Abstract
一种具有在球形自由微粒体作用下经过压实的塑性变形表面的钛植入物。这种结构的表面,是通过在离心式——行星加工装置中对植人物毛坯的加工来达到的,其中的毛坯是放入充填有球和肥皂水溶液的工作室中,在工作室旋转的过程中,球和毛坯表面碰撞,进行加工,籍以保证新要求的表面结构。所获得的植入物,具有增强了的抗血栓性和很强的生物惰性。
Description
本发明属于医学领域,更确切地说,属于心血管外科用的植入物及其制造方法,它可以非常成功地结合心脏瓣膜假体使用。
近年来,在心脏外科中愈来愈广泛地使用金属植入物,其中包括钴和钛,而且由于钴的价格高,就使钛的应用更加广泛。在心脏外科中,广泛采用心脏瓣膜假体作为植入物,它包括一个带有导血孔的瓣膜环,其中装有闭锁器官(参见美国专利4713071,Iofis等等)。对于这种心脏瓣膜假体来说,在杜绝因其表面影响而形成血栓方面提出了很严格的要求,还要求它的表面具有非常低的生物活性或者很高的生物惰性。
为了保证这些对表面非常高的要求,已经提出要在予加工好的一个表面或多个表面敷上不同的膜层,其中包括敷光谱纯碳膜(例如可以参见a.c.cccp № II55263)。在这种情况下得到的心脏瓣膜假体,其表面上有厚度为6~8微米的高质量碳膜,实际上它在起始状态就能阻止血栓的形成,而且具有很低的生物活性。然而这种方法並未得到广泛应用,因其具有一系列缺点。对其中某些缺点指出如下:在瓣膜整个表面上极难达到均匀的厚度,而厚度不均会使血液流动的动力学质量变坏。制做这种膜层需要使用复杂而昂贵的设备,以保证10-6毫米汞柱数量级的高真空,並要求精确地控制温度。最后,所得到的膜层会随着时间的推移分离和“脱皮”,而这将导致不良的后果,特别是在考虑到不可能在所使用的植入物上进行修
复工作时。正是由于上述情况的存在,此方法以及其他一系列方法目前未被采用。
实践中采用的是这些已知方法的各种变形,它们使用不带任何膜层但表面质量非常高的钛植入物,这种高质量的表面是靠多级加工达到的,其中包括如研磨、抛光、使用研磨膏进行精细抛光这样一些步骤以及其他一些特殊加工形式,它能保证获得0.16微米数量级的粗糙度,按照专家们的意见,就能保证很高的抗血栓性。然而正如实践所证实的那样,这样高的加工光洁度,对阻止血栓的形成並不起决定性影响,而从另一方面看,这种多级加工过程周期长而昂贵。在此应当注意的是,使植入物趋向于形成血栓的主要因素是由于在其表面上存在多孔疏松结构,这种结构在传统的加工过程中仍然保留着。
因此应当承认,在研制心血管外科用植入物方面需要开拓新的途径,这种新的途径具有简单的结构,然而又有高的抗血栓性能,采用简单的方法就能制造出这种结构。
作为本发明基础提出的任务是要制作一种钛植入物表面,以其所具有的结构能够保证增强了的抗血栓性及降低了的生物活性,並且拟定一种既简单又经济的加工方法,以便在植入物上能够得到这种表面。
所提出的任务是这样解决的:在心血管外科用钛植入物中(该植入物具有加工好的抗血栓生物惰性表面),根据本发明,被加工出来的表面是一个塑性变形表面,而且是在自由的粒状体的作用下被压实了的。
类似这种表面结构的优点在于,其中不存在一般钛所具有的孔隙,于是在很大程度上就能降低形成血栓的可能性,而随后将要详细介绍的我们所做的实验表明,带有这种表面的植入物具有极低的生物活性。
植入物表面的类似结构,可以借助包括有毛坯的初加工和最后加工的方法来获得。按照本发明,在最后加工阶段,是将毛坯放在装填有回转体形式的抛光微粒和肥皂水溶液的工作室内,並使该工作室以一定的速度作行星式旋转,此速度足以使毛坯的表面通过与微粒的相互作用而产生塑性变形。
该方法的优点在于能使被加工表面达到很高的光洁度,而且钛表面实际上完全不存在孔隙,从而也就保证了很高的抗血栓性。除此而外,在利用此方法进行加工时,还可以降低对初加工质量的要求,因为最终达到的表面光洁度实际上与初加工关系很小。
这种抛光微粒可以是任意一种回转体,但最合适的作法是采用球状体,因为在这种情况下能够保证其与被加工表面的点接触,並使被加工表面由于塑性变形而得到最好的压实。
