Prothese für den Ersatz eines Gelenks Beschreibung
Die Erfindung betrifft eine Prothese für den Ersatz eines Gelenks mit Bandlängenausgleich, vorzugsweise für ein
Fingergelenk oder Zehengelenk, bestehend aus zwei mittels eines Gelenks miteinander verbundenen Teilen, die jeweils einen Schaft zur Verankerung in den angrenzenden Knochen aufweisen.

Aus dem deutschen Gebrauchsmuster G 92 16 202.9 ist eine Finger-Mittelgelenkprothese bekannt, die aus zwei gelenkig miteinander verbundenen Teilen besteht, an deren gegenüberliegenden freien Enden jeweils ein Schaft zum Einsetzen in einen Fingerknochen sich befindet und die Teile mittels Scharniergelenke verbunden sind. Dieses Scharniergelenk besteht aus einer Gabel mit zwei beabstandeten Ansätzen mit Bohrungen und einem zweiten Teil, ebenfalls mit einer Bohrung, der in den Zwischenraum der Gabel einsetzbar ist und mittels eines Stiftes über die fluchtenden Bohrungen verbunden werden kann. Nachteilig an dieser Erfindung ist u.a., daß die einzusetzende Prothese nach der Resektion an den Knochen genau die Länge des Bandapparates aufweisen muß, da die Operation sonst nicht ausreichend befriedigend verläuft.
Es gibt bei diesem Stand der Technik nur unbefriedigende Möglickeiten während der Operation auf die Länge des Bandapparates einzugehen.
Der Erfindung liegt deshalb die Aufgabe zu Grunde, eine Prothese für den Ersatz eines Gelenks zu schaffen, die einfacher bei der Anpassung des Implantates an den vorhandenen Bandapparat und seinen unterschiedlichen Längen zu handhaben ist und gleichzeitig die natürliche Gelenkkinematik weitestgehend wiederherstellt. 15
Weiterhin ist es Aufgabe der Erfindung bei Erreichen der Endpunkte der Beweglichkeit einen weichen Anschlag zu erhalten.
Die Aufgabe wird bei der Prothese für den Ersatz eines Gelenks erfingungsgemäß dadurch gelöst, daß die beiden in den Knochen zu verankernden Teile je ein Gelenkteil mit kugelförmig ausgearbeiteten Segment als Gelenkpfannen besitzen, in denen ein kugelförmiges Verbindungselement mit zwei kugelförmigen Segmenten als Gelenkköpfe beweglich gelagert ist und so die beiden Gelenkteile über das Verbindungselement verbunden werden. Das kugelförmige Verbindungselement besteht aus zwei miteinander über eine Scheibe verbundene Gelenkköpfe.
Das erfindungsgemäße Gelenk bietet den Vorteil, daß es unter den verhältnismäßig schwierigen Operationsbedingungen eine einfache und relativ schnelle gelenkige Verbindung gibt. Während der Operation werden zunächst einzeln in den

