Description
技术领域
本发明通常涉及一种用于治疗肥胖病的装置。尤其涉及一种易插入和可移动的胃用收扎带装置,其可围绕并挤压胃部从而形成一个具有缩减直径的气孔口。
背景技术
近年来,已经有很多方法被用于治疗肥胖病。其中较具前景的一种方法是环绕胃的一部分放置收扎带,从而挤压胃部以产生小于胃的正常内径的气孔口,以此限制食物进入胃的下部消化区。
这种收扎带被Kuzmak等人在美国专利US4,592,339中作了描述。Kuzmak描述了一种气孔可调节的胃用收扎带,其包括一个可通过远端注入部位膨胀和收缩的气囊部分。气囊膨胀部分可在手术中和手术后调节开口的气孔。
在过去的几年中,现有技术的收扎带的制造者已经改进了这些收扎带的气囊设计。然而,进一步改进的一个重要方面是气孔可调胃用收扎带的改进,这种收扎带包括比现有技术的收扎带更适合病人组织的气囊部分,这一改进将进一步确保气囊充填均匀,且气囊壁的所有部分都不会承受高压。
发明内容
本发明的目的在于提供一种用于克服现有技术中上面指出的和其他的缺陷的可调节气孔的胃用收扎带,这种收扎带可被配置为没有褶皱和折痕,从而可靠地充满填充溶液。
本发明提供一种胃用收扎带,包括:一弹力装载带;附于所述弹力装载带上的气囊;以及与所述气囊流体连通的一流体供给管,其附于所述的弹力装载带;其中,所述的气囊包括界定多个内腔的多个隔板;所述流体供给管还包括多个进水口,各进水口与所述气囊的对应内腔和所述流体供给管流体连通;所述流体供给管的进水口的其中两个或多个具有不同的尺寸。
在本发明的一个实施例中,胃用收扎带同样包括一弹力装载带,其上排列一流体供给管和一气囊。本实施例中的气囊有一位于气囊内壁顶部的加强部分。从而,收扎带环绕胃放置时气囊适合于在加强部分间弯曲。

具体实施方式
现参考附图,其中相同的数字标记表示同样的构件。图1和图2描绘了具有现有技术中可由流体膨胀的限制带12的胃10,与带12流体连通的一流体供给管14和远端的流体注入口15流体连通。带12一般包括一外部弹力装载带,其内部附有一可膨胀气囊18。膨胀后的气囊18限制胃10的容积。要使气囊18膨胀,将填充溶液注入注入口15,且通过供给管14将填充溶液输送至气囊18即可。
当将收扎带12环绕胃10放置时,现有技术收扎带12的气囊18具有折叠且形成折痕的倾向。这些折痕在某些情况下会限制填充溶液向气囊18的所有区域流动。这将影响气囊18的几何结构,并可能损坏气囊18。
现参考图3至5,显示了本发明的分段胃用收扎带20。与现有技术的胃用收扎带相似,分段胃用收扎带20有一弹力装载带22、与气囊28流体连通的一流体供给管24和一远端流体注入口(图中未示)。气囊28优选长度为大约8至大约15cm,更优选为大约11cm;但是应该意识到,气囊28可以是能对胃部提供足够挤压的任何长度。气囊28优选由具有大约0.3mm至大约0.7mm厚度的材料组成,更优选的是厚度大约0.5mm的材料。显然,材料的厚度取决于气囊的材料,且应意识到气囊的厚度可因气囊组分不同而改变。优选弹力装载带22比气囊28长,也可以是足以适应所用锁紧机构50的任何合适的长度。分段胃用收扎带的所有构件优选由医用等级的硅聚合物组成,但也可以由任何柔韧的生物相容材料组成,包括可植入聚氨酯。
如图4中清楚显示的,分段胃用收扎带20也有一个或多个位于气囊28内的隔板30,将气囊28的内部空间分割为一个或多个腔室32。隔板可以在气囊中均匀的间隔排列(如图所示),或不均匀的间隔排列,隔板垂直于弹力装载带22的最长边或与弹力装载带22最长边呈一定角度。同样,隔板30可以用其它材料结构或特征如多层材料来加固,从而辅助气囊28在这些部位弯曲,多层材料包括一种或多种材料、加厚材料、织构化材料或一种致密材料。
流体供给管24有进水口34,每一个进水口与一个腔室32相通,将填充溶液分配到各腔室中。进水口34可以具有基本相同的直径。可选地,进水口34可以沿着管24从最接近流体注入口的端部开始按最小直径到最大直径有序排列,从而最接近流体注入口的腔室可以与远离流体注入口的腔室以基本相同的速率充填。而且,在有些应用中,如图7所示,最好在进水口34处设置一单向止回阀,从而使填充溶液可进入腔室而不能通过进水口34流出腔室。例如,图7显示的具有进水口34的流体供给管包括鸭嘴型止回阀36,从而使填充溶液可进入腔室而不能通过进水口34流出腔室。尽管在本描述性实施例中显示的是鸭嘴型止回阀,但是,任何允许填充溶液进入腔室但不能通过进水口流出腔室的单向止回阀都是可以的。可选地,由于在有些情况下填充溶液可能需要从气囊中排出,那么使用双向止回阀可能更有利,双向止回阀仅允许在流体供给管中的压力低于腔室中填充溶液的压力时使填充溶液排出腔室。优选的,这一压差通过使用注射器从注入口抽出流体而形成。
