Abstract

L'implant (1) comprend une tige effilée (2) destinée à être mise en place dans le canal médullaire du fémur et un embout proximal (3) destiné à recevoir une tête sphérique d'articulation (4), ladite tige effilée (2) comprenant une partie métaphysaire (2a) délimitée, du côté proximal, par une face proximale (2b) et délimitée par des côtés interne et externe et par des faces antérieure et postérieure.Selon l'invention, ladite tige effilée (2) comprend :- une rainure (5) aménagée dans la partie métaphysaire (2a) de l'implant (1), à partir de ladite face proximale de celui-ci, orientée dans le sens transversal par rapport à cette partie métaphysaire (2a), c'est-à-dire reliant ladite face antérieure à ladite face postérieure ; et- un trou taraudé (6) débouchant dans le fond de cette rainure (5).L'instrument comprend deux pièces indépendantes, dont une comprend une partie pouvant être aménagée dans la rainure (5) et de l'autre comprend une partie pouvant être engagée dans le trou taraudé (6).

Description

1 La présente invention concerne un implant fémoral de prothèse de hanche
et un instrument pour la mise en place de cet implant sur un fémur. Comme cela est bien connu, une prothèse de hanche comprend généralement un implant fémoral et un implant cotyloïdien. L'implant fémoral comprend une tige effilée destinée à être mise en place dans le canal médullaire du fémur et un embout proximal destiné à recevoir une tête sphérique d'articulation. L'implant cotyloïdien comprend une coque destinée à être mise en place dans la cavité cotyloïdienne du bassin et un insert en matériau favorisant le glissement, notamment en polyéthylène, destiné à être reçu dans cette coque, cet insert délimitant une cavité sphérique de réception de la tête prothétique de l'implant fémoral. Pour la bonne reconstitution de l'articulation, le positionnement adéquat de l'implant fémoral par rapport au fémur est un point particulièrement critique. La position de cet implant doit notamment être réglée aussi précisément que possible en pivotement selon l'axe longitudinal du fémur, afin de régler l'orientation dudit embout vers l'avant (antéversion) ou vers l'arrière (rétroversion), par rapport au plan sagittal, selon l'anatomie du patient, en même temps qu'est réalisé l'enfoncement de l'implant dans le fémur par impaction.
Pour mettre en place un implant fémoral de prothèse de hanche, il est connu d'utiliser divers instruments, notamment des râpes permettant de creuser de manière adéquate l'épiphyse fémorale, des implants d'essai, des instruments d'impaction. Cependant, avec les matériels connus, le réglage du positionnement adéquat de l'implant fémoral par rapport au fémur n'est pas toujours réalisé de manière particulièrement précise et fait largement appel à l'expérience du praticien. La présente invention vise à remédier à cet inconvénient en fournissant un implant fémoral et un instrument pour la mise en place de cet implant, permettant de régler précisément la position de l'implant dans le fémur, en .30 particulier angulairement. L'implant concerné comprend, de manière connue en soi, une tige effilée destinée à être mise en place dans le canal médullaire du fémur et un embout proximal destiné à recevoir une tête sphérique d'articulation, ladite tige effilée comprenant une partie métaphysaire délimitée, du côté proximal, par une face 2907664 2 proximale et délimitée par des côtés interne et externe et par des faces antérieure et postérieure. Selon l'invention, ladite tige effilée comprend : - une rainure aménagée dans la partie métaphysaire de l'implant, à partir 5 de ladite face proximale de celui-ci, orientée dans le sens transversal par rapport à cette partie métaphysaire, c'est-à-dire reliant ladite face antérieure à ladite face postérieure ; et - un trou taraudé débouchant dans le fond de cette rainure. L'instrument selon l'invention comprend : 10 - une première pièce comprenant, à son extrémité distale, une nervure destinée à être engagée de manière ajustée dans ladite rainure, de manière à permettre une liaison en rotation entre cette première pièce et ladite tige effilée, cette première pièce comprenant également un alésage longitudinal débouchant dans ladite nervure ; et 15 - une deuxième pièce, indépendante de ladite première pièce, comprenant une tige pouvant être engagée dans l'alésage longitudinal de la première pièce et comprenant, à son extrémité distale, une extrémité filetée, cette extrémité filetée pouvant, indépendamment de l'engagement de ladite nervure dans ladite rainure, être vissée dans le trou taraudé de ladite tige 20 effilée, et - une tête de frappe solidaire