Abstract

Hüftgelenk-Endoprothese mit einem im Oberschenkelknochen verankerten Prothesenschaft (4) mit einem Anschlußzapfen (5) auf dem ein Kopplungselement (1) angeordnet ist, auf dem ein Kugelkopf (3) befestigt ist, der in einer Pfanne (6) oder einem Pfanneneinsatz gelagert ist, der wiederum im Beckenknochen verankert ist, dadurch gekennzeichnet, daß zwischen dem Kopplungselement (1) und dem Kugelkopf (3) eine biokompatible Zwischenschicht (2) zur form- und/oder kraftschlüssigen Verbindung angeordnet ist.

Description

  • Die Erfindung betrifft eine Hüftgelenk-Endoprothese nach dem Oberbegriff des Anspruchs 1.
  • Aus der DE 40 08 563 A1 ist eine Hüftgelenk-Endoprothese mit einem im Oberschenkelknochen verankerten Prothesenschaft mit einem Anschlußzapfen bekannt, auf dem ein Kopplungselement angeordnet ist. Auf diesem Kopplungselement ist dann der Kugelkopf aufgesetzt, der in einer Pfanne gelagert ist, die wiederum im Beckenknochen verankert ist. Ein Beispiel für das Einsatzgebiet eines derartigen Kopplungselements ist eine Prothese, deren Anschlußzapfen aus einem anderen Material als der Kugelkopf besteht. Außerdem kann dieses Kopplungselement in einem Modularsystem eingesetzt werden, bei dem das Kopplungselement die Anpassung von diversen prothetischen Kugelköpfen und Prothesenschäften ermöglicht.
  • Der Erfindung liegt die Aufgabe zugrunde, eine Hüftgelenk-Endoprothese nach dem Oberbegriff des Anspruchs 1 so zu verbessern, daß eine verbesserte Verbindung des Zwischenelements mit dem Kugelkopf erreicht ist.
  • Erfindungsgemäß wird diese Aufgabe dadurch gelöst, daß zwischen dem Kopplungselement und dem Kugelkopf eine biokompatible Zwischenschicht zur form- und/oder kraftschlüssigen Verbindung angeordnet ist.
  • Diese Zwischenschicht kann z.B. aus Knochenzement (PMMA = Polymethylmethacrylate), einem Edelmetall wie Gold oder aus Polyethylen bestehen. In vorteilhafter Ausführungsform besteht die Zwischenschicht aus einer durch Oberflächenoxidation erzeugten nicht leitenden Schicht. Mit dieser Zwischenschicht ist erreicht, daß eine op timale Kraftübertragung gegeben ist. Wenn die Zwischenschicht nicht leitend, d.h. elektrisch isolierend ist, so ist sie zur Unterbindung von Frettingkorrosion geeignet.
  • Vorteilhafterweise besteht das Kopplungselement aus dem Material des Prothesenschafts, damit keine Frettingkorrosion zwischen dem Anschlußzapfen und dem Kopplungselement auftreten kann.
  • In bevorzugter Ausführungsform ist der Kugelkopf als Schale ausgebildet und weist eine sphärische Innenkontur auf, wobei die Außenkontur des Kopplungselements eine hieran angepaßte sphärische Außenkontur aufweisen muß.
  • Der Kugelkopf besteht bevorzugt aus einem für die Gleitpaarung optimalen Werkstoff, also beispielsweise aus Aluminiumoxid, wenn der Gleitpartner, d.h. die Pfanne, aus Polyethylen oder Aluminiumoxid besteht oder aus einer Kobalt-Chrom-Legierung, wenn der Gleitpartner Polyethylen oder eine Kobalt-Chrom-Legierung ist.
  • Weitere Ausgestaltungen der Erfindung ergeben sich aus der Zeichnung, die nachfolgend beschrieben ist.
  • Mit dem Bezugszeichen 4 ist ein Prothesenschaft bezeichnet, der im Oberschenkelknochen verankert ist. An dem zum Beckenknochen gewandten Ende des Prothesenschaftes 4 weist dieser einen konisch geformten Anschlußzapfen 5 auf, auf dem ein Kopplungselement 1 mit einer angepaßten konischen Bohrung 7 aufgesetzt ist. Das Kopplungselement 1 hat eine sphärische Außenkontur auf die ein Kugelkopf 3 aufgesetzt ist. Der Kugelkopf 3 ist als Schale ausgebildet und weist eine sphärische Innenkontur auf, die an die Außenkontur des Kopplungselements 1 angepaßt ist.
  • Zwischen dem Kopplungselement 1 und dem Kugelkopf 3 ist eine biokompatible Zwischenschicht 2 zur form- und/oder kraftschlüssigen Verbindung angeordnet. Diese Zwischenschicht 2 kann z.B. aus Knochenzement, einem Edelmetall wie Gold oder aus Polyethylen bestehen. Auch kann diese Zwischenschicht aus einer durch Oberflächenoxidation erzeugten elektrisch nicht leitenden Schicht bestehen. Besonders wichtig ist die Kraftübertragung und gleichmäßige Verteilung der Kraft, so daß keine Spannungsspitzen auftreten.
  • Der Kugelkopf 3 ist in einer Pfanne 6 bzw. einem Pfanneneinsatz gelagert. Die Pfanne 6 kann direkt oder über eine weitere Schale im Beckenknochen verankert sein.

Claims (5)

  1. Hüftgelenk-Endoprothese mit einem im Oberschenkelknochen verankerten Prothesenschaft (4) mit einem Anschlußzapfen (5) auf dem ein Kopplungselement (1) angeordnet ist, auf dem ein Kugelkopf (3) befestigt ist, der in einer Pfanne (6) oder einem Pfanneneinsatz gelagert ist, der wiederum im Beckenknochen verankert ist, dadurch gekennzeichnet, daß zwischen dem Kopplungselement (1) und dem Kugelkopf (3) eine biokompatible Zwischenschicht (2) zur form- und/oder kraftschlüssigen Verbindung angeordnet ist.
  2. Hüftgelenk-Endoprothese nach Anspruch 1, dadurch gekennzeichnet, daß die Zwischenschicht (2) aus Knochenzement, Edelmetall wie Gold oder Polyethylen besteht.
  3. Hüftgelenk-Endoprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Zwischenschicht (2) aus einer durch Oberflächenoxidation erzeugten nicht leitenden Schicht besteht.
  4. Hüftgelenk-Endoprothese nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß das Kopplungselement (1) aus dem Material des Prothesenschaftes (4) hergestellt ist.
  5. Hüftgelenk-Endoprothese nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß der Kugelkopf (3) als Schale ausgebildet ist und eine sphärische Innenkontur aufweist und die Außenkontur des Kopplungselementes (1) eine hieran angepaßte sphärische Außenkontur aufweist.