angrenzenden resezierten Knochen die Teile fixiert und verankert und anschließend über ein kugelförmiges Verbindungselement gelenkig miteinander verbunden. Durch die Auswahl eines in ihren Abmessungen passenden Verbindungselementes wird dabei die natürliche Gelenkkinematik weitestgehend wieder hergestellt. Dieses wird auf die besonders einfache Art erreicht, in dem bei der Anpassung des Implantates an den vorhandenen Bandapparat die unterschiedlichen Längen durch ein Verbindungselement mit einer passenden Scheibendicke zwischen den beiden Gelenkköpfen als Distanzstück, ausgewählt wird. 15
In einer vorteilhaften Ausgestaltung sind die Gelenkteile an den Schäften so ausgebildet, daß in den Flächen der Gelenkteile, die gegenüber den Schäften liegen, jeweils ein kugelförmiges Segment als Gelenkpfanne zur Aufnahme des kugelförmigen Verbindungselementes mit seinen beiden Gelenkköpfen vorhanden ist. Damit die beiden zu verbindenden Teile auch beweglich sind, sind die Flächen an den Gelenkteilen, die gegenüber den Schäften für die Verankerung in den resezierten Knochen liegen, abgewinkelt. Die Abwinkelung erfolgt möglichst im Drehpunkt der sich gegenüberliegenden Teile in einem Winkel entsprechend dem Anwendungsgebiet.
Bei den erfindungsgemäßen Ausführungen handelt es sich dabei bevorzugt um einen Winkel von bis zu 45° in der Fläche in Richtung der Schäfte der jeweiligen Teile. Als besonders vorteilhaft hat sich dabei eine Neigung von 35° bis 45° erwiesen, so daß das Gelenk insgesamt um einen Winkel bis zu 90°, bevorzugt bis zu 74° aus der Axialen in einer Richtung ausgelenkt werden kann.
AIs vorteilhaft hat sich für ein nach der Operation verträgliches Implantat erwiesen, wenn die Flächen an den Gelenkteilen, die an der Seite der Schäfte angeordnet sind, einen Winkel von bis zu 40°, bevorzugt 20° bis 30° zu der den Schäften abgewandten Seite hin aufweisen.
Das erfindungsgemäß zur Anwendung kommende kugelförmige Verbindungselement besteht aus mehreren miteinander verbundenen Teilen. Jeweils ein kugelförmig gearbeitete s Kugelsegment befindet sich mittig als Gelenkkopf auf den Flächen einer Scheibe, deren Dicke den Erfordernissen angepaßt werden kann. Diese Anordnung kann auf bekannter Art und Weise fest miteinander verbunden werden oder es hat sich als besonders vorteilhaft gezeigt, wenn das Verbindungselement bestehend aus Scheibe und den beiden Gelenkköpfen aus einem Stück gefertigt ist.
In einer bevorzugten Ausführungsform ist am Umfang der Scheibe eine Vertiefung eingearbeitet. Es hat sich als vorteilhaft erwiesen, wenn die Vertiefung in Form einer Rille vorhanden ist, die nach Möglichkeit umlaufend sein sollte. Die Rille am Umfang der Scheibe des Verbindungselementes kann einen Röntgenkontrastring aufnehmen.
Das kugelförmige Verbindungelement besitzt durch die spezielle Gestaltung zwei Funktionen, die es in sich vereinigt. Es dient als Reib- und Kugelgelenkteil auf der einen Seite und bewirkt durch die Zwischenscheibe mit variabler Dicke, daß der Operateur nach der Resektion der beiden Knochenenden auf die Länge des Bandapparates reagieren kann und entsprechend der Länge des Bandapparates ein

passendes Verbindungselement mit der erforderlichen Scheibendicke auswählen kann, um damit die Bandspannung zu beinflussen.
Der Anschlag der Anschlagflächen bei Erreichen der Endpunkte der Winkelbewegung auf die Flächen der Scheibe erfolgt durch die erfindungsgemäße Lösung weich, da die Scheibe des Verbindungselementes vorteilhafterweise aus einem Kunststoff gefertigt wird und somit eine Berührung der Flächen der sich gegenüberliegenden Gelenkteile, die bevorzugt aus einem harten gut implantierbaren und verträglichen Material gefertigt wurden, ausgeschlossen ist.
Die Schäfte der Teile sind vorteilhafterweise mit Schaftvertiefungen versehen. Dieses hat den Vorteil, daß die Lage der Prothese sich genau fixieren und befestigen läßt.
Damit ist eine spätere Drehung der Prothese nahezu ausgeschlossen.
In einer bevorzugten Ausführung, werden die Schaftvertiefungen in Form von Nuten in der Längsrichtung der Schäfte eingearbeitet. Besonders vorteilhaft ist es, wenn mehrere Nuten in einen Schaft eingebracht werden. Dabei kann die Anordnung beliebig erfolgen.
Es hat sich auch von Vorteil gezeigt, wenn die Schäfte, die in den Knochen eingelassen und befestigt werden eine Konizität aufweisen. Besonders vorteilhaft ist es, wenn der Anstieg vom Ende des Schaftes zum Gelenkteil dabei 3° bis 4° beträgt, aber auch andere Winkel sind möglich. Durch die vorhandene Konizität wird bei der einzusetzenden Prothese in die resezierten Knochen ein fester Preßsitz erreicht.

