Description

  • In einer absolut bekannten Weise ist ein Femurschaft dazu vorgesehen, in den Markkanal des Femurs impaktiert zu werden. Das proximale Ende des Schafts nimmt unmittelbar oder auf angesetzte Weise einen Femurkopf auf, der mit dem Gelenkpfannenhohlraum des Hüftknochens zusammenwirkt. Bei diesen unterschiedlichen Femurschäften ist zu unterscheiden zwischen denjenigen, die dazu ausgebildet sind, mit Zement impaktiert zu werden und denjenigen, die dazu ausgebildet sind, ohne Zement impaktiert zu werden. Die zementierten Schäfte werden entweder von einigen Ärzten systematisch, oder von anderen Ärzten abwechselnd mit unzementierten Schäften oder höchstens im Notfall verwendet, wenn die Form oder die Qualität des Knochens nicht für eine Implantation ohne Zement geeignet sind.
  • Da der Operateur vor dem Eingriff nicht immer vorhersehen kann, welche Art von Implantation, mit oder ohne Zement, er durchführen muss, hat es sich als wichtig herausgestellt, bei dem gleichen Instrumentarium von Raspeln über die 2 Schaftausführungen verfügen zu können, die mit oder ohne Zement und damit in jedem Fall mit der besten Art der Fixierung des Implantats am Knochen impaktierbar sind.
  • Der Schwierigkeitsgrad beim Führen der Raspeln für die Knochenvorbereitung und die Stabilität der zuletzt verwendeten Raspel zeigen nämlich an, ob es notwendig ist, das endgültige Implantat zu zementieren oder nicht.
  • Eine vorteilhafte Lösung geht aus der Lehre des Patents FR 2.793.675 hervor. Dieses Patent bezieht sich auf einen Satz von Femurschäften, der für eine gleiche Größe einen Schaft umfasst, der dazu angepasst ist, mit Zement impaktiert zu werden und einen Schaft, dazu angepasst, ohne Zement impaktiert zu werden. Die Querschnitte jedes der Schäfte sind im metaphysären Bereich betrachtet unterschiedlich und so bestimmt, dass sie ihre Impaktierung mit einer gleichen Raspel ermöglichen, deren Abmessungen identisch sind mit denen des Schafts ohne Zement. Der Querschnitt des Schafts ohne Zement ist so definiert, dass er eine optimale metaphysäre Ausfüllung sicherstellt, während der Querschnitt des zementierten Schafts so definiert ist, dass er seine Stabilität und zugleich die örtliche Kompression einer gleichmäßig dicken Zementschicht im Bereich seitlicher Ausnehmungen sicherstellt.
  • Nach dieser technischen Lösung scheint kein Mittel für die Verbesserung von 2 weiteren Parametern eingesetzt zu werden, die für die Lebensdauer der Fixierung eines zementierten Schafts wichtig sind: seine diaphysäre Selbstzentrierung und die homogene Zusammensetzung der zementierten Schicht. Der Schaft wird nämlich ohne kalibrierte diaphysäre Führung im zuvor nach dem herkömmlichen Zementierverfahren mit Zement gefüllten Femur platziert. Bei dieser Art der Zementierung bleiben Luftblasen sowie Blut- und Fettrückstände in der Zementhülle eingeschlossen, die im Laufe der Zeit die Festigkeitseigenschaften der Hülle abschwächen.
  • Außerdem kann die Lehre des Patents FR 2728160 , das ein Hüftprothesensystem mit zementierten und unzementierten Schäften unter Verwendung einer universellen Raspel beschreibt, angeführt werden. Die Schäfte sind dazu vorgesehen, beliebig in die Markhöhle des Femurs implantiert zu werden, die mittels der Raspel erzeugt worden ist, welche einen Körper mit einem bestimmten Querschnitt aufweist und an der Reibezähne ausgebildet sind. Dieses Dokument liefert keinerlei Hinweis auf eine optimale Zentrierung des Schafts im metaphysären Abschnitt, bei der der diaphysäre und der metaphysäre Abschnitt dennoch mit Zement ausgefüllt werden können.
  • Ausgehend von diesem Stand der Technik hat es sich insbesondere zur Vermeidung der Bildung von Inhomogenitäten in der Zusammensetzung und der Dicke der Zementschicht und um die Abscherung dieser Zementschicht wesentlich zu verringern, als vorteilhaft erwiesen, in einem ersten Schritt den Femurschaft zu platzieren und in einem zweiten Schritt das Zement zu injizieren und den Zwischenraum zwischen dem Schaft und dem Knochen zu füllen, der distal von einem Zentrier- und Diffusionsstopfen begrenzt wird und proximal von einem Abschnitt des Schafts, der den intramedullären Raum vollständig ausfüllt.
