Abstract

Anneau de soutien de cotyle pour prothèse de hanche, caractérisé par le fait qu'il est constitué d'une pièce en acier inoxydable d'épaisseur comprise entre 0,8 mm et 1,5 mm, ladite pièce étant formée d'un hémisphère ajouré, tronqué au niveau de son sommet, destiné à s'adapter dans la cavité cotylodienne, muni sur une partie de sa périphérie d'un bord recourbé formant une jupe destinée à s'appliquer sur le sourcil cotylodien et présentant sur le reste de sa périphérie une échancrure destinée à se situer au niveau de la face antérieure du cotyle, de l'échancrure ischiopubienne, et de la face postérieure du cotyle.

Description

Anneau de soutien de cotyle osseux pour prothèse de hanche
La présente invention concerne un anneau de soutien de cotyle osseux pour prothèse de hanche.
L'idée d'un anneau de soutien au niveau du cotyle osseux dérive des conséquences de la chirurgie de la hanche dont l'explosion quantitative débute vers les années 1950. En fait, ce n'est vraiment qu' & partir des années 70 - 75 que la chirurgie de la prothèse de hanche prend véritablement son essor et améliore ses possibilités techniques quant à la qualité des matériaux utilisés et des ciments. A partir des années 1970, les prothèses de hanche sont posées en grand nombre chez des sujets de moins de 65 ans.
La durée de vie de ces prothèses est d'environ 10 à 20 ans en l'absence d'accident intercurant, tel qu'infection du matériel ou descellement, qu'il s'agisse d'un descellement septique, aseptique ou traumatique. Le descellement de ces prothèses de hanche peut se faire soit au niveau fémoral, soit au niveau cotyloldien.
La chirurgie de la prothèse de hanche a donc commencé à partir des années 1980 à prendre un second essor, à savoir celui d'une chirurgie de reprise pour changement de matériel descellé ou cassé. Un certain nombre de problèmes ont dû être résolus au niveau fémoral et cotyloldien.
Plusieurs situations peuvent se présenter, selon que l'on est en présence d'un cotyle dysplasique, d'un cotyle protrusif ou d'un cotyle béant en cas de grand descellement et de migrations de matériel.
On peut observer un cotyle dysplasique, soit chez les patients n'ayant jamais subi de chirurgie lorsqu'une partie du toit supéro-externe du cotyle est manquant congénitalement, soit chez des patients où l'usure du cotyle est due à une prothèse ayant commencé à erroder le toit de ce cotyle.
Le cotyle protrusif se voit presque exclusivement dans l'arthrose de hanche ; il s'agit d'un enfoncement progressif de la tête fémorale dans le bassin qui pousse devant elle une mince pellicule osseuse représentée par l'arrière-fond du cotyle.
Quant au cotyle béant, il s'agit de la forme majeure des destructions du cotyle que l'on peut observer comme séquelles de descellement de prothèse totale.
Peu à peu, au cours des années, la pièce cotyloidienne descellée du fait de micro-mouvements quotidiens use peu à peu tout son entourage osseux. Lors de l'extraction du matériel au cours de la réintervention chirurgicale, on constate alors l'existence de pertes de substance osseuse plus ou moins importantes, nécessitant dans certains cas le renforcement par un anneau de soutien armé.
Les anneaux actuellement connus présentent de manière générale trop d'encombrement et surtout une trop grande rigidité empêchant leur adaptabilité aux diverses morphologies rencontrées, et nécessitant des efforts mécaniques trop importants de la part du chirurgien en cours d'opération.
La présente invention a pour but d'éviter Ces inconvénients.
La présente invention a pour objet un anneau de soutien de cotyle pour prothèse de hanche, caractérisé par le fait qu'il est constitué d'une pièce en acier inoxydable d'épaisseur comprise entre 0,8 mm et 1,5 mm, ladite pièce étant formée d'un hémisphère ajouré, tronqué au niveau de son sommet, destiné à s'adapter dans la cavité cotyloidienne, muni sur une partie de sa périphérie d'un bord recourbé formant une jupe destinée s'appliquer sur le sourcil cotyloldien, et présentant sur le reste de sa périphérie une échancrure destinée à se situer au niveau de la face antérieure du cotyle, de l'échancrure ischiopubienne, et de la face postérieure du cotyle.
Selon un autre mode de réalisation, ladite jupe est prolongée par deux pattes de fixation formant un angle avec ladite jupe et destinées à être appliquées respectivement contre l'aile iliaque et la face postérieure du cotyle.
Selon un autre mode de réalisation ledit anneau comporte deux pattes de fixation formant un angle avec ladite jupe, l'une des pattes étant destinée à être appliquée contre l'aile iliaque et l'autre patte étant destinée à être appliquée sur la tubérosité ischiatique.
De préférence, la largeur de ladite jupe est comprise entre 8 mm et 12 mm.
D'autres caractéristiques et avantages de la présente invention apparaitront au cours de la description suivante de modes de réalisation donnés à titre illustratif mais nullement limitatif. Dans le dessin annexé - La figure 1 est une vue schématique en perspective d'une partie de l'os iliaque muni d'une première variante d'anneau selon l'invention.