de l'une ou l'autre desdites première et deuxième pièces, permettant l'impaction de ladite tige effilée dans le canal médullaire du fémur. La tige effilée peut ainsi être reliée à l'instrument par vissage de ladite 25 extrémité filetée dans ledit trou taraudé puis ladite nervure peut être introduite dans ladite rainure. La tige effilée est ensuite introduite dans le canal médullaire du fémur, après résection adéquate de l'os, au moyen de l'instrument, et sa position angulaire par rapport à ce dernier peut être réglée au moyen de ladite première pièce. La tige effilée est ensuite impactée dans le fémur ; au cours de cette opération, ladite première pièce peut être utilisée pour conserver l'orientation de la tige effilée, par maintien angulaire de cette première pièce, jusqu'à réalisation de l'impaction cornplète. L'invention permet ainsi de réaliser, de manière précise, le réglage de 35 l'orientation du col, et de conserver la position réglée, tout au long de 2907664 3 l'impaction, et donc d'obtenir un positionnement angulaire précis de cet implant fémoral en même temps qu'est réalisé l'enfoncement de l'implant dans le fémur. La rainure peut avoir une largeur égale au diamètre du trou taraudé, ou 5 légèrement supérieure à ce diamètre, afin que les parois qui la délimitent latéralement favorisent la présentation de ladite extrémité filetée en regard de ce trou taraudé. L'engagement de cette extrémité filetée dans ce trou taraudé s'en trouve facilitée.
10 Selon une forme de réalisation préférée de l'invention, - les parois délimitant latéralement ladite rainure comprennent, aménagés en elles, deux évidements arrondis centrés sur l'axe du trou taraudé. L'instrument comprend, dans ce cas, une portion de forme cylindrique 15 destinée à être engagée de manière ajustée dans l'espace délimité entre ces deux évidements. Lorsque l'instrument est relié à ladite tige effilée, la venue de cette portion cylindrique de l'instrument à proximité immédiate des parois délimitant latéralement ladite rainure permet d'élargir la zone d'appui de l'instrument 20 contre la tige effilée et donc de renforcer la liaison de cette tige effilée avec l'instrument dans le sens transversal. Cette liaison renforcée assure la transmission de l'énergie d'impaction dans le sens longitudinal de la tige effilée, sans risque de déformation de l'extrémité filetée de l'instrument. Avantageusement, le filet de ladite extrémité filetée présente une section 25 transversale trapézoïdale, et le taraudage du trou taraudé présente une section transversale correspondante. Cette section transversale permet à ce filet et à ce taraudage de résister aux efforts exercés pendant l'impaction. L'instrument peut avantageusement comprendre des moyens de 30 maintien permettant de maintenir ladite nervure de ladite première pièce en engagement dans la rainure de la tige effilée lorsque ladite extrémité filetée est engagée dans ledit trou taraudé. Ces moyens de maintien peuvent notamment être sous forme d'une portion filetée aménagée sur ladite deuxième pièce, sur laquelle peut être vissé un écrou moleté d'appui contre ladite première pièce.

5 La figure 1 est une vue de cet implant de côté ; la figure 2 en est une vue partielle, de côté, avec une partie en coupe selon le plan sagittal médian de l'implant ; la figure 3 en est une vue partielle, en perspective ; la figure 4 est une vue en perspective de l'instrument, et l0 la figure 5 est une vue en perspective de cet instrument relié à un implant selon l'invention. Les figures 1 à 3 représentent un implant fémoral 1 de prothèse de hanche. Cet implant 1 comprend une tige effilée 2 destinée à être mise en place 15 dans le canal médullaire du fémur et un embout proximal 3 destiné à recevoir une tête sphérique d'articulation 4. La tige effilée 2 comprend une partie métaphysaire 2a délimitée, du côté proximal, par une face proximale 2b et délimitée par des côtés interne 2c et externe 2d et par des faces antérieure 2e et postérieure 2f (dans l'exemple 20 représenté, l'implant 1 est un implant de côté droit). Dans l'exemple représenté, la tige 2 comprend, sur ses faces antérieure 2e et postérieure 2f des rainures et/ou nervures de prise d'appui contre le fémur. Elle peut également en être dépourvue (cf. figure 5). Ainsi que cela apparaît, la tige effilée 2 comprend une rainure 25 proximale 5 et un trou taraudé 6. La rainure 5 est aménagée dans la partie métaphysaire 2a, à partir de la face proximale 2b, et est orientée dans le sens transversal par rapport à cette partie métaphysaire 2a, c'est-à-dire qu'elle relie la face antérieure 2e à la face postérieure 2f de la tige 2.
