Abstract

Cette cupule est du type non cimenté et non vissé, comprenant, d'une part, une calotte extérieure (2) en un métal biocompatible tel qu'un acier inoxydable ou un alliage de titane ou autre similaire, munie d'au moins une patte radiale de fixation pourvue de perforations pour le passage de vis et dont la paroi sphérique présente une pluralité de trous (4) pour le passage de vis de fixation (5) et, d'autre part, une calotte intérieure (3) en une matière plus douce que le métal, comme une matière plastique biocompatible, telle qu'un polyéthylène haute densité. D'une part, la calotte intérieure (3), qui est engagée dur dans la calotte extérieure (2), présente le long de son bord supérieur et postérieur et sur un secteur d'au moins 180degré , un prolongement (3a) formant auvent antiluxation et, d'autre part, de segment annulaire bordant l'ouverture de la calotte intérieure (3) est muni, sur sa face externe, de facettes (3b) lui conférant un contour polygonal tandis que le segment annulaire correspondant de la calotte extérieure (2) présente, sur sa face interne, le même nombre de facettes (2b) lui conférant le même contour polygonal.

Description

CUPULE ANCREE POUR PROTHESE TOTALE DE HANCHE
La présente invention concerne une cupule ancrée pour prothèse totale de hanche du type non cimenté et non vissé, comprenant, d'une part, une calotte extérieure en un métal biocompatible tel qu'un acier inoxydable ou un alliage de titane ou autre similaire, munie d'au moins une patte radiale de fixation pourvue de perforations pour le passage de vis et dont la paroi sphérique présente une pluralité de trous pour le passage de vis de fixation et, d'autre part, une calotte intérieure en une matière plus douce que le métal, telle qu'une matière plastique biocompatible, telle q'un polyéthylène haute densité.
La difficulté majeure recontrée avec ces cupules est l'élimination ou, tout au moins, la réduction des risques de luxation. La présente invention vise justement à atteindre ce but. A cet effet, dans la prothèse qu'elle concerne et qui est du type précité, d'une part, la calotte intérieure, qui est engagée dur dans la calotte extérieure, présente le long de son bord supérieur et postérieur et sur un secteur d'au moins 1800, un prolongement formant auvent antiluxation et, d'autre part, de segment annulaire bordant l'ouverture de la calotte intérieure est muni, sur sa face externe de facettes lui conférant un contour polygonal tandis que le segment annulaire correspondant de la calotte extérieure présente, sur sa face interne, le même nombre de facettes lui conférant le même contour polygonal.
Cette disposition permet d'orienter à volonte la calotte intérieure par rapport à la calotte extérieure et donne ainsi au praticien qui pose cette prothèse ou la corrige, d'orienter l'auvent antiluxation de la calotte intérieure dans la direction la plus propice à éviter une luxation.
Suivant une autre caractéristique de l'invention visant à procurer, à cette cupule, une meilleure stabilité, la calotte extérieure métallique est équipée de deux pattes radiales de fixation, diamétralement opposée, dont l'une, inférieure, en forme de crochet est engageable sur le bord inférieur du cotyle et dont l'autre, supérieure en forme de raquette, présente quatre perforations disposées en croix permettant la mise en place de quatre vis de fixation.
Suivant encore une autre caractéristique de l'invention visant à améliorer la tenue de cette cupule dans le cotyle du patient, outre les perforations pour le passage de vis de fixation, la calotte métallique extérieure présente des perforations pour des pions cylindroconiques annelé extérieurement et filetés intérieurement.
De toute façon, l'invention sera bien comprise, à l'aide de la description qui suit, en référence au dessin schématique annexé représentant, à titre d'exemple non limitatif, une forme d'exécution de cette cupule