  • Zur Lösung eines solchen Problems wurde ein Set aus Femurschäften, die den Merkmalen des Anspruchs 1 entsprechen, vorgesehen und geschaffen.
  • Das Problem, die Versteifung des beim Einführen in den Knochenhohlraum am Ende des Implantats befestigten Zentrier- und Diffusionsstopfens sicherzustellen, wird dadurch gelöst, dass der Zentrier- und Diffusionsstopfen einen Versteifungsschaft demontierbar aufnimmt, der für das Zusammenwirken mit dem zentralen Kanal des Femurschafts vorgesehen ist und der in der Mitte des Stopfens eingeschraubt ist.
  • Das Problem, den Zwischenraum zwischen dem Schaft und dem Knochen zu füllen, wird dadurch gelöst, dass der Stopfen mit der Spitze des distalen Endes des Femurschafts verbunden ist und Löcher für den Ausfluss des Zements und dessen Aufsteigen im Knochenhohlraum aufweist, wobei die Löcher zwischen dem distalen Ende und Zentrierungs- und Verschlussflügeln, die der Stopfen aufweist, angeordnet sind.
  • Das Problem, das Injizieren des Zements als solches sicherzustellen, wird dadurch gelöst, dass der zentrale Kanal für das Injizieren des Zements am proximalen Ende des Schafts durch eine Bohrung mit Innengewinde zum Einschrauben eines Verbindungsstücks verlängert ist, das geeignet ist, das Injizieren des Zements zu ermöglichen, indem das Ende der Injektionsspritze für die Zementinjektion mit dem zentralen Loch des Implantats verbunden wird.
  • Nach einem weiteren Merkmal nimmt die Bohrung mit Innengewinde durch Einschrauben ein Schließorgan des zentralen Kanals auf, das durch einen Schaft verlängert ist, der dazu geeignet ist, nach dem Abfließen des Zements die Reinigung des zentralen Kanals des Femurschafts sicherzustellen.
  • Das Problem, jegliche Verunreinigung aus der zementierten Schicht zu entfernen, wird dadurch gelöst, dass der metaphysäre Block für die Zentrierung und den Druck des Schafts eine bogenförmige untere Begrenzung aufweist, wobei am Scheitelpunkt der Konkavität Abflussöffnungen für das Abfließen des Restzements und anderer, die homogene Zusammensetzung der Zementschicht gefährdende Verunreinigungen (Luft, Fett, Blut etc.) ausgebohrt sind.
  • Was den Femurschaft betrifft, der dazu bestimmt ist, ohne Zement impaktiert zu werden, so ist letzterer im Querschnitt so geformt, dass eine optimale metaphysär-diaphysäre Ausfüllung des Femurs und eine unmittelbare Selbstblockierung bei der Impaktierung sichergestellt sind.
  • Die Erfindung wird nachfolgend näher erklärt anhand der Figuren der anhängenden Zeichnungen, wobei:
  • die 1 eine Frontansicht des zementierten Femurschafts ist
  • die 2 eine entsprechend der Linie 2-2 der 1 betrachtete Längsschnittansicht ist
  • die 3 eine Frontansicht des unzementierten Femurschafts ist
  • die 4 eine Frontansicht des zementierten Femurschafts und der superponierten Raspel (mit derselben Größe wie der unzementierte Schaft) entsprechender Größe ist, die den für das Zement ausgesparten Raum zeigt
  • die 5, 6, 7, 8 die wesentlichen Operationsphasen für die Platzierung der zementierten Prothese zeigen.
  • Gemäß der Erfindung umfasst das Set aus Femurschäften für jede Raspelgröße einen Schaft (1), der dazu angepasst ist, mit Zement impaktiert zu werden und einen Schaft (2), der dazu ausgebildet ist, ohne Zement impaktiert zu werden.
  • Was den Schaft (2) betrifft, der dazu ausgebildet ist, ohne Zement impaktiert zu werden, so ist letzterer von dem im oben genannten Patent FR 2.793.675 beschriebenen und dargestellten Typ. Die 3 zeigt ein Beispiel eines Femurschafts (2) der dazu ausgebildet ist, ohne Zement impaktiert zu werden. Die Schnittprofile dieses Schafts sind so bestimmt, dass sie eine optimale metaphysär-diaphysäre Ausfüllung und eine unmittelbare Selbstblockierung bei der Impaktierung sicherstellen.
  • Der Schaft (1), der erfindungsgemäß dazu bestimmt ist, mit Zement impaktiert zu werden, ist im metaphysären Abschnitt selbstzentrierend durch einen Abschnitt (1d) im Bereich des Zervikalschnitts, dessen frontale und sagittale Abmessungen mit denen der Raspel identisch sind, und ist im diaphysären Abschnitt durch einen Verschlussstopfen (3) zentriert. Im Übrigen weist der Femurschaft (1) einen zentralen ausmündenden Kanal (1a) auf, der für das Injizieren des Zements und seine Verdrängung nach außen zum Ausfüllen des Zwischenraums zwischen dem Schaft (1) und dem Knochen mit dem Verschlussstopfen (3) zusammenwirkt.