30 Le trou taraudé 6 débouche dans le fond de cette rainure 5. Le filet qu'il comporte présente une section transversale trapézoïdale. La rainure 5 a une largeur légèrement supérieure au diamètre du trou 6, et les parois qui la délimitent latéralement comprennent deux évidements arrondis 7 aménagés en elles, centrés sur l'axe du trou 6.
2907664 5 L'implant 1 peut être mis en place sur un fémur au moyen de l'instrument 10 représenté sur les figures 4 et 5. Cet instrument 10 comprend une première pièce 11 est une deuxième pièce 12 indépendante l'une de l'autre.
5 La première pièce 11 comprend, à son extrémité distale, une nervure 13 destinée à être engagée de manière ajustée dans la rainure 5, afin de permettre une liaison en rotation entre cette première pièce 11 et la tige effilée 2 par insertion de cette nervure 13 dans la rainure 5. Cette première pièce 11 comprend également un alésage longitudinal débouchant dans ladite 10 nervure 13 et une portion cylindrique 14 destinée à être engagée de manière ajustée dans l'espace délimité entre les deux évidements 7. La deuxième pièce 12 comprend une tige 15 pouvant être engagée de manière coulissante dans l'alésage longitudinal de la première pièce 11. Elle comprend, à son extrémité distale, une extrémité filetée 16 pouvant, 15 indépendamment de l'engagement de ladite nervure 13 dans ladite rainure 5, être vissée dans le trou taraudé 5 de ladite tige effilée 2. Le filetage de cette extrémité 16 est complémentaire de celui du trou 5, c'est-à-dire présente également un filet de section transversale trapézoïdale. La deuxième pièce 12 comprend aussi une portion filetée 17 solidaire 20 d'elle, sur laquelle peut être vissé ou dévissé un écrou moleté 18 d'appui contre l'extrémité proximale de la pièce 11. Cet écrou 18 permet, en position de vissage, et lorsque l'extrémité 16 est vissée dans le trou 5, d'assurer l'immobilisation axiale de la pièce 11 par rapport à la pièce 12, en position d'engagement de la nervure 13 dans la rainure 5.
25 En outre, la deuxième pièce 12 comprend une tête de frappe 20 solidaire d'elle, permettant l'impaction de la tige effilée 2 dans le canal médullaire du fémur. En pratique, ainsi que cela se comprend, la tige effilée 2 est reliée à l'instrument 10 par vissage de l'extrémité filetée 16 dans le trou taraudé 6 puis 30 la nervure 13 est introduite dans la rainure 5. Les parois qui délimitent latéralement la rainure 5 favorisent la présentation de l'extrémité filetée 16 en regard du trou taraudé 6, de sorte que l'engagement de cette extrémité 16 dans ce trou 6 s'en trouve facilitée. Au cours de cet assemblage, la portion cylindrique 14 vient à proximité :35 immédiate des parois délimitant latéralement la rainure 5, au niveau des 2907664 6 évidements 7. Il en résulte une liaison renforcée de la tige effilée 2 à l'instrument 10 dans le sens transversal, qui permet d'assurer la transmission de l'énergie d'impaction dans le sens longitudinal de la tige effilée 2, sans risque de déformation de l'extrémité filetée 16 de l'instrument 10.
5 La tige effilée 2 est ensuite introduite dans le canal médullaire du fémur, après résection adéquate de l'os, au moyen de l'instrument 10, puis est orienté de manière adéquate par orientation de la pièce 11. La tige 2 est ensuite impactée dans l'os par frappe sur la tête 20 ; au cours de cette impaction, ladite première pièce 11 peut être maintenue angulairement pour 10 conserver l'orientation du col 3, jusqu'à réalisation de l'impaction complète. Comme cela apparaît de ce qui précède, l'invention fournit un implant fémoral 1 et un instrument 10 pour la mise en place de cet implant sur un fémur, qui présentent l'avantage déterminant de permettre de réaliser, de manière précise, le réglage de l'orientation du col 3 d'un implant, et de 15 conserver la position réglée, tout au long de l'impaction. L'invention permet donc d'obtenir un positionnement angulaire précis de cet implant fémoral en même temps qu'est réalisé l'enfoncement de l'implant 1 dans le fémur. Il va de soi que l'invention n'est pas limitée à la forme de réalisation décrite ci-dessus à titre d'exemple mais qu'elle s'étend à toutes les formes de 20 réalisations couvertes par les revendications ci-annexées.

