Description
上述したように、健常者においては、他動的な動的伸展相及び動的屈曲相の各ばね係数は近位側と遠位側とで異なる値をとる。そこで、健常者の動的伸展相及び動的屈曲相においてばね係数を近位側と遠位側とに二分するための最適な関節角度を求めた。
健常者20名(男性15名、女性5名、年齢55〜85歳)の両肘関節について、図1に示した筋トーヌス計測装置を用いて関節角度及び関節トルクを計測した。
上記の計測で得た520回分のデータ(生波形図)のそれぞれについて、以下の解析を行った。
結果を表1にまとめて示す。
動的伸展相及び動的屈曲相の関節角度−トルク変化曲線を実施例1で求めた分割角度60度で二分して得られる近位側及び遠位側のばね係数が、UPDRS筋強剛スコアとどの程度相関するかを調べた。
厚生省特定疾患神経変性疾患調査研究斑パーキンソン病診断基準(1995年)によりパーキンソン病と診断された患者24名(男性17名、女性7名、年齢47〜85歳)と前述の実施例1を行った健常者20名(男性15名、女性5名、年齢55〜85歳)に対して図1に示した筋トーヌス計測装置を用いて関節角度及び関節トルクを計測した。
上記の74データのそれぞれについて、以下の解析を行った。
UPDRS筋強剛スコアと、屈曲相における全範囲、近位、遠位の各ばね係数との関係を図5A、図5B、図5Cに示す。各図中の「r」は、UPDRS筋強剛スコアとの相関係数を意味する。UPDRS筋強剛スコアに対しては、全範囲ばね係数と遠位ばね係数とが同程度で良い相関を示した。また、いずれにおいてもUPDRS 0とUPDRS 1とで有意差は認められなかった。

Claims (7)
- 被験者の関節を他動的に屈曲伸展運動させるための関節トルク及び前記関節の関節角度を検出する検出部と、前記検出部からの出力信号を演算処理する演算部とを備え、
前記演算部は、伸展相及び屈曲相のうちの少なくとも一方における前記関節トルクと前記関節角度との関係を関節角度によって近位側部分及び遠位側部分を含む2以上の部分に分割し、前記遠位側部分及び前記近位側部分のうちの少なくとも一方での前記関節トルクと前記関節角度との関係から前記関節のばね係数を求めることを特徴とする筋トーヌス計測装置。 - 前記演算部は、前記伸展相における前記関節トルクと前記関節角度との関係を関節角度によって近位側部分及び遠位側部分を含む2以上の部分に分割し、前記遠位側部分での前記関節トルクと前記関節角度との関係から前記関節の伸展相遠位ばね係数を求める請求項1に記載の筋トーヌス計測装置。
- 前記演算部は、前記伸展相及び前記屈曲相のそれぞれにおける前記関節トルクと前記関節角度との関係を関節角度によって近位側部分及び遠位側部分を含む2以上の部分に分割し、
前記伸展相の前記近位側部分及び前記遠位側部分での前記関節トルクと前記関節角度との関係から前記関節の伸展相近位ばね係数及び伸展相遠位ばね係数と、
前記屈曲相の前記近位側部分及び前記遠位側部分での前記関節トルクと前記関節角度との関係から前記関節の屈曲相近位ばね係数及び屈曲相遠位ばね係数と、
前記伸展相における前記関節トルクと前記関節角度との関係から前記遠位側部分及び前記近位側部分を含む範囲での前記関節の伸展相全範囲ばね係数と、
前記屈曲相における前記関節トルクと前記関節角度との関係から前記遠位側部分及び前記近位側部分を含む範囲での前記関節の屈曲相全範囲ばね係数とを求める請求項1に記載の筋トーヌス計測装置。 - 前記近位側部分と前記遠位側部分との間の関節角度が59度以上63度以下の範囲内にある請求項1〜3のいずれかに記載の筋トーヌス計測装置。
- 前記演算部は、前記関節トルクと前記関節角度との関係を関節角度によって前記近位側部分と前記遠位側部分とに二分する請求項1〜4のいずれかに記載の筋トーヌス計測装置。
- 前記演算部は、屈曲相における前記関節トルクと伸展相における前記関節トルクとの差を複数の関節角度について算出し、それらを合計することによって得られるバイアス差の和を更に求める請求項1〜5のいずれかに記載の筋トーヌス計測装置。
- 前記関節が肘関節である請求項1〜6のいずれかに記載の筋トーヌス計測装置。