  • Der Stopfen (3) ist vorteilhafterweise aus Polyethylen hergestellt. Dieser Stopfen weist einen zylindrischen Abschnitt auf, der innen dazu ausgebildet ist, auf das distale Ende des Femurschafts (1) aufgesteckt zu werden. Die untere Wandung des Abschnitts (3a) ist mit kreisförmig (3b) angeordneten Löchern versehen, um den Ausfluss des Zements (c) und seine Verdrängung in der Markhöhle des Femurs zu ermöglichen, wie in der Folge der Beschreibung angegeben wird. Der zylindrische Abschnitt (3a) ist durch Flügel (3c) verlängert, die dazu ausgebildet sind, die Markhöhle des Femurs perfekt zu verschließen und dabei gleichzeitig die Zentrierung des Schafts (1) in dem Kanal sicherzustellen.
  • Nach der Platzierung des Stopfens (3) am distalen Endes des Femurschafts (1) sind die Löcher (3b) zwischen dem durchbrochenen distalen Ende des Kanals für den Durchfluss des Zements und den Verschlussflügeln (3c) des Stopfens (3) angeordnet.
  • Die Platzierung des zementierten Femurschafts (1) erfolgt wie nachstehend unter besonderer Bezugnahme auf die 5, 6, 7 und 8 angegeben.
  • Der Femurschaft (1) wird zuvor mit dem Verschlussstopfen (3) ausgestattet, dessen Größe durch eine Ausmessung der Diaphyse des Femurs bestimmt worden ist. Dann wird ein Versteifungsschaft (4) in den zentralen Kanal (1a) des Femurschafts (1) eingesetzt, damit er mit seinem Gewindeende (4a), in ein Gewindeloch (3d) in der Mitte des Stopfens (3) eingeschraubt werden kann. Dieser Versteifungsschaft (4) kann eine Verlängerung (4b) außerhalb des Knochenschafts aufweisen, die dazu dient, den Femurschaft im Knochenschaft zu führen und zu impaktieren.
  • Nachdem der Femurschaft platziert ist, kann der Operateur den Versteifungsschaft (4) herausschrauben, um die Injektion des Zements (C) durch den Kanal (1a) (6) hindurch vorzunehmen. Hierzu verwendet der Operateur ein Verbindungsstück (5), das in eine Bohrung mit Innengewinde (1b) am proximalen Ende des zentralen Kanals des Implantats eingeschraubt wird. Das Verbindungsstück (5) wird an seinem anderen Ende mit dem Ansatzstück einer Zementspritze, einer nach dem herkömmlichen Zementierverfahren bekannten Gerätschaft, verbunden. Das Zement (C) wird also in den zentralen Kanal (1a) injiziert und dabei im Bereich des distalen Endes des Schafts (1) durch die Löcher (3b) des Verschlussstopfens (3) gedrängt. Da die Flügel (3c) den Verschluss des Knochenhohlraums sicherstellen, wird das Zement natürlicherweise zum Aufsteigen neigen, um den Zwischenraum zwischen dem Schaft (1) und dem Knochen auszufüllen, bevor es auf das Hindernis auftrifft, das aus dem Abschnitt (1d) des Schafts besteht, der den gesamten intramedullären Raum ausfüllt und auf diese Weise einen Gegendruckblock bildet (7).
  • Der zentrale Kanal (1a) kann anschließend gereinigt und durch einen Schaft (6) verschlossen werden, dessen proximales Ende, das als Stopfen ausgebildet ist, in die Bohrung mit Innengewinde (1b) des Femurschafts (1) eingeschraubt wird.
  • Es ist zu beachten, dass der Abschnitt (1d), der ein das Implantat zentrierendes und auf die Zementhülle Druck ausübendes Element bildet, am Scheitelpunkt der Konkavität seiner unteren bogenförmigen Begrenzung (1e), Abflussöffnungen (1c) für das Abfließen des Restzements und anderer Verunreinigungen (Luft, Fett, Blut etc.) aufweist.
  • Es sei daran erinnert, dass der Querschnitt des zementierten Schafts (1) in einem metaphysären Abschnitt in Höhe des Zervikalschnitts dem Querschnitt einer Raspel gleicher nominaler Größe entspricht, der seinerseits dem Querschnitt des Schafts ohne Zement (2) gleicher Größe entspricht (4).
