Abstract

L'invention concerne une prothèse de hanche, de type modulaire, prévue pour être adaptée en fonction des caractéristiques géométriques particulières de chaque articulation à opérer. La prothèse comporte une tige 1 de longueur appropriée, dont la partie supérieure comprend un logement pour un collet 14 conçu pour recevoir une tête sphérique, et un cotyle coopérant avec la tige et pouvant être orienté ou ancré dans l'acétabule.

Description

PROTHESE DE HANCHE MODULAIRE.
La présente invention concerne une prothèse de hanche. de type modulaire. pouvant être utilisée indifféremment en fonction des conditions opératoires et de la forme spécifique de l'artlculatson des patients.
En cas de fracture. d'arthrite ou autre affection touchant les hanches et leurs articulations avec le fémur, on utilise habituellement des prothèses de hanches pour permettre au patient de conserver sa mobilité.
D'une manière connue. ces prothèses sont formées d'une pièce et comportent une tige surmontée d'un collet sur lequel est disposée une tête sphérique de diamètre approprié.
Ces prothèses présentent l'inconvénient de ne pouvoir être utilisées indifféremment quel que soit le patient. à moins d'avoir des dimensions adaptées à la structure osseuse à opérer.
En effet. même s les dimensions de la prothèse sont adaptées au patient, la courbure de la tête n'est pas toujours convenable et le collet peut être trop long ou trop petit. ce qui empêche de réaliser une articulation satisfaisante.
Dans ce cas, la tête de la prothèse ne s'adapte pas de manière satisfaisante avec l'acétabule du bassin avec laquelle doit coopérer ladite prothèse.
La présente invention se propose d'éliminer les inconvénients mentionnés ci-dessus en réalisant une prothèse de hanche comportant un collet de longueur variable selon les caractéristiques physiques propres à chaque patient.
Dans ce but. la présente invention se propose de réaliser une prothèse de hanche pouvant être reliée directement au fémur ou fixée à l'os.
n autre but de l'invention est de réaliser une prothèse de hanche comportant un collet dont l'axe peut suivre une direction quelconque, même une ligne brisée.
Les buts mentionnés ci-dessus sont atteints par une prothèse de hanche de type modulaire. caractérisée en ce qu'elle comprend pour l'essentiel une tige de longueur appropriée comportant dans sa partie supérieure un logement pour un collet conçu pour recevoir une tête sphérique, et un cotyle coopérant avec la tige et pouvant être cimenté ou ancré dans l'acetabu le.
D'autres caractéristiques et avantages de la prothèse de hanche modulaire selon la présente invention ressortiront mieux de la description donnée ci-après d'un mode de réalisation préférentiel, et en référence au dessin annexé dans lequel

En se référant aux figures annexées et J us particulièrement aux figures 1 à 5, la prothèse de hanche de type modulaire selon la présente invention comporte pour l'ressentie une tige I pourvue d'une partie allongée 2 cylindrique et légèrement conique vers le bas, et d'une partie supérieure recourbée 3.
La partie recourbée 3 présente en section deux grands côtés 4 aplatis et deux côtés courbés 5 et 6 convexes. Bien qu'elle n'en comporte pas habituellement, la tige I peut recevoir un collier d'appui 7.
D'un point de vue pratique, cette structure permet au chirurgien de décider au cours de l'opération, si va appuyer la prothèse sur l'os ou l'insérer dans la cavité fémorale et la fixer au niveau de sa partie conique.
Le collet de la prothèse, qui s'étenc sur environ deux millimetres, comporte des cavités S dans lesquelles sont logées des goupilles 9 orientées vers te haut à partir d'un collier de dimension plus grande lr# pouvant être appuyé sur l'os.
Bien entendu, la tige I est de dimension différente selon le patient, de manière à pouvoir être fixée ou insérée en contact direct avec l'os.
Dans ce dernier cas, la partie supérieure de la tige reçoit un revêtement approprié, en alumine ou en titane, afin de favoriser le contact entre la prothèse et l'os du patient. D'une manière avantageuse, ce revêtement peut être réalisé dans une matière comportant des particules sphériques afin d'augmenter la surface de contact entre l'os et la prothèse.
La partie supérieure de la tige 1 comporte une cavité Il éliptique, et un trou borgne 12 formant logement dans la partie centrale de la cavité 11.
Dans ce logement est ancrée l'une des extrémités 13 d'un collet 14, dont l'autre extrémité 15 est conçue pour être reliée à un cotyle approprié 16 prévus pour être engagés dans l'acétabule du bassin. On remarquera a ce sujet que la longueur du collet 18 peut varier en fonction des exigences spécifiques d'utilisation de la prothèse.
De même, l'inclinaison de l'axe dudit collet par rapport à son extrémité ancrée dans le logement 11 de la tige 1 peut varier, l'axe de ladite extrémité coïncidant bien entendu avec celui dudit logement. L'inclinaison peut être quelconque dans le plan correspondant à l'axe du logement Il.
En particulier, ains#i que cela est représenté à la figure 5. le collet peut comporter une extrémité d'insertion qui est décalée par rapport à son axe longitudinal.
On peut ainsi obtenir une prothèse comportant un collet qui prolonge sensiblement la courbure de la partie supérieure de la tige 1.
En se référant également aux figures 6 à 10, on remarquera en outre que le cotyle 16, qui forme partie intégrante de la prothèse, peut être réalisé selon deux variantes différentes, en vue d'être, soit ancré dans l'acétabule par cimentation ou simplement inséré par forcement.
Plus précisément, on prévoit un premier type de cotyle 17 comportant une demi-sphère en polyéthylène ou similaire 1S, logé dans une coquille 19 en titane de configuration identique. Ladite coquille comporte deux séries d'ergots 20 disposés en correspondance de l'extrémité ouverte de la coquille,-dans un plan parallèle à celui de ladite extrémité. Les ergots 20 ont pour fonction de comprimer le ciment disposé entre le cotyle et l'acétabule pour empêcher ledit cotyle de sortir dudit acétabule et ainsi assurer une fixation optimale de ces deux parties.
Selon une autre forme de réalisation, on peut utiliser un second cotyle 21, de forme également demi-sphérique et pouvant être fixé dans l'acétabule sans qu'il soit nécessaire d'utiliser des ciments.
Ce second cotyle est réalisé en titane et comporte une série d'orifices radiaux 22 et des ergots 23 sur sa circonférence. Les orifices radiaux 22 ont pour fonction d'assurer l'élasticlté du cotyle, les ergots 23 étant destinés à assurer une fixation optimale du cotyle dans l'acétabule.
A l'intérieur du cotyle 21. on a inséré un élément en polyéthylène (de préférence du type RCH 15 ) comportant une partie façonnée en forme de calotte sphérique 24 et une partie annulaire 25 légèrement conique.
Dans la pratique, la conicité de ladite partie annulaire permet d'assurer une liaison stable et parfaite entre la surface extérieure en titane et l'élément Intérieur en polyéthylène.
En outre, la liaison entre les parties mentionnées ci-dessus est renforçee par la présence d'une série de dents 26 formées sur la circonférence du cotyle dans des plans parallèles à celui définissant l'ouverture dudit cotyle.
La description qui précède et les figures annexees permettent de remarquer le caractère fonctionnel et la facilité d'utilisation qui caractérisent la prothèse de hanches de type modulaire selon la présente invention.
Cette prothèse a été décrite et illustrée en se référant à des modes de réalisation préférentiels mais elle peut subir de nombreuses modifications et variations évidentes pour l'homme de l'art sans que l'on sorte du cadre de l'invention.