Die Schaftlänge an den beiden Teilen sollte aufgrund der anatomischen Gegebenheiten der resezierten Knochen nach Möglichkeit unterschiedlich sein.
Während bei dem einen Schaft als bevorzugte Länge sich der 3-fache Durchmesser vom größten Durchmesser des Schaftes bewährt hat, sollte der Schaft des anderen dazugehörigen Teiles eine Länge von dem 1,2 bis 1,4-fachen der Länge des anderen Schaftes aufweisen. Aber auch andere Abmessungen sind möglich.
Um bei der Operation an die konkret vorliegenden anatomischen Verhältnisse der Gelenke und Personen die Prothese anzupassen, werden alle Maße im gleichen Verhältnis vergrößert oder verkleinert.
Die erfindungsgemäße Prothese sollte aus Metall, Kunststoff oder Keramik bestehen. Als bevorzugtes Material kommt für die Teile Ti6A14V zum Einsatz. Aber auch andere implantierbare Metalle mit guter Biokompatibilität sind möglich. Die Oberflächen dieser Implantate können durch Titan-Niob-Beschichtungen, Reinsttitan-Plasmabeschichtungen mit extra rauher Oberflächenstruktur oder mit einer Hydroxy-Apatitbeschichtung, die besonders gewebeverträglich sind, ausgestattet sein. Die Ausführung läßt auch generell unbeschichtete Teile zu.
Das Verbindungselement sollte vorteilhafterweise aus einem Kunststoff gefertigt werden. Als besonders vorteilhaft hat sich dabei Polyethylen erwiesen, insbesondere das Vollmaterial UHMWPe (Chirulen), aber auch andere Materialien sind möglich.
Weitere Vorteile, Einzelheiten und erfindungswesentliche Merkmale ergeben sich unter Bezugnahme auf die beigefügten Zeichnungen.

Nach der Figur 1 besteht die Prothese aus einem ersten Teil 1 und einem zweiten Teil 2 mit jeweils einem sich gegenüberliegenden Gelenkteil 5,6.
An den sich gegenüberliegenden Enden der Gelenkteile 5 und befindet sich jeweils ein Schaft 3,4, der nach außen gerichtet ist.
Die Fläche 12 am Gelenkteil 5 und die Fläche 13 am Gelenkteil 6 befinden sich jeweils an der gegenüberliegenden Seite der jeweiligen Schäfte 3,4. Die Flächen 12 und 13 sind dabei so angeordnet, daß sie bis zur Achse 14 in einem Winkel von 90° verlaufen und die Fläche 12 in der Höhe der Achse 14 in einem Winkel von 40° zum Schaft 3 wegknickt und die Fläche 13 am Gelenkteil 6 in der gleichen Weise wegknickt, jedoch nur in Richtung des Schaftes 4.
In die Gelenkteile 5,6 ist jeweils mittig in den Flächen 12 und 13 eine Gelenkpfanne 7,8 fluchtend zu den Schäften 3 und 4 eingearbeitet.
Nach dem Einsetzen der Schäfte 3 und 4 in die Knochen wird das kugelförmige Verbindungsstück 9 in den Zwischenraum in
die Gelenkpfannen 7 und 8 der beiden Gelenkteile 5 und 6 eingesetzt. Dabei bilden die Oberflächen der Gelenkköpfe 20 und 21 mit den Oberflächen der jeweiligen Gelenkpfannen 7 und 8 ein Reib- und Kugelgelenklager. Durch die Scheibe 10, die sich mittig zwischen den beiden Gelenkköpfen 20 und 21 befindet, erfolgt der Anschlag der Flächen 12 und 13 nur gegen die Flächen des Scheibe 10.
Als besonders vorteilhaft hat es sich dabei erwiesen, wenn die Scheibe 10 aus einem Kunststoff besteht, der eine leichte federnde Wirkung besitzt, so daß der Anschlag der Flächen am Ende der Bewegung weich erfolgt.
Da die Scheibe 10 eine unterschiedliche Dicke aufweisen kann, ohne die Funktion des Gelenks zu beeinflussen, besteht die Möglichkeit durch Auswahl der Dicke der Scheibe 10 ein entsprechend der Länge des Bandapparates erforderliche Prothese zu erhalten.
In der dargestellten Figur ist der Umfang der Scheibe 10 umlaufend mit einer Vertiefung 11 versehen. Diese Vertiefung kann zur Aufnahme eines Röntgenkontrastmitteis dienen.
In der in der Figur 1 dargestellten Prothese sind die Flächen 15 und 16 an den Gelenkteilen in einem Winkel von 24° aus der Senkrechten gegenüber der Achse 14 von den Schäften 3 und 4 weg geneigt.
Die Schäfte 3 und 4 weisen jeweils eine Konizität auf, die beim Einsatz in die Knochen einen ausreichenden festen Sitz ermöglichen.
In der Figur 2 ist die erfindungsgemäße Gelenkprothese in der Draufsicht dargestellt. Deutlich ist der konische Schaft 3,4 und das Gelenkteil 5,6 zu erkennen. Dieser Schaft 3,4 besitzt in dem Ausführungsbeispiel drei Schaftvertiefungen 17, 18 und 19.'
In diesem Fall sind diese Schaftvertiefungen vorteihafter in Form einer Nut in Längsrichtung eingearbeitet, wobei die drei Schaftvertiefungen in gleichmäßigen Abstanden um den Schaft angeordnet sind.
In der Figur 3 ist eine Prothese in einem eingebauten Zustand dargestellt. Deutlich sind die beiden Teile mit den Gelenkteilen 5 und 6, die auf den resezierten Knochen 22 und 23 sitzen und durch die Schäfte 3 und 4 in diese fixiert und fest verankert wurden, zu erkennen.
Da in dieser Figur das Gelenk in leicht gebeugter Haltung dargestellt ist, wird das Zusammenwirken der Gelenkteile 5 und 6 und des kugelförmigen Verbindungselementes mit seiner Scheibe 10 besonders deutlich.
Durch den leicht geöffneten Zustand des Gelenkes in der Figur 3, sind die Anschlagflächen 12 und 13 und die Flächen der Scheibe 10 nicht aufliegend.