Comme le montre le dessin, cette cupule, qui est du type non vissé et non scellé, comprend une calotte extérieure 2 sensiblement hémiphérique, en métal biocompatible, à l'intérieur de laquelle est logée une calotte intérieure 3 en une matière plus douce que le métal, et notamment en une matière plastique biocompatible telle qu'un polyéthylène haute densité.
Un métal convenant parfaitement à la constitution de la calotte extérieure 2 est un acier inoxydable ou un alliage de titane.
Comme le montre le dessin, la calotte intérieure 3, qui est destinée à être engagée dure dans la calotte extérieure 2 présente, le long de son bord supérieur et postérieur et sur un secteur d'au moins 1800, un prolongement 3a formant un auvent antiluxation. En outre, le segment annulaire bordant l'ouverture de la calotte intérieure 3 est muni, sur sa face externe, de facettes 3b lui conférant un contour polygonal, tandis que le segment annulaire correspondant de la calotte extérieure 2 présente, sur sa face interne, le même nombre de facettes 2b lui conférant le même contour polygonal.
Comme cela se conçoit aisément, cette disposition permet au praticien qui effectue la pose de cette prothèse ou sa correction, d'orienter l'auvent anti-luxation 3a dans la direction la plus propice à éviter une luxation.
Pour permettre un choix convenable de l'orientation angulaire de l'auvent anti-luxation 3a de la calotte intérieure 3, le nombre de facettes 2b et 3b des contours polygonaux des segments annulaires précités de la calotte extérieure 2 et de la calotte intérieure 3 est avantageusement de douze.
Suivant une autre caractéristique de l'invention visant à ameliorer la stabilité de cette cupule, outre les perforations 4 prévues dans la paroi sphérique de la calotte extérieure 2 pour le passage de vis de fixation 5 implantées dans le cotyle du patient, cette calotte extérieure 2 est équipée de deux pattes radiales diamétralement opposées, respectivement une patte supérieure 6 en forme de raquette et une patte inférieure 7 en forme de crochet engageable sur le bord inférieur du cotyle.
La patte radiale supérieure 6 en forme de raquette présente quatre perforations 6a disposées en croix pour le passage de vis de fixation, tandis que la patte radiale inférieure 7 en forme de crochet présente une seule perforation 7a pour le passage d'une vis de fixation.
Suivant encore une autre caractéristique visant à améliorer la stabilité de cette cupule dans le cotyle du patient, outre les perforations 4 prévues dans la paroi sphérique de la calotte métallique extérieure 2 pour le passage de vis de fixation 5, sont prévues d'autres perforations 8, deux dans cet exemple, pour le passage de pions cylindro-coniques 9 annelés extérieurement et filetés intérieurement. Le filetage intérieur de ces pions 9 est destiné à permettre au praticien de les mettre en place et de les retirer en cas de nécessité.

Claims (4)

REVENDICATIONS
1. Cupule ancrée pour prothèse totale de hanche, du type non cimenté et non vissé, comprenant, d'une part, une calotte extérieure (2) en un métal biocompatible tel qu'un acier inoxydable ou un alliage de titane ou autre similaire, munie d'au moins une patte radiale de fixation pourvue de perforations pour le passage de vis et dont la paroi sphérique présente une pluralité de trous (4) pour le passage de vis de fixation (5) et, d'autre part, une calotte intérieure (3) en une matière plus douce que le métal, comme une matière plastique biocompatible, telle q'un polyéthylène haute densité, caractérisée en ce que, d'une part, la calotte intérieure (3), qui est engagée dur dans la calotte extérieure (2), présente le long de son bord supérieur et posterieur et sur un secteur d'au moins 1800, un prolongement (3a) formant auvent antiluxation et, d'autre part, de segment annulaire bordant l'ouverture de la calotte intérieure (3) est muni, sur sa face externe, de facettes (3b) lui conférant un contour polygonal tandis que le segment annulaire correspondant de la calotte extérieure (2) présente, sur sa face interne, le même nombre de facettes (2b) lui conférant le même contour polygonal.

Cupule selon la revendication 1, caractérisée en ce que la calotte extérieure métallique (2) est équipée de deux pattes radiales de fixation (6 et 7), diamétralement opposée, dont l'une, inférieure (7), en forme de crochet est engageable sur le bord inférieur du cotyle et dont l'autre, supérieure (6) en forme de raquette, présente quatre perforations (6a) disposées en croix, permettant la mise en place de quatre vis de fixation.
3. Cupule selon la revendication 1 ou 2, caractérisée en ce qu'outre les perforations (4) pour le passage de vis de fixation, la calotte métallique extérieure (2) présente des perforations (8) pour des pions cylindro-coniques (9) annelés extérieurement et filetés intérieurement.
4. Cupule selon l'une quelconque des revendications précédentes, caractérisée en ce que le nombre des côtés des polygones formés par les faces (2b, 3b) des calottes extérieure 2 et intérieure 3 est de douze.