  • Ohne deshalb den Rahmen der Erfindung zu verlassen, kann der Schaft ohne Zement (2) einen zentralen ausmündenden Kanal für das Abfließen einer Füll- oder Fixationsflüssigkeit aufweisen. In Wiederaufnahmefällen aufgrund einer Lockerung oder einer Knochenfraktur kann es nämlich vorteilhaft sein, ein biologisches Knochenzement in den zentralen Kanal des Schafts ohne Zement injizieren zu können, um Knochendichteverluste auszugleichen oder die Synthese des frakturierten Knochens zu fördern.
  • Die Vorteile gehen deutlich aus der Beschreibung hervor.

Claims (8)

  1. Set aus Femurschäften, einen Schaft (1) umfassend, der dazu angepasst ist, mit Zement in einen Knochenhohlraum impaktiert zu werden, einen Schaft (2), der dazu angepasst ist, ohne Zement in diesen Hohlraum impaktiert zu werden und eine Raspel, deren Größe identisch ist mit der des Schafts ohne Zement (2), wobei die Querschnitte der Schäfte (1) und (2) unterschiedlich sind und so bestimmt, dass sie die Platzierung der Schäfte nach dem Ausraspeln ermöglichen und wobei der Querschnitt des Schafts ohne Zement (2) so bestimmt ist, dass seine unmittelbare Selbstblockierung bei der Impaktierung sichergestellt ist, dadurch gekennzeichnet, dass der Schaft (1), der dazu angepasst ist, mit Zement impaktiert zu werden, im metaphysären Abschnitt selbstzentrierend ist durch einen Teil im Bereich des zervikalen Schnitts, dessen frontale und sagittale Abmessungen identisch sind mit denen der Raspel und dass dieser Schaft im diaphysären Abschnitt zentrierbar ist durch einen Verschlussstopfen (3), wobei der Schaft (1) einen zentralen ausmündenden Kanal (1a) aufweist für das Injizieren des Zements und seine Verdrängung nach außen zum Füllen des Zwischenraums zwischen dem Schaft und dem Knochen.
  2. Set nach Anspruch 1, dadurch gekennzeichnet, dass das Set einen Verschlussstopfen (3) umfasst und der Stopfen (3) einen Schaft zur Versteifung (4) demontierbar aufnimmt, der für das Zusammenwirken mit dem zentralen Kanal (1a) des Femurschafts (1) vorgesehen ist und der in der Mitte des Stopfens (3) eingeschraubt ist.
  3. Set nach Anspruch 1, dadurch gekennzeichnet, dass das Set einen Verschlussstopfen (3) umfasst und der Stopfen (3) mit der Spitze des distalen Endes des Femurschafts (1) verbunden ist und Löcher (3b) für den Ausfluss des Zements oder jeglicher Art von Produkt zur Knochenfüllung oder Knochenfixation in den Knochenhohlraum und das Aufsteigen in dem Knochenhohlraum aufweist, wobei die Löcher zwischen dem distalen Ende und den Zentrierungs- und Verschlussflügeln (3c), die der Stopfen aufweist, angeordnet sind.
  4. Set nach Anspruch 1, dadurch gekennzeichnet, dass der zentrale Kanal (1a) für das Injizieren des Zements am proximalen Ende des Schafts (1) durch eine Bohrung mit Innengewinde (1b) zum Einschrauben eines Verbindungsstücks (5) verlängert ist, das geeignet ist, das Injizieren des Zements zu ermöglichen, indem es das Ende einer Spritze für die Zementinjektion mit dem zentralen Kanal (1a) des Implantats verbindet.
  5. Set nach Anspruch 4, dadurch gekennzeichnet, dass die Bohrung mit Innengewinde (1b) durch Einschrauben ein durch einen Schaft (6) verlängertes Schließorgan (6a) [sic] des zentralen Kanals aufnimmt, wobei der Schaft dazu geeignet ist, nach dem Abfließen des Zements die Reinigung des zentralen Kanals (1a) des Femurschafts sicherzustellen.
  6. Set nach Anspruch 1, dadurch gekennzeichnet, dass der metaphysäre Block für die Zentrierung und den Druck des Schafts (1) eine bogenförmige untere Begrenzung (1e) [sic] aufweist, wobei am Scheitelpunkt der Konkavität Abflussöffnungen (1c) für das Abfließen des Restzements und anderer, die homogene Zusammensetzung der Zementschicht gefährdende Verunreinigungen ausgebohrt sind.
  7. Set nach Anspruch 1, dadurch gekennzeichnet, dass der Schaft (2) im Querschnitt so geformt ist, dass eine optimale metaphysär-diaphysäre Ausfüllung des Femur und eine unmittelbare Selbstblockierung bei der Impaktierung sichergestellt sind.
  8. Set nach Anspruch 1, dadurch gekennzeichnet, dass der Schaft ohne Zement (2) alternativ einen zentralen ausmündenden Kanal für das Abfließen einer Füll- oder Fixationsflüssigkeit aufweist.
