Description

Estente de válvula cardiaca
La invención se refiere a un estente de válvula cardiaca con una sección instalada para el alojamiento de un implante de válvula cardiaca y con una pluralidad de elementos de anclaje dispuestos próximos.
Tales estentes de válvulas cardíacas se conocen en una pluralidad de configuraciones para la sustitución de válvulas cardiacas configuradas defectuosas o modificadas patológicamente. En este caso, el implante quirúrgico de prótesis de válvulas cardíacas se realiza regularmente en el corazón cardioplégico. La válvula cardiaca antigua, perjudicada en su función es seccionada y se cose la prótesis de válvula cardiaca a implantar.
En presencia de enfermedades de la válvula mitral, en cambio, se trata de mantener en la mayor medida posible la válvula antigua a pesar de la función errónea aparecida, para que no se destruya todo el aparato dinámico de la válvula mitral. Esto se debe a que, por ejemplo, los tendones (chordae tendineae) que se colocan en la válvula mitral son muy importantes para la función ventricular y, por lo tanto, no deberían desprenderse, a ser posible, fuera de la válvula mitral antigua.
Por lo tanto, es ideal desplazar la válvula mitral (cuando no se puede reconstruir) en la mayor medida posible hacia el lado, pata dejar espacio para la prótesis de la válvula. El espacio no juega aquí un papel tan agravante como en el anillo de la aorta, en el que un estrechamiento conduce muy fácilmente a paradas (por ejemplo, en el caso de desplazamiento de la válvula de la aorta antigua con implantación puramente percutánea).
Los tendones (chordae tendineae) de la válvula mitra deberían mantenerse en este caso a ser posible en su estructura, para mantener o bien alcanzar en gran medida la mejor función posible de la cámara izquierda. En este caso, tienen una enorme importancia que la valva de la válvula mitral delantera (anterior) no sea presionada fácilmente en la dirección del espacio libre, sino que se adhiera al anillo de la válvula mitral, para que se impida un desplazamiento hacia delante de la válvula de la válvula mitral anterior en el tracto de flujo de salida del ventrículo izquierdo (fenómeno SAM: Movimiento Sistólico Anterior). Esto es extraordinariamente importante, puesto que de lo contrario se puede producir rápidamente una descompensación del corazón izquierdo (comportamiento defectuoso masivo del ventrículo izquierdo).
Por lo tanto, quirúrgicamente se cose la válvula mitral antigua en el anillo antiguo, de manera que existe un flujo de sangre ininterrumpido a través de la válvula y a través de las dos cámaras cardíacas adyacentes. Después del retroceso (la adhesión de la válvula al anillo) se implanta la prótesis de válvula cardiaca quirúrgicamente en este anillo a través de costura.
Este método costoso se realiza obligatoriamente con la ayuda de la máquina cardioneumónica y regularmente no es cuestionable para pacientes de alto riesgo, de manera que se buscan métodos mínimos invasivos o percutáneos para el implante de una válvula cardiaca.
Así, por ejemplo, se conoce a partir de los documentos DE 195 46 692 C2 o EP 1 469 797 B1 una prótesis de válvula cardiaca auto-expansible para el implante en el cuerpo humano a través de un sistema de catéter con una válvula cardiaca y con un estente comprimible y expansible conectado con la válvula cardiaca. Una prótesis de válvula cardiaca auto-expansible de este tipo se puede conducir con la ayuda de un sistema de catéter a través de una arteria inguinal hasta el lugar del implante en el corazón. Después de alcanzar el lugar del implante se puede desplegar sucesivamente entonces tal estente, que está compuesto en su dirección longitudinal por varios segmentos auto-expansibles plegables relativamente entre sí. Después del despliegue se puede amarrar la prótesis de válvula cardiaca con el apoyo de ganchos de anclaje al menos en la zona próxima al corazón en el vaso sanguíneo respectivo.
Otro dispositivo para la fijación y anclaje de prótesis de válvulas cardiacas se describe en el documento DE 100 10 074 A1, que se forma esencialmente por elementos conectados entre sí en forma de alambre. En este caso, se emplean abrazaderas configuradas de forma diferente para conseguir una fijación y un apoyo seguros de una válvula cardiaca.