Claims (9)

REVENDICATIONS
1 û Implant fémoral (1) de prothèse de hanche, comprenant une tige effilée (2) destinée à être mise en place dans le canal médullaire du fémur et un embout proximal (3) destiné à recevoir une tête sphérique d'articulation (4), ladite tige effilée (2) comprenant une partie métaphysaire (2a) délimitée, du côté proximal, par une face proximale (2b) et délimitée par des côtés interne (2c) et externe (2d) et par des faces antérieure (2e) et postérieure (2f), caractérisé en ce que ladite tige effilée (2) comprend : - une rainure (5) aménagée dans la partie métaphysaire (2a) de l'implant (1), à partir de ladite face proximale de celui-ci, orientée dans le sens transversal par rapport à cette partie métaphysaire (2a), c'est-à-dire reliant ladite face antérieure (2e) à ladite face postérieure (2f) ; et -un trou taraudé (6) débouchant dans le fond de cette rainure (5).
2 û Implant (1) selon la revendication 1, caractérisé en ce que la rainure (5) a une largeur égale au diamètre du trou taraudé (6), ou légèrement supérieure à ce diamètre.
3 û Implant (1) selon la revendication 1 ou la revendication 2, caractérisé en ce que les parois délimitant latéralement ladite rainure (5) comprennent, aménagés en elles, deux évidements arrondis (7) centrés sur l'axe du trou taraudé (6).
4 û Implant (1) selon l'une des revendications 1 à 3, caractérisé en ce que le taraudage du trou taraudé (6) présente une section transversale trapézoïdale.
5 û Instrument pour la mise en place sur un fémur de l'implant fémoral (1) selon l'une des revendications 1 à 4, caractérisé en ce qu'il comprend : - une première pièce (11) comprenant, à son extrémité distale, une nervure (13) destinée à être engagée de manière ajustée dans ladite rainure (5), de manière à permettre une liaison en rotation entre cette première pièce (11) et ladite tige effilée (2), cette première pièce (11) :30 comprenant également un alésage longitudinal débouchant dans ladite nervure (13) ; et - une deuxième pièce (12), indépendante de ladite première pièce (11), comprenant une tige (15) pouvant être engagée dans l'alésage longitudinal de la première pièce (11) et comprenant, à son extrémité distale, une extrémité :35 filetée (16), cette extrémité filetée (16) pouvant, indépendamment de 2907664 8 l'engagement de ladite nervure (13) dans ladite rainure (5), être vissée dans le trou taraudé (6) de ladite tige effilée (2), et - une tête de frappe (20) solidaire de l'une ou l'autre desdites première et deuxième pièces (11, 12), permettant l'impaction de ladite tige effilée (2) dans 5 le canal médullaire du fémur.
6 ù Instrument selon la revendication 5, pour la mise en place de l'implant (1) selon la revendication 3, caractérisé en ce qu'il comprend une portion de forme cylindrique (14) destinée à être engagée de manière ajustée dans l'espace délimité entre les deux évidements arrondis (7) que 10 comprennent les parois délimitant latéralement ladite rainure (5).
7 ù Instrument selon la revendication 5 ou la revendication 6, caractérisé en ce que le filet de ladite extrémité filetée (16) présente une section transversale trapézoïdale.
8 ù Instrument selon l'une des revendications 5 à 7, caractérisé en ce 15 qu'il comprend des moyens de maintien (17, 18) permettant de maintenir ladite nervure (13) de ladite première pièce (11) en engagement dans la rainure (5) de la tige effilée (2) lorsque ladite extrémité filetée (16) est engagée dans ledit trou taraudé (6).
9 ù Instrument selon la revendication 8, caractérisé en ce que les 20 moyens de maintien sont sous forme d'une portion filetée (17) aménagée sur ladite deuxième pièce (12), sur laquelle peut être vissé un écrou moleté (18) d'appui contre ladite première pièce (11).