Claims (24)

Anwaltsakte 0302-OST OHST U SOHN GbR Falkenweg D-14712 Rathenow Prothese für den Ersatz eines Gelenks Schutzansprüche
1. Prothese für den Ersatz eines Gelenks bestend aus zwei mittels eines Gelenks miteinanderverbundenen Teilen, die jeweils einen Schaft zur Verankerung in den angrenzenden Knochen und ein Verbindungselement aufweisen, dadurch gekennzeichnet, daß der erste Teil (1) und der zweite Teil (2) aus je einem Schaft (3,4) und einem Gelenkteil (5, 6) bestehen, wobei in den Gelenkteilen (5,6) auf der den Schäften (3,4) abgewandten Seiten jeweils ein kugelförmiges Segment als Gelenkpfanne (7,8) zur Aufnahme eines kugelförmigen Verbindungselementes (9) mit einer Scheibe (10) zur beweglichen Verbindung der Teile (1,2) angeordnet ist.
2. Prothese nach Anspruch 1, dadurch gekennzeichnet, daß die Anschlagflächen (12,13) der Gelenkteile (5,6) an ihren den Schäften (3,4) abgewandten Seiten bis zur Achse (14) in einem Winkel von 90° zur Achse (14) angeordnet und danach abgewinkelt sind.

3. Prothese nach Anspruch 1,
dadurch gekennzeichnet, daß der Bewegungswinkel der Teile 1 und 2, die über das kugelförmige Verbindungselement (10) verbunden sind, bis 90°, bevorzugt bis 74° aus der Axialen beträgt. 10
4. Prothese nach Anspruch 2 , dadurch gekennzeichnet,
daß die Abwinklung der Anschlagflächen (12,13) bis betragen.

