Abstract

Cette prothèse articulée comprend au moins un élément fixe (12) destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile (4) apte à se déplacer par rapport au ou à chaque élément fixe, ainsi qu'au moins un élément intermédiaire (20) permettant l'articulation du ou de chaque élément mobile par rapport au ou à chaque élément fixe.Le ou chaque élément intermédiaire est reçu dans un volume intérieur de l'élément fixe, cet élément intermédiaire (20) possédant au moins un degré de liberté en rotation par rapport à cet élément fixe, alors que l'élément mobile (4) est reçu au moins partiellement dans un volume intérieur de l'élément intermédiaire (20), en possédant au moins un degré de liberté en rotation par rapport à cet élément intermédiaire, et il est prévu des moyens permettant la solidarisation en translation, dudit autre élément par rapport audit élément intermédiaire.

Description

La présente invention concerne une prothèse articulée.
L'invention vise plus particulièrement une telle prothèse qui, une fois implantée, relie deux parties du corps humain, entre lesquelles s'exercent des contraintes mécaniques. Il peut s'agir notamment, d'une part du cotyle de la hanche ou de l'épaule et, d'autre part, du canal médullaire du fémur ou
de l'humérus.
L'invention se propose de réaliser une prothèse articu-
lée, dont la structure est simple, dont le montage est aisé et qui est implantée de façon fiable dans les organes qu'elle relie. A cet effet, elle a pour objet une prothèse articulée, caractérisée en ce qu'elle comprend au moins un élément fixe destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile apte à se déplacer par rapport au ou à chaque élément fixe, ainsi qu'au moins un élément intermédiaire permettant l'articulation du ou de chaque élément mobile par rapport au ou à chaque élément fixe, en ce que le ou chaque élément intermédiaire est reçu dans un volume intérieur de l'élément fixe, cet élément intermédiaire possédant au moins un degré de liberté en rotation par rapport à cet élément fixe, des moyens étant prévus, pour permettre la solidarisation en translation dudit élément intermédiaire par rapport audit élément fixe, en ce que l'élément mobile est reçu au moins partiellement dans un volume intérieur de l'élément intermédiaire, en possédant au moins un degré de liberté en rotation par rapport à cet élément intermédiaire, et en ce qu'il est prévu des moyens permettant la solidarisation en translation dudit élément
mobile par rapport audit élément intermédiaire.
Selon d'autres caractéristiques de l'invention: - les moyens de solidarisation en translation dudit
élément intermédiaire par rapport audit élément fixe compren-
nent un pourtour, sensiblement rigide, du débouché du volume intérieur de cet élément fixe; - les moyens de solidarisation en translation dudit élément mobile par rapport audit élément intermédiaire comprennent un pourtour sensiblement rigide, du débouché du volume intérieur de cet élément intermédiaire; - l'élément intermédiaire possède une surface extérieure sphérique tronquée, et coopère avec une surface intérieure sphérique correspondante, dont est pourvu ledit élément fixe;
- l'élément fixe possède une échancrure d'intro-
duction de l'élément intermédiaire dans le volume intérieur de cet élément fixe; - l'élément mobile possède une tête sphérique, apte
à coopérer avec une surface intérieure sphérique correspon-
dante de l'élément intermédiaire; - la tête sphérique dudit élément mobile est pourvue d'un méplat équatorial d'introduction de cette tête dans le volume intérieur de l'élément intermédiaire; - l'élément intermédiaire est muni d'un organe de butée, notamment amovible, permettant de limiter le pivotement de l'élément mobile par rapport à cet élément intermédiaire; la prothèse forme une prothèse d'épaule ou de hanche, l'élément fixe étant destiné à être rendu solidaire du cotyle de l'épaule ou de la hanche, alors que l'élément mobile comporte une tige apte à être introduite dans le canal médullaire de l'humérus ou du fémur, ainsi qu'une tête prolongeant cette tige et apte à être introduite dans ledit
volume intérieur de l'élément intermédiaire.
- la figure 1 est une vue en coupe longitudinale, illustrant les différents éléments constitutifs d'une prothèse articulée conforme à l'invention; - les figures 2, 2A et 2B sont des vues analogues à la figure 1, illustrant trois étapes du montage d'un élément intermédiaire de la prothèse de la figure 1, dans le volume intérieur d'un élément fixe de cette prothèse; et - les figures 3 et 3A sont des vues analogues à la figure 1, illustrant l'introduction d'un élément mobile de la prothèse de la figure 1, dans le volume intérieur de l'élément
intermédiaire de cette prothèse.
La figure 1 illustre une prothèse articulée conforme à l'invention, qui est une prothèse d'épaule. A titre de variante, l'homme du métier pourra réaliser une prothèse conforme à l'invention, dont le type est différent, par
exemple une prothèse de hanche.
La prothèse 2 illustrée sur cette figure 1 comprend un élément mobile, désigné dans son ensemble par la référence 4, qui comporte une tige 6, de profil connu, qui est destinée à être introduite dans le canal médullaire de l'humérus non représenté. Cette tige 6 est prolongée par une collerette 7, ainsi que par une tête 8 sphérique de diamètre d, qui est
pourvue d'un méplat équatorial s'étendant de façon inclinée.
