PROTHESE CARACTERISEE EN CE QUE SA TIGE EST MUNIE D'AU MOINS DEUX SURFACES OPPOSEES 1 A MACROSTRUCTURE EN POINTES DE DIAMANT 2, EN CE QUE SES SURFACES 1 ET 3 S'ELARGISSENT PROGRESSIVEMENT A PARTIR DE LA POINTE 4 DE LA TIGE EN DIRECTION DE LA COLLERETTE DU CONE 5 POUR REALISER UN AUTOBLOCAGE SIMULTANE TRIDIMENSIONNEL DANS LE PLAN AXIAL, DANS LE PLAN FRONTAL ET DANS LE PLAN SAGITTAL, EN CE QU'UNE LAME ANTIROTATION 6 EQUIPE LA PARTIE TROCHANTERIENNE DE LA TIGE, CETTE PARTIE TROCHANTERIENNE ETANT MUNIE D'UN TARAUDAGE 7 SUR SA FACE SUPERIEURE, ET EN CE QUE LE CONE 5 EST UN CONE MORSE MUNI D'UN MICROFILETAGE.

Description

La présente invention concerne le domaine de la chirurgie, en particulier des prothèses, et a pour objet une prothèse fémorale. L'invention a également pour objet un ancillaire correspondant.
Actuellement, pour réaliser des prothèses de hanches, on utilise généralement des prothèses fémorales présentant, soit une surface lisse, soit une surface cruantée. Les prothèses fémorales à surface lisse sont fixées dans le fût fémoral au moyen d'un ciment. Ce dernier assure, lors de l'implantation, le comblement entre l'os et l'implant. Cependant, une prothèse de hanche cimentée comporte à terme un risque de descellement aseptique, et les difficultés opératoires se pré- sentant en cas de reprises chirurgicales, notamment l'ablation du ciment, occasionnent de très déllcats problèmes techniques.
Les prothèses fémorales à surface cruante, à savoir, par exemple, à couverture madréporique, ctestçà-dire présentant un surfaçage micro- et macrobillè ont permis d'obvier aux in convénients des prothèses a surface lisse en permettant une fixation sans ciment, la couverture madréporique favorisant une réhabitation osseuse sur la prothèse.
Toutefois, ces prothèses fémorales à couverture ma dréporique ne permettent pas une stabilité immédiate par autoblocage, la stabilisation définitive n'étant effective qu'après réhabitation osseuse et ossification secondaire. En outre, en cas de reprise chiruzgicale, la dépose d'une telle prothèse est rendue très difficile du fait de l'intégration intime du tissu osseux dans la surface madréporique, en particulier dans les macrobilles dont on risque un arrachement ainsi qu'une détérioration du tissu osseux.
La présente invention a pour but de pallier ces inconvénients.
Elle a, en effet, pour objet une prothèse fémorale caractérisée en ce que sa tige est munie d'au moins deux surfaces opposées à macrostructure en pointes de diamant, en ce que ses surfaces s'élargissent progress#ivement à partir de la pointe de la tige en direction de la collerette du cône pour réaliser un autoblocage simultané tridimensionnel dans le plan axial, dans le plan frontal et dans le plan sagittal, en ce qu'une lame anti-rotation équipe la partie trochantérienne de la tige, cette partie trochantérienne étant munie d'un taraudage sur sa face supérieure, et en ce que le cône est un cône morse muni d'un microfiletage.
L'invention sera mieux comprise grâce à la description ci-après, qui se rapporte à un mode de réalisation préféré, donné à titre d'exemple non limitatif, et expliqué avec référence aux dessins schématiques annexés, dans lesquels la figure 1 est une vue en élévation frontale d'une prothèse conforme à l'invention la figure 2 est une vue en élévation latérale de la prothèse suivant la figure 1 la figure 3 est une vue partielle en plan, à plus grande échelle, de la macrostructure de surface, et la figure 4 est une vue en élévation frontale et partiellement en coupe d'un ancillaire correspondant à la prothèse.
Conformément à l'invention, et comme le montrent plus particulièrement, à titre d'exemple, les figures 1 et 2 des dessins annexés, la prothèse fémorale présente une tige qui est munie de deux surfaces opposées 1 à macrostructure en pointes de diamant 2, et ses surfaces 1 et 3 s'élargissent progressivement à partir de la pointe 4 de la tige en direction de la collerette du cône 5. En outre, la partie trochantérienne de la tige est pourvue d'une lame anti-rotation 6, et, sur sa face supérieure, d'un taraudage 7, et le c8ne 5, qui est un cône morse, est muni d'un microfiletage.
La macrostructure en pointes de diamant 2 est réalisée par un striage croisé des surfaces 1 (figure 3), et lesdites pointes de diamant 2 sont munies, en outre, d'une microstructure de surface microbillée. Cette microstructure de surface est du type à rugosité contrôlée et assure, ensemble avec la macrostructure 2, la réhabitation osseuse et l'ossifi- cation secondaire, la surface de contact avec l'os spongieux étant fortement augmentée par rapport aux prothèses connues.
Les surfaces 1 et 3, qui s'élargissent progressive ment à partir de la pointe 4 en direction de la collerette du cône 5, assurent simultanément un autoblocage dans le plan frontal sur les surfaces 3, c'est-a-dire sur les surfaces interne et externe, dans le plan sagittal sur les surfaces 1 avant et arrière, et, en coopération, un autoblocage axial.
Ainsi, la coopération de ces surfaces 1 et 3 avec la maero- structure 2 permet l'obtention d'un autoblocage tridimensionnel primitif favorisant une réhabitation osseuse secondaire
La lame anti-rotation 6 équipant la partie trochantérienne de la tige est destinée a' améliorer la fixation primaire de la prothèse dans le fut fémoral, c'est-à-:lire à assurer une stabilité immédiate au niveau du fémur.
Le taraudage 7 sur la face supérieure de la partie trochantérienne est prévu comme auxiliaire de positionnement lors de la mise en place de la prothèse et sert, en outre, de point d'attache d'un extracteur en cas de reprise chirurgicale éventuelle.
En outre, le microfiletage équipant le cane morse 5 est destin a assurer un blocage immédiat de la bille 5', tant en rotation que dans le sens d'une extraction, grâce à un écrasement des microfilets lors de l'impaction de ladite bille 5' sur le cône 5.
L'invention a également pour objet un ancillaire fémoral sous forme d'une râpe correspondant a la prothèse décrite ci-dessus, représenté a' la figure 4 des dessins annexés, et caractérisé en ce qu'il est constitué par un corps de rape 8 présentant â sa partie supérieure une face 9 d'application d'un manche 10 ou d'une tete école 11 avec une bille fantôme 12, l'assemblage entre le corps de rSpe 8 et le manche 10 ou la tête école Il étant assuré par un dispositif démontable a' coopération de forme 13 du type & encliquetage, à verrouillage, à vissage, ou autre.
Ce mode de realisation d'ancillaire permet la mise en oeuvre de râpes de tailles variables avec un manche unique pour la préparation du fflt fémoral, la râpe servant, en outre, après détermination de la taille de tige adaptée, de prothèse fantôme, En effet, la râpe peut être désolidarisée de son manche 10 après mise en place dans le fût fémoral, et une tête école il peut ttre montée sur la face 9 du corps de rSpe 8 par simple encliquetage, verrouillage, vissage, ou autre, de sorte qu'est réalisée une prothèse pante reproduisant exactement les conditions de positionnement de la future prothese.
Ainsi la prothèse peut être disposée définitivement, la prothèse fantôme ayant permis de vérifier au préalable l'exacti- tude de sa position ainsi que le bon choix de la bille d'articulation.
Grâce a l'invention, il est possible de réaliser une prothèse fémorale et un ancillaire correspondant permettant une adaptation facilitée au fût fémoral et un maintien amélio- ré, tant au niveau de l'autoblocage immédiat qu'au niveau de la réhabitation osseuse et de l'ossification secondaire, tout en autorisant une extraction plus facile#en cas de reprise chirurgicale. En effet, du fait de la disposition légèrement inclinée des surfaces 1 et 3, un léger déplacement de la tige en direction d'extraction du fflt fémoral a pour conséquence une désolidarisation du tissu spongieux de l'os et de la ma- crostructure 2 en pointes de diamant à surface microbillée.
Bien entendu, l'invention n'est pas limitée au mode de réalisation décrit et représenté aux dessins annexés. Des modifications restent possibles, notamment du point de vue de la constitution des divers éléments, ou par substitution d'équivalents techniques, sans sortir pour autant du domaine de protection de 1 t invention.