5. Prothese nach Anspruch 1 , dadurch gekennzeichnet, daß die Gelenkteile (5,6) an der den Schäften (3,4) zugewandten Flächen (15,16) einen Winkel aus der rechtwinkligen Ebene zur Achse (14) von bis zu 40°, bevorzugt 20° bis 30° zu der den Schäften (3,4) abgewandten Seite hin aufweisen.
6. Prothese nach Anspruch 1 , dadurch gekennzeichnet, daß das kugelförmige Verbindungselement (9) aus zwei Gelenkköpfen (20,21) und einer dazwischen angeordneten Scheibe (10) besteht.
7. Prothese nach Anspruch 1 und 6 , dadurch gekennzeichnet, daß das kugelförmige Verbindungselement (9) aus mehreren miteinander verbundenen Teilen, bevorzugt aus einem Teil besteht.

8. Prothese nach Anspruch 1 und 6,
dadurch gekennzeichnet, daß die Dicke des Scheibe (10) variabel ist.
9. Prothese nach Anspruch 1 und 6, dadurch gekennzeichnet, daß am Umfang der Scheibe (10) eine Vertiefung (11), bevorzugt eine Rille vorhanden ist.
10. Prothese nach Anspruch 9, dadurch gekennzeichnet, daß die Vertiefung (11) umlaufend ist.
11. Prothese nach Anspruch 6,9 und 10, dadurch gekennzeichnet, daß die Vertiefung (11) am Umfang der Scheibe (10) einen Röntgenkontrastring aufnimmt.
12. Prothese nach Anspruch 1, 6 und 7, dadurch gekennzeichnet, daß das kugelförmige Verbindungselement (10) als Reib- und Gelenkteil ausgebildet ist.
13. Prothese nach Anspruch 1-4 und 6-8, dadurch gekennzeichnet, daß der Anschlag der Anschlagflächen (12,13) an die Flächen der Zwischenscheibe (10) nach einer Bewegung weich ist.
14. Prothese nach Anspruch 1 , dadurch gekennzeichnet, daß die Schäfte (3,4) mindestens jeweils eine Schaftvertiefung (17, 18, 19) aufweisen.

15. Prothese nach Anspruch 1 und 14,
dadurch gekennzeichnet, daß die Anordnung der Schaftvertiefungen {17, 18,19) an den Schäften {3,4) beliebig ist.
16. Prothese nach Anspruch I1 14 und 15
dadurch gekennzeichnet, daß die Anordnung der Schaftvertiefungen (17,18,19) bevorzugt in Form von Nuten in Längsrichtung und/oder in gleichmäßigen Abständen ist.
17. Prothese nach Anspruch 1 und 14 - 16, dadurch gekennzeichnet, daß die Schäfte {3,4) rund oder oval und zylindrisch oder konisch sind.
18. Prothese nach Anspruch 1 und 17 , dadurch gekennzeichnet, daß die Konizität der Schäfte (3,4) bevorzugt einen Anstieg von 3° bis 4° aufweisen
19. Prothese nach Anspruch 1 , dadurch gekennzeichnet, daß der Schaft {4} die dreifache Länge des größten Durchmessers des Schaftes besitzt und der Schaft (3) bevorzugt die 1,2 bis 1,4-fache Länge des anderen Schaftes (4) aufweist.
20. Prothese nach Anspruch 1 - 19 , dadurch gekennzeichnet, daß das Implantat aus einem Metall, Kunststoff oder Keramik besteht.
21. Prothese nach Anspruch 1 - 20 , dadurch gekennzeichnet, daß die Prothese aus einem anderen Material mit guter Biokompatibilität, bevorzugt aus dem Material Ti6A14V besteht.
22. Prothese nach Anspruch 1 - 21, dadurch gekennzeichnet,
daß die Oberfläche eine Titan-Niob-Beschichtung oder eine Reinsttitan-Plasmabeschichtung mit extra rauher Oberfläche oder eine Hydroxy-Apatitbeschichtung erhält. 15
23. Prothese nach Anspruch 1, 6-10 und 12 ,
dadurch gekennzeichnet, daß das Verbindungselement (9) aus einem Kunststoff, bevorzugt aus Polyethylen, insbesondere aus dem Vollmaterial UHMWPe (Chirulen) besteht.
24. Prothese nach Anspruch 1, dadurch gekennzeichnet, daß die Prothese als Scharniergelenk, bevorzugt als Fingergelenk oder Zehengelenk, insbesondere als Mittelgelenk eines Fingers angewendet wird.