Claims (7)

Rex endl-ations
1. Prothèse de hanche modulaire caracterisée en ce qu'elle comporte une tige (I) de longueur appropriée, comportant dans sa partie supérieure (3) un logement (I1) pour recevoir un collet (14) prévu pour être fixé à une tête spherique, et un cotyle coopérant avec ladite tige et conçu pour être cimente ou ancré dans l'acétabule.
2. Prothèse selon la revendication 1, caractérisé en ce que la tige (1) comporte une partie allongée (2) cylindrique et légèrement conique vers le bas. et une partie supérieure (3) recourbée, ladite partie supérieure présentant en section deux côtés (4) aplatis et deux côtés courbés (5.6) conx exes.

Prothèse selon la revendication 1 ou 2, caractérisée en ce que la tige (I) comporte un collet s'étendant sur environ deux millimètres à partir de la partie supérieure (3), ledit collet présentant des cavités (8) dans lesquelles sont logées des goupilles (9) orientées vers le haut à partir d'un collet (10) de dimensions plus importantes pouvant être appuyée sur l'os.

Prothèse selon l'une quelconque des revendications précédentes caractérisée en ce que la tige (1) est de dimension différente selon les patients. de manière à pouvoir être cimentée ou insérée directement sur l'os des patients, la partie supérieure de ladite tige recevant alors un revêtement approprié en alumine ou en titane, afin d'améliorer le contact entre la prothèse et l'os, ledit revêtement étant réalisé dans une matière comportant des particules sphériques pour augmenter la surface de contact entre la prothèse et l'os.
5.Prothèse selon l'une quelconque des revendications précédentes, caractérisée en ce que la partie supérieure (3) de la tige (1) comporte une cavité (Il) sensiblement élliptique, un trou borgne (12) formant logement étant ménagé dans la partie centrale de ladite cavité, l'une des extrémités (13) d'un collet (14) étant ancrée dans ledit logement et l'autre extrémité (15) dudit collet étant conçue pour être reliée à une tête sphérique prévue pour coopérer avec un cotyle (16) pc:#vant être fixé dans l'acétabule du bassin.
6. Prothèse selon l'une quelconque des revendications recedentes caractérisée en ce que la longueur du collet (14) peut varier en fonction des exigences particulières d'utilisation de la prothèse. l'axe dudit collet pouvant avoir une inclinaison différente de celle de son extrémité
Insérée dans le logement (Il) de la tige (1).

Prothèse selon la rexendication 6, caractérisée en ce que le cotyle (17) de la prothèse cl porte une partie (18) en forme de den##-sphêre en polyéthylène ou similaire. logée dans une coquille (191 de forme identique en titane, ladite coquille comportant deux séries d'ergots (20) disposés en correspondance de l'extrémité ouverte de la coquille. le long d'un plan parallèle à celui de ladite extrémité.
S. Prothèse selon la rexendication 6, caractérisée en ce que le cotyle (21) de la prothèse est réalisé en titane et comporte une série d'orifices radiaux (22) et d'ergots (23) sur sa circonférence, un élément en polyéthylène étant inséré à l'intérieur dudit cotyle, ledit élément comportant une partie façonnée en forme de calotte sphérique (24) et une partie annulaire (25) légèrement conique. ladite partie annulaire présentant en outre une suite de dents (26) formées sur sa circonférence, d'un des plans parallèles à celui définissant l'ouverture dudit cotyle.