Ceci signifie que ce méplat n'est pas perpendiculaire à l'axe principal A de la collerette 7. La dimension transversale de ce méplat 10, qui est notée d', est inférieure au diamètre _ précité. La prothèse illustrée à la figure 1 comprend également un élément fixe 12, destiné à être solidarisé au cotyle non représenté de l'épaule. Cet élément fixe 12 possède une surface extérieure sphérique, ainsi qu'une surface intérieure sphérique, de diamètre D. Cette surface intérieure s'étend sur un secteur angulaire supérieur à 180 , de sorte que la dimension transversale L, correspondant au débouché 14 du volume intérieur 16 de l'élément 12, est inférieure au
diamètre D précité.
La paroi intérieure de l'élément fixe 12 peut en outre être creusée d'une échancrure non représentée, permettant
d'élargir localement la section transversale du logement 16.
Ceci garantit une introduction plus aisée d'un élément intermédiaire dans ce logement, comme cela est décrit dans ce
qui suit.
La prothèse comprend enfin un élément intermédiaire 20, dont la surface extérieure 22 définit une portion de sphère tronquée, de diamètre identique à celui D du logement 16. Cet élément intermédiaire possède une surface intérieure 24 également sphérique, dont le diamètre est égal à celui d de
la tête 8.
La surface intérieure 24 de l'élément intermédiaire 20 s'étend selon un secteur angulaire supérieur à 1800. De la sorte, la dimension transversale du débouché 26 du logement 28 de cet élément 20 est sensiblement égale à la dimension
transversale d' du méplat 10.
Le montage de la prothèse illustrée à la figure 1 va être
décrit maintenant en référence aux figures 2 et 3.
Il s'agit, dans un premier temps, d'introduire l'élément intermédiaire 20 dans le volume intérieur 16 de l'élément fixe 12. A cet effet, on dispose cet élément intermédiaire, de sorte que sa surface extérieure sphérique 22 se trouve au voisinage du débouché 14 du logement 16. Puis, on rapproche axialement cet élément intermédiaire de l'élément fixe, de manière à le faire pénétrer dans le logement 16, comme
représenté à la figure 2B.
On fait ensuite pivoter l'élément intermédiaire 20, de manière que sa surface extérieure 22 s'étende au voisinage des parois intérieures de l'élément fixe, comme le montre la figure 2C. Une fois ces opérations réalisées, l'élément intermédiaire 20 ne possède aucun degré de liberté en translation par rapport à l'élément fixe 12. En effet, la dimension transversale L du débouché 14 est inférieure au diamètre extérieur D de cet élément intermédiaire 20. En revanche, il existe trois degrés de liberté en rotation entre
l'élément intermédiaire et l'élément fixe.
Comme le montre la figure 3, il s'agit ensuite d'intro-
duire la tête sphérique 8 de l'élément mobile 4 dans le volume
intérieur 28 de l'élément intermédiaire 20.
A cet effet, on incline tout d'abord la tête 8, de sorte que le méplat 10 s'étende perpendiculairement à l'axe A. On rapproche alors axialement, selon cet axe A, l'élément mobile de l'élément intermédiaire. Etant donné que la dimension transversale d' du méplat 10 est sensiblement identique à celle du débouché 26, ceci permet l'introduction de la tête
8 dans le logement 28, comme illustré à la figure 3.
Puis, une fois la tête 8 en butée contre la surface intérieure 24 de l'élément intermédiaire, on fait pivoter l'élément mobile 4, de manière à lui conférer l'agencement illustré à la figure 3A. Etant donné que la dimension transversale d' du débouché 26 est inférieure au diamètre d de la tête 8 de l'élément mobile 4, cette dernière ne peut ressortir du volume intérieur 28, dans l'agencement de la
figure 3A.
Dans le cas o l'élément mobile 4 n'est pas libre de pivoter autour de l'axe A, il est possible de basculer l'élément intermédiaire 20 au sein du logement 16 de l'élément fixe 12. Puis, on rapproche axialement l'élément mobile 4 de l'élément intermédiaire ainsi basculé, afin que le méplat 10 coopère avec le pourtour du débouché 26, de façon analogue à
ce qui est illustré aux figures 3 et 3A.
Le pourtour du débouché 26 est sensiblement rigide. A cet effet, l'élément intermédiaire peut être réalisé entièrement en un matériau rigide, notamment métallique. A titre de variante, cet élément intermédiaire peur être réalisé en un matériau déformable, tel que du polyéthylène, une bague rigide
étant alors rapportée au voisinage du débouché précité.
Une fois réalisé l'agencement de la figure 3A, on peut rapporter sur le méplat 10 un moyen de butée avantageusement amovible, telle une vis 30. Cette dernière, en limitant le pivotement de la tête 8 par rapport à l'élément intermédiaire 20, empêche la tête de recouvrer sa position de la figure 3, ce qui évite toute désolidarisation intempestive entre cet
élément intermédiaire 20 et cet élément mobile 4.
L'invention permet de réaliser les objectifs précédemment mentionnés. En effet, les différents éléments constitutifs de la prothèse articulée de l'invention possèdent une structure
relativement simple.
L'assemblage de ces éléments est particulièrement aisé et peut être réalisé par un utilisateur, sans que ce dernier
n'ait à mettre en oeuvre une force physique importante.
Le montage de ces différents éléments n'induit par ailleurs quasiment aucune déformation de ceux-ci, ce qui est
avantageux en termes de fiabilité mécanique.
Il est à noter que la présence de l'élément intermédiaire autorise le montage mutuel des éléments fixes et mobiles, quand bien même il n'existe pratiquement aucun débattement en
rotation entre ces deux éléments.
Enfin, une fois la prothèse articulée de l'invention mise en place, elle possède une résistance élevée à l'égard des contraintes mécaniques, s'exerçant notamment en traction. En effet, la présence d'éléments intermédiaires permet de ne transmettre que dans une très faible mesure les éventuels efforts auxquels est soumise la prothèse articulée de l'invention.