随后,本发明将通过具体的实施例和附图加以说明,该图是获取具有塑性变形的植入物表面的方法实施装置的草图。
借助图中给出的装置,可以得到带有塑性变形表面层的或者直接带有塑性变形表面的心血管外科用的植入物。这种装置包括可以沿箭头A方向旋转的回转部分1,其中装有两个工作室2,可与回转部分1一起沿箭头B方向绕自身轴旋转。毛坯3放在工作室2内,工作室2为自由装在其中的微粒4及注入其中的肥皂水所充填。微粒4应做成回转体的形式,以保证能同被加工的植入物毛坯3是点接触或线接触。在回转部分1和工作室2按箭头所示方向同时旋转时,处在工作室2中的毛坯3和微粒4具备了动能並互相碰撞,结果就使植入物毛坯3的表面发生塑性变形,在加工结束时得到的表面具有平滑光亮的外观。加工出来的表面没有孔隙,对于抗血栓是有利的,实际上排除
了形成血栓的可能性。
尽管利用上述方法可以得到各种心脏外科用的植入物,其中包括心脏瓣膜、血管用的管形元件等等,但最令人感兴趣的是加工心脏瓣膜用的植入物,具体说来就是加工心脏瓣膜的本体(kopnyc)和闭锁器官。现在看一下加工心脏瓣膜植入物本体的几个例子。首先由纯钛制作出植入物本体的毛坯。对每一个这样的本体予先进行加工,以使其达到给定的表面光洁度,然后进行脱脂。经过这种予加工之后的毛坯,按照以下援引的本发明的实施例进行加工。
例1
将予加工好的心脏瓣膜植入物的本体放进由莫斯科金属切削机床实验科学研究所(ЭНИМС)生产及出售的离心式行星加工装置的容积为1升的工作室中。此工作室50%的容积装上直径0.8~1.0毫米混合的抛光淬硬铬钢球,以保证工件表面的各个部分都能被加工到。然后向工作室中注入0.5升肥皂水溶液。让工作室以60转/分的牵连转动角速度和相对转动角速度作行星式旋转。加工进行了3个小时。在加工之后,对另件进行外观检验,並对表面的粗糙度作出评价,其Ra值在0.32微米左右,表面上不存在孔隙。
我们曾在较低的相对旋转速度条件下对同样参数的钢球进行过实验。表面是没有光泽的,保留有大量的孔隙。
例2
重复例1所述的方法,其差别仅在于转速为150转/分。植入物本体具有光亮的表面,不存在孔隙。表面粗糙度值Ra=0.20微米。
例3
重复例1所述的方法,其差别仅在于角速度为200转/分。植入物本体具有特别光亮的表面,表面粗糙度值Ra为0.18微米。在此实验中曾经在高于200转/分的速度下额外地加工一对植入物本体。但所得到的本体沿被加工表面边缘有一些瘤,是由于塑性变形过大造成的。
为了确定所得工件生物学惰性的程度,我们按照例1~3的条件另外加工一些钛的薄片。然后将这些薄片在恒温37℃条件下在样品介质(生理溶液,血浆和血清)中进行了几乎1年的试验。借助电子显微镜对这些薄片进行过研究,没有发现任何细微缺陷;而与此同时,对于未经上述加工的类似钛薄片,经过半年就发现了细微缺陷,而且数量是随时间增加而增加的。
除此而外,按照例1~3加工出来的薄片曾被植入实验用的小白鼠大腿肌肉中,随后进行了植入物组织形态学研究。此研究表明,按照上述方法加工出来的钛薄片,实际上对周围的组织没有任何生物作用。
Claims (6)
1、一种心血管外科用的钛植入物,具有经过加工的抗血栓的生物惰性表面,其特征在于,被加工出来的表面是一个在自由的微粒体作用下经过压实的塑性变形表面。
2、一种按照权利要求1的要求制造钛植入物的方法,包括植入物毛坯的成形及随后其表面的初步加工和最后加工,其特征在于:在最后加工阶段是将毛坯放进装有抛光微粒状自由介质及肥皂水溶液的工作室中,此抛光微粒是一些回转体;使该工作室以一定的速度作行星式旋转,此速度足以使毛坯通过与微粒的相互作用而产生塑性变形。
3、如权利要求2所述的方法,其特征在于回转体是一些球体。
4、如权利要求3所述的方法,其特征在于,上述球体是由铬钢制做的。
5、如权利要求4所述的方法,其特征在于,球体的直径为0.8~1.0毫米。
6、如权利要求5所述的方法,其特征在于,在钢球直径为0.8~1.0毫米的情况下,使工作室作行星式旋转的合成速度是60~200转/分,转动时间为2~4小时。
