Description

用于释放分子筛材料以形成血液凝块的装置和方法
技术领域
本发明通常涉及血液凝结装置,更具体地涉及血液凝结材料、结合这种材料的装置以及用于释放这种材料以用作出血控制装置的方法。
背景领域
血液是包含分散于液相中的红细胞、白细胞、血球以及血小板的液态组织。所述液相是血浆,其包括酸性物质、脂类、可溶的电解质以及蛋白质。蛋白质悬浮于液相中,并且可通过任意的各种方法从液相中分离出来,诸如过滤、离心分离、电泳以及免疫化学技术。悬浮于液相中的一种独特蛋白质是纤维蛋白原。当发生出血时,纤维蛋白原与水和凝血酶(一种酶)反应以形成纤维蛋白,其不能溶于血液中并聚合以形成凝块。
在各种情况下,包括人类的动物可能会受伤。所述创伤经常伴随出血。在一些情况下,创伤和出血是较轻微的,除了进行简单的急救之外,需要正常的血液凝结功能。但是不幸的是在其它情况下会发生大出血。这些情况通常需要专门器材和材料以及受过训练的人员来给予合适的救助。如果不能迅速获得这种救助,会发生过多的失血。当出血非常严重时,有时立即获得器材和受训人员的救助仍不足以及时止住血流。
此外,严重创伤通常在偏远地区或诸如战场上的地点发生,在所述地方不能立即获得足够的医疗救助。在这些情况下,重要的是止住出血即使是不严重创伤中的出血足够长的时间以允许受伤的人或动物来接受医疗。
在致力于解决所述问题的过程中,对材料进行研发以便控制在不能获得常规救助或不是最有效救助的情况下的过多出血。虽然已经显示这些材料有一定的成功性,但是有时它们对外伤没有足够的有效性,并且费用趋于偏高。另外,这些材料在一些情况下有时是无效的,并且可能难于施用到伤口以及从伤口移除。
另外或可替换地,以前研发的材料会产生不希望的副作用。例如现有技术的血液凝结材料通常是粉末或细小的微粒,其中材料的表面区域通常在将材料施用到血液时产生放热反应。时常会不必要地将过多的材料倒在伤口上,其会加剧该放热作用。依赖于材料的独特性质,最终的放热量会足以导致患者的不舒适或甚至会烧伤患者。虽然一些现有技术的专利明确记载最终放热量为希望的特征,其可提供给伤口类似于烧灼的凝结效果,但是存在伤口位置处和伤口位置周围的组织会受到不希望影响的可能性。
此外,为了将所述材料从伤口移除,通常需要冲洗伤口。如果给予的材料量导致不舒适或灼烧,会需要立即冲洗伤口。在受伤的人或动物还没有被运输到能够提供所需冲洗设施的情况下,会导致不希望的作用或对伤口的过度处理。
在手术过程中出血也会是一个问题。除了对切口或内部出血区域进行缝合或钉住之外,经常使用在出血位置上施加压力和/或吸收血液的纱布或其它材料来控制出血。但是,当出血变得过多时,这些措施会不足以止住血流。另外,任意高度放热的出血控制材料会伤害出血位置周围的组织并且其设计会导致在使用后不容易被移除。
基于前述,本发明的一个总的目的是提供用于控制出血的装置和方法,使用它们可克服现有技术的问题或在现有技术上进行改进。
发明内容
根据一方面,本发明提供一种用于促进血液凝结从而控制出血的装置。该装置包括用于在其中保持微粒形式的分子筛材料的容器。该容器的至少一部分由其中具有开孔的网状物限定,这样当装置应用到出血位置时,微粒分子筛材料通过开孔与血液进行接触。