Claims (9)

REVEND I CATIONS
1. Prothèse articulée, caractérisée en ce qu'elle comprend au moins un élément fixe (12) destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile (4) apte à se déplacer par rapport au ou à chaque élément fixe, ainsi qu'au moins un élément intermédiaire (20) permettant l'articulation du ou de chaque élément mobile par rapport au ou à chaque élément fixe, en ce que le ou chaque élément intermédiaire est reçu dans un volume intérieur (16) de l'élément fixe, cet élément intermédiaire (20) possédant au moins un degré de liberté en rotation par rapport à cet élément fixe, des moyens étant prévus, pour permettre la solidarisation en translation dudit élément intermédiaire (20) par rapport audit élément fixe (12), en ce que l'élément mobile est reçu au moins partiellement dans un volume intérieur (28) de l'élément intermédiaire (20), en possédant au moins un degré de liberté en rotation par rapport à cet élément intermédiaire, et en ce qu'il est prévu des moyens permettant la solidarisation en translation dudit élément mobile (4) par rapport audit élément intermédiaire (20).
2. Prothèse articulée selon la revendication 1, caracté-
risée en ce que les moyens de solidarisation en translation dudit élément intermédiaire (20) par rapport audit élément fixe (12) comprennent un pourtour, sensiblement rigide, du
débouché (14) du volume intérieur (16) de cet élément fixe.
3. Prothèse articulée selon l'une des revendications 1
ou 2, caractérisée en ce que les moyens de solidarisation en translation dudit élément mobile (4) par rapport audit élément intermédiaire (20) comprennent un pourtour sensiblement rigide, du débouché (26) du volume intérieur (28) de cet
élément intermédiaire.
4. Prothèse articulée selon l'une des revendications 1
à 3, caractérisée en ce que ledit élément intermédiaire (20) possède une surface extérieure (22) sphérique tronquée, et coopère avec une surface intérieure sphérique correspondante,
dont est pourvu ledit élément fixe (12).
5. Prothèse articulée selon la revendication 4, caracté-
risée en ce que ledit élément fixe possède une échancrure d'introduction de l'élément intermédiaire dans le volume intérieur de cet élément fixe.
6. Prothèse articulée selon l'une des revendications
précédentes, caractérisée en ce que ledit élément mobile (4) possède une tête sphérique (8), apte à coopérer avec une surface intérieure (24) sphérique correspondante de l'élément
intermédiaire (20).
7. Prothèse articulée selon la revendication 6, caracté-
risée en ce que la tête sphérique (8) dudit élément mobile (4) est pourvue d'un méplat équatorial (10) d'introduction de
cette tête (8) dans le volume intérieur de l'élément intermé-
diaire (20).
8. Prothèse articulée selon l'une des revendications
précédentes, caractérisée en ce que l'élément intermédiaire (20) est muni d'un organe de butée (30), notamment amovible, permettant de limiter le pivotement de l'élément mobile (4)
par rapport à cet élément intermédiaire.
9. Prothèse articulée selon l'une quelconque des
revendications précédentes, caractérisée en ce qu'elle forme
une prothèse d'épaule ou de hanche, l'élément fixe (12) étant destiné à être rendu solidaire du cotyle de l'épaule ou de la hanche, alors que l'élément mobile (4) comporte une tige (6) apte à être introduite dans le canal médullaire de l'humérus ou du fémur, ainsi qu'une tête (8) prolongeant cette tige et apte à être introduite dans ledit volume intérieur (28) de l'élément intermédiaire (20)
