Abstract

Cet ensemble comprend : - une tige médullaire (2) destinée à être fixée à l'os métacarpien au moyen de ciment synthétique et comprenant un moyen de fixation (7); - une tige médullaire (3) destinée à être fixée à l'os métacarpien par ostéogenèse, et présentant un moyen de fixation (8) identique à celui de la première tige (2) citée; - une cupule (4) destinée à être fixée au trapèze au moyen de ciment synthétique, délimitant une cavité hémisphérique (10); - une cupule (5) destinée à être fixée au trapèze par ostéogenèse, délimitant une cavité hémisphérique (14) identique à celle de la cupule (4) précitée, et- au moins deux pièces intermédiaires (6) de longueurs différentes, comprenant chacune, à une extrémité, un moyen de fixation (15) complémentaire de celui de l'une ou l'autre des tiges médullaires (2, 3) précitées et, à son autre extrémité, une tête sphérique (16) pouvant être reçue à pivotement dans la cavité (10, 14) de l'une ou l'autre des cupules (4, 5) précitées.

Description

La présente invention concerne un ensemble d'éléments permettant la constitution d'une prothèse d'articulation trapézo-métacarpienne.
L'articulation trapézo-métacarpienne est particulièrement sollicitée lors des mouvements du pouce et constitue l'une des articulations devant fréquemment faire l'objet d'un remplacement par une prothèse.
Les prothèses articulaires trapézo-métacarpiennes existantes ont pour inconvénient de n'être pas parfaitement adaptables à toutes les situations pouvant se présenter, selon l'âge, la taille et l'anatomie du patient.
La présente invention vise à remédier à cet inconvénient pratique essentiel.
L'ensemble d'éléments qu'elle concerne comprend
- une tige médullaire destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci au moyen de ciment synthétique, cette tige comprenant un moyen de fixation aménagé dans son extrémité proximale
- une tige médullaire destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci par ostéogenèse, cette tige présentant une forme qui correspond sensiblement à la forme du canal médullaire de cet os de telle sorte qu'elle puisse être coincée dans celui-ci, et présentant une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, ce qui réalise l'ancrage définitif de cette tige, laquelle présente en outre, aménagé dans son extrémité proximale, un moyen de fixation identique à celui de la première tige citée
- une cupule destinée à être fixée au trapèze au moyen de ciment synthétique, cette cupule comprenant au moins un moyen aménagé dans sa face extérieure pour recevoir du ciment afin d'assurer la liaison entre ladite cupule et ce ciment, et délimitant une cavité hémisphérique
- une cupule destinée à être fixée au trapèze par ostéogenèse, comprenant des moyens assurant sa fixation provisoire à l'os et une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, ce qui réalise l'ancrage définitif de cette cupule, laquelle délimite une cavité hémisphérique identique à celle de la cupule précitée, et
- au moins deux pièces intermédiaires de longueurs différentes, comprenant chacune, à une extrémité, un moyen de fixation complémentaire de celui de l'une ou l'autre des tiges médullaires précitées et, à son autre extrémité, une tête sphérique pouvant être reçue à pivotement dans la cavité de l'une ou l'autre des cupules précitées.
Ainsi, ces pièces sont modulaires et permettent de constituer selon les besoins soit une prothèse cimentée, soit une prothèse ancrée par ostéogenèse, soit une prothèse cimentée sur un os et ancrée par ostéogenèse sur l'autre os ; les pièces intermédiaires précitées, du fait de leurs longueurs différentes, permettent d'adapter la prothèse obtenue à la distance anatomique existant entre les deux os à appareiller.
De préférence, le moyen de fixation de chacune des tiges médullaires est constitué par une cavité proximale présentant une forme conique à faible pente, et le moyen de fixation complémentaire de chacune des pièces intermédiaires est constitué par un embout de forme conique ayant une pente correspondante.
Selon une forme de réalisation préférée de l'invention, la tige médullaire destinée à être fixée à l'os métacarpien au moyen de ciment synthétique présente une queue médullaire cylindrique, une portion métaphysaire sensiblement conique ou évasée et une collerette proximale.
De préférence, les moyens que comprend la cupule destinée à être ancrée par ostéogenèse pour sa fixation provisoire à l'os sont constitués par une forme tronconique de cette cupule et par un filet aménagé dans la paroi de cette cupule, des encoches diamétralement opposées étant aménagées dans la paroi proximale de cette cupule pour la prise d'appui d'un instrument de vissage.
Pour sa bonne compréhension, l'invention est à nouveau décrite ci-dessous en référence au dessin schématique annexé représentant, à titre d'exemple non limitatif, une forme de réalisation préférée de l'ensemble d'éléments qu'elle concerne.
La figure 1 est une vue en perspective de ces différents éléments, et
la figure 2 est une vue en coupe et à échelle agrandie de l'un de ces éléments.
La figure 1 représente un ensemble d'éléments permettant la constitution d'une prothèse d'articulation trapézo-métacarpienne.
Cet ensemble d'éléments comprend deux tiges médullaires 2, 3, deux cupules trapéziennes 4, 5 et un jeu de trois pièces intermédiaires 6.
La tige 2 est destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci au moyen de ciment synthétique, après résection adéquate. Elle présente une queue médullaire cylindrique 2a, une portion métaphysaire 2b sensiblement conique ou évasée et une collerette proximale d'appui 2c. La tige 2 comprend en outre une cavité proximale 7 aménagée coaxialement à elle dans ses collerette 2c et portion métaphysaire 2b, qui présente une forme conique à faible pente.
La tige 3 est destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci par ostéogenèse. Elle présente une forme qui correspond sensiblement à la forme du canal médullaire de cet os, à savoir une queue médullaire 3a de forme légèrement courbe et une section qui augmente progressivement de l'extrémité distale vers l'extrémité proximale, de telle sorte qu'elle puisse être engagée dans ce canal médullaire et être retenue dans celui-ci par coincement.
La tige 3 comprend de plus une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, ce qui réalise son ancrage définitif à l'os.
En outre, cette tige 3 présente une cavité 8 identique à la cavité 7, aménagée dans son extrémité proximale.
La cupule 4 est destinée à être fixée au trapèze au moyen de ciment synthétique. Elle comprend deux gorges 9 aménagées dans sa face extérieure pour recevoir du ciment afin d'assurer la liaison entre ladite cupule et ce ciment, et délimite une cavité hémisphérique 10.
La cupule 5 est destinée à être fixée provisoirement au trapèze par vissage puis définitivement par ostéogenèse. Elle présente une forme tronconique et un filet 11, ainsi qu'un alésage axial 12 permettant son engagement sur l'extrémité d'un outil d'impaction et deux encoches 13 diamétralement opposées, aménagées dans sa paroi proximale 5a, pour la prise d'appui d'un instrument de vissage.
Cette cupule 5 présente de plus une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, et délimite une cavité hémisphérique 14 identique à celle de la cupule 4.
Chacune des trois pièces intermédiaires 6, comprend, à une extrémité, un embout de forme conique 15 à faible pente, complémentaire de l'une ou l'autre des cavités 7 et 8 et, à son autre extrémité, une tête sphérique 16 pouvant être reçue à pivotement dans l'une ou l'autre des cavités 7 et 8.
Ces pièces 6 présentent en outre des parties centrales Ga de longueurs différentes d'une pièce 6 à une autre.
Les différents éléments 2 à 6 constituant ledit ensemble sont ainsi modulaires et permettent de constituer selon les besoins soit une prothèse cimentée, soit une prothèse ancrée par ostéogenèse, soit une prothèse cimentée sur un os et ancrée par ostéogenèse sur l'autre os. Les pièces 6, du fait de leurs longueurs différentes, permettent d'adapter la prothèse obtenue à la distance anatomique existant entre les deux os à appareiller.

