Abstract
Una grapadora quirúrgica (10), que comprende: un efector final (20) que tiene un brazo del cartucho (24) para contener al menos una grapa y un yunque (22) acoplado giratoriamente al brazo del cartucho (24) y eficaz para deformar la al menos una grapa, caracterizado por un soporte (30) acoplado al efector final (20) y que puede moverse entre una posición proximal en la que el yunque (22) y el cartucho (24) pueden girar entre sí y una posición distal en la que el soporte (30) conecta con al menos en una parte media del efector final (20) para evitar la desviación del yunque (22) respecto al brazo del cartucho (24) cuando al menos una grapa se dirige desde el brazo del cartucho (24) y se deforma contra el yunque (22).
Description
Grapadora quirúrgica con un soporte efector
final.
La presente invención se refiere en líneas
generales a dispositivos quirúrgicos y, en particular, a una
grapadora y/o cortadora quirúrgica que tiene un soporte para un
efector final.
Los instrumentos quirúrgicos endoscópicos y
laparoscópicos a menudo se prefieren respecto a los dispositivos
quirúrgicos abiertos tradicionales, ya que el uso de orificios
naturales (endoscópicos) o incisiones más pequeñas (laparoscópicos)
tiende a reducir el tiempo de recuperación postoperatorio y provocan
complicaciones.
En consecuencia, se ha realizado un desarrollo
significativo en diversos instrumentos quirúrgicos endoscópicos y
laparoscópicos que son adecuados para la colocación precisa de un
efector final distal en un sitio quirúrgico deseado. Los efectores
finales distales conectan el tejido de numerosas maneras para
conseguir un efecto diagnóstico o terapéutico (por ejemplo,
endocortadora, mordaza, cortadora, grapadoras, aplicadores de
pinzas, dispositivo de acceso, dispositivo de suministro de
fármaco/terapia génica y dispositivo energético que usa
ultrasonidos, RF, láser, etc.).
Típicamente, las grapadoras quirúrgicas
endoscópicas y laparoscópicas tienen un mecanismo de grapado, el
efector final, que está adaptado para ajustarse a través de una
cánula para situar al efector final respecto al tejido a grapar. El
efector final tiene un brazo del cartucho que contiene un cartucho
con grapas y un yunque que está conectado giratoriamente al brazo
del cartucho. El brazo del cartucho y el yunque actúa cada uno como
una mordaza separada para cerrar y mantener unido el tejido a
grapar. Tras el accionamiento del cartucho, las grapas se dirigen
desde el brazo del cartucho a través del tejido y contra los surcos
formadores de grapa formados en el yunque y alineados con las grapas
del brazo del cartucho, formando de esta manera las grapas que
mantienen el tejido unido.
El documento EP1400206 describe un dispositivo
quirúrgico para manipular tejido. El dispositivo incluye un árbol
alargado que tiene un extremo proximal y un extremo distal que se
extiende desde el mismo. Hay también un efector final alargado que
tiene extremo proximal unido al extremo distal del árbol,
extendiéndose desde el mismo un extremo distal. El efector final
tiene primera y segunda mordazas opuestas que pueden moverse una con
respecto a la otra de una posición abierta, en la que las mordazas
están espaciadas hasta una posición cerrada en la que las mordazas
están en una aproximación cercana entre sí. Al menos una de las
mordazas tiene un miembro expansible dispuesto sobre la misma para
aumentar selectivamente el tamaño de la mordaza en una dirección
perpendicular al eje longitudinal de manera que se aumenta la
rigidez del efector final.
Cuando una grapadora dispara simultáneamente
múltiples grapas en una disposición lineal, puede ocurrir la
desviación del yunque. En particular, la fuerza requerida para
dirigir las grapas a lo largo de una longitud no soportada
relativamente larga del yunque puede crear un par de torsión
sustancial en el extremo distal del yunque, que da como resultado
una desviación. Esto puede dar como resultado grapas mal formadas,
especialmente hacia el extremo distal de los efectores finales
relativamente largos. Como los procedimientos endoscópicos o
laparoscópicos utilizan pequeños orificios o puertos de acceso, el
espesor o diámetro externo del efector final está necesariamente
limitado. Incluso con el uso de materiales muy tenaces para el
yunque (por ejemplo, acero o titanio) la desviación aún no es
observable tras el accionamiento de la grapadora.