- La figure 3 est une vue en coupe selon la ligne III-III de la figure 2.
- La figure 4 est une vue en élévation d'une seconde variante d'anneau selon l'invention.
- La figure 5 est une vue en coupe selon la ligne V-V de la figure 4.
- La figure 6 est une vue schématique en perspective d'une troisième variante d'anneau selon l'invention en position dans l'os iliaque.
On a illustré schématiquement dans la figure 1 un os iliaque 10 avec ses différentes parties référencées de la manière suivante - Tubérosité ischiatique (ischion) 1 - Branche pubienne 2 - Echancrure ischio pubienne 3 - Face antérieure du cotyle (corne antérieure) 4 - Face postérieure du cotyle (corne postérieure) 5 - Aile iliaque 6 - Sourcil cotyloidien 7 - Cavité cotyloidienne (cotyle) 8.
L'anneau de soutien 20 visible dans cette figure et également dans les figures 2 et 3, est particulièrement utilisé dans le cas des dysplasies. Il est constitué d'une pièce métallique de préférence en acier inoxydable homologué pour les applications médicales ; son épaisseur est comprise entre 0,8 mm et 1,5 mm. Elle comporte un hémisphère 21 tronqué à son sommet 22 et ajouré en 23 de manière à permettre le passage du ciment de fixation. Les trous 23 doivent être d'un diamètre suffisamment important afin d'éviter tout laminage du ciment lors de l'impaction du cotyle. L'hémisphère 21 est largement échancré sur sa périphérie au niveau de trois zones successives - la zone 24 correspondant la face antérieure 4 du cotyle - la zone 25 correspondant à l'échancrure ischio pubienne 3 - la zone 26 correspondant à la face postérieure 5 du cotyle.
Ces trois échancrures forment une échancrure continue et sont destinées à éviter toute gêne mécanique pouvant autre de la présence de l'anneau et tout risque de luxation.
L'hémisphère est prolongé dans sa zone non échancrée par un bord recourbé ou jupe 27 de manière à venir au contact avec l'aile iliaque 6.
La fonction de cette jupe est également de rigidifier la structure de l'anneau. Quatre trous 28 situés au niveau de la jupe 27 permettent la fixation de l'anneau par des vis 29 dans le sourcil cotyloidien 7.
Afin de pouvoir adapter les anneaux 20 aux diverses morphologies rencontrées, les diamètres de l'hémisphère 21 sont par exemple de 50 mm, 56 mm et 60 mm. On voit dans la figure 3 un cotyle artificiel 30 destiné à s'emboîter dans l'anneau 20.
Les figures 4 et 5 montrent une seconde variante d'anneau 40 selon l'invention particulièrement adaptée aux cas de protrusions.
On retrouve un hémisphère 41 tronqué à son sommet 42 et ajouré au niveau des trous 43 ; l'anneau 40 présente également une échancrure correspondant à trois zones 44, 45 et 46 au niveau de la face antérieure 4 du cotyle, de l'échancrure ischiopubienne 3 et de la face postérieure 5 du cotyle. Son bord replié ou jupe 47 est prolongé par deux pattes de fixation 48 et 49 ; la patte 49 est destinée å s'appliquer contre l'aile iliaque 6, tandis que la patte 48 est destinée à s'appliquer contre la face postérieure du cotyle 5. Pour cela les pattes forment un angle avec le bord 47 proprement dit, comme cela est clairement visible dans la figure 5. Outre des perçages 51 prévus dans le bord 47, les pattes comportent des perçages 52 et 53 respectivement. Ainsi il est possible de fixer l'anneau 40 dans le cotyle hors d'une zone à densité osseuse faible.
La mise en oeuvre des pattes 48 et 49 assouplit la structure de l'anneau ; l'angle des pattes peut être aisément modifié par le chirurgien au cours de l'opération pour une parfaite adaptation et un ancrage optimal lors du vissage sans risque de verticalisation.
Plusieurs diamètres d'hémisphère peuvent être prévus, par exemple 50 mm, 56 mm et 60 mm.
La troisième variante d'anneau visible dans la figure 6 est destinée aux reconstructions cotyloidiennes en cas de cotyle béant. Sa conception de base est identique à celle des deux autres variantes.
L'anneau 60 a son hémisphère 61 avec son ouverture supérieure 62 et sa jupe 67 présentant des perçages 68 permettant une fixation dans le sourcil cotyloidien 7. Mais les deux pattes 71 et 72 prolongeant la jupe sont fixées plus loin, l'une sur l'aile iliaque 6 et l'autre sur la tubérosité ischiatique 1, au niveau de perçages 73. Les pattes peuvent faire une pièce unitaire avec l'hémisphère ou être rapportées par soudure à la jupe 67. Ces pattes aisément déformables permettent d'adapter l'implant aux morphologies rencontrées.
L'anneau de la figure 6 comporte en outre un crochet 74 destiné à être retenu au niveau de l'échancrure ischiopubienne 3.
L'anneau selon l'invention permet de traiter tous les cas, quelle que soit la difficulté chirurgicale.
Bien entendu l'invention n'est pas limitée aux trois modes de réalisation illustrés. On pourra, sans sortir du cadre de l'invention, remplacer tout moyen par un moyen équivalent.