Claims (4)

REVENDICATIONS
1 - Ensemble d'éléments permettant la constitution d'une prothèse d'articulation trapézo-métacarpienne, caractérisé en ce qu'il comprend
- une tige médullaire (2) destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci au moyen de ciment synthétique, cette tige (2) comprenant un moyen de fixation (7) aménagé dans son extrémité proximale ;
- une tige médullaire (3) destinée à être insérée dans le canal médullaire de l'os métacarpien et à être fixée à celui-ci par ostéogenèse, cette tige (3) présentant une forme qui correspond sensiblement à la forme du canal médullaire de cet os de telle sorte qu'elle puisse être coincée dans celui-ci, et présentant une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, ce qui réalise l'ancrage définitif de cette tige (3), laquelle présente en outre, aménagé dans son extrémité proximale, un moyen de fixation (8) identique à celui de la première tige (2) citée
- une cupule (4) destinée à être fixée au trapèze au moyen de ciment synthétique, cette cupule comprenant au moins un moyen (9) aménagé dans sa face extérieure pour recevoir du ciment afin d'assurer la liaison entre ladite cupule (4) et ce ciment, et délimitant une cavité hémisphérique (10)
- une cupule (5) destinée à être fixée au trapèze par ostéogenèse, comprenant des moyens (11) assurant sa fixation provisoire à l'os et une surface poreuse dont les pores sont destinés à être envahis par les cellules osseuses lors de leur croissance, ce qui réalise l'ancrage définitif de cette cupule (5), laquelle délimite une cavité hémisphérique (14) identique à celle de la cupule (4) précitée, et
- au moins deux pièces intermédiaires (6) de longueurs différentes, comprenant chacune, à une extrémité, un moyen de fixation (15) complémentaire de celui de l'une ou l'autre des tiges médullaires (2, 3) précitées et, à son autre extrémité, une tête sphérique (16) pouvant être reçue à pivotement dans la cavité (10, 14) de l'une ou l'autre des cupules (4, 5) précitées.
2 - Ensemble d'éléments selon la revendication 1, caractérisé en ce que le moyen de fixation de chacune des tiges médullaires (2, 3) est constitué par une cavité proximale (7, 8) présentant une forme conique à faible pente, et en ce que le moyen de fixation complémentaire de chacune des pièces intermédiaires (6) est constitué par un embout (15) de forme conique ayant une pente correspondante.
3 - Ensemble d'éléments selon la revendication 1 ou la revendication 2, caractérisé en ce que la tige médullaire (2) destinée à être fixée à l'os métacarpien au moyen de ciment synthétique présente une queue médullaire cylindrique (2a), une portion métaphysaire (2b) sensiblement conique ou évasée et une collerette proximale (2c).
4 - Ensemble d'éléments selon l'une des revendications 1 à 3, caractérisé en ce que les moyens que comprend la cupule (5) destinée à être ancrée par ostéogenèse pour sa fixation provisoire à l'os sont constitués par une forme tronconique de cette cupule (5) et par un filet (11) aménagé dans la paroi de cette cupule (5), des encoches (13) diamétralement opposées étant aménagées dans la paroi proximale (5a) de cette cupule (5) pour la prise d'appui d'un instrument de vissage.