Por consiguiente, existe una necesidad de
dispositivos mejorados para grapar y/o cortar tejido.
La presente invención proporciona en general
dispositivos para soportar un yunque sobre una grapadora y/o
cortadora quirúrgica que comprende las características de la
reivindicación 1. Las realizaciones preferidas se indican en las
reivindicaciones dependientes.
En una realización la grapadora quirúrgica puede
incluir un efector final que tiene un brazo del cartucho que
contiene una o más grapas y un yunque acoplado giratoriamente al
brazo del cartucho y eficaz para deformar las grapas. El dispositivo
puede incluir también un soporte adaptado para contactar con el
efector final y reducir la desviación del yunque cuando una o más
grapas se dirigen desde el brazo del cartucho y se deforman contra
el yunque. En una realización ejemplar, el brazo del cartucho puede
adaptarse para mantener una pluralidad de grapas en al menos una
fila lineal. El efector final y el soporte, en otras realizaciones,
pueden tener también un diámetro externo máximo que es menor de
aproximadamente 16 mm cuando el soporte está en una posición plegada
para permitir que el efector final se ajuste dentro de pequeños
orificios o cánulas.
El soporte puede tener diversas configuraciones,
aunque en una realización puede estar adaptado para contactar con el
yunque en una localización distal respecto al punto de giro. Por
ejemplo, el soporte puede acoplarse de forma deslizable al yunque y
moverse entre una posición proximal en la que el yunque puede
moverse alrededor del punto de giro y una posición distal en la que
el soporte evita que el yunque se mueva alrededor del punto de giro.
En una realización ejemplar, el soporte puede incluir al menos dos
uniones acopladas giratoriamente entre sí, pudiendo una de las
uniones acoplarse de forma deslizable al yunque. En particular, el
soporte puede incluir una unión distal acoplada de forma deslizable
al yunque, una unión proximal acoplada giratoriamente a un extremo
de la unión distal y en un extremo opuesto a un árbol alargado que
se extiende proximalmente desde el brazo del cartucho y una unión
media acoplada giratoriamente en un extremo del árbol alargado y
acoplada giratoriamente en un extremo opuesto a la unión distal y
la unión proximal. En otra realización, el soporte puede estar en
forma de un manguito que puede moverse entre una primera posición en
la que el yunque puede girar respecto al brazo del cartucho y la
segunda posición en la que se evita que el yunque gire respecto al
brazo del cartucho. En otras realizaciones el dispositivo puede
incluir un miembro de retén para limitar el movimiento distal del
soporte. Por ejemplo, el yunque puede incluir al menos una ranura
alargada para recibir de forma deslizable al menos un miembro de
clavija formado sobre el soporte. La ranura puede terminar en una
parte sustancialmente media del yunque para limitar el movimiento
distal del soporte.
La invención se entenderá mejor a partir de la
siguiente descripción detallada tomada junto con los dibujos
adjuntos, en los que:

En general, se proporciona una grapadora y/o
cortadora quirúrgica que tiene un efector final para suministrar
grapas al tejido, y un soporte para evitar la desviación del efector
final. En particular, el efector final puede incluir un brazo del
cartucho adaptado para contener grapas y un yunque acoplado
giratoriamente al brazo del cartucho y adaptado para deformar las
grapas dirigidas desde el brazo del cartucho. El soporte puede estar
acoplado al efector final y puede moverse entre una primera posición
plegada en la que el brazo del yunque puede girar respecto al brazo
del cartucho y una segunda posición extendida en la que el soporte
se conecta con el yunque para limitar sustancialmente la desviación
del yunque cuando una o más grapas se dirigen desde el brazo del
cartucho y se deforman contra el yunque. El soporte es
particularmente eficaz para usar con grapadoras y/o cortadoras
quirúrgicas lineales que tienen un efector final relativamente largo
para suministrar una o más filas lineales o líneas de grapas, puesto
que el soporte puede evitar sustancialmente la desviación de un
extremo distal del yunque durante la aplicación de líneas de grapas
largas. Por ejemplo, la grapadora quirúrgica puede tener un efector
final que tenga una longitud que está en el intervalo de
aproximadamente 60 mm a 90 mm. El soporte también es particularmente
eficaz para usar procedimientos endoscópicos y laparoscópicos,
puesto que el soporte puede ser capaz de plegarse para permitir que
el efector final y el soporte se introduzcan a través de orificios
naturales (por ejemplo, la garganta) o a través de pequeños puertos
de acceso tales como cánulas u otras rutas pequeñas. Por ejemplo, en
una realización ejemplar, el efector final y el soporte pueden tener
un diámetro externo máximo que es menor de aproximadamente 16 mm
cuando el soporte está en una posición plegada para permitir que el
soporte se inserte a través de rutas que tienen un diámetro de
aproximadamente 16 mm o menor. Una persona especialista en la
técnica entenderá que el soporte puede usarse con diversos
dispositivos de suministro de sujeción y que la grapadora lineal
descrita en este documento simplemente se muestra con fines de
referencia.
