Description

c)第一のインピーダンスパラメータおよび第二のインピーダンスパラメータを使用して、インピーダンスパラメータの少なくとも変化を決定するか、または少なくとも第一もしくは第二のインピーダンスパラメータを閾値と比較するステップと、を含み、請求項1に従って本明細書で提案される。
-第一の時点での第一のインピーダンスパラメータおよび第二の時点での第二のインピーダンスパラメータを決定するように構成される、測定装置と、
-少なくとも第一のインピーダンスパラメータおよび第二のインピーダンスパラメータを使用して、(経時的な)インピーダンスパラメータの変化を決定するように、または少なくとも第一もしくは第二のインピーダンスパラメータを閾値と比較するように構成される、処理ユニットと、を備える。

・補助システムに厳密に組み込まれたセンサの使用が可能になる。例えば、大動脈の周囲に別個の超音波カフは不要である。
・この方法は、総心臓時間容積の決定ならびに補助レベル(総心臓時間容積のポンプ体積流量の割合)の決定を可能にする。
・電気インピーダンス測定技術の統合は、大動脈超音波カフまたは類似のものと比較して実施が容易である。
・総心臓時間容積は、システムによって連続的に測定されるため、LVADは、心臓時間容積ならびに補助レベルの両方に基づいて調整することができ、これは短期補助システム(キーワード離脱)では特に有利である。
・この方法は、例えば、最小心室容積に達していないときに、補助システムを調整できるように、心室容積を決定する。
Claims (10)
- 埋め込み可能な心室補助システム(2)であって、
第一の時点での第一のインピーダンスパラメータおよび第二の時点での第二のインピーダンスパラメータを決定するように構成された、測定装置(9)と、
前記第一のインピーダンスパラメータおよび前記第二のインピーダンスパラメータの間の変化を決定し、前記第一のインピーダンスパラメータおよび前記第二のインピーダンスパラメータの間の前記変化に基づいて心室容積の変化を決定するように構成された、処理ユニット(8)と、を備え、
前記測定装置(9)が、前記第一および第二のインピーダンスパラメータを決定するためのインピーダンスの測定に使用され得る少なくとも二つの電極(4、5)を備え、前記電極が前記補助システムの外面の円周方向に配置された心室インピーダンスを測定することを目的とすることを特徴とし、
前記処理ユニットは、前記第一および/または第二のインピーダンスパラメータを心室の虚脱、前記心室補助システムの心室内壁への吸引、または心室壁の過度な拡張のリスクに関連する閾値と比較するようにさらに構成されている、補助システム。 - 前記測定装置(9)が、インピーダンスを測定することができる四つの電極(4、5、27、28)を備える、請求項1に記載の補助システム。
- 前記測定装置(9)は、異なる周波数でのインピーダンス測定のために設計される、請求項1または2に記載の補助システム。
- 流体を供給するためのカニューレ(13)と、流体流量を生成するための流量機械(14、16)とをさらに備える、請求項1~3のいずれか一項に記載の補助システム。
- 前記電極が、前記補助システムの前記カニューレ(13)の外面の円周方向に配置される、請求項4に記載の補助システム。
- 前記電極(4、5,27,28)が、心臓の心室(20)領域に位置付けられた埋め込まれた状態にある、請求項1~5のいずれか一項に記載の補助システム。
- 流体体積流量(7)を決定するための装置をさらに備える、請求項1~6のいずれか一項に記載の補助システム。
- 前記処理ユニットは、前記第一のインピーダンスパラメータに基づいて第一の心室容積を決定し、前記第二のインピーダンスパラメータに基づいて第二の心室容積を決定するようにさらに構成されている、請求項1に記載の補助システム。
- 前記心室補助システム(2)は、前記心室補助システム(2)に流体を供給するためのカニューレ(13)を含み、前記カニューレは、入口開口部(12)を備え、前記少なくとも二つの電極(4、5)の第一の電極(4)は前記補助システム(2)の外面の前記入口開口部(12)の上流に配置され、前記少なくとも二つの電極(4、5)の第二の電極(5)は前記補助システム(2)の外面の前記入口開口部(12)の下流に配置されている、請求項1に記載の補助システム。
- 埋め込み可能な心室補助システム(2)であって、
第一の時点での第一のインピーダンスパラメータおよび第二の時点での第二のインピーダンスパラメータを決定するように構成された、測定装置(9)と、
前記第一のインピーダンスパラメータおよび前記第二のインピーダンスパラメータの間の変化を決定し、前記第一のインピーダンスパラメータおよび前記第二のインピーダンスパラメータの間の前記変化に基づいて心室容積の変化を決定するように構成された、処理ユニット(8)と、を備え、
前記測定装置(9)が、前記第一および第二のインピーダンスパラメータを決定するためのインピーダンスの測定に使用され得る少なくとも二つの電極(4、5)を備え、前記電極が前記補助システムの外面の円周方向に配置された心室インピーダンスを測定することを目的とすることを特徴とし、
前記心室補助システム(2)は、前記心室補助システム(2)に流体を供給するためのカニューレ(13)を含み、前記カニューレは、入口開口部(12)を備え、前記少なくとも二つの電極(4、5)の第一の電極(4)は前記補助システム(2)の外面の前記入口開口部(12)の上流に配置され、前記少なくとも二つの電極(4、5)の第二の電極(5)は前記補助システム(2)の外面の前記入口開口部(12)の下流に配置されている、心室補助システム。
