Description

  • Spannstückverbindungen für Implantate Die Erfindung betrifft eine Vorrichtung zum dauerhaften, zementfreien Befestigen von Implantaten im Inneren von Röhrenknochen.
  • Knochenendoprotbesen werden immer häufiger in der Humanmedizin eingesetzt. Verschiedene Substitutionsprodukte sind bereits für Implantate vorhanden. Als Werkstoffe kommen Metallegierungen, Kunststoffe und technische Keramik-infrage, aber jedes dieser Materialien besitzt gewisse Nachteile, wenn man es als Implantat einsetzt. Der Kunststoff z. B. verträg sich oft sehr schlecht mit dem Organismus, so daß er schon nach kürzester Zeit wieder aus dem Körper entfernt werden muß. Außerdem besitzt er keine besonders hohe chemische Widerstandsfähigkeit und ist auch nicht genügend volumenbaständig. Bei den Metallverbindungen gibt es bereits Werkstoffe, z. B. Kobalt-Chromstahl, die weit besser mit dem lebenden Organismus verfraglich sind als Kunststoffe, aber zum größten Teil müssen solche Implantate aus Metallen bzw. Metallegierungen mit Zementen oder Klebern in oder an den Knochen gekittet werden. Die keramischen Werkstoffe sind zwar inert und korrosionsbeständig gegenüber dem Organismus, aber man befürchtet, daß ein solches Material, wie es in der deutschen Offenlegungsschrift 2 134 316 beschrieben wird, aufgrund seines spröden Charakters, z. B. beim Einschlagen eines Aluminiumoxid-Schaftes, beschädigt oder sogar zerstört wird. Außerdem benötigt man ebenfalls Kitte, Zemente oder Kleber, um gewisse Dauerverbindungen herzustellen.
  • Daher ist es die Aufgabe der vorliegenden Erf ndung, bei Endoprothesen die Verwendung von Klebern und Zementen zum Befestigen der Implantat-Teile aus den in der Fachwelt bekannten Gründen zu vermeiden, und daß beim Einbau keine Gefahr einer Beschädigung des Implantat-Materials besteht.
  • Die erfindungsgemäße Spannstückverbindung für ein Implantat ist als Lösung der vorstehend genannten Aufgabe dadurch gekennzeichnet, daß in den Raum zwischen Corticalis und dem teilweise hohlen Implantatschaft ein Kugelhaufwerk eingebracht ist und dieses in etwa senkrechter Richtung zur Corticaliswand anpressend wirkt, indem ein in einem Längsschlitz beweglicher Querstift im hohlen Teil des Schaftes auf das Haufwerk drückt.
  • Auf den Zeichnungen ist dieser Erfindungsgedanke beispielsweise dargestellt, und zwar zeigen in Teil-L-ngsschnitten Fig. 1: ein Ausführungsbeispiel für den Fall eines Knochens mit gekrümmter Längsachse, beispielsweise eines Oberschenkelknochens.