Sin embargo, en estas soluciones conocidas existe el peligro de una implantación errónea de válvulas cardiacas debido a posicionamiento defectuoso y a alineación angular errónea de la prótesis de válvulas cardiacas a implantar.
Un posicionamiento mejorado y una alineación angular mejorada se puede conseguir a través del estente conocido a partir del documento EP 1 469 797 B1, en el que están configuradas las llamadas abrazaderas de apoyo, que se pueden insertar en las bolas de la aorta y de esta manera establecen un distanciamiento definido con respecto a la válvula de la aorta. Además, existe también la posibilidad de interrumpir una implantación fijada erróneamente de una prótesis de válvula cardiaca e insertar el estente de válvula de nuevo en el sistema de catéter o bien en el llamado cartucho. En este caso, se pliega entonces todo el estente y se introduce de nuevo en el cartucho. En este caso es posible hacer deslizar el estente de nuevo hacia fuera, de manera que se puede conseguir un buen
posicionamiento para el estente de válvula después de varios intentos de posicionamiento (“sliding technique”).
Además, se conocen también estentes de válvulas cardiacas a partir de los documentos US 2005/075727 A1, US 2007/050020 A1 o US 2003/078652 A1, en los que el estente se alinea y se fija, entre otras cosas, también con hilos de anclaje en el corazón.
No obstante, un problema todavía mayor para el emplazamiento óptimo de la nueva válvula cardiaca en el estente (o bien estente de válvula) consiste en que la válvula nativa antigua no debe retirarse en la mayoría de los casos de la técnica de implantación descrita anteriormente.
Esto conduce, sin embargo, a que la válvula nueva, que se introduce a presión (parcialmente aplastada en el interior) en la válvula antigua deformada, sea modificada en su forma de estente original. Esto se debe a que el lugar de la implantación para el estente de válvula se determina a través de la anatomía, el estado y la naturaleza de la válvula nativa antigua (por ejemplo, en el caso de esclerosos o calcificación de la válvula nativa).
Por lo tanto, el anillo de válvula antiguo con las válvulas/bolsas modificadas de forma correspondiente determina hasta qué punto y hacia donde se puede desplegar la válvula nueva y se puede desarrollar en su forma. Por lo tanto, con ello no sólo es importante el anclaje/posicionamiento, para obtener una función óptima de la válvula así como la función auricular y ventricular, sino también en ajuste del estente de válvula en el neo-anillo (el anillo de válvula antiguo lo configura con válvula antigua) y, por lo tanto, el desplazamiento hacia atrás de la válvula antigua.
Partiendo de los problemas que se plantean en los estantes de válvula cardiaca conocidos, el cometido de la invención es, por lo tanto, crear un estente de válvula cardiaca, en particular un estente de válvula mitral para el transplante invasivo mínimo, que posibilita un ejercicio de la función lo más natural posible del corazón.
De acuerdo con la invención, el cometido se soluciona a través del estente de válvula cardiaca con las características de la reivindicación 1. Las reivindicaciones dependientes reproducen configuraciones ventajosas de la invención.
La idea básica de la invención es crear un estente de válvula cardiaca, que crea las condiciones previas anatómicas para un ejercicio de la función natural, similar a un corazón sano. En este caso, con el estente de válvula cardiaca de acuerdo con la invención en la configuración plegable auto-expansible se posibilita una operación invasiva mínima, que garantiza un posicionamiento exacto y una fijación segura del estente de válvula. En este caso, especialmente en el caso de la configuración del estente de válvula cardiaca de acuerdo con la invención como estente de válvula mitral, se genera una tensión entre la válvula mitral y el ventrículo similar a la tensión de los cordones naturales (los chordae tendineae) y al mismo tiempo se procura que las porciones de la válvula mitral antigua (especialmente de la valva anterior de la válvula mitral) no perturben ya el flujo de paso de la sangre.
Por lo tanto, está previsto que el estente de válvula de acuerdo con la invención sea introducido, apoyado por catéter, a través de una de las cámaras cardiacas o a través de uno de los vasos sanguíneos grandes adyacentes del corazón, que entonces se puede desplegar en una de las cámaras del corazón, siendo fijados sus elementos de anclaje en el tejido. Finalmente, se fija el estente en la pared subvalvular de las cámaras del corazón opuesta al estente, bajo la configuración de una tensión entre la pared de la cámara del corazón y los elementos de anclaje próximos amarrados en posición supravalvular con hilos de anclaje (designados a continuación también como neochordae).
