Relatório Descritivo da Patente de Invenção para GRAMPEADOR CIRÚRGICO COM SUPORTE PARA EFEITO DE EXTREMIDADE.
Campo da Invenção [001] A presente invenção refere-se, de forma ampla, a dispositivos cirúrgicos, e em particular a um grampeador e/ou cortador cirúrgico possuindo um suporte para um efeito de extremidade.
Fundamentos da Invenção [002] Instrumentos cirúrgicos endoscópicos e laparoscópicos são frequentemente preferidos ao invés de dispositivos cirúrgicos abertos tradicionais visto que o uso de orifícios naturais (endoscópicos) ou incisões menores (laparoscópicos) tende a reduzir o tempo e as complicações de pós-operatório. Consequentemente, desenvolvimentos significativos adentraram a faixa de instrumentos cirúrgicos endoscópicos e laparoscópicos que são adequados para a colocação precisa de um efeito de extremidade distal em um local cirúrgico desejado. Esses efeitos de extremidade distal engatam o tecido de várias formas para alcançar um efeito diagnóstico ou terapêutico (por exemplo, endocortador, agarre, cortador, grampeadores, aplicador de prendedor, dispositivo de acesso, dispositivo de distribuição de terapia de drogas/gene, e dispositivo de energia utilizando ultra-som, RF, laser, etc.).
[003] Tipicamente, os grampeadores cirúrgicos endoscópicos e laparoscópicos possuem um mecanismo de grampear, o efeito de extremidade, que é adaptado para encaixar através de uma cânula para posicionar o efeito de extremidade com relação ao tecido a ser grampeado. O efeito de extremidade possui um braço de cartucho contendo um cartucho com grampos, e uma placa de apoio que é conectada de forma articulada ao braço do cartucho. [004] Quando um grampeador dispara simultaneamente múltiplos grampos em uma disposição linear, a deformação da placa de apoio pode ocorrer. Em particular, a força necessária para se acionar os grampos juntamente com um comprimento não suportado relativamente longo da placa de apoio pode criar um torque substancial na extremidade distal da placa de apoio, resultando em deformação. Isso pode resultar em grampos mal formados, especialmente na direção da extremidade distal dos efeitos de extremidade relativamente longos. Visto que os procedimentos endoscópicos ou laparoscópicos utilizam pequenos orifícios ou portas de acesso, a espessura ou diâmetro externo do efeito de extremidade é necessariamente limitado. Sumário da Invenção [006] A presente invenção fornece geralmente métodos e dispositivos para suportar uma placa de apoio em um grampeador e/ou cortador cirúrgico. Em uma modalidade, o grampeador cirúrgico pode incluir um efeito de extremidade que possui um braço de cartucho para reter um ou mais grampos, e um aplaca de apoio acoplada de forma articulada ao braço de cartucho e eficiente para deformar os grampos. O dispositivo também pode incluir um suporte adaptado para contatar o efeito de extremidade e reduzir a deformação da placa de apoio quando um ou mais grampos são acionados a partir do braço de cartucho e

O efeito de extremidade e o suporte também podem, em outras modalidades, ter um diâmetro externo máximo que seja inferior a cerca de 16 mm. quando o suporte está em uma posição desmontada para permitir que o efeito de extremidade encaixe dentro de orifícios ou cânulas pequenos.
[007] O suporte pode ter uma variedade de configurações, mas em uma modalidade pode ser adaptado para contatar a placa de apoio em uma localização distai com relação ao ponto de articulação. [008] Conforme indicado previamente uma variedade de técnicas pode ser usada para mover o suporte 30 entre as posições proximal e distal. Por exemplo, em uma modalidade a conexão proximal pode ser acoplada de modo articulado a uma vareta impulsora 50 (FIG.2D) que s estende através do eixo alongado 12 e adaptada para mover-se de maneira deslizante em relação ao eixo alongado 12 para avançar ou retrair a conexão proximal 32, movendo por este meio o suporte 30 entre as posições proximal e distal. Uma alavanca, gatilho, ou outro mecanismo formado no cabo do dispositivo pode ser provido para efetuar o movimento da vareta impulsionadora 50. [009] Métodos ilustrativos para se grampear o tecido também são fornecidos. Em uma modalidade, o método pode incluir o posicionamento do tecido entre um braço de cartucho e uma placa de apoio de um grampeador cirúrgico, e o avanço de um suporte com relação à placa de apoio para impedir a deformação da placa de apoio com relação ao braço de cartucho quando um ou mais grampos são acionados do cartucho, através do tecido posicionado entre a placa de apoio e o braço de cartucho, e são deformados contra a placa de apoio para grampear o tecido.