本发明的其它方面包括一种用于控制出血的垫和一种能施用到出血伤口的绷带。在垫和绷带两者中,具有网状结构和保持在其中的分子筛材料微粒。在垫的实施例中,具有附连到网状结构的刚性或半刚性的支撑物,以便于将压力施加到垫和伤口上。在绷带中,具有附连到网状结构的柔性基体,所述基体可以是粘合地附连以覆盖伤口的织物或塑料元件。在任意的实施例中,网状结构可由相互连接并布置成限定开孔的多个元件(合成材料或天然材料的线、细丝或带)来限定。所述开孔的尺寸制成允许在分子筛材料微粒和血液之间保持接触。
在本发明的又一方面,一种包扎出血伤口的方法,包括提供微粒形式的分子筛材料并且将所述材料保持在网状结构中,将网状结构放置在出血伤口上,这样分子筛材料与从伤口流出的血液进行接触,施加压力到网状结构以确保所述材料与血液接触,以及将网眼材料从伤口移除。
本发明的一个优点在于:在完成将本发明的任意装置施加到出血伤口之后,可容易移除所述装置。尤其是,由于沸石材料为颗粒、珠子或弹丸的形式并封装入袋子或网状结构中,所述材料可清洁地从治疗伤口移开并丢弃。因此,需要对伤口进行较少冲洗或不需要冲洗伤口来将剩余的沸石冲洗掉。在其中容纳沸石材料的袋子结合到粘性绷带中,所述装置可放置到伤口上持续导致凝结的必要时间量。
另一优点在于:微粒形式的沸石材料允许材料较少放热地与血液反应。当微粒尺寸增加时(例如从细微到粗糙),微粒与血液可接触的表面积增加。材料的多孔性质还允许液态血液组分被芯吸掉以导致血液增稠,从而促进凝结形成。由于暴露到血液的微粒表面积减少了,实现了从血液中较少侵略性的汲取水分,由此缓和了在伤口位置处经受的所述放热作用。
本发明的又一优点在于:合适剂量的分子筛材料可容易施加到开放伤口。尤其是当所述装置是容纳沸石材料的多孔袋子时,所述装置可容易地从消毒的包装中取出并直接保持在血液从其喷出的位置处,在不将粉末或弹丸泄漏到伤口区域外部的情况下促进血液的凝结。消除了用于施加到出血伤口的分子筛材料量的估计、预算或计算。因此,浪费较少分子筛材料或不浪费分子筛材料。

具体实施方式
在此公开的是用于将材料释放到伤口以促进血液凝结以及包扎伤口的装置和方法。所述装置通常包括可施加到出血伤口的应急手段或装置,这样所述材料接触伤口组织以通过吸收至少部分液相的血液而减少或止住血流,从而促进凝结。一种装置包括容器,用于在其中保持微粒形式的分子筛材料。至少一部分容器由其中具有开孔的网状物限定,并且至少一部分微粒分子筛材料通过开孔与血液直接接触。
在本发明中使用的分子筛材料可以是合成聚合物凝胶、纤维素材料、多孔硅胶、多孔玻璃、氧化铝、羟磷灰石、硅酸钙、氧化锆、沸石等。示范性的合成聚合物包括但不限于苯乙烯(stylene)-二乙烯苯共聚物、交联聚乙烯醇、交联聚丙烯酸酯、交联乙烯基醚-马来酐共聚物、交联苯乙烯(stylene)-马来酐共聚物或交联聚酰胺及其组合。
分子筛材料优选为沸石。可用的其它分子筛材料包括但不限于八面沸石。在此所使用的术语“沸石”指的是铝矽酸盐的晶体形式,具有脱水能力而在结晶结构中不经受明显的变化。沸石可包括一种或多种离子种类,诸如像钙和钠两部分组成。典型地,沸石是由大约90%重量的钙和大约10%重量的钠组成的易碎材料。钙部分包含大约5埃大小的晶体,以及钠部分包含大约4埃大小的晶体。沸石的优选分子结构是“A型”晶体,即具有限定圆形或基本圆形开孔的立方晶体结构的一种晶体。
沸石可与其它材料混合或以另外的方式与其它材料结合使用,所述材料具有脱水能力而在结晶结构中不经受明显的变化。所述材料包括但不限于硫酸镁、偏磷酸钠、氯化钙、糊精、多醣、前述材料的组合以及前述材料的氢氧化物。