La fijación de los hilos de anclaje en la pared distan de las cámaras del corazón representa, por lo tanto, un contra apoyo para los elementos de anclaje próximos, que se forma, por ejemplo, a través de un nudo o a través de otro elemento que actúa como contra apoyo. Este contra apoyo puede estar configurado con preferencia también como elemento de ajuste de la longitud del hilo.
Las ventajas del estente válvula cardíaca de acuerdo con la invención son, por lo tanto, una fijación exacta y sencilla del estente de válvula cardiaca en una operación invasiva mínima y contractilidad mejorada del corazón frente a los estentes de válvulas convencionales.
Con preferencia, los hilos de anclaje alineados axialmente con relación al eje longitudinal del estente de válvula de acuerdo con la invención se fijan con uno de sus extremos en el anillo del implante de válvula cardiaca, de manera que en el caso de la configuración de la tensión entre el estente y la pared de válvula cardíaca se puede conseguir una acción directa sobre el posicionamiento y la alineación angular de la válvula. Sin embargo, los hilos de anclaje pueden estar conectados también en la periferia distal del estente de válvula con el estente. No obstante, la conexión entre los hilos de anclaje y el estante debe realizarse de tal manera que se puede configurar una tensión, que se extiende esencialmente con relación al eje longitudinal del estente en dirección axial entre elementos próximos del anclaje y e contra apoyo distal.
De acuerdo con otra configuración preferida de la invención, los hilos de anclaje presentan elementos (neo chordae) para el ajuste de la longitud de los hilos de anclaje, con los que se puede ajustar la longitud de los hilos de anclaje
para el ajuste de una tensión determinada entre el estente de la válvula cardiaca y la pared del corazón. En este caso, por ejemplo, puede estar previsto un elemento de ajuste de la longitud de los hilos para cada hilo o por ejemplo un elemento de ajuste de la longitud de los hilos en común para todos los hilos. El elemento de ajuste de la longitud de los hilos está configurado con preferencia pequeño y puede estar ajustado, por ejemplo, de tal forma que el elemento acorta el hilo a la longitud deseada a través de arrollamiento de una longitud excesiva del hilo.
Se prefiere también la configuración de hilos de anclaje elásticos en dirección axial, que pueden reaccionar a movimientos de contracción del corazón, sin que exista una longitud demasiado grande de los hilos, que podría influir negativamente sobre la función cardiaca. En este caso, la longitud de los hilos se puede seleccionar de tal manera que la elasticidad no se consigue totalmente a costa de la tensión entre los elementos de anclaje y la pared del corazón.
Si el contra apoyo está configurado como elemento de ajuste de la longitud de los hilos, resulta una configuración especialmente ventajosa, puesto que de esta manera es posible también un ajuste posterior de la tensión entre los elementos de anclaje y el contra apoyo, es decir, un retensado de los hilos de anclaje, sin que deba abrirse el corazón.
De manera especialmente preferida, la estructura del estente de válvula mitral en el plano del anillo de la válvula mitral está configurada esencialmente ovalada o en forma de U, de tal manera que no se ejerce ninguna presión sobre al LVOT (tracto de flujo de salida del ventrículo izquierdo) y/o sobre el anillo de la aorta y de esta manera se evita un perjuicio de la función cardiaca (Ma L, Tosi P, Huber CH, Taub S, Gerelle G, von Segesser LK. Doublecrowned valved stents for off-pump mitral valve replacement. Eur J. Cardiothorac Surg. Agosto de 2005; 28(2):194-8; discusión 198-9.). También el aparato subvalvular se mantiene completamente en su anatomía natural y no se perjudica (Boudjemline Y, Agnoletti G, Bonnet D, Behr L, Borenstein N, Sidi D, Bonhoeffer P. Steps toward the percutaneous replacement of atrioventricular valves and experimental study. J Am Coll Cardiol. Julio de 2005 19;46(2):360-5).