流体供给管24可以附着于气囊28内带部22的表面顶部,或者嵌入气囊28内的带部22,如图5所示。流体供给管24也可以附着于气囊28外部带部22的表面顶部,如图6所示,或被嵌入气囊28外的带部22。
可选地,每个隔板都可以有一小孔,使填充溶液可通过该小孔从一个腔室流动到隔壁腔室。这样的话流体供给管将只需要一个通向腔室的进水口,优选是在气囊的两端的其中一个腔室;尽管多个入口仍能使用。
分段胃用收扎带20也包括一锁紧机构50,从而该分段胃用收扎带可以可释放地固定在绕胃部的一环绕位置。索紧机构50可以是将分段胃用收扎带20保持在环绕位置的任何合适结构,例如导向片和锁扣结构、滑动片和槽结构、钩扣结构、或者如图3、4、8所示将片缝合在一起,但并不仅限于此。
分段胃用收扎带20的安装这样进行:首先将收扎带穿过套管针插入病人的腹部;下一步使用顿器解剖器械在食管胃结附近的胃后面切出一个通道;然后通过一器械例如夹钳或钝器解剖器械夹住分段胃用收扎带20,通过切出的的通道将其围绕病人胃部卷绕;然后合上锁紧机构50;然后将注入口附结于胃用收扎带之上,并固定在腹部或其他合适位置的皮下;然后将一种合适的填充溶液例如盐水注入注入口并通过流体输送管24的注入口34将溶液输送到气囊28的腔室32中。如果需要在当前或将来某时间安装胃用收扎带,可通过将注射器插入注入口抽出溶液而从气囊28中排出预定数量的填充溶液。
图8显示了本发明胃用收扎带的另一实施例。在该实施例中,胃用收扎带100同样包括了一弹力装载带105、一与气囊115流体连通的流体供给管110和一远端设置的流体注入口(未示出)。在本实施例中,气囊115有一个和多个加强部分120。优选地,加强部分120沿着气囊115的内部长度均匀间隔排列,加强部分120可由较厚的材料、织构化材料、硬的或软的材料或者其他相似的结构组成,其有助于气囊115环绕胃部放置时在相邻的加强部分120之间的弯曲。在图8所示的本优选实施例中,每个加强部分120每端厚度与气囊115的厚度基本相同;然而每个加强部分在其中点最厚,并且比气囊厚。每个加强部分120的中点优选为气囊115厚度的大约2至8倍,更优选为气囊115厚度的大约4倍。气囊115环绕胃部时,可以使用加强部分120将气囊分为很多腔室,也可以不用加强部分。
尽管本发明已经通过一些实施例进行了描述,而且示例性实施方式已经描述的相当详细,申请人不希望如此详细描述附属权利要求以约束或在某些方面限制权利要求的保护范围。对本领域技术人员而言,另外的优点和改进是显而易见的。
例如,代替在气囊中使用隔板以在气囊中制造腔室。分段胃用收扎带由隔开的多个气囊组成,其附于弹力装载带上。
此外,对本领域技术人员而言,显然上述发明同样适用于其它类型的可植入收扎带。例如,收扎带可用于治疗粪便失禁,美国专利6,461,292中公开了这样一种收扎带,其内容在此引入作为参考。收扎带可用于治疗尿失禁,美国专利2003/0105385中公开了这样一种收扎带,其内容在此引入作为参考。收扎带可用于治疗胃痛和胃酸,美国专利6,470,892中公开了这样一种收扎带,其内容在此引入作为参考。收扎带可用于治疗阳痿,美国专利2003/0114729中公开了这样一种收扎带,其内容在此引入作为参考。
Claims (8)
1.一种胃用收扎带,包括:
一弹力装载带;
附于所述弹力装载带上的气囊;以及
与所述气囊流体连通的一流体供给管,其附于所述的弹力装载带;
其特征在于:
所述的气囊包括界定多个内腔的多个隔板;
所述流体供给管还包括多个进水口,各进水口与所述气囊的对应内腔和所述流体供给管流体连通;
所述流体供给管的进水口的其中两个或多个具有不同的尺寸。
2.如权利要求1所述的胃用收扎带,其特征在于:所述流体供给管包括多个止回阀,各设置在所述众多进水口中的其中一个内。
3.如权利要求2所述的胃用收扎带,其特征在于:所述多个止回阀各包括一个单向止回阀。
4.如权利要求2所述的胃用收扎带,其特征在于:所述多个止回阀各包括一个双向止回阀。
5.如权利要求1所述的胃用收扎带,其特征在于:所述多个进水口由小到大排列在所述流体供给管上,使所述气囊的多个腔室以基本相同的速率充填。
6.如权利要求1所述的胃用收扎带,其特征在于:所述气囊长度在大约8cm和大约15cm之间。
7.如权利要求1所述的胃用收扎带,其特征在于:所述弹力装载带还包括一锁紧机构。
8.如权利要求7所述的胃用收扎带,其特征在于:所述锁紧机构包括一个或多个缝合带。
