 출원은 전체적으로 본발명의 개시에 참조로써 포함되어 있다. 아울러, 부품의 크기가 다르거나 부품 내의 개구 크기가 다른 경우도 본 발명의 범위에 포함된다.

Claims (15)

  1. 하나 이상의 뼈를 따라 인접하여 위치하고 길이방향으로 하나의 축을 가지는 판 부재로서, 상기 판 부재는 상기 축에 평행한 관통 슬롯을 포함하고, 상기 슬롯은 상기 판부재의 전체 길이에 걸쳐 연장된 채널부를 포함하는 판부재;
    벽에 의해 한정되고 하나의 길이방향 축을 가진 관통 개구를 포함한 스테빌라이져(stabilizer)로서, 상기 스테빌라이져는 추가로 상기 개구 축에 대해 측면으로 연장된 1 이상의 핑거부를 포함하고, 이 때, 상기 핑거부는 상기 채널내에 수용되고 상기 개구는 상기 슬롯과 통하는 스테빌라이져;
    뼈 맞물림 부, 매개부 및 나사산이 있는 포스트부(threaded post portion)를 포함한 뼈 볼트로서, 상기 볼트는, 상기 매개부가 상기 스테빌라이져의 상기 벽에 인접하도록, 상기 스테빌라이져의 상기 개구 및 상기 판 부재의 상기 슬롯을 관통하여 뻗어나온 뼈 볼트;

    를 포함하고, 이 때,상기 너트 및 상기 와셔는 상기 볼트에 맞물리기 전에 결합된 변형교정 삽입물 장치.
  2. 제 1 항에 있어서, 상기 너트 및 상기 와셔는 상호 회전가능하고, 서로에 대해 병진적으로 움직일 수 있는 것을 특징으로 하는 삽입물 장치.
  3. 제 2항에 있어서, 상기 너트는, 부분적으로 상기 가장자리부 내에 있고 상기 구멍과 중심을 같이 하는, 슬리브부를 포함하는 것을 특징으로 하는 삽입물 장치.
  4. 제 3항에 있어서, 상기 와셔는 상기 개구내에 제 1 직경을 가진 언더컷을 포함하고, 상기 너트 및 상기 와셔는, 상기 슬리브부를 상기 개구 내에 삽입하고 상기 슬리브부의 일부를 상기 제 1 직경보다 큰 제 2 직경까지 확장함에 의해, 결합된 것을 특징으로 하는 삽입물 장치.
  5. 제 4항에 있어서, 상기 슬리브부는 상기 본체부로부터 원거리인 말단을 포함하고, 상기 말단이 상기 슬리브부의 상기 확장부를 형성하는 것을 특징으로 하는 삽입물 장치.
  6. 제 1항에 있어서, 상기 뼈 볼트는 상기 뼈내에서 상기 볼트를 끼워넣기 위해 사용하는 공구 맞물림 홈(tool engaging recess)을 추가로 포함하는 것을 특징으로 하는 삽입물 장치.
  7. 제 1항에 있어서, 상기 볼트의 상기 매개부는, 수 개의 다른 배향으로 상기 스테빌라이져의 상기 개구내에 수용될 수 있는 형상의 확대부(enlarged portion)를 포함하는 것을 특징으로 하는 삽입물 장치.
  8. 제 7항에 있어서, 상기 확대부는 구형부(spherical portion)를 가진 표면을 포함하는 것을 특징으로 하는 삽입물 장치.
  9. 제 1항에 있어서, 상기 판 부재는 복수개의 관통 슬롯을 포함하는 것을 특징으로 하는 삽입물 장치.
  10. 제 9항에 있어서, 상기 판은 굽어진 것을 특징으로 하는 삽입물 장치.
  11. 제 1항에 있어서, 상기 너트는 길이방향 축을 가지고, 상기 너트로부터 뻗어나와 너트의 축을 향하여 기울어진 탭을 추가로 포함하며, 상기 탭은 상기 너트가 느슨해지는 것을 방지하기 위해 상기 볼트의 나사산 있는 포스트 부분과 상호 작용하도록 되어 있는 것을 특징으로 하는 삽입물 장치.
  12. 제 1항에 있어서, 상기 스테빌라이져는 팔각형의 상부표면 및 둥근 하부표면을 가진 것을 특징으로 하는 삽입물 장치.
  13. 제 1항에 있어서, 적어도 제 2의 상기 뼈 볼트, 상기 제 2의 뼈 볼트에 관련된 적어도 제 2의 상기 스테빌라이져, 상기 제 2의 뼈 볼트와 관련된 적어도 제 2의 상기 워셔 및, 상기 제 2의 뼈 볼트와 관련된 적어도 제 2의 상기 너트를 추가로 포함하고, 이 때 상기 제 2의 워셔 및 상기 제 2의 너트는 전기 제 2의 뼈 볼트와 맞물리기 전에 결합되는 것을 특징으로 하는 삽입물 장치.