用于公开应用中的沸石可天然生成或合成制造。各种天然生成的沸石在沉积环境以及其它地点中以沉积物存在。可应用于在此描述的合成物的天然生成的沸石包括但不限于方沸石、菱沸石、片沸石、钠沸石、辉沸石以及thomosonite。还会发现,用于在此描述的合成物和方法中的合成制造的沸石通过下述工艺生产,其中稀土元素的氧化物由硅酸盐、氧化铝或与碱或碱土金属氧化物组合的氧化铝取代。
各种材料可与沸石混合、联合或结合入沸石中以在伤口位置处保持无菌环境或提供补足沸石凝结功能的作用。可用的示范性材料包括但不限于药剂活性的化合物,诸如抗生素、抗真菌试剂、抗菌剂、抗炎试剂、止痛剂(例如甲氰咪胍、马来酸氯苯那敏、盐酸苯海拉明以及盐酸异丙嗪)、细菌抑制剂、包含银离子的化合物等。可结合用来提供附加止血功能的其它材料包括但不限于抗坏血酸、凝血酸、芸香苷以及凝血酶。也可以加入在伤口位置上具有希望作用的植物制剂。
在本发明的一个实施例中,参照图1示出用于直接在伤口位置处促进血液凝结的装置。该装置是渗透性袋子,其允许液体进入以接触保持于其中的血液凝结沸石(或其它分子筛)材料。密封的包装(未示出)提供用于储存所述装置的消毒环境直到所述装置会被使用。整体用10示出并在此后称为“袋子10”的所述装置包括网筛或网状物12以及由网筛或网状物保持于其中的沸石微粒14。网状物12在所有的面上都封闭,并且限定能够将沸石微粒14保持于其中的开孔,同时允许液体流过。如图所示,示出网状物12为展平的状态,并且只示出少许沸石微粒14。
沸石微粒14基本为球形或不规则性状(例如球形、珠子、弹丸等),并且直径为约0.2毫米(mm)到约10mm,优选直径为约1mm到约7mm,更优选直径为约2mm到约5mm。在任意实施例中(球形、珠子、弹丸等),随着微粒尺寸增加,可获得由血液接触的较小微粒表面积。因此通过改变微粒尺寸可控制凝结速度。此外,也可控制水分的吸收(水分也具有沸石的放热作用)。
网状物12由互连的材料线、细丝或带形成。所述线、细丝或带可以任意一种方式或多种方式的组合互连,所述方式包括但不限于编织成纱布、缠绕、整体形成等。优选地,所述互连使得网状物可弯曲,同时基本保持由其限定的开孔的尺寸。制成线、细丝或带的材料可以是聚合物(例如尼龙、聚乙烯、聚丙烯、聚酯等)、金属、玻璃纤维或有机物(例如棉、羊毛、丝等)。
现在参照图2,由网状物12限定的开孔的尺寸制成为保持沸石微粒14,但是适于血液流动通过开孔。由于网状物12可围绕沸石微粒14拉紧,微粒可延伸通过开孔一距离d。如果沸石微粒14延伸通过开孔,微粒则能够直接接触其上施加袋子10的组织。这样从组织喷出的血液立即接触沸石微粒14,并且其水相被芯吸到沸石材料中,由此促进血液凝结。但是,本发明没有必要使得沸石微粒通过网状物伸出。
为了将袋子10施加到出血伤口,从包装中取出所述袋子并放置到出血伤口上。处于网状物12中的沸石微粒14接触伤口的组织和/或血液,并且血液的至少一部分液相由沸石材料吸收,由此促进血液凝结。
本发明的另一实施例是参照图3的垫,其以20示出,并且此后称为“垫20”。垫20包括网状物12、由网状物12在其中保持的沸石(或其它分子筛)微粒14、以及在将垫20施加到出血伤口时施加压力的支撑物22。如上所述,网状物12具有能够将沸石微粒14保持于其中同时允许血液流过其的开孔。
