Beschreibung
Die Erfindung bezieht sieh auf eine Schaftprothese der im 5 Oberbegriff des Anspruchs 1 angegebenen Art sowie ein entsprechendes Bin' bzw. Ausschlagewerkzeug.
Bei den bekannten Konussystemen sind verschiedentlich Aussparungen zur Aufnahme eines Sicherungselements im HaIsbereich angebracht, welche zur Verdrehsicherung eines Ein bzw. Ausschlägers für Konusschäfte dienen.
Derartige Aussparungen schwächen den Halsbereich der Prothese und wurden bisher nicht im Hinblick auf eine ins- gesamt zu erreichende maximale Festigkeit ausgelegt. Weiterhin besteht bei den bekannten Protehesen der Nachteil, daß durch die räumliche Entfernung der Aussparung vom Konus zusätzliche Toleranzen auftreten, welche dazu führen können, daß, trotz Eingriffs des Sicherungselementes in die Aussparung, der Gegenkonus des Ein- bzw. Ausschlagwerkzeugs nicht korrekt auf dem Konus sitzt und bei der schlagartigen Beanspruchung die zu übertragenen Kräfte sich nicht gleichmäßig auf den Konusbereich verteilen. Außerdem müssen für verschiedene Halslängen wegen der dar— aus resultierenden unterschiedlichen Anordnung des Lochs in bezug auf den Konus verschiedene Werkzeuge vorrätig gehalten werden.
Der in Anspruch 1 angegebenen Erfindung liegt die Aufgabe
zugrunde, bei derartigen Prothesen unter Erzeilung einer heraufgesetzten Festigkeit des Prothesenhalses einerseits

eine sicherere übertragung der schlagartigen Beanspruchung beim Ein- oder Austreiben der Prothese zu bewirken und zum anderen eine Vereinheitlichung zu ermöglichen.
Die Erfindung beruht auf der Erkenntnis, daß wegen des in Richtung des Konusendes der Prothese abnehmenden Biegemoments bei Belastung die durch die Aussparung im Halsbereich unterhalb der Konusfläche dann am geringsten ist, wenn diese möglichst nahe an der Konusfläche gelegen ist.
Auf diese Weise bleibt die kegelige Ansehlußfläehe für das Werkzeug unbeeinträehtigt und steht vollständig zur Kraftübertragung 2ur Verfügung. Andererseits wird gleichzeitig bewirkt, daß die Sicherung des an den Konus anzupassenden Werkzeugs möglichst nahe am erweiterten Ende des Konus er folgt, um hier Toleranzen auszuschalten, welche möglicher weise ein Verkanten des Innenkonus des Werkzeugs in bezug auf den Außenkonus des Prothesenhalses zur Folge haben.
Bei einer bevorzugten Weiterbildung der Erfindung zur Anwendung bei einer Hüftgelenkprothese ist die runde Ausspa rung an der Innenseite der gelenknahen Krümmung des Schaftes angeordnet.
Dabei ist in vorteilhafter Weise dafür gesorgt, daß beim Ausschlagen einer Prothese die auftretenden Zugkräfte nur
ein tieferes Eindringen des Sicherungselementes in die entsprechende Aussparung (in Querrichtung bezogen auf den
Prothesenhals) zur Folge haben, so daß insoweit keine Lockerung des Sitzes des Werkzeugs auf dem Konus eintritt
und insoweit auch keine Beeinträchtigung der übertragung der schlagartigen Zugkräfte eintritt.

Das Sicherungselement ist bevorzugt um einen im Bereich des erweiterten Konusendes gelegenen Drehpunkt aus der Aussparung ausschwenkbar gelagert, so daß das Werkzeug ohne Schwierigkeiten aufgesetzt werden kann und nach dem Aufsetzen lediglich das Sicherungselement einzuschwenken iet, was durch geeignete Hebelübertragungen auch durch im Handgriff des Instrumentes angeordnete Betätigungselemente erfolgen kann, so dad Einhandbedienung möglich ist.
Die Erfindung bezieht sich ebenfalls auf ein entsprechendes Ein- oder Aussehlagwerkzeug.
Sie läßt sich insbesondere vorteilhaft anwenden bei zementfreien Prothesen, bei denen der Prothesenschaft zum Erreichen eines festen Sitzes im Markraum eingeschlagen werden muß. Vorteilhaft ist auch, daß die Gestaltung des unteren Halsbereiches relativ unabhängig gewählt werden kann, da dort nicht eine Aussparung und die diese umgebende Flächengestaltung berücksichtigt werden müssen.
Bevorzugt wird zum Ein- und Ausschlagen der Prothese ein einziges Werkzeug benutzt, da durch die Sicherung des Konussitzes durch die erfindungsgemäßen Maßnahmen sowohl Zug- als auch Druckkräfte - gegebenenfalls alternierend - sicher übertragen werden.
Vorteilhafte Weiterbildungen der Erfindung sind in den Unteransprüchen gekennzeichnet bzw. werden bei der nachstehenden Darstellung zusammen mit einer bevorzugten Aüsführung der Erfindung näher beschrieben. Es zeigen:

In Figur 1 ist eine mit den erfindungsgemäßen Maßnahmen versehene HÜftqelenkprothese einschließlich Werkzeug in Seitenansicht dargestellt. Der Prothesenschaft 1 trägt einen Hals 2, an dessen freiem Ende ein Konus 3 angeordnet ist, auf das ein entsprechender Gegegenkonus 4 eines Einbzw. Ausschlagwerkzeugs aufgesetzt ist.
Eine im Querschnitt kreisförmige Aussparung 5 befindet sich am Hals 2 in unmittelbarer Nachbarschaft des erweiterten Endes des Konus 3. Durch diese Anordnung wirkt sich die systematisch bedingt Schwächung des Materials in möglichst geringer Weise auf das Übertragbare Biegemoment aus. Die erreichbare Dauerfestigkeit der Prothese im Halsbereich ist gegenüber anderen Lösungen wesentlich heraufgesetzt.
In diese Aussparung 5 greift ein entsprechendes Sicherungselement 6 ein, welches nahe der Aussparung 5 im Prothesenhals um eine tangential gerichtete Achse 7 drehbar gelagert ist. Der Schaft 8 des Werkzeugs ist in der Zeichnung teilweise im Schnitt dargestellt, wobei ein Handgriff 9 und ein Schlagbereich 10 für den Gebrauch zweckmäßig angeordnet sind.

Ein am Schaft 8 schwenkbar gelagerter Betätigungshebel 11 §' für das S icher ung se leinen t 6 ist mittels einer geeigneten 'i\ Kraftübertragung mit diesem derart verbunden, daß - durch JI eine geeignete Umlenkung der Bewegung - im am Schaft anliegenden Zustand das Sicherungselement 6 sich - gegebe- j nenfalls unter Vorspannung - zur Arretierung der Schlagübertragung innerhalb der Aussparung 5 befindet, so daß eine toleranzarme Sicherung des Kegelsitzes (Elemente 3 und 4) gewährleistet ist. ,
Auf diese Weise ist verhindert, daß trotz eingesetzten Sicherungselements 5 der Konussitz Spiel hat, so daß die schlagartige Beanspruchung sich nicht gleichmäßig verteilt und somit möglicherweise zur Verformungen des Konus bei trägt, welche seine Maßhaltigkeit in bezug auf die später , aufzusetzende Gelenkkugel beeinträchtigen. Weiterhin weist das ausschwenkbare Sicherungselement 6 in bezug auf die nächstliegende Unterstützung im Bereich des Außenkonus 4 des Werkzeugs einen kurzen Hebelarm auf, so daß die dort auftretenden Biegemomente klein sind und die in diesem Bereich zu übertragenden Kräfte keine zusätzliche Verstärkung der Lagerung etc. dieses Elementes erfordern.
Der Hebel IX ist um eine Achse 12 gelagert und weist ein Maul 13 auf, welches um einen am Ende des Sicherungshebels 6 befindlichen Stift 14 greift und somit in Form einer Steuerkurve eine mechanische Übersetzung bildet.
Aus Figur 2 ist ersichtlich, daß bei vom Schaft 8 abgeschwenkten Griff des Hebels 11 der Sicherungshebel 6 aus schwenkt ist und den Prothesenhals freigibt, so daß der

Konus 3 aus der entsprechenden Aufnahme 4 herausgezogen werden kann.
Zwischen einer unterhalb des Griffes 9 vorgesehenen Anschlagfläche 15 und einem oberhalb des Griffendes 11 vorgesehenen - ebenfalls als Anschlagfläche dienenden - Bund 16 ist eine Schlagmasse 17 auf dem Schaft in Längsrichtung beweglich angeordnet, welche zum Ausschlagen der Prothese dient. Bei nur kleineren notwendigen Kräften kann sie aber auch während des Einschlagvorgangs benutzt werden. Auf diese Weise ist das dargestellte Ein- und Ausschlagwerkzeug universell verwendbar.
Die Erfindung beschränkt sich in ihrer Ausführung nicht auf das vorstehend angegebene Beispiel. Vielmehr sind eine Vielzahl von Varianten denkbar, welche von der dargestellten Lösung auch bei grundsätzlich anders gearteten Ausführungen Gebrauch machen.

1. Konusanschluß für eine_Gelenkprothese mit einer Aussparung im Halsbereich zur Sicherung eines auf den Konus aufgesetzten Ein- bzw. Ausschlagwerkzeugs, mit einem in die Aussparung eingreifenden Stift,
dadurch gekennzeichnet,

daß die Aussparung (5) in unmittelbarer Nachbarschaft des erweiterten Endes des Konus (3) vorgesehen ist.
2. Konusanschluß Anspruch 1, dadurch gekennzeichnet, daß bei einer Hüftgelenkprothese die Aussparung (5) an der Innenseite der gelenknahen Krümmung vorgesehen ist.
3. Konusanschluß nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Aussparung (5) rund ausgebildet ist.
4. Ein- bzw. Ausschlagwerkzeug für eine Prothese mit einem Konusanschluß nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß der Stift (6) an die Aussparung angepaßt ist und daß die Aussparung in unmittelbarer Nachbarschaft des erweiterten Konusendes vorgesehen ist.

8. Werkzeug nach den Ansprüchen 6 und 7, dadurch
gekennzeichnet, daß ein weiterer Anschlag (16) für die (Aus-)schlagmasse (17) unmittelbar oberhalb des am Werkzeugschaft (8) anliegenden Hebels (11) angeordnet ist.