Este estente de válvula obtiene una forma extraordinariamente ajustada suave, adaptada al aparato de la válvula mitral natural, de manera que este estente de válvula que termina cónicamente (eje craneal-caudal) no totalmente circular (casi ovalado en el eje transversal) se puede apoyar y adaptar muy fuertemente a la forma natural de la válvula mitral. En la zona del anillo anterior de la válvula mitral, el estente de la válvula es plano y no provoca casi ninguna presión ni ningún estrechamiento sobre el LVOT. En la zona del anillo posterior de la válvula mitral es ovalado y reproduce la forma del anillo posterior. Este estente de válvula forma una estructura fina, limitada en la longitud (craneal-caudal), que se adapta en su forma totalmente a la válvula mitral y, por lo tanto, aparece como una copia negativa en la zona del anillo de la válvula mitral natural. El estente de la válvula contacta, en efecto, con la válvula mitral antigua y su anillo, pero los mantiene casi totalmente inalterados en su anatomía.
La invención se explica en detalle a continuación con la ayuda de ejemplos de realización especialmente preferidos, reproducidos en los dibujos adjuntos. En este caso:
La figura 1 muestra un ejemplo de realización preferido del estente de válvula de acuerdo con la invención en una vista lateral esquemática.
La figura 2 muestra el ejemplo de realización mostrado en la figura 1 en vista en planta superior desde arriba.
La figura 3 muestra una vista en planta superior sobre varios estentes de válvula configurados de manera especialmente preferida de acuerdo con la invención.
La figura 4 muestra una vista en planta superior sobre un ejemplo de realización desde abajo.
La figura 5 muestra una vista esquemática para la ilustración del transplante invasivo mínimo de un estente de válvula mitral de acuerdo con la invención en una primera fase durante la introducción del estente de válvula mitral de acuerdo con la invención en el lugar del transplante.
La figura 6 muestra una vista esquemática para la ilustración del transplante invasivo mínimo de un estente de válvula mitral de acuerdo con la invención es una segunda fase después del posicionamiento de la válvula mitral.
La figura 7 muestra una vista esquemática para la ilustración del transplante invasivo mínimo de un estente de válvula mitral de acuerdo con la invención al término de la fijación de los hilos de anclaje fuera del apéndice de la pared cardiaca ventricular izquierda.

Las figuras 1 a 8 muestran el estente de acuerdo con la invención para el implante y fijación de prótesis de válvula cardiaca en diferentes vistas para la ilustración de la estructura del estente y de las relaciones espaciales de las secciones individuales del estente entre sí en el estado desplegado (figuras 1-4 y 6-8) y en el estado plegado (figura 5).
La figura 1 muestra un estente de válvula mitral plegable 10 de acuerdo con la invención en una vista lateral en perspectiva. El estente 10 presenta esencialmente tres secciones: en la sección próxima (superválvula) en el estente 10 están dispuestos de forma circular una pluralidad de elementos de anclaje 20 en forma de dientes o en forma de arco (figura 3), que amarran el estente de válvula supravalvularmente (o bien atrial) en el estado implantado. Dispuesto adyacente a la zona distal se encuentra el cuerpo de estente 30 formado cónico y ovalado en la sección transversal, con preferencia aplanado hacia el LVOT (ver la figura 2).
El cuerpo de estente 30 forma una figura en forma de cesto o trapezoidal, que se adapta al anillo de la válvula mitral y se extiende en la dirección del ventrículo izquierdo. Este estente 10 es retenido en el atrio a través de su forma que termina cónicamente y a través de los elementos de anclaje atriales 20. En este cuerpo de estente 30 puede estar integrada una válvula tricúspide 50.
En el cuerpo del estente 30 se encuentran en la zona distal (hacia el ventrículo izquierdo) unos hilos de anclaje 40, que están instalados en el lado distal de este cuerpo de estente 30 para el anclaje de todo el estente 10. Estos hilos de anclaje 40 proporcionan un anclaje en la pared ventricular opuesta o, por ejemplo, en la zona del músculo papilar (porción próxima, media o distal del músculo papilar); ver las figuras 7 y 8. Los hilos de anclaje 40 se pueden posicionar por medio de un elemento regulable de ajuste de la longitud del hilo 70 y se pueden ajustar con la longitud óptima, de manera que se puede fijar y amarrar entonces el estente de válvula cardiaca 10.