La Figura 1 ilustra una realización ejemplar de
la grapadora quirúrgica 10 que tiene un soporte 30 para limitar o
prevenir la desviación de un yunque. Como se muestra, el dispositivo
10 generalmente incluye un mango 14 que tiene un árbol alargado 12
que se extiende desde el mismo e incluye extremos proximal y distal
12a, 12b. Se muestra una carcasa del árbol 12 dispuesta sobre una
parte del árbol alargado 12. Un ensamblaje de aplicación de grapas o
efector final 20 se forma sobre el extremo distal 12b del árbol
alargado 12. El efector final 20 incluye brazos opuestos,
denominados brazo del cartucho 24 y yunque 22, que están acoplados
giratoriamente entre sí en sus extremos proximales mediante un punto
de giro 26 y que están adaptados para recibir tejido entre ellos. El
brazo del cartucho 24 está adaptado para contener un cartucho de
grapas que tiene múltiples grapas dispuestas en su interior y que
está configurado para dirigirse hacia el tejido y el yunque 22
incluye una pluralidad de surcos formados en su interior para
deformar las grapas. El dispositivo 10 puede incluir también uno o
más gatillos acoplados al mismo para accionar el efector final 20,
por ejemplo para abrir y cerrar mandíbulas opuestas del efector
final y/o dirigir una cuchilla de corte a través del efector final
20. La Figura 1 ilustra un primer gatillo 14a acoplado de forma
móvil al mango 14 para cerrar las mandíbulas opuestas del efector
final 20. El mango 14 incluye también un segundo gatillo 14b
acoplado de forma móvil al mismo para disparar el cartucho de grapas
para suministrar una o más grapas al tejido. El segundo gatillo 14b
puede ser también eficaz para hacer avanzar una cuchilla distalmente
a través del cartucho de grapas para cortar el tejido grapado.
Aunque se muestran dos gatillos 14a, 14b, el mango 14 puede incluir
adicional o alternativamente otros mecanismos de accionamiento tales
como un tirador rotatorio, una palanca, un tirador deslizante,
etc., para accionar el efector final 20. Una persona especialista en
la técnica entenderá que el dispositivo puede incluir otras diversas
características no descritas en este documento. Por ejemplo, el
dispositivo puede incluir una junta articulada formada entre el
árbol alargado 12 y el efector final 20 para permitir el movimiento
angular del efector final 20 respecto al árbol alargado 12. Puede
proporcionarse un gatillo u otro mecanismo al mango para ajustar
angularmente el efector final 20.
El soporte 30 se muestra con más detalle en las
Figuras 2A-2D. Como se muestra, el soporte 30 está
en forma de un ensamblaje de unión que está acoplado de forma móvil
entre el árbol alargado 12 y el yunque 22. En particular, el soporte
30 incluye una unión proximal 32, una unión distal 34 y una unión
media 36 que están acopladas de forma giratoria entre sí y que
pueden moverse respecto al árbol alargado 12 y el efector final 30.