Descrição Detalhada da Invenção [0011] Em geral, o grampeador e/ou cortador cirúrgico é fornecido possuindo um efeito de extremidade para distribuir grampos para o tecido, e um suporte para impedir a deformação do efeito de extremidade. Em particular, o efeito de extremidade pode incluir um braço de cartucho adaptado para reter os grampos, e uma placa de apoio acoplada de forma articulada ao braço de cartucho e adaptada para deformar os grampos acionados a partir do braço de cartucho. O suporte pode ser acoplado ao efeito de extremidade e pode ser movido entre uma primeira posição desmontada na qual o braço da placa de apoio pode articular com relação ao braço de cartucho, e uma segunda posição estendida na qual o suporte engata a placa de apoio para limitar substancialmente a deformação da placa de apoio quando um ou mais grampos são acionados a partir do braço de cartucho e deformados contra a placa de apoio. O suporte é particularmente eficiente para uso com grampeadores e/ou cortadores cirúrgicos lineares possuindo um efeito de extremidade relativamente longo para distribuição de uma ou mais fileiras lineares ou linhas de grampos, visto que o suporte pode impedir substancialmente a deformação de uma extremidade distai da placa de apoio durante a aplicação de linhas longas de grampos. [0012] A figura 1 ilustra uma modalidade ilustrativa de um grampeador cirúrgico 10 possuindo um suporte 30 para limitar ou impedir a deformação de uma placa de apoio. Como ilustrado, o dispositivo 10 inclui geralmente um cabo 14 possuindo um eixo alongado 12 que se estende a partir do mesmo, e incluindo extremidades proximal e distai 12a, 12b. Um alojamento de eixo 13 é ilustrado disposto sobre uma parte do eixo alongado 12. Um conjunto de aplicação de grampos ou efeito de extremidade 20 é formado na extremidade distai 12b do eixo alongado 12. O efeito de extremidade 20 inclui braços opostos, referidos como um braço de cartucho 24 e uma placa de apoio 22, que são acoplados de forma articulada um ao outro em suas extremidades proximais por um ponto de articulação 26, e que são adaptados para receber tecido entre os mesmos. A fenda pode incluir um número de posições nas quais um pino batente pode ser inserido, e a posição do pino batente pode determinar a extensão na qual a conexão distai pode deslizar na direção da extremidade distai da placa de apoio antes de não ocorrer mais qualquer deslizamento. Os versados na técnica apreciarão que uma variedade de outras técnicas podem ser utilizadas para se criar batentes ou outros mecanismos que limitem o movimento distai do suporte.
[0018] O suporte também pode ter uma variedade de outras configurações para impedir a deformação de uma placa de apoio em um grampeador cirúrgico. Em outra modalidade, como ilustrado nas figuras 3A a 3C, o suporte pode estar na forma de uma manga que é móvel entre uma primeira posição e uma segunda posição com relação a um efeito de extremidade. Claims (9)
- REIVINDICAÇÕES1. Grampeador cirúrgico (10), compreendendo:um efeito de extremidade (20) possuindo um braço de cartucho (24) para reter pelo menos um grampo, e uma placa de apoio (22) acoplada de forma articulada ao braço de cartucho (24) e eficiente para deformar o pelo menos um grampo; e caracterizado pelo fato de que compreende ainda: um suporte (30) acoplado ao efeito de extremidade (20) e móvel entre uma posição proximal na qual a placa de apoio (22) e o braço de cartucho (24) podem articular com relação um ao outro, e uma posição distal na qual o suporte (30) engata pelo menos em uma parte intermediária do efeito de extremidade (20) para impedir a deformação da placa de apoio (22) com relação ao braço de cartucho (24) quando pelo menos um grampo é acionado a partir do braço de cartucho (24) e deformado contra a placa de apoio (22).
- 2. Grampeador cirúrgico, de acordo com a reivindicação 1, caracterizado pelo fato de que o suporte (30) é alinhado axialmente com o efeito de extremidade (20) quando o suporte (30) está na posição proximal, e onde o suporte (30) se estende de forma transversal ao efeito de extremidade (20) quando o suporte (30) está na posição distal.
- 3. Grampeador cirúrgico, de acordo com a reivindicação 1, caracterizado pelo fato de que o suporte (30) compreende uma conexão (34) acoplada de forma deslizante na placa de apoio (22).

- 5. Grampeador cirúrgico, de acordo com a reivindicação 4, caracterizado pelo fato de que a conexão distai (34) é acoplada à placa de apoio (22) em uma posição distai em uma junta articulada (26) formada entre a placa de apoio (22) e o braço de cartucho (24).
- 6. Grampeador cirúrgico, de acordo com a reivindicação 1, caracterizado pelo fato de que o suporte compreende uma manga (340) deslizantemente desdobrada sobre pelo menos uma parte da placa de apoio (320).
- 7. Grampeador cirúrgico, de acordo com a reivindicação 1, caracterizado pelo fato de que compreende adicionalmente um acionador acoplado ao efeito de extremidade (20) e adaptado para empurrar a placa de apoio (22) na direção do braço de cartucho (24).