La figura 2 muestra el estente 10 en una vista en planta superior. En este caso se puede reconocer que el estente 10 configura un neo-anillo o bien un cuerpo de estente 30, en el que se puede insertar la prótesis de válvula cardiaca 50 y se puede fijar en él. Además, se puede reconocer que el estente 10 de acuerdo con la invención puede estar configurado asimétrico con relación a una pluralidad de abrazaderas de estente 20 supravalvulares (atriales). Esto se puede reconocer en que el cuerpo de estente 30 está configurado de forma ovalada en este figura y está aplanado en un lado, es decir, que está configurado esencialmente en forma de U, de manera que se puede insertar en este lugar aplanado en la dirección del LVOT. Este aplanamiento provoca que en este lugar no se pueda ejercer ninguna presión sobre el LVOT y la válvula aorta a través del estente auto-expansible, cuando el estente 10 se coloca, por ejemplo en la posición mitral. Otras configuraciones ventajosas del estente 10 de acuerdo con la invención se muestran también en la figura 3.
La figura 4 muestra el estente 10 de acuerdo con la invención en una vista inferior. A partir de ella se deduce claramente que el diámetro se reduce desde la porción atrial hacia la porción ventricular del cuerpo de estente 30, de manera que éste, visto desde el lateral, aparece como un tronco de cono (ver la figura 1). Tanto los elementos de fijación 20 como también el cuerpo de estente 30 pueden estar revestidos con un material (por ejemplo, plástico, Perikard, PTFE o Goretex, etc.), para conseguir una obturación mejorada entre la prótesis de válvula cardiaca 50, el cuerpo de estente 30 y la estructura circundante del corazón. Esta membrana de obturación se inserta o bien se aplica entonces entre la prótesis de la válvula cardiaca 50 y el cuerpo del estente 30 o sobre los tirantes atriales del estente 20 para conseguir una obturación óptima de la válvula entre las dos cámaras cardíacas.
En las figuras 5 a 7 y 8 se describe la implantación transapical retrógrada de un estente de válvula. El recorrido de acceso transaortal retrógrado como también transatrial integrado se pueden realizar de manera alternativa. El emplazamiento del estente de válvula con un estente de válvula plegado sobre el anillo de válvula mitral antiguo se muestra en la figura 5. Una vez realizada la orientación por medio de marcas en el estente de válvula 10 (no mostrado), se puede comenzar lentamente con el despliegue (por ejemplo con preferencia de forma auto-expansiva) de los elementos de anclaje atriales 20. El posicionamiento en el atrio izquierdo debería realizarse de tal forma que el lado aplanado del cuerpo de estente 30 se coloca en la dirección del LVOT (válvula de aorta). Entonces se expande adicionalmente el estente.
La figura 6 muestra el estente de válvula 10 expandido en el tracto de flujo de entrada atrio ventricular izquierdo. Los hilos de anclaje 40 son ajustados junto, en o fuera de la pared del corazón y posteriormente son fijados- como se representa en la figura 7 – con la ayuda del contra apoyo 80 configurado con preferencia como elemento de ajuste de la longitud del hilo. Durante el ajuste de la longitud del hilo de anclaje 40 se lleva a cabo, entre otras cosas, también una visualización del aparato de la válvula mitral (por ejemplo, Echo, CT y NMR),. De manera que el anillo se puede estirar por medio del estente nuevo 10 de una manera óptima en la dirección de la paren ventricular, no existe ya ninguna fuga paravalvular, el estente 10 está bien fijado y el anillo y aparato de la válvula mitral apoyan de
5 manera ventajosa la función del ventrículo izquierdo.
De manera alternativa a la figura 7, los hilos de anclaje 40 se pueden fijar también en los músculos papilares (ver la figura 8, de manera que estos hilos 40 representan neo-chordae y pueden asumir la función de los chordae tendineae puestos fuera de función. La fijación de los hilos de anclaje 40 en la pared del corazón se realiza en cada caso por medio de un contra apoyo 80, que puede estar configurado como nudo o también como elemento
10 autónomo. También es posible que los hilos de anclaje ventriculares 40 sean colocados no sólo en el cuerpo de estante 30, sino también en la válvula integrada propiamente dicha. También estas superficies de anclaje caudales 40 se pueden fijar en cualquier otro punto en el ventrículo.