La unión proximal 32 tiene un extremo proximal 32 (Figura 2D) que es
giratoria y que está acoplada de forma deslizable a una parte del
árbol alargado 12 y a un extremo final 32b que está acoplado
giratoriamente a un extremo proximal 34a de la unión distal 34. La
unión distal 34 se extiende distalmente desde el extremo distal 32b
de la unión proximal 32 e incluye un extremo distal 34b que está
acoplado giratoria y deslizablemente al yunque 22. La conexión
deslizante y giratoria entre el extremo proximal 32a de la unión
proximal 32 y el árbol alargado 12b, y entre el extremo distal 34b
de la unión distal 34 y el yunque 22, puede formarse usando diversas
técnicas. Por ejemplo, la unión proximal 32 puede acoplarse a una
parte del árbol alargado 12 que se desliza, por ejemplo una varilla
de empuje deslizante 50 (Figura 2D), y que permite el movimiento
giratorio de la unión proximal 32 respecto a la misma. La unión
distal 34 puede incluir un miembro de clavija que se desliza dentro
de una ranura 22c formada en el yunque y que permite el movimiento
giratorio de la unión distal 34a respecto al yunque 22. Como se ha
indicado anteriormente, el soporte 30 puede incluir también una
unión media 36. Como se muestra en las Figuras 2C y 2D, la unión
media 36 está acoplada giratoriamente a un primer extremo 36a del
extremo distal 32b de la unión proximal 32 y al extremo proximal 34a
de la unión distal 34, estando acopladas estas tres dichas uniones
32, 34, 36 giratoriamente entre sí en una parte media del soporte
30. El segundo extremo 36b de la unión media 36 está acoplado
giratoriamente al árbol alargado 12 en un punto de giro fijo 36c, de
manera que la unión media 36 no puede deslizarse respecto al árbol
alargado 12.
Durante el uso, la conexión entre las uniones
32, 34, 36 permite que el soporte 30 se mueva entre una posición
proximal plegada, en la que el yunque 22 puede girar respecto al
brazo del cartucho 24 y una posición distal extendida, en la que el
soporte 30 es eficaz para limitar o prevenir la desviación del
yunque 22 cuando una o más grapas se dirigen desde el brazo del
cartucho 24 y se deforman contra el yunque 22. La posición proximal
plegada se muestra en las Figuras 2A y 2B y, como se muestra las
uniones 32, 34, y 36, está alineada esencialmente axialmente con el
efector final 20 de manera que el diámetro externo máximo del árbol
alargado 12, el efector final 20 y el soporte 30 es sustancialmente
constante a lo largo de una longitud del mismo para permitir que una
parte distal del dispositivo 10 se introduzca a través de una
trayectoria pequeña, por ejemplo de 16 mm o menor de diámetro. En
particular, la unión proximal 32 está totalmente replegada en una
dirección proximal, por ejemplo usando un gatillo u otro mecanismo
formado sobre el mango de la grapadora como se analizará con más
detalle a continuación, para tirar de la unión distal 34 en una
dirección proximal de manera que el extremo distal 34b de la unión
distal 34 esté en una posición más proximal respecto al yunque 22.
El repliegue proximal de la unión proximal 32 hace girar también el
primer extremo 36a de la unión media 36 (no mostrado en las Figuras
2A y 2B) en una dirección proximal de manera que la unión media 36
se recibe dentro de una abertura formada en el árbol alargado 12. En
esta posición, el yunque 22 es libre de abrirse y cerrarse respecto
al brazo del cartucho 24. La Figura 2A ilustra el yunque 22 en una
posición abierta para recibir tejido a través del mismo y la Figura
2B ilustra el yunque 22 en una posición cerrada para conectar tejido
a través del mismo. Puede usarse un mecanismo de cierre diferente
para mover el yunque 22 a la posición cerrada mostrada en la Figura
2B o, como alternativa, el soporte 30 puede usarse para impulsar el
yunque 22 hacia el brazo del cartucho 24 cuando el soporte 30 se
mueve hacia la posición distal extendida.
El soporte 30 se muestra en la posición distal
extendida en las Figuras 2C y 2D. Como se muestra, la unión proximal
32 se hace avanzar, por ejemplo usando un gatillo u otro mecanismo
formado en el mango de la grapadora como se analizará con más
detalle a continuación, en una dirección distal hacia el efector
final 20. El movimiento distal de la unión proximal 32 provocará que
el primer extremo 36a de la unión media 36 gire alrededor del punto
de giro fijo 36c formado entre el segundo extremo 36b de la unión
media 36 y el árbol alargado 12. Como el primer extremo 36a de la
unión media 36 se mueve lejos del árbol alargado 12 y gira alrededor
del punto de giro fijo 36c, el extremo proximal 34a de la unión
distal 34 se moverá igualmente lejos del mismo, es decir, girará
respecto al árbol alargado 12 y el extremo distal 34b de la unión
distal 34 se deslizará distalmente dentro de la ranura alargada 22c
formada en el yunque 22. Cuando la unión distal 34 alcanza una
posición más distal, la fuerza conferida a la unión proximal 32 ya
no hace avanzar más la unión distal 34 si no que en lugar de ello da
como resultado una fuerza hacia abajo aplicada por la unión distal
34 al yunque 22 para limitar o prevenir la desviación del yunque 22
respecto al brazo del cartucho 24 cuando las grapas se están
dirigiendo desde el brazo del cartucho 24 contra el yunque 22.