Claims (3)

  1. - REVENDICATIONS-
    1. Prothèse fémorale, caractérisée en ce que sa tige est munie d'au moins deux surfaces opposées (1) à macrostrucu- re en pointes de diamant (2), en ce que ses surfaces (1 et 3) s'élargissent progressivement à partir de la pointe (4) de la tige en direction de la collerette du cône (5) pour réaliser un autoblocage simultané tridimensionnel dans le plan axial, dans le plan frontal et dans le plan sagittal, en ce qu'une lame anti-rotation (6) équipe la partie trochantérienne de la tige, cette partie trochantérienne étant munie d'un taraudage (7) sur sa face supérieure, et en ce que le cône (5) est un cône morse muni d'un microfiletage.
  2. 2. Prothèse, suivant la revendication 1, caractérisée en ce que la macrostructure en pointes de diamant (2) est réalisée par un striage croisé des surfaces (1), et lesdites pointes de diamant (2) sont munies, en outre, d'une microstructure de surface microbillée.

Ancillaire fémoral sous forme d'une rape correspondant à la prothèse fémorale suivant l'une quelconque des revendications l et 2, caractérisé en ce qu t il est constitué par un corps de râpe (8) présentant â sa partie supérieure une face (9) d'application d'un manche (lO) ou d'une tête école (ll) avec une bille fantôrne (12), l'assemblage entre le corps de râpe (8) et le manche (10) ou la tête école (ll) étant assuré par un dispositif démontable à coopération de forme (13) du type d encliquetage, à verrouillage, a vissage, ou autre.