La figura 7 muestra el posicionamiento terminado y la fijación del estente 10. Después de ajustar la longitud y después de la determinación de la posición de los hilos de anclaje 40 individuales, éstos son fijados por medio del
15 elemento de ajuste de la longitud del hilo 70 regulable, por ejemplo en la pared ventricular izquierda. El elemento de ajuste de la longitud del hilo 70 sirve para el ajuste óptimo de la longitud y de la posición del estente de válvula 10 y, por lo tanto, de la prótesis de válvula 50. Hilos 40 diferentes pueden presentar diferentes longitudes y diferentes posiciones de fijación en el ventrículo.
Las figuras 9 y 11 muestran otros ejemplos para el empleo del estente de válvula cardiaca 10, donde el estente 10
20 está adaptado a la anatomía respectiva (para la posición en la válvula de la aorta y de la válvula pulmonar es adecuada más bien una forma circular (ver la figura 3) y para la posición tricúspide es adecuada más bien una forma ovalada).
La figura 12 muestra un estente de válvula en una vista lateral esquemática, que se representa para mayor claridad sin válvula cardiaca y sin hilos de anclaje.
25 Para la ilustración de la posición del estente de válvula de la figura 12 en su lugar, la figura 13 muestra una vista esquemática, doral, intercardial de un estente de válvula cardiaca fijado en la posición mitral. Prestar atención a la buena adaptación del estente de válvula al entorno atrial izquierdo. De esta manera, se evitan las distancias entre la pared atrial izquierda / anillo atrial y el estente de válvula. La válvula cardiaca y los hilos de anclaje hacia la punta ventricular se han omitido para la simplificación de la representación.

Claims (6)

  1. REIVINDICACIONES
    1.-Estente de válvula cardiaca (10) con una sección instalada para el alojamiento de un implante de válvula mitral
    (30) y con una pluralidad de elementos de anclaje (20) dispuestos próximos, y con una pluralidad de hilos de anclaje (40), que están fijados con uno de sus extremos en el estente (10), y con un contra apoyo (80) que fija los hilos de
    5 anclaje (40) con su otro extremo en la pared distal de la cámara cardiaca bajo la configuración de una tensión entre la pared de la cámara cardiaca y los elementos de anclaje (20) amarrados próximos, caracterizado porque el estente de válvula mitral (10) está configurado como tronco de cono.
  2. 2.-Estente de válvula cardiaca (10) de acuerdo con la reivindicación 1, caracterizado porque los hilos de anclaje (40) están fijados en el anillo del implante de válvula mitral (30).
    10 3.- Estente de válvula cardiaca (10) de acuerdo con una de las reivindicaciones anteriores, caracterizado por al menos un elemento de ajuste de la longitud del hilo (70) instalado para el ajuste de la longitud de al menos un hilo de anclaje (40).
  3. 4.- Estente de válvula cardiaca (10) de acuerdo con una de las reivindicaciones anteriores, caracterizado porque el contra apoyo (80) está configurado como elemento de ajuste de la longitud del hilo.
    15 5.-Estente de válvula cardiaca (10) de acuerdo con la reivindicación 4, caracterizado porque el estente de válvula cardiaca (10) está configurado en el plano del anillo de válvula mitral esencialmente ovalado o en forma de U.
  4. 6.- Estente de válvula cardiaca (10) de acuerdo con una de las reivindicaciones anteriores, caracterizado porque entre el implante de válvula cardiaca (30) y el estente (10) está dispuesta una membrana de obturación.
  5. 7.-Estente de válvula cardiaca (10) de acuerdo con una de las reivindicaciones anteriores, caracterizado porque
    20 entre los elementos de anclaje (20) y el implante de válvula cardiaca (30) está dispuesta una membrana de obturación.
  6. 8.- Estente de válvula cardiaca (10) de acuerdo con una de las reivindicaciones anteriores, caracterizado porque sal menos un elemento de anclaje (20) está constituido de una aleación de memoria de forma.