  • Die Einzelheiten und die Merkmale der Erfindung sind aus der nachfolgenden Beschreibung zu entnehmen.
  • Nachdem das Implantat 2 nebst Kugelhaufwerk 6, 7 und die Zylinderkörper 5 in den vorher ausgefrästen Hohlraum des Röhrenknochens ohne Anwendung von Zwang gebracht worden ist,und man mittels eines Werkzeugs, vorzugsweise eines Drehrnomentenschlüssels, die Schraube 9, vorzugsweise in Gestalt einer Inbusschraube 11, am Kopf des Implantates angezogen hat, drückt diese auf die stark abgerundeten Zylinderkörper 5 im hohlen Implantatteil, und diese wiederum übertragen den Druck auf den freibeweglichen Querstift 4, der in einen Längsschlitz 3 eingepaßt ist, sowie weiterhin auf die konische Hülse 13.
  • Der Raum zwischen dem unteren Verschlußstück 8 und der Hülse 13 ist mit Kugeln aus Keramik oder Metallen ausgefüllt. Dieses Kugelhaufwerk zeigt die wirkung einer Pseudo-Flüssigkeit, wenn die Einzelelemente gleich oder annähernd gleich im Durchmesser sind und ein Druck ausgeübt; wird. Dadurch besteht aber die Gefahr, daß die Kugeln am unteren Verschlußstück 8 bzw. an der Hülse 13 vorbeigleiten können; deshalb whlt man für die untere und obere Querschicht des Kugelhaufwerks etwas größere Kugeln 7. Der Druck in Richtung der Knochenlängsachse auf den Kugelhaufen bewirkt ein Ausweichen und Anpressen der kugeln in Richtung zur Corticaliswand 1. Damit ist ein Verkeilen des Implantats im Röhrenknochen erreicht.
  • Der obere Kugelhaufen 12 dient im allgemeinen lediglich zur Wahrung der Abstände zwischen Implantat und Corticalis. Gegebenfalls kann eine zusätzliche Sicherung am Kopf des Implantates durch ein weiteres Druckelement 10 erfolgen, indem sich nach dem gleichen Prinzip die Kugeln 12 im Knochengewebe fest verankern.
  • Als Materialien für die erfindungsgemäße Vorrichtung sind keramische Werkstoffe, vorzugsweise Aluminiumoxid oder Verbundwerkstoffe vorgesehen, die entweder dicht, porös oder zumindest oberflächenporös sind.
  • Damit nach dem ärztlichen Eingriff Entzündungen weitgehend vermieden werden, ist an geeigneter Stelle ein Pharmaka-Depot angebracht.
  • Durch die erfindungsgemäßen konstruktiven Maßnahmen hat die Vorrichtung Vorteile gegenüber den bereits bekannten Einbaumöglichkeiten von Implantaten in Röhrenknochen, insbesondere ein Vermeiden schlagartiger Beanspruchungen wehren des Operierens. Ferner, daß eine leichte und sichere Verankerung des Implantats durch ein Kugelhaufwerk im Inneren des Röhrenknochens erfolgt,wobei Zemente, Kitte oder dgl. nicht benötigt sind. Außerdem hat das Kugelhaufwerk noch den Vorteil, daß neu gebildetes Organgewebe leicht in die Hohlraume des Laufwerkes einwachsen kann. Diesen Vorgang kann man noch unterstützen, indem die Kugeln ebenfalls eine-poröse Oberflache aufw#eisen.

Claims (8)

  1. Patent- bzw. Schutzansprüche 1. Vorrichtung zum dauerhaften, zementfreien Befestigen von Implantaten im Inneren von Röhrenknochen, dadurch gekennzeichnet, daß in den Raum zwischen Corticalis (1) und dem teilweise hohlen Tmplantatschaft (2) ein Kugelhaufwerk (6;7) eingebracht ist und dieses in etwa senkrechter Richtung zur Corticaliswand anpressend wirkt, indem ein in einem Langsschlltz (3) beweglicher Querstift (4) im hohlen Teil des Schaftes auf das Haufwerk druckt.
  2. 2. Vorrichtung nach Anspruch 1 dadurch gekennzeichnet, daß die Druckübertragung auf den Querstift (4) durch eine Schraube (9), vorzugsweise einer Inbusschraube (11), mittels der stark abgerundeten Zylinderkörper (5) im hohlen Implantatteil erfolgt.
  3. 3. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß die Kugeln des Kugelhaufwerkes an der Hülse (13i und am Verschlußstück (8) einen größeren Durchmesser (7) als in der Mitte (6) haben.
  4. 4. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß durch ein weiteres Druckelement (10) und an den Kugeln (7 ;12) eine zusätzliche Verankerung mit dem Knochengewebe erzielbar ist.
  5. 5. Vorrichtung nach Anspruch 1 bis 4, dadurch gekennzeichnet, daß als Werkstoff für das Implantat Keramik, vorzugsweise Aluminiumoxid, eingesetzt ist.
  6. 6. Vorrichtung nach Anspruch 1 bis 4, dadurch gekennzeichnet, daß als Material für das Implantat ein Verbundwerkstoff, vorzugsweise ein keramischer Verbundwerkstoff, gewahrt ist.
  7. 7. Vorrichtung nach mindestens einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß für das Implantat an denjenigen Stellen ein poröses Material eingesetzt ist, an denen dieses direkt mit dem Knochengewebe in Berührung kommt.
  8. 8. Vorrichtung nach Anspruch 1 bis 7, dadurch gekennzeichnet, daß ein Pharmaka-Depot im oder am Implantat eingesetzt ist.