网状物12缝合、胶合、夹紧或以另外的方式安装在支撑物22上。支撑物22包括底表面24和顶表面26,沸石微粒14由容器12保持接触底表面24。底表面24不透过沸石微粒14(防止微粒迁移入支撑物22中),并且进一步抵抗水或其它流体的吸收。顶表面26能够具有压力,该压力由人们将垫20施加到出血伤口或由顶表面26上支撑的重体施加到其上。整个支撑物22是刚性的或半刚性的,以允许施加压力,同时减少对患者造成的不舒适性。
为了将垫20施加到出血伤口,从其包装中取出垫20并放置到出血伤口上。由于具有图1和图2实施例的袋子,沸石微粒14或者与伤口组织接触或者与血液直接接触。用手对顶表面26的加压或将重体施加到该表面可将压力施加到伤口,由此促进沸石微粒14和伤口之间的接触并促进血液液相的吸收。使用捆扎装置诸如带子、弹性装置、尼龙搭扣材料、前述装置和材料的组合等也可将垫20(具有或不具有重体)保持到伤口上。
现在参照图4,本发明的另一实施例为绷带,其以50示出,所述绷带包括保持在网状物12中的沸石微粒14(或一些其它分子筛材料)并安装到可施加到伤口(例如使用压敏粘合剂将绷带50施加到使用者的皮肤)的柔性基体52。网状物12缝合、胶合或以另外的方式安装到基体52以形成绷带50。
基体52是塑料或织物元件,其是传导性的以便被保持在受伤人或动物的出血伤口上或出血伤口附近的皮肤上。粘合剂54设置在接合受伤人或动物皮肤的基体52表面上。具体地,如果基体52是非透气性塑料材料的话,基体可包括孔56,以允许湿气从皮肤表面消散。
在制备用于本发明装置的沸石材料(也就是材料形成微粒形式)的过程中,在材料形成微粒之前或之后,通过将热量施加到沸石材料可控制沸石水合的初始水平。但是,还惊奇地发现当沸石的微粒尺寸增加时,水分含量对作为将沸石微粒混合到血液中产生的任意放热具有较小的相关作用。这样,可通过挤出、碾磨、铸造等将沸石材料形成沸石微粒(在图1-4中以14示出)。

Claims (19)

1.一种用于促进血液凝结的装置,包括:
容器,在所述容器中保持微粒形式的分子筛材料,所述容器的至少一部分由网状物限定,所述网状物在其中具有开孔;
其中在处理出血伤口时,所述装置的应用致使至少一部分所述微粒分子筛材料能够通过所述开孔与血液进行接触。
2.如权利要求1所述的用于促进血液凝结的装置,其中所述分子筛材料是沸石。
3.如权利要求2所述的用于促进血液凝结的装置,其中所述沸石包括具有约0.2mm到约10mm直径的微粒。
4.如权利要求2所述的用于促进血液凝结的装置,其中所述沸石包括具有约1mm到约7mm直径的微粒。
5.如权利要求2所述的用于促进血液凝结的装置,其中所述沸石包括具有约2mm到约5mm直径的微粒。

9.如权利要求8所述的垫,其中所述分子筛材料是沸石。
10.如权利要求9所述的垫,其中所述沸石包括具有约0.2mm到约10mm直径的微粒。
11.如权利要求9所述的垫,其中所述沸石包括具有约1mm到约7mm直径的微粒。

15.如权利要求14所述的绷带,进一步包括位于所述基体上的粘合剂,所述粘合剂构造成方便将所述绷带保持在使用者的皮肤上。
16.如权利要求14所述的绷带,其中所述基体包括孔。
17.如权利要求14所述的绷带,其中所述分子筛材料是沸石。
18.一种包扎出血伤口的方法,所述方法包括下述步骤:
提供微粒形式的分子筛材料并且将所述材料保持在网状结构中;
将所述网状结构放置在出血伤口上,这样所述分子筛材料与所述出血伤口的受伤组织接触;
施加压力到所述网状结构;以及
将所述网眼材料从所述伤口移除。
19.如权利要求18所述的方法,进一步包括使用捆扎装置将所述网状结构保持在所述出血伤口上。