Claims (8)

REVENDICATIONS
1/ Anneau de soutien de cotyle pour prothèse de hanche, caractérisé par le fait qu'il est constitué d'une pièce en acier inoxydable d'épaisseur comprise entre 0,8 mm et 1,5 mm, ladite pièce étant formée d'un hémisphère ajouré, tronqué au niveau de son sommet, destiné à s'adapter dans la cavité cotyloidienne, muni sur une partie de sa périphérie d'un bord recourbé formant une jupe destinée à s'appliquer sur le sourcil cotyloidien et présentant sur le reste de sa périphérie une échancrure destinée à se situer au niveau de la face antérieure du cotyle, de l'échancrure ischiopubienne, et de la face postérieure du cotyle.
2/ Anneau de soutien de cotyle selon la revendication i, caractérisé par le fait qu'il comporte dans ladite jupe des trous en vue d'une fixation sur ledit sourcil cotyloidien.
3/ Anneau de soutien selon la revendication 1, caractérisé par le fait que ladite jupe est prolongée par deux pattes de fixation formant un angle avec ladite jupe et destinées à être appliquées respectivement contre l'aile iliaque et la face postérieure du cotyle.
4/ Anneau de soutien selon la revendication 1, caractérisé par le fait qu'il comporte deux pattes de fixation formant un angle avec ladite jupe, l'une des pattes étant destinée à être appliquée contre l'aile iliaque et l'autre patte étant destinée à être appliquée sur la tubérosité ischiatique.
5/ Anneau de soutien selon l'une des revendications 3 et 4, caractérisé par le fait que lesdites pattes forment une pièce unitaire avec l'hémisphère.
6/ Anneau de soutien selon l'une des revendications 3 et 4, caractérisé par le fait que lesdites pattes sont liées à L'hémisphère par soudure.
7/ Anneau de soutien selon la revendication 4, caractérisé par le fait qu'il comporte en outre un crochet ischiopubien.
8/ Anneau de soutien selon l'une des revendications précédentes, caractérisé par le fait que la largeur de ladite jupe est comprise entre 8 mm et 12 mm.