Aunque el soporte 30 puede conectar con
cualquier parte del yunque 22, en una realización ejemplar, el
soporte 30 puede adaptarse para conectarse a una parte
sustancialmente media del yunque 22. Dicha configuración puede
asegurar que la unión distal 34 aplica una fuerza hacia abajo sobre
el yunque 22, que puede ser opcionalmente eficaz para mover el
yunque a la posición cerrada. La conexión de la parte media del
yunque 22 puede conseguirse usando, por ejemplo, un miembro de retén
para limitar el movimiento distal del soporte 30 respecto al yunque
22. En la realización mostrada en las Figuras 2C y 2D, un punto
final 22 de la ranura alargada 22c formada en el yunque 22 funciona
como un miembro de retén, a medida que la ranura 22c termina próxima
al extremo más distal del yunque 22. De esta manera, el punto final
22d evitará que la unión distal 34 se deslice distalmente más allá
del extremo más distal de la ranura 22c, causando de esta manera que
la unión distal 34 se conecte con la parte media del yunque 22. El
punto final de la ranura puede ajustarse también usando una clavija
de retén móvil. La ranura puede incluir numerosas posiciones en las
que una clavija de retén puede insertarse, y la posición de la
clavija de retén puede determinar la extensión en la que la unión
distal puede deslizarse hacia el extremo distal del yunque antes de
que no ocurra deslizamiento adicional. Una persona especialista en
la técnica entenderá que pueden usarse otras diversas técnicas para
crear retenes u otros mecanismos que limiten el movimiento distal
del soporte.
Como se ha indicado previamente, pueden usarse
otras diversas técnicas para mover el soporte 30 entre las
posiciones proximal y distal. Por ejemplo, en una realización la
unión proximal puede acoplarse giratoriamente a una varilla de
empuje 50 (Figura 2D) que se extiende a través del árbol alargado 12
y está adaptada para moverse deslizablemente respecto al árbol
alargado 12 para hacer avanzar y replegar la unión proximal 32,
moviendo de esta manera el soporte 30 entre las posiciones proximal
y distal. Puede proporcionarse una palanca, gatillo u otro mecanismo
formado en el mango del dispositivo para efectuar el movimiento de
la varilla de empuje 50. Una persona especialista en la técnica
entenderá que el movimiento de soporte y/o el giro del yunque y el
brazo del cartucho puede conseguirse simultáneamente o
independientemente entre sí e independientemente de disparar las
grapas desde el brazo del cartucho 34.
El soporte puede tener también otras diversas
configuraciones para evitar la desviación de un yunque sobre una
grapadora quirúrgica. En otra realización, como se muestra en las
Figuras 3A-3C, el soporte puede estar en forma de
manguito que puede moverse entre una primera posición y una segunda
posición respecto al efector final. Como se muestra, la grapadora
quirúrgica 300 generalmente incluye un árbol alargado 310 que tiene
un efector final formado sobre un extremo distal del mismo y que
incluye un yunque 320 y un brazo del cartucho 330 acoplado
giratoriamente al mismo. Un manguito de 340 se dispone de forma
deslizable dentro de la carcasa externa del árbol alargado 310 y
puede moverse entre una posición proximal replegada (mostrada en la
Figura 3A y una posición distal extendida (mostrada en la Figura
3B). El manguito 340 puede tener diversas formas y tamaños y puede
ser sustancialmente tubular o, como se muestra en las Figuras 3A y
3B, puede tener una forma generalmente alargada con una sección
transversal sustancialmente semicircular. Cuando el manguito 340
está en la posición proximal, como se muestra en la Figura 3A, el
manguito 340 está totalmente replegado respecto al yunque 320 de
manera que permite que el yunque 320 gire respecto al brazo del
cartucho 330. Cuando el manguito 340 se hace avanzar a la posición
distal, el manguito 340 se desliza sobre y contacta con el yunque
320 en una localización distal respecto al punto de giro 350,
impidiendo de esta manera o evitando el movimiento giratorio entre
el yunque 320 y el brazo del cartucho 330. Por consiguiente, cuando
se dispara una grapa, el manguito 340 limita o evita la desviación
del yunque 320.
Como con la realización mostrada las Figuras
2A-2D, el movimiento del manguito del soporte 340
puede conseguirse de diversas maneras. En una realización, como se
muestra en la Figura 3, el mango 370 del dispositivo 300 incluye un
botón de deslizamiento 360 formado sobre el mismo y acoplado a un
extremo proximal del manguito 340. De esta manera, el movimiento del
botón de deslizamiento 360 entre las posiciones proximal y distal es
eficaz para mover el manguito 340 entre las posiciones proximal y
distal.
Una persona especialista en la técnica entenderá
que el manguito puede tener otras diversas configuraciones y no
necesariamente moverse entre una posición replegada y extendida. Por
ejemplo un manguito puede hacerse girar entre una primera posición
en la que el manguito es adyacente al brazo del cartucho y permite
girar el yunque y un brazo del cartucho y una segunda posición en la
que el manguito está adyacente al yunque y limita o previene que el
yunque gire con respecto al brazo del cartucho.
Un especialista en la técnica entenderá otras
características y ventajas adicionales de la invención basándose en
las realizaciones descritas anteriormente. Por consiguiente, la
invención no tiene que limitarse por lo que se ha mostrado y
descrito particularmente, excepto lo indicado en las
reivindicaciones adjuntas.
Claims (9)
1. Una grapadora quirúrgica (10), que
comprende:
un efector final (20) que tiene un brazo del
cartucho (24) para contener al menos una grapa y un yunque (22)
acoplado giratoriamente al brazo del cartucho (24) y eficaz para
deformar la al menos una grapa,
caracterizado por
un soporte (30) acoplado al efector final (20) y
que puede moverse entre una posición proximal en la que el yunque
(22) y el cartucho (24) pueden girar entre sí y una posición distal
en la que el soporte (30) conecta con al menos en una parte media
del efector final (20) para evitar la desviación del yunque (22)
respecto al brazo del cartucho (24) cuando al menos una grapa se
dirige desde el brazo del cartucho (24) y se deforma contra el
yunque (22).
2. La grapadora quirúrgica (10) de la
reivindicación 1, en la que el soporte (30) está alineado axialmente
con el efector final (20) cuando el soporte (30) está en la posición
proximal y en la que el soporte (30) se extiende sustancialmente
transversalmente respecto al efector final (20) cuando el soporte
(30) está en la posición distal.
3. La grapadora quirúrgica (10) de la
reivindicación 1, en la que el soporte (30) comprende una unión (34)
acoplada de forma deslizable al yunque (22).
4. La grapadora quirúrgica (10) de la
reivindicación 3, en la que la unión (34) incluye una unión proximal
(32) acoplada giratoriamente a un árbol alargado (12) que se
extiende proximalmente desde el efector final (20) y una unión
distal (34) acoplada giratoriamente en un extremo a la unión
proximal (32) y en un extremo opuesto al yunque (22).
5. La grapadora quirúrgica (10) de la
reivindicación 4, en la que la unión distal (34) está acoplada al
yunque (22) en una posición distal respecto a un punto de giro (26)
formada entre el yunque (22) y el brazo del cartucho (24).
6. La grapadora quirúrgica (300) de la
reivindicación 1, en la que el soporte comprende un manguito (340)
que puede acoplarse de forma deslizable sobre al menos una parte del
yunque (320).
7. La grapadora quirúrgica (10) de la
reivindicación 1 que comprende adicionalmente un accionador acoplado
al efector final (20) y adaptado para impulsar el yunque (22) hacia
el brazo del cartucho (24).
8. La grapadora quirúrgica (10) de la
reivindicación 1, en la que el soporte (30) está adaptado para
impulsar el yunque (22) hacia el brazo del cartucho (24).
9. La grapadora quirúrgica (10) de la
reivindicación 1, en la que un extremo proximal del yunque que está
acoplado al brazo del cartucho en un punto de giro (26) y al soporte
(30) puede moverse entre una posición proximal en la que el yunque
(22) puede moverse alrededor del punto de giro (26) y una posición
distal en la que el soporte (30) evita que el yunque (22) se mueva
alrededor del punto de giro (26).
